    AIDS Surveillance in Africa

    Surveillance of the AIDS pandemic in Africa has always posed formidable problems for epidemiologists. Diagnostic accuracy-according to the case definitions for AIDS used in industrialised countries-is impossible to achieve in all but a few places with the right diagnostic facilities. Responding to the urgent need for surveillance, the World Health Organisation drew up a clinical case definition (the WHO/Bangui definition), which depended on clinical criteria without the need for serological verification.'2 Judged by its use, the WHO/Bangui definition has been successful-52 African countries have reported cases ofAIDS using mainly this definition.3 Some countries have modified it to fit local circumstances, removing a defining symptom here, adding the need for an extra sign there, and many now accept or encourage a positive result of an HIV test as supportive evidence. (At least one, COte d'Ivoire, requires such a result.2) Inevitably the definition has its limitations, and two papers in this week's journal discuss these at length (p 11852, p 11894). Because of limited laboratory facilities published evaluations of the WHO/Bangui definition have been mainly restricted to groups of sick patients using HIV positivity as the reference standard. The definition's sensitivity and specificity have been calculated as being between 60% and 90%2 5- useful for purposes of surveillance, but leaving uncertainty over whether this surveillance tool is intended to monitor trends in cases of AIDS or HIV infection. Other problems exist with the WHO/Bangui definition. Because many doctors lack diagnostic facilities they use the definition for diagnosis. The title "clinical case definition" encourages this confusion. The misuse is disturbing as the probability that a patient who fulfils the WHO/Bangui definition tests positive for HIV may fall well below 50% when seroprevalence is low.5 Another problem of using the definition is the delayed and incomplete picture that it gives of the spread of infection.6 Far preferable for surveillance of infection is the unlinked anonymous testing for HIV of sentinel groups attending health services67 (such as pregnant women and people with sexually transmitted diseases), which has now begun in several African countries89 using the same methods as in industrialised countries.'° Where does this leave the WHO/Bangui definition? De Cock and colleagues rehearse the overwhelming case for AIDS reporting to continue and suggest a thoughtful redesign of the definition, which includes the requirement for a positive HIV test result.3 Insisting on positive test results in all circumstances, however, is impractical: HIV tests are already limited and are lioely to become more so as AIDS funding to Africa inevitably falls. As a provisional solution to the problem of surveillance the WHO/Bangui definition has been useful, but the time has come for its reappraisal

    The distinctive challenges and opportunities for creating leadership within social enterprises

    Purpose The purpose of this study is to present a systematic assessment of the distinctive challenges and opportunities associated with creating leadership within the realm of social enterprise. A modified and expanded form of Grint’s leadership lenses heuristic framework (i.e. person, position, process, performance, purpose and place) is used to examine and highlight what is particular about creating leadership in social enterprises by virtue of their distinctive missions, strategic contexts, legal forms and organisational structures and cultures. Based on this initial exploration, five research priorities are identified to better understand and then develop leadership practice in the social enterprise realm. Design/methodology/approach An enhanced heuristic framework for systematically examining leadership within the social enterprise research literature has been applied, drawing on the leadership practice literature. The application is illustrated through six instrumental case studies. Findings While there are a number of similarities between leading in the social enterprise realm and leading within the private, public and not-for-profit sectors, the levels of complexity, ambiguity and the lack of an established theoretical and practical knowledge base makes creating leadership in the social enterprise sector that much more challenging. On the positive side of the ledger, the fact that the purpose is at the core of social enterprise means that it is relatively easier to use the purpose to create a basis for common meaningful action, compared to leadership within the private and public sectors. Related to this, given the strongly local or “glocal” nature of social enterprise, a ready opportunity exists for leaders to draw upon a place as a strategic resource in mobilising followers and other stakeholders. The novel, uncertain and pioneering nature of a social enterprise is also arguably more tolerant and accommodating of a leadership mindset that focuses on posing questions regarding “wicked” problems compared to public, private for-profit and, indeed, traditional not-for-profit sector organisations. Originality/value As far as we can ascertain, this is the first systematic attempt to examine the distinctive challenges and opportunities associated with creating leadership within the social enterprise realm. The application of the heuristic framework leads to the identification of five key inter-related lines of empirical research into leadership practices within social enterprises. </jats:sec

    Testing the Hubble Law with the IRAS 1.2 Jy Redshift Survey

    We test and reject the claim of Segal et al. (1993) that the correlation of redshifts and flux densities in a complete sample of IRAS galaxies favors a quadratic redshift-distance relation over the linear Hubble law. This is done, in effect, by treating the entire galaxy luminosity function as derived from the 60 micron 1.2 Jy IRAS redshift survey of Fisher et al. (1995) as a distance indicator; equivalently, we compare the flux density distribution of galaxies as a function of redshift with predictions under different redshift-distance cosmologies, under the assumption of a universal luminosity function. This method does not assume a uniform distribution of galaxies in space. We find that this test has rather weak discriminatory power, as argued by Petrosian (1993), and the differences between models are not as stark as one might expect a priori. Even so, we find that the Hubble law is indeed more strongly supported by the analysis than is the quadratic redshift-distance relation. We identify a bias in the the Segal et al. determination of the luminosity function, which could lead one to mistakenly favor the quadratic redshift-distance law. We also present several complementary analyses of the density field of the sample; the galaxy density field is found to be close to homogeneous on large scales if the Hubble law is assumed, while this is not the case with the quadratic redshift-distance relation.Comment: 27 pages Latex (w/figures), ApJ, in press.     A web-oriented framework for the development and deployment of academic facing administrative tools and services

    The demand for higher education has increased dramatically in the last decade. At the same time, institutions have faced continual pressure to reduce costs and increase quality of education, while delivering that education to greater numbers of students. The introduction of software systems such as virtual learning environments, online learning resources and centralised student record systems has become routine in attempts to address these demands. However, these approaches suffer from a variety of limitations: They do not take all stakeholders’ needs into account. They do not seek to reduce administrative overheads in academic processes. They do not reflect institution-specific academic policies. They do not integrate readily with other information systems. They are not capable of adequately modelling the complex authorisation roles and organisational structure of a real institution. They are not well suited to rapidly changing policies and requirements. Their implementation is not informed by sound software engineering practises or data architecture design. Crucially, as a consequence of these drawbacks such systems can increase administrative workload for academic staff. This thesis describes the research, development and deployment of a system which seeks to address these limitations, the Module Management System (MMS). MMS is a collaborative web application targeted at streamlining and minimising administrative tasks. MMS encapsulates a number of user-facing tools for tasks including coursework submission and marking, tutorial attendance tracking, exam mark recording and final grade calculation. These tools are supported by a framework which acts as a form of “university operating system”. This framework provides a number of different services including an institution abstraction layer, role-based views and privileges, security policy support integration with external systems

    Trace element and isotope constraints on crustal anatexis by upwelling mantle melts in the North Atlantic Igneous Province: an example form the Isle of Rum, NW Scotland

    Sr and Nd isotope ratios, together with lithophile trace elements, have been measured in a representative set of igneous rocks and Lewisian gneisses from the Isle of Rum in order to unravel the petrogenesis of the felsic rocks that erupted in the early stages of Palaeogene magmatism in the North Atlantic Igneous Province (NAIP). The Rum rhyodacites appear to be the products of large amounts of melting of Lewisian amphibolite gneiss. The Sr and Nd isotopic composition of the magmas can be explained without invoking an additional granulitic crustal component. Concentrations of the trace element Cs in the rhyodacites strongly suggests that the gneiss parent rock had experienced Cs and Rb loss prior to Palaeogene times, possibly during a Caledonian event. This depletion caused heterogeneity with respect to 87Sr/86Sr in the crustal source of silicic melts. Other igneous rock types on Rum (dacites, early gabbros) are mixtures of crustalmelts and and primarymantle melts. Forward Rare Earth Element modelling shows that late stage picritic melts on Rum are close analogues for the parent melts of the Rum Layered Suite, and for the mantle melts that caused crustal anatexis of the Lewisian gneiss. These primary mantle melts have close affinities to Mid-Oceanic Ridge Basalts (MORB), whose trace element content varies from slightly depleted to slightly enriched. Crustal anatexis is a common process in the rift-to-drift evolution during continental break-up and the formation of Volcanic Rifted Margins systems. The ‘early felsic–later mafic’ volcanic rock associations from Rum are compared to similar associations recovered from the now-drowned seaward-dipping wedges on the shelf of SE Greenland and on the Vøring Plateau (Norwegian Sea). These three regions show geochemical differences that result from variations in the regional crustal composition and the depth at which crustal anatexis took place

    Missing in Action? Electronic Gaming Machines in Gambling Studies Research

    In the past thirty years casinos across the world have become dominated by the rise of “electronic gaming machines” (EGMs). Expanding with tremendous speed, this technology has arguably become the dominant form of non-online gambling around the world at time of writing (DeMichele, 2017; Schwartz, 2018). EGMs are also noted as being one of the most harmful forms of gambling, with significant numbers of players betting beyond their financial limits (MacLaren et al, 2012; Stewart & Wohl, 2013), spending a disproportionate amount of time playing (Cummings, 1999; Ballon, 2005; Schüll, 2012; cf. Dickerson, 1996), becoming disconnected from the world outside of the “zone” (Schüll, 2012) of gambling play, and even becoming bankrupt or otherwise financially crippled as a result of their use (Petry, 2003; Scarf et al, 2011). Using metadata from Web of Science and Scopus databases, we analysed peer-reviewed gambling research produced in Australia, New Zealand, North America and the UK published between 1996 and 2016. Surprisingly, we found that the overwhelming of majority of articles do not specifically address EGMs as the most popular and pervasive gambling technology available. Our paper teases out some concerning implications of this finding for the interdisciplinary field of gambling studies

    Bonneville basin analogues for large lake processes & chronologies of geomorphic development on Mars

    Journal ArticlePleistocene Lake Bonneville was a large (~50,000 sq km) terrestrial closed lake system in Utah, USA that developed during the Last Glacial Maximum (~20 ka BP), and persisted at highstand until a catastrophic outburst flood event ~17.4 ka cal BP and warming climate significantly lowered its volume [1]. Lake Bonneville and its modern relict Great Salt Lake (fig. 1) is one of the most extensive, well preserved, and best dated lake systems on Earth and can serve as an analogue for deducing the style of development and age of similar features imaged on Mars [2]. Lake Bonneville exhibits prominent shorelines, spits, bay mouth barriers, deltas, gullies, outburst channels, and playa lake features, including patterned grounds and downwind aeolian systems, all of which are features inferred from imagery of Mars landforms

    An investigation of herpes simplex virus promoter activity compatible with latency establishment reveals VP16-independent activation of immediate-early promoters in sensory neurones

    Herpes simplex virus (HSV) type-1 establishes lifelong latency in sensory neurones and it is widely assumed that latency is the consequence of a failure to initiate virus immediate-early (IE) gene expression. However, using a Ore reporter mouse system in conjunction with Ore-expressing HSV-1 recombinants we have previously shown that activation of the IE ICPO promoter can precede latency establishment in at least 30 % of latently infected cells. During productive infection of non-neuronal cells, IE promoter activation is largely dependent on the transactivator VP16 a late structural component of the virion. Of significance, VP16 has recently been shown to exhibit altered regulation in neurones; where its de novo synthesis is necessary for IE gene expression during both lytic infection and reactivation from latency. In the current study, we utilized the Ore reporter mouse model system to characterize the full extent of viral promoter activity compatible with cell survival and latency establishment. In contrast to the high frequency activation of representative IE promoters prior to latency establishment, cell marking using a virus recombinant expressing Ore under VP16 promoter control was very inefficient. Furthermore, infection of neuronal cultures with VP16 mutants reveals a strong VP16 requirement for IE promoter activity in non-neuronal cells, but not sensory neurones. We conclude that only IE promoter activation can efficiently precede latency establishment and that this activation is likely to occur through a VP16-independent mechanism
