Effects of treatment with HM insulin produced by Bioton, NovoNordisk and Eli Lilly in children with type 1 diabetes diagnosed in 2001 and 2002
WSTĘP. Celem pracy jest ocena wyrównania metabolicznego u dzieci leczonych
insulinami firm: Bioton, NovoNordisk oraz Eli Lilly.
MATERIAŁ I METODY. Badaniem objęto 142 dzieci w wieku 2-18 lat (śr. 11,5
roku) leczonych z powodu cukrzycy typu 1 w latach 2001 i 2002. U badanych analizowano:
wiek zachorowania, długość leczenia cukrzycy, występowanie ostrych powikłań oraz
stężenie HbA1c.
WYNIKI. Średnie stężenie HbA1c w grupie 1. wynosiło 7,28%, w grupie 2. — 6,68%, zaś w 3. — 7,64%. U żadnego z badanych dzieci w okresie obserwacji nie
stwierdzono ciężkich niedocukrzeń. Kwasicę metaboliczną, która wystąpiła w trakcie
infekcji wirusowej wymagającej podania płynów, rozpoznano u 6 najmłodszych dzieci
(< 5 rż.). U wszystkich badanych obserwowano zmniejszenie zapotrzebowania na insulinę,
u żadnego z nich nie stwierdzono pełnej remisji.
WNIOSKI. Leczenie insulinami firm Bioton, NovoNordisk oraz Eli Lilly
spowodowało dobre wyrównanie metaboliczne cukrzycy. Pacjenci leczeni metodą intensywną
za pomocą wstrzykiwaczy penów w celu poprawy wyrównania metabolicznego cukrzycy
powinni przyjmować insulinę o pośrednim czasie działania przynajmniej w 2 dawkach
na dobę.INTRODUCTION. The aim of the study was the appreciation of the degree of
metabolic control in children treated with Bioton, Novo Nordisk or Eli Lilly insulin.
MATERIAL AND METHODS. Examined were 142 children with type 1 diabetes treated
in the years 2001 and 2002, aged 2–18 years (mean 11,5 years). Analyzed were age,
duration of the diabetes therapy, frequency of acute complications, HbA1c.
RESULTS. The mean levels of HbA1c were in the group 1 - 7,28%, group 2
- 6,58%, group 3 - 7,64%. In the investigated children severe hypoglycemia was
not observed. Only in the group of 6 youngest children (< 5 years of age) in the
course of acute infections observed were metabolic ketosis. In all the children
observed was a decrease of insulin requirement but in none of them a complete
remission.
CONCLUSION. The treatment with insulin Bioton, Novo Nordisk and Eli Lilly
restored a good metabolic control in patients with type 1 diabetes. Patients treated
with an intensive insulin therapy with a pen for an optimalisation of the metabolic
control should receive an intermediate insulin at least in two daily doses
Comparison of effects of insulin Gensulin therapy and insulin analogues administred by the personal insulin pump in children and adolescents with type 1 diabetes
WSTĘP. Prawidłowa kontrola metaboliczna jest
jedynym udowodnionym sposobem opóźnienia wystąpienia
późnych powikłań u chorych na cukrzycę,
a intensywna czynnościowa terapia oraz częste monitorowanie
glikemii stwarzają szansę optymalizacji
wyrównania metabolicznego. Celem pracy jest
ocena wyrównania metabolicznego u dzieci i młodzieży
chorych na cukrzycę typu 1 leczonych insuliną
Gensulin R (IG-R) oraz analogami insuliny podawanymi
w ciągłym wlewie podskórnym za pomocą
osobistej pompy insulinowej (OPI).
MATERIAŁ I METODY. Badaniem objęto 86 dzieci
w wieku 5,0-16,9 roku, leczonych z powodu cukrzycy
typu 1 przez 2,8-8,7 roku (X - 4,5 roku), a z zastosowaniem
OPI 18-55 miesięcy (X - 25 miesięcy). Pacjentów
podzielono na 3 grupy: grupa 1 (16 dzieci)
- otrzymująca insulinę Gensulin R, grupa 2 (28 dzieci)
- przyjmująca NovoRapid, grupa 3 (42 dzieci)
- stosująca Humalog. Do oceny wyrównania metabolicznego
wykorzystano stężenie HbA1c oznaczone
w latach 2003 (231 oznaczeń) i 2004 (278 oznaczeń)
oraz wyniki badań glukozy uzyskane w 2004 roku
u 22 dzieci przy zastosowaniu systemu ciągłego monitorowania
glikemii (CGMS, continuous glucose
monitoring system).
WYNIKI. W 2003 roku średnia wartość HbA1c w grupie
1 statystycznie była istotnie wyższa od stężenia
notowanego w 2004 roku (p = 0,02). Takiej różnicy
nie wykazano w grupach 2 i 3 (0,7 vs. 0,2). W 2003
roku średnia wartość HbA1c w grupie 1 była wyższa
od stężenia HbA1c w grupach 2 i 3, różniąc się
w sposób istotny statystycznie (p = 0,000 vs. p = 0,03).
W 2004 roku różnicę znamienną statystycznie wykazano
tylko w grupach 1 i 2 (p = 0,01). W badaniu
CGMS hiperglikemię po śniadaniu stwierdzono
w grupach 1 i 3, natomiast przed obiadem i kolacją
oraz po nich stężenia glukozy tylko u pojedynczych
badanych były nieprawidłowe (> 160 mg/dl).
W godzinach nocnych stężenia glukozy w grupie 1
i 2 były prawidłowe (do 130 mg/dl), natomiast
w grupie 3 w pierwszej połowie nocy stwierdzono
hipoglikemię, natomiast w drugiej - stany hiperglikemii.
Hipoglikemie bezobjawowe w badaniu
CGMS (stężenie glukozy < 60 mg/dl) zanotowano
u 13 spośród 22 dzieci (59%), w grupie 1 u 5 spośród
10 pacjentów, natomiast w grupach 2 i 3
u 8 z 12 pacjentów.
Niedocukrzenia trwały 0-120 min,
średnio 95 minut na dobę na pacjenta. Najdłuższe
hipoglikemie obserwowano w pierwszej połowie nocy, najczęściej u chorych z grupy 3. W latach 2003
i 2004 u żadnego badanego dziecka nie odnotowano
hipoglikemii objawowej wymagającej podania glukagonu.
Z powodu kwasicy metabolicznej leczono
6 dzieci, w tym 2 osoby dwukrotnie. Najczęściej (12%)
rozpoznano ją w grupie 3. Jej przyczyną były infekcje
wirusowe przewodu pokarmowego, układu oddechowego,
a w jednym przypadku - blokada pompy.
WNIOSKI. W celu optymalizacji stężenia glukozy insulinę
Gensulin należy podać 30 minut przed posiłkiem.
Warunkiem prawidłowego wyrównania metabolicznego
jest przestrzeganie zaleceń dietetycznych
oraz godzin podania insuliny i spożycia posiłków.
Podanie IG-R nie zwiększa ryzyka występowania
hipoglikemii bezobjawowych oraz jawnych. Przy
ocenie skuteczności terapii cukrzycy u dzieci najlepszą
metodą kontroli glikemii jest badanie wykorzystujące
system ciągłego monitorowania.INTRODUCTION. Proper metabolic control is the only
proved method of delaying occurrence of complications
in diabetes at a later stage. Intensive insulin therapy
with frequent monitoring of blood glucose
level enables optimal metabolic control. The aim of
the study is to evaluate metabolic control in children
and adolescents with diabetes type 1 treated with
Gensulin R and analogues of insulin administered in
continuous subcutaneous insulin infusion (CSII).
MATERIAL AND METHODS. The study was conducted
in 86 patients aged 5.0-16.9 years, with duration
of diabetes type 1 2.8-8.7 (X - 4.5) years and using
the CSII from 18 to 55 (X - 25) months. Patients were
divided into 3 groups: group 1 - treated with
Gensulin R - 16 children, group 2 - NovoRapid
- 28 children and group 3 - Humalog - 42 children.
HbA1c in 2003 (231 samples) and 2004 (278 samples)
and glucose outcome in 22 patients in the
2004 year was examined.
RESULTS. In 2003 mean HbA1c value in the first group
was statistically significantly higher than in 2004
(p = 0.02). No statistically significant differences
were found in groups 2 and 3 (0.7 vs. 0.2). Statistically
significant difference occured between groups
1-2 and 1-3 (p = 0.000 vs. p = 0.03) in 2003, and in
2004 only between groups 1 and 2 (p = 0.01). In
continuous glucose monitoring system (CGMS)
examination, hyperglycemia was found in group 1
and 3 before breakfast while before and after dinner
and supper only few measures were abnormal
(> 160 mg/dl). Subjects in groups 1 and 2 had lower
blood glucose levels during nightime, patients in
group 3 displayed nocturnal hypoglicemia in the first
half of the night and in the second half of the night
blood glucose also increased. Asymptomatic hypoglycemia
was noted in CGMS examination (glucose
level < 60 mg/dl) in 13/22 of children (59%) in group
1 - 5/10 and in groups 2 and 3 together - 8/12 patients.
Hypoglycemia lasted from 0-120 min; on
average 95 min/24 hours/patients. The longest hypoglycemia
was observed in the first half of the night,
mostly in group 3. Symptoms requiring administration
of glukagon were not noticed in the year
2003 and 2004. Six children, 2 of them twice, were
hospitalized, because of ketoacidosis. Episodes of
ketoacidosis were most frequently (12%) diagnosed
in group 3, they were caused by virus infections of
the digestive tract, respiratory tract and in one case
by clogging in the distal tubing.
CONCLUSIONS. Insulin Gensulin should be given
30 min before mealtime for optimal glucose-lowering
effect. In order to achieve proper metabolic control
maintaining shick regime of insulin treatment as well
as keeping to time schedules and adherence to
a diabetic diet is necessary. Administration of insulin
Gensulin R does not increase the risk of hypoglycemia.
Continuous glucose monitoring system is the
best way of glucose levels monitoring in children
treated with insulin
Associations between adipocytokines and metabolic disturbances in obese adolescents - preliminary results
Wstęp Celem prezentowanych badań była ocena zależności
między obecnością składowych zespołu metabolicznego
u otyłych nastolatków a występowaniem
insulinooporności, z uwzględnieniem roli wybranych
adipocytokin.
Materiał i metody Badaniem objęto 99 nastolatków
w wieku 10-18 lat (średnia wieku 14,3 ± 1,8 roku)
diagnozowanych w Klinice Endokrynologii i Diabetologii
Wieku Rozwojowego we Wrocławiu z powodu
otyłości. U pacjentów określono: wskaźnik masy
ciała (BMI), obwód talii, zmierzono ciśnienie tętnicze,
wykonano krzywą cukrową (OGTT) z insuliną,
wyliczono HOMA-IR, oceniono gospodarkę tłuszczową,
zbadano stężenie leptyny, adiponektyny i rezystyny.
Insulinooporność rozpoznano, gdy: HOMA-IR było
większe od 2,5; zespół metaboliczny ustalono według
kryteriów IDF.
Wyniki W grupie badanej zespół metaboliczny rozpoznano
u 36,36% pacjentów. Insulinooporność
stwierdzono u 67,67% badanych, wśród pacjentów
z rozpoznanym zespołem metabolicznym u 86,11%,
bez zespołu metabolicznego u 55,5%. W grupie pacjentów
z ZM stwierdzono istotnie statystycznie niższe
stężenie adiponektyny w stosunku do grupy pacjentów
bez tych zaburzeń.
Wnioski
1. U otyłych nastolatków obserwuje się insulinooporność,
częściej u dzieci, które spełniają kryteria zespołu
metabolicznego.
2. Adiponektyna jest czułym markerem insulinooporności
i zespołu metabolicznego.Background The purpose of our study was to evaluate
associations between metabolic syndrome’s components,
insulin resistance and adipocytokines in obese teenagers.
Material and methods We examined 99 overweight and
obese children, aged 10-18 years (mean 14.3 ± 1.8) diagnosed
and treated in Department of Pediatric
Endocrinology and Diabetology, Medical University of
Wrocław. BMI, waist circumference, blood pressure,
OGTT with insulin levels, HOMA-IR, total cholesterol,
HDL, LDL and triglycerides levels were taken into account.
Moreover adiponectin and resistin were estimated.
Insulin resistance was diagnosed if HOMA-IR > 2.5. Metabolic
syndrome (MetS) was diagnosed according to IDF.
Results MetS was diagnosed in 36.36% of children. Insulin
resistance was diagnosed in 67.67% of children: in
86.11% children with MetS and 55.5% children without
MetS. In the MetS group lower concentration of
adiponectin was observed.
Conclusions
1. Insulin resistance is observed in obese adolescents, more
frequently in patients with metabolic syndrome.
2. Adiponectin is a sensitive marker for insulin resistance
and metabolic syndrome
Polymorphism of the FTO
The objective was to compare the impact of clinical and genetic factors on body mass index (BMI) in children with type 1 diabetes (T1DM) without severe obesity. A total of 1,119 children with T1DM (aged 4–18 years) were qualified to take part in the study. All children were genotyped for variants of FTO, MC4R, INSIG2, FASN, NPC1, PTER, SIRT1, MAF, IRT1, and CD36. Results. Variants of FTO showed significant association with BMI-SDS in the T1DM group. The main factors influencing BMI-SDS in children with T1DM included female gender (P=0.0003), poor metabolic control (P=0.0001), and carriage of the A allele of the FTO rs9939609 gene (P=0.02). Conclusion. Our research indicates, when assessing, the risk of overweight and obesity carriage of the A allele in the rs9939609 site of the FTO gene adds to that of female gender and poor metabolic control. Treatment of severe primary IGF-1 deficiency using rhIGF-1 preparation — first three years of Polish experience
Introduction: The objective of this study was to analyse the effects of the first three years of treatment with recombinant human insulinlike growth factor 1 (rhIGF-1) in patients from the Polish population.
Material and methods: Twenty-seven children (22 boys and five girls) aged 2.8 to 16.0 years old were qualified for treatment with rhIGF-1 (mecasermin) in different treatment centres, according to Polish criteria: body height below –3.0 SD and IGF-1 concentration below percentile 2.5 with normal growth hormone (GH) levels. Mecasermin initial dose was 40 μg/kg bw twice a day and was subsequently increased to an average of 100 μg/kg bw twice a day. Body height, height velocity, weight, body mass index (BMI), and adverse events were measured.
Results: Mecasermin treatment resulted in a statistically significant increase in body height (1.45 ± 1.06 SD; p < 0.01) and height velocity in comparison with pre-treatment values. The biggest change in height velocity happened during the first year and diminished during subsequent years. Body weight and BMI also increased significantly after treatment (1.16 ± 0.76 SD and 0.86 ± 0.75 SD, respectively; p < 0.01). Eight patients reported adverse events. These were mild and temporary and did not require treatment modification except in two patients.
Conclusions: Treatment with rhIGF-1 was effective and safe in Polish patients with primary IGF-1 deficiency. It had a clear beneficial effect on the height of the patients and significantly accelerated the height velocity, particularly in the first year of treatment
