Comparative motion analysis in healthy subjects with different alterations of substantia nigra echogenicity and Parkinsonian patients in early stages
In der präklinischen Phase des idiopathischen Parkinson-Syndroms (IPS) zeigen sich häufig Frühzeichen, wie z.B. olfaktorische Dysfunktionen, depressive Verstimmungen oder ein einseitig verminderter Armschwung, die jeweils nicht für eine Diagnose ausreichend sind. Bei mehr als 90% der Patienten mit IPS findet man mit dem transkraniellen Ultraschall (TCS) im Bereich der Substantia nigra (SN) ein hyperechogenes Areal. Jedoch kann auch bei etwa 9% der gesunden, unauffälligen Probanden aus der Normalbevölkerung eine SN-Hyperechogenität beobachtet werden, von denen allerdings nur ein geringer Teil in den folgenden Jahren ein IPS entwickelt.
Ziel dieser Studie war der Nachweis von möglichen subtilen motorischen Bewegungsauffälligkeiten bei Probanden mit hyperechogener SN in der Bewegungsanalyse und der Vergleich von Bewegungsparametern mit bekannten präklinischen Markern.
Es wurden insgesamt 60 Probanden und 19 Parkinson-Patienten (Gruppe PD) in die Studie eingeschlossen. Die 60 Probanden wurden in 3 Gruppen eingeteilt, Kriterien hierfür waren der Ultraschallbefund und der neurologische Status (Gruppe SN-, SN+, SN++). Die Bewegungsanalyse umfasste die drei Bewegungen Finger-Tapping, alternierende Hand-Armbewegung und den Gang. Darüber hinaus wurden neben der Erhebung der Familienanamnese noch weitere spezielle neurologische Untersuchungen durchgeführt, wie die Riechprüfung mit den Sniffin-Sticks, die Unified Parkinson's Disease Rating Scale (UPDRS) und das Becks Depressions-Inventar (BDI).
Als Grundlage für die folgenden Ergebnisse konnten mit der Bewegungsanalyse deutliche Bewegungsunterschiede zwischen SN-Unauffälligen und Parkinson-Patienten gezeigt werden. Bei der Gruppe SN+ zeigten sich nur bei feinmotorischen Übungen subtile Bewegungsstörungen, dagegen konnte bei der Gruppe SN++ auch bei grobmotorischen Bewegungen wie dem Armschwung ein Defizit nachgewiesen werden.
Motorisch auffällige Probanden mit mindestens einem auffälligen Bewegungsparameter haben im Mittel deutlich asymmetrische Armschwünge, eine verminderte Riechfunktion und überdurchschnittlich häufig eine positive Familienanamnese. Betrachtet man motorisch auffällige Probanden mit manifester Riechstörung, so sind bereits grobmotorische Bewegungen wie der rechte Armschwung oder die Gehgeschwindigkeit signifikant vermindert. 20% dieser Probanden sind in einem weiteren präklinischen Marker auffällig. Sollten diese Risikopersonen in der funktionellen Bildgebung parkinson-ähnliche Muster zeigen, so wäre gegebenenfalls in Zukunft eine frühe protektive Behandlung zum Schutz verbleibender dopaminerger Neuronen auf der Basis der Fusion mehrerer einfacher klinischer Tests möglich.In the preclinical phase of Parkinson's Disease (PD), there are frequently signs which are not sufficient for the diagnosis of PD, for example olfactory dysfunction, depressive mood or arm swing asymmetry.
Substantia nigra (SN) hyperechogenicity is found by transcranial ultrasound in more than 90% of PD patients. However, this feature is also found in about 9% of healthy subjects but just a small number of them will develop PD in the following years.
The aim of this study was detecting subtle motor dysfunctions in subjects with hyperechogenic SN by motion analysis and comparing motion parameters with established preclinical features.
The study included 60 probands, classified in 3 groups, and 19 PD patients. Classifying criteria were SN echomorphology and neurological signs (SN-, SN+, SN++). Motion analysis included finger tapping, diadochokinesis and gait. Other parameters were olfactory dysfunction (Sniffin sticks), UPDRS (Unified Parkinson's Disease Rating Scale) and BDI (Beck Depression Inventory).
Basically obvious motion differences were detected by comparing healthy subjects without SN hyperechogenicity (SN-) to PD patients. SN+ just showed subtle differences in fine motor skills whereas SN++ showed differences in gross motor skills like arm swing. Subjects with motor dysfunction in at least one parameter frequently show obvious arm swing asymmetry, olfactory dysfunction and positive family history for PD.
Probands with motor and olfactory dysfunction show already significantly reduced arm swing and gait velocity. 20% of those indicate at least one more preclinical sign. If these subjects at risk showed PD-like patterns in functional neuro-imaging, the fusion of various simple clinical tests could possibly permit an early protective treatment
National Recommendations for Physical Activity and Physical Activity Promotion
Always and at any age, regular physical activity can act as a powerful elixir with a beneficial effect on health and well-being. The wide variety of health effects that physical activity can have, for example on our cardiovascular system, back and joints, is scientifically well proven. At the same time, we spend most of our time sitting – at school, at the office or in the car. Our bodies, however, want to be on the move! This fundamental instinct is deeply rooted in human nature and this basic urge to move does not really suit our modern sedentary lifestyle.That is why it is important to know what kind of, and how much, physical activity is most effective in improving the health of which sectors of the population. However, we also need to know the specific ways in which we can most effectively motivate people to become physically active. For the very first time, this publication sets out National Recommendations for Physical Activity and Physical Activity Promotion for the Federal Republic of Germany. The recommendations apply to children and adolescents, adults and older persons as well as to adults with chronic diseases. In their present form, they are geared particularly to experts in different areas and institutions.
The recommendations are characterized by their solid scientific grounding. Highly respected scientists from the fields of sports science, sports medicine and public health were involved in drawing them up. As with their creation, the recommendations’ further development and implementation will be accompanied closely by the working group on physical activity promotion in everyday life (“Bewegungsförderung im Alltag”) at the Federal Ministry of Health. This platform provides a forum for numerous organizations involved in health promotion, physical activity and sport in Germany and is part of the National Action Plan “IN FORM – German national initiative to promote healthy diets and physical activity”

Somatic mosaicism and common genetic variation contribute to the risk of very-early-onset inflammatory bowel disease
Very-early-onset inflammatory bowel disease (VEO-IBD) is a heterogeneous phenotype associated with a spectrum of rare Mendelian disorders. Here, we perform whole-exome-sequencing and genome-wide genotyping in 145 patients (median age-at-diagnosis of 3.5 years), in whom no Mendelian disorders were clinically suspected. In five patients we detect a primary immunodeficiency or enteropathy, with clinical consequences (XIAP, CYBA, SH2D1A, PCSK1). We also present a case study of a VEO-IBD patient with a mosaic de novo, pathogenic allele in CYBB. The mutation is present in ~70% of phagocytes and sufficient to result in defective bacterial handling but not life-threatening infections. Finally, we show that VEO-IBD patients have, on average, higher IBD polygenic risk scores than population controls (99 patients and 18,780 controls; P < 4 × 10-10), and replicate this finding in an independent cohort of VEO-IBD cases and controls (117 patients and 2,603 controls; P < 5 × 10-10). This discovery indicates that a polygenic component operates in VEO-IBD pathogenesis

The transverse-momentum (pT) spectra and coalescence parameters B2 of (anti)deuterons are measured in pp collisions at s√=13 TeV for the first time in and out of jets. In this measurement, the direction of the leading particle with the highest pT in the event (pleadT>5 GeV/c) is used as an approximation for the jet axis. The event is consequently divided into three azimuthal regions and the jet signal is obtained as the difference between the Toward region, that contains jet fragmentation products in addition to the underlying event (UE), and the Transverse region, which is dominated by the UE. The coalescence parameter in the jet is found to be approximately a factor of 10 larger than that in the underlying event. This experimental observation is consistent with the coalescence picture and can be attributed to the smaller average phase-space distance between nucleons inside the jet cone as compared to the underlying event. The results presented in this Letter are compared to predictions from a simple nucleon coalescence model, where the phase space distributions of nucleons are generated using PYTHIA 8 with the Monash 2013 tuning, and to predictions from a deuteron production model based on ordinary nuclear reactions with parametrized energy-dependent cross sections tuned on data. The latter model is implemented in PYTHIA 8.3. Both models reproduce the observed large difference between in-jet and out-of-jet coalescence parameters, although the almost flat trend of the BJet2 is not reproduced by the models, which instead give a decreasing trend
Measurements of azimuthal anisotropies at forward and backward rapidity with muons in high-multiplicity p–Pb collisions at √sNN = 8.16 TeV
The study of the azimuthal anisotropy of inclusive muons produced in p-Pb collisions at sNN−−−√=8.16 TeV, using the ALICE detector at the LHC is reported. The measurement of the second-order Fourier coefficient of the particle azimuthal distribution, v2, is performed as a function of transverse momentum pT in the 0-20% high-multiplicity interval at both forward (2.032 GeV/c. The v2 coefficient of inclusive muons is extracted using two different techniques, namely two-particle cumulants, used for the first time for heavy-flavour measurements, and forward-central two-particle correlations. Both techniques give compatible results. A positive v2 is measured at both forward and backward rapidities with a significance larger than 4.7σ and 7.6σ, respectively, in the interval 2<pT<6 GeV/c. Comparisons with previous measurements in p-Pb collisions at sNN−−−√=5.02 TeV, and with AMPT and CGC-based theoretical calculations are discussed. The findings impose new constraints on the theoretical interpretations of the origin of the collective behaviour in small collision systems
