    Evaluation de l'ITT par les médecins généralistes (Etude prospective interventionnelle auprÚs de praticiens de la région Stéphanoise)

    Etablir un certificat mĂ©dical pour coups et blessures et fixer une incapacitĂ© totale de travail est un acte mĂ©dico-judiciaire difficile qui peut ĂȘtre confiĂ© Ă  tout mĂ©decin. Dans l'activitĂ© quotidienne des unitĂ©s de mĂ©decine lĂ©gale est constatĂ©e une inhomogĂ©nĂ©itĂ© franche entre les Ă©valuations d'ITT faites par les mĂ©decins gĂ©nĂ©ralistes et celles des mĂ©decins lĂ©gistes. Cette disparitĂ© a entraĂźnĂ© une perte de confiance des autoritĂ©s judiciaires envers les mĂ©decins gĂ©nĂ©ralistes pour un travail qui peut ĂȘtre, pourtant de leur ressort. L'objectif de notre Ă©tude Ă©tait de dĂ©terminer si le manque de formation Ă©tait un des facteurs intervenant dans la difficultĂ© qu'Ă©prouvent les mĂ©decins gĂ©nĂ©ralistes Ă  fixer une ITT. Et si il Ă©tait possible d'y pallier par un mode de formation accessible et efficace sur ce sujet qui reste soumis Ă  beaucoup de questionnement. Dans ce but, une enquĂȘte prospective interventionnelle a Ă©tĂ© menĂ©e auprĂšs de 41 mĂ©decins gĂ©nĂ©ralistes de la rĂ©gion StĂ©phanoise. Notre enquĂȘte a montrĂ© la possibilitĂ© d'Ă©tablir un modĂšle de formation, amĂ©liorant la pratique mĂ©dicale, mais Ă©galement la rĂ©ticence persistante de certain mĂ©decin devant cette activitĂ©. Des perspectives d'avenir peuvent en ĂȘtre dĂ©duites en terme de formation mĂ©dicale continue et de e-learning . La perte de confiance des autoritĂ©s judiciaires pour les Ă©valuations d'ITT, autres que celles Ă©tablies par un mĂ©decin lĂ©giste, et une dĂ©sappropriation de cet acte mĂ©dicolĂ©gal de la part des mĂ©decins gĂ©nĂ©ralistes restent problĂ©matiques.     Transmission of force to the hyoid bone during manual strangulation: Simulation using finite element numerical models

    Introduction Strangulation is a medicolegal form of mechanical asphyxia, and can be difficult to identify if cutaneous damage to the neck is limited. We began by creating a numerical model of a hyoid bone with adjustable anthropometric parameters and then subjected our model to compression simulating a precision grip on the neck from the front. Materials and methods We selected six bones from the 77 hyoid bones contained in the database we created during a previous study led by our laboratory, in which we demonstrated the sexual dimorphism of this bone and identified the minimal force required for fracture. The anthropometric characteristics of these six bones (angle, length and width) corresponded to those of the 10th, 50th and 90th percentiles from cluster 1 (male individuals) and cluster 2 (female individuals), respectively. After enhancing, developing and meshing the selected 3D models, we analysed and reproduced simulation conditions that were as close as possible to the in vivo conditions of neck strangulation from the front (area of pressure on the bone, tissue environment, and biological variability of this bone). Results We modelled the six numerical hyoid bone models using the finite element method. For all models, the simulation of mechanical pressure applied to simulate anterior strangulation resulted in fractures. The forces required to produce these fractures matched the results obtained in the experimental testing of dissected hyoid bones. Conclusion Six finite element numerical models were created, covering the anthropometric morphological variability of the hyoid bone. These six models enabled numerical simulation of the in vivo behaviour of a hyoid bone subjected to one-handed strangulation
