Légzőszervi megbetegedések primer immundefektusokban = Respiratory manifestations in primary immunodeficiencies
Absztrakt:
Veleszületett immundefektusokban a légutak fertőzései a leggyakoribb prezentációs
jelek és egyben a vezető halálok. Az egyes immundefektusokra jellemző lehet a
domináns kórokozó vagy kórokozóspektrum, amelynek ismerete a fertőzések
empirikus kezelésében és prevenciójában játszik döntő szerepet, vagy éppen
fordítva, a szokványostól eltérő patogén azonosítása terelheti gyanúnkat
immundefektus irányába. Az akut légúti betegségek mellett a primer immundefektus
krónikus tüdőbetegségekre is gyakran hajlamosít. A visszatérő fertőzések
következtében az alsó légutak visszafordíthatatlan szerkezeti károsodása,
bronchiectasia alakulhat ki, amely további fertőzések kiindulási gócaként
szolgálhat. A krónikus tüdőbetegségek másik csoportja, az interstitialis
tüdőbetegségek az infekcióktól függetlenek, és az egyes immundefektusokban
jellemző immunregulációs zavar megnyilvánulásaként foghatók fel. Az életminőség,
a hosszú távú prognózis tekintetében döntő jelentőségű a másodlagos
tüdőkárosodás szempontjából rizikócsoportot jelentő betegek azonosítása és a
rendszeres szűrővizsgálatok elvégzése az elváltozások korai felismerése

Abstract:
The respiratory infections are the most common presentations and leading cause of
morbidity and mortality in primary immunodeficiencies. The pathogen or spectrum
of pathogens may be characteristic for the underlying primary immunodeficiency,
and that knowledge plays an important role in the empirical treatment or
prevention of the infections, or, conversely, the identification of an unusual
pathogen may raise the suspicion on an immunodeficiency. Apart from the acute
respiratory infections, chronic lung diseases are also common complications. The
recurrent infections result in bronchiectasis, an irreversible structural damage
of the respiratory tract, that frequently serve as locus for subsequent
infections. The other group of chronic pulmonary complications are the
interstitial lung diseases, which seem to be independent from infections and can
be regarded as the pulmonary manifestation of the immune dysregulation. It is
important to identify those patients with immunodeficiency who are at increased
risk of secondary lung complications and to regularly screen this group for the

A szöveti hidráció szabályozásának experimentális vizsgálata - különös tekintettel a perinatális időszakra = Experimental studies on the regulation of tissue hydratation with particular reference to the perinatal period
1. A vízterek perinatális átrendeződésének új koncepcióját dolgoztuk ki. Újszülött nyulakban és foetusokban igazoltuk, hogy a különböző mobilitású szöveti vízfrakciók aránya az érés során sajátosan változnak. Igazoltuk, hogy a foetális/neonatális agy- és tüdőszövetben a fizikai vízterek átrendeződését anyai steroid, terbutalin és amilorid kezelése nem befolyásolja. 2. Humán anyagban 14-40 hetes gesztációs korú magzatokban vizsgáltuk az agyszövet-specifikus aquaporin-4 (AQP4) és aquaporin1 (AQP1) membrán-csatorna fehérjék expresszióját és leírtuk azok intracellularis elhelyezkedésének és mennyiségének semi-kvantitatív változásait. Felhívtuk a figyelmet az AQP-k élettani és klinikai jelentőségére az agy fiziológiás dehidrációjának folyamatában. 3. Kimutattuk, hogy az angiotenzin konvertáló enzim (ACE) DD genotipusa protektív hatással bír alacsony súlyú koraszülöttekben a keringési elégtelenség kialakulása szempontjából. 4. Kongentiális szívfejlődési rendellenességgel született újszülöttek vérmintáinak retrospektív analízisével megállapítottuk, hogy a vaszkuláris endotheliális növekedési faktor (VEGF) génpolymorfizmusa növeli a szívfejlődési rendellenességek kialakulásának kockázatát. 5. Megállapítottuk, hogy az éretlen vese beszűkült koncentrálóképessége az eddigi ismert mechanizmusok melett a vese papilla/medulla magas hialuronsav tartalmával magyarázható. A szöveti hialuronsav az ADH-mediálta ozmotikus vízmozgást gátolja. | 1. A new concept of perinatal redistribution of body fluid compartments has been developed. In fetal/neonatal rabbit pups we demonstrated marked changes in the various physical water compartments which remained unaffected by maternal administration of steroid, terbutaline and amiloride. 2. The developmental changes in aquaporin-1 (AQP1) and AQP4 were investigared in human brain tissue during the period of 14-40 weeks of gestation. We demonstrated the progressive increase of their expression and the pattern of their distribution as the gestation advances. Furthermore, the role of AQP-s in the physiological brain dehydration has been established. 3. Evidences have been provided to indicate the protective effects of ACE DD genotype on the development of circulatory failure in low-birthweight neonates. 4. A gyermekkori uveitis klinikai jellemzői és terápiája = Clinical features and therapy of uveitis in childhood
Absztrakt:
Bevezetés: Az uveitis a szem középső burkát alkotó képletek
gyulladásával járó betegség, melynek incidenciája alacsony. Hátterében a
leggyakrabban autoimmun megbetegedések és infekciók állnak. Az autoimmun
kórképek közül a juvenilis idiopathiás arthritishez asszociáltan előforduló
uveitis a leggyakoribb. A betegek jelentős része a lokális szemészeti kezelésre
kevésbé mutat javulást, így szükség lehet immunmoduláló terápia bevezetésére.
Célkitűzés és módszer: Retrospektív vizsgálatunkban
összesen 33 gyermek adatait elemeztük, akiket az elmúlt 5 év során a Pécsi
Gyermekgyógyászati és Szemészeti Klinikán uveitisszel diagnosztizáltak és
kezeltek. Eredmények: A betegek átlagéletkora 9,3 (0,3–17,8) év
volt. A nemi megoszlásban lényeges különbséget nem észleltünk, de juvenilis
idiopathiás arthritishez társult uveitis esetén női túlsúlyt találtunk. Az
esetek 60%-ában (20/33) sikerült a betegség háttérében etiológiai faktort
azonosítani. 12 betegnél juvenilis idiopathiás arthritishez, két esetben
Behcet-kórhoz, egy gyermeknél gyulladásos bélbetegséghez társultan alakult ki
uveitis. 5 betegnél az uveitis hátterében infekció igazolódott. A kórkép az
autoimmun betegségek többségében anterior, ezzel szemben infekciók esetén
posterior lokalizációjú volt. A betegek többsége a lokális szerek mellett
szisztémás terápiában is részesült. 3 gyermek szisztémás szteroidot kapott, 18
esetben betegséglefolyást módosító reumaellenes szer, per os
metotrexátkezelés került bevezetésre. 13 betegnél a súlyos betegségaktivitásra
való tekintettel biológiai terápiát (adalimumabinjekció) indítottunk. A
gyógyszer bevezetését követően átlagosan 1,45 (0,75–2,5) hónapon belül remisszió
alakult ki. Következtetés: A gyermekkori uveitis kiemelt
jelentőséggel bír. A korai diagnózis, a megfelelő terápia, a rendszeres
kontrollvizsgálatok a társszakmák szoros együttműködését igénylik. Orv Hetil.

Abstract:
Introduction: Uveitis is characterized by inflammation of the
middle layer of the eye. Its overall incidence is low. Autoimmune diseases and
infections are the most common underlying diseases. Out of the autoimmune
diseases, juvenile idiopathic arthritis is associated most frequently with
uveitis. The topical ophthalmological treatment may fail in a significant
proportion of the patients and immunomodulatory therapy may be required.
Aim and method: In a retrospective study, data of 33
children diagnosed and treated with uveitis at the Department of Pediatrics and
Ophthalmology, University of Pécs during the last 5 years were collected and
analyzed. Results: The mean age of the patients was 9.3
(0.3–17.8) years. Boys and girls were equally affected with an exception of
patients with juvenile idiopathic arthritis where female predominance was found.
An underlying disease could be identified in 60% of the cases (20/33). Uveitis
was associated in 12 patients with juvenile idiopathic arthritis, in 2 patients
with Behcet’s disease and in a single case with inflammatory bowel disease.
Infections have been proven in 5 patients. The autoimmune diseases caused an eye
inflammation typically in anterior localization, in contrast to the infections
that resulted in posterior uveitis. The majority of the patients required
systemic treatment. 3 of them received systemic corticosteroid and 18 patients
methotrexate as disease-modifying antirheumatic drug. 13 children with severe
disease activity required biological therapy (adalimumab injection). Remission
could be achieved in 1.45 (0.75–2.5) months. Conclusion:
Pediatric uveitis is of great importance. Early diagnosis, adequate therapy and

A humán parechovírusok klinikai jelentősége súlyos újszülött- és csecsemőkori fertőzésekben hazánkban = The clinical significance of severe human parechovirus infections in newborns and infants in Hungary
Absztrakt:
Bevezetés: A humán parechovírus (HPeV,
Picornaviridae víruscsalád) okozta fertőzések többsége
tünetmentes, de gyermekeknél hasmenéssel járhatnak. Újabb megfigyelés, hogy a
HPeV-ok a csecsemők körében súlyos, központi idegrendszeri megbetegedést és
szepszisszerű kórképet is okozhatnak. A HPeV-ok szerepe a hazai klinikumban nem
ismert. Célkitűzés: A HPeV-ok kimutatása, meghatározása
gyermekektől származó székletmintákból és a klinikai tünetek elemzése.
Módszer: A HPeV-ok kimutatása és tipizálása a reverz
transzkripció–polimeráz-láncreakció és a szekvenálás módszereivel történt három
betegcsoportban: A) 10 év alatti kórházban kezelt, n = 75; B) 1 év alatti, n =
237; és C) szisztémás/idegrendszeri tüneteket is mutató hasmenéses gyermekek
(<18 év), n = 105 székletmintáiból. Eredmények: Az A
csoportban 4% (3/75) volt a HPeV-pozitív minták aránya (két HPeV1, egy nem
tipizálható HPeV), mind gastroenteralis tüneteket mutató csecsemőkből (7–11
hónapos). A B csoportban a HPeV-fertőzések aránya 6,8% (16/237) volt, melyek
közül öt HPeV1, hat HPeV3 és öt nem tipizálható HPeV volt. E csoportban a
HPeV1-fertőzések többsége (4/5) nem igényelt kórházi ellátást, míg a
HPeV3-fertőzéses esetek 83%-a (5/6) igényelte azt. A szisztémás/idegrendszeri
tüneteket is mutató betegekből (C csoport) azonosított HPeV-pozitív minták
(4,8%) két genotípusba tartoztak (három HPeV1 és két HPeV3), és mind kórházban
kezelt fiatal (≤2 hónapos) csecsemőktől származtak.
Következtetés: A HPeV1-fertőzések általában enyhébb
lefolyásúak, és ritkábban igényelnek kórházi ellátást. A HPeV1-fertőzés vezető
tünete a hasmenés, azonban 1–2 hónapos csecsemőknél idegrendszeri érintettség
(aluszékonyság, bágyadtság) is tapasztalható volt. Az újszülöttek körében a
HPeV3-megbetegedések gyakoribbak. A súlyos HPeV3-fertőzésre jellemzőek a
gastroenteralis tünetek (7/8), a ≥38 °C-os láz (6/7), az étvágytalanság (6/7),
az exanthema (4/7), az aluszékonyság/bágyadtság (3/7), a szepszisszerű állapot

Abstract:
Introduction: Most human parechovirus (HPeV, family
Picornaviridae) infections are asymptomatic but may cause
gastroenteritis in children. New reports show that HPeVs can be associated with
severe central nervous system symptoms and sepsis-like syndromes in infants. The
clinical significance of HPeVs in Hungary has not been investigated before.
Aim: The aim of this study was to detect genotype HPeV in
faecal samples of children and analysis of the clinical symptoms.
Method: For the detection and genotyping of HPeV strains,
reverse transcription–polymerase chain reaction and sequencing methods were used
from faecal samples of children with gastroenteritis divided into three groups:
group A) hospitalised children younger than 10 years (n = 75); group B) 0–12
months infants (n = 237) and group C) children less than 18 years of age with
sepsis-like/neurological symptoms (n = 105) were tested.
Results: Three HPeV positive samples (3/75, 4%) were found
in group A, two of them belong to the HPeV type 1, the third was non-typeable.
All positive samples were from infants of 7 to 11 months of age. In group B,
HPeV was detected in 6.8% (16/237) of the samples. Five were HPeV1, six were
HPeV3 and five were non-typeable. While most of the infants with HPeV1 (4/5) did
not require hospitalisation, 83% of the HPeV3 infected infants (5/6) did. Five
(4.8%) HPeV strains detected from children less than 18 years of age with
sepsis-like/neurological symptoms (group C) belonged to HPeV1 (three) and HPeV3
(two). All positive samples were from hospitalised infants less than 2 months of
age. Conclusion: HPeV1 infections are less severe in infants
than HPeV3 infections. The leading symptom of HPeV1 was diarrhoea, although in
infants less than 1–2 months neurological symptoms (somnolence, lassitude) were
also present. HPeV3 infections were more common among newborns. The main
symptoms of severe HPeV3 infection are: gastroenteritis (7/8), fever ≥38 °C
(6/7), loss of appetite (6/7), rash (4/7), somnolence/lassitude (3/7),
sepsis-like syndrome (3/7) and respiratory symptoms (2/7). Orv Hetil. 2019;

Severe acrocyanosis precipitated by cold agglutinin secondary to infection with Mycoplasma pneumoniae in a pediatric patient
This is the first report describing a severe form of cold agglutinin-
induced acrocyanosis with cutaneous necrosis after
Mycoplasma infection in a 9-year-old patient without
any other severe symptoms and laboratory alterations.
We also present the results of two non-invasive methods
used to determine the viability of tissues, degree of tissue
perfusion impairment, and the responsiveness of the microvasculature.
Laser Doppler flowmetry and laser speckle
contrast imaging, both suitable to measure tissue blood
perfusion non-invasively, have been used in the diagnosis
and follow-up of various peripheral vascular diseases.
In our patient, we demonstrated remarkably reduced microcirculation
before the treatment and a significant perfusion
increase in the acral regions after pentoxifylline
therapy. The investigational techniques were useful tools
to assess and quantify the severity of peripheral perfusion
disturbances and to monitor the efficacy of the treatment
in our patient
