Immunotherapy of transitional cell carcinoma of urinary bladder
Karcinom prijelaznog epitela mokraÄnog mjehura najÄeÅ”Äi je karcinom mokraÄnog sustava. KliniÄki razlikujemo dva oblika ove bolesti: miÅ”iÄno-neinvazivni i miÅ”iÄno-invazivni oblik. Temeljna kliniÄka odrednica miÅ”iÄno-neinvazivnih tumora su recidivne novotvorinske promjene; 60 ā 90 % miÅ”iÄno-neinvazivnih tumora recidivirat Äe ako se lijeÄe samo transuretralnom resekcijom (TUR). Upravo stoga se nakon TUR-a u pacijenata u kojih postoji visoki rizik od ponovne pojave ili progresije bolesti provodi intravezikalna imunoterapija BCGom (bacillus Calmette-Guerin). BCG predstavlja živi atenuirani soj Mycobacterium bovis. U
primjeni BCG-a intravezikalne terapije razlikujemo indukcijsku terapiju i terapiju održavanja. Intravezikalna aplikacija BCG-a uzrokuje masivan ulazak upalnih stanica u stijenku mokraÄnog mjehura te produkciju citokina detektibilnih u sluznici mjehura i u urinu, Å”to dovodi do imunog odgovora protiv tumora. Äinjenica je kako BCG uzrokuje dugotrajan i dugodjelujuÄi imuni odgovor. Do eradikacije tumorskih stanica dolazi zbog celularnih fokusa u stijenci mjehura, a kao direktan antitumorski efektorski mehanizam navodi se direktna antitumorska aktivnost IFN (interferon) i citotoksiÄnost NK (engl. natural killer) stanica.Bladder cancer is the most common cancer in urinary system. There are two clinical aspects of this disease: muscle non-invasive and muscle invasive disease. Basic characteristic of non-muscle invasive disease is tumor recurrence; 60-90 % of tumors will recurr if treated by transurethral resection (TUR) only. That is the reason why patients in whom exists the risk of recurrence or progression undergo intravesical bacillus Calmette-Guerin (BCG) immunotherapy. BCG represents live attenuated Mycobacterium bovis. There are two phases in BCG therapy: induction and maintenance therapy. Intravesical BCG application causes massive agregation of immune cells in bladder wall and producton of cytokins which causes cytotoxic tumor response. Direct effector mechanism is achieved by IFN (interferon) and NK (natural killer) cell cytotoxicity

Hypofrontality is a state of decreased cerebral blood
flow in the prefrontal cortex during executive function
performance; it is commonly observed in patients with schizophrenia. Cognitive
dysfunction, as well as the psychological symptoms of schizophrenia, influences
the ability of patients to reintegrate into society. The current study
investigated the effects of an interactive sports video game (IVG; Nintendo
Wiiā¢ Sports Resort) on frontal lobe function of patients with schizophrenia. A
sample of eight patients (6 male and 2 female; mean age = 46.7 years, standard
deviation (SD) = 13.7) engaged in an IVG every week for 3 months in a controlled,
single-blind, crossover study. Before and after the intervention we examined
frontal lobe blood-flow volume using functional near-infrared spectroscopy
(fNIRS), and assessed functional changes using the Frontal Assessment Battery, Health-Related
Quality of Life scale, and behaviorally-assessed physical
function tests. fNIRS revealed that prefrontal activity during IVG performance
significantly increased in the IVG period compared with the control period.
Furthermore, significant correlations between cerebral blood flow changes in different
channels were observed during IVG performance. In addition, we observed
intervention-related improvement in health-related quality of life following IVG.
IVG intervention was associated with increased prefrontal cortex activation and
improved health-related quality of life performance in patients with
schizophrenia. Patients with chronic schizophrenia are characterized by
withdrawal and a lack of social responsiveness or interest in others.
Interventions using IVG may provide a useful low-cost rehabilitation method for
such patients, without the need for specialized equipment
