Early hematopoietic stem cell transplantation in a patient with severe mucopolysaccharidosis II : 7 years follow-up
Mucopolysaccharidosis type II (MPS II - Hunter syndrome) is an X-linked lysosomal storage disorder caused by a deficiency in the enzyme iduronate-2 sulfatase (I2S), leading to the accumulation of the glycosaminoglycans, affecting multiple organs and systems. Enzyme replacement therapy does not cross the blood brain barrier, limiting results in neurological forms of the disease. Another option of treatment for severe MPS, hematopoietic stem cell transplantation (HSCT) has become the treatment of choice for the severe form of MPS type I, since it can preserve neurocognition when performed early in the course of the disease. To date, only few studies have examined the long-term outcomes of HSCT in patients with MPS II. We describe the seven-year follow-up of a prenatally diagnosed MPS II boy with positive family history of severe MPS form, submitted to HSCT with umbilical cord blood cells at 70 days of age. Engraftment after 30 days revealed mixed chimerism with 79% donor cells; after 7 years engraftment remains at 80%. I2S activity 30 days post-transplant was low in plasma and normal in leukocytes and the same pattern is observed to date. At age 7 years growth charts are normal and he is very healthy, although mild signs of dysostosis multiplex are present, as well as hearing loss. The neuropsychological evaluation (Wechsler Intelligence Scale for Children - Fourth Edition - WISC-IV), disclosed an IQ of 47. Despite this low measured IQ, the patient continues to show improvements in cognitive, language and motor skills, being quite functional. We believe that HSCT is a therapeutic option for MPS II patients with the severe phenotype, as it could preserve neurocognition or even halt neurodegeneration, provided strict selection criteria are followed

fragilidade óssea e acarreta importantes prejuízos funcionais aos pacientes. Seu
tratamento requer uma abordagem multidisciplinar, conjugando tratamento
medicamentoso, de reabilitação e ortopédico. A fisioterapia é considerada
fundamental no tratamento integral desta enfermidade e na maximização das
capacidades funcionais desses indivíduos. Contudo, é desconhecido se esta
população está sendo contemplada pela rede de assistência fisioterapêutica do
Sistema Único de Saúde (SUS) do Município do Rio de Janeiro, e qual o tipo de
tratamento prestado. Objetivo: Descrever a assistência fisioterapêutica oferecida
no SUS do município do Rio de Janeiro a pacientes com OI acompanhados pelo
Centro de Referência em Tratamento da Osteogenesis Imperfecta do Instituto
Fernandes Figueira/ Fiocruz (CROI-IFF). Materiais e métodos: Trata-se de um
estudo descritivo e transversal, que utilizou dois questionários semi-estruturados
aplicados aos responsáveis por crianças com OI e a fisioterapeutas de instituições
com fisioterapia motora pediátrica. Trinta e um pacientes (31) preencheram todos
os critérios do estudo, além de sete (7) instituições com fisioterapia motora
pediátrica do SUS do município do Rio de Janeiro. Resultados: Houve
prevalência na amostra do sexo masculino (64,5%), com média de idade elevada
(64meses) e baixa renda familiar. O tipo de OI mais freqüente foi o III e quase a
metade da amostra (48,4%) não realizava fisioterapia. O motivo principal alegado
pelos responsáveis foi a falta de vaga nas instituições (33%), com procura em 3 a
5 instituições em 32,5% dos casos. Foi observado que as crianças mais novas
estavam menos inseridas no tratamento fisioterapêutico, e que a maioria delas
(64%) já havia perdido a vaga no tratamento. O motivo principal do desligamento
foi o excesso de faltas justificadas decorrentes das fraturas e cirurgias a que se
submetem. Nas instituições, as práticas fisioterapêuticas encontradas estavam de
acordo com a literatura, no entanto, foi encontrado alto índice (42,9%) de não
aceitação para o tratamento, e o maior empecilho relatado pelos profissionais para
a inserção destas crianças no tratamento foi a falta de capacitação para este
atendimento (66,7%). Os critérios para desligamento da vaga em 42,9% foi o
excesso de faltas justificadas. Conclusão: Foi encontrada baixa inserção no
tratamento fisioterapêutico e prática restritiva em parte das instituições
pesquisadas. Faz-se necessária a adequação dos serviços a fim de promover a
integralidade da assistência às crianças com OI, a melhora da qualidade de vida e
sua efetiva inclusão social.Introduction: Osteogenesis Imperfecta (OI) is a genetic disorder that leads to
bone fragility and involves significant functional impairment to the patients. Its
treatment requires a multidisciplinary approach, combining drug treatment,
rehabilitation and orthopedic surgeries. Physical therapy is considered essential in
the comprehensive treatment of this desease, aiming to maximize functional
abilities of these individuals. However, it is unknown whether this population is
being contemplated by the Brazilian public health system (SUS) network of
physical therapy in the city of Rio de Janeiro, and the kind of treatment provided.
Objective: To describe the physical therapy offered in SUS network in the city of
Rio de Janeiro to patients with OI followed at the Reference Center for
Osteogenesis Imperfecta treatment in Instituto Fernandes Figueira/ Fiocruz (CROIIFF).
Methods: A descriptive and cross-sectional study, which used two semistructured
questionnaires applied to parents of children with OI and to physical
therapists of rehabilitation services. Thirty-one patients (31) met all study criteria,
and seven (7) SUS services with pediatric physical therapy in the municipality of
Rio de Janeiro were included. Results: There was a prevalence in the sample of
males (64.5%), with OI type III (42.5%), elevated mean age (64months) and low
family income. Almost half of the sample (48.4%) were not under physical therapy
and the main reason was an absence of available and accessible physical therapy
providers lack of vacancy in the services (33%); families had tried to enroll patients
in 3-5 institutions in 32.5% of cases. It was observed that the younger children
were less embedded in physical therapy, and most of them (64%) were excluded
from treatment. The main reason for such exclusions was excessive justified
absences resulting from fractures and underlying surgeries. Among the institutions,
physical therapy practices were in agreement with the literature, however, a high
rate (42.9%) of non-acceptance for treatment has been found, and the largest
obstacle reported by professionals to insert these children in treatment was lack of
expertise for such service (66.7%). Conclusion: We found low participation in
physical therapy and restrictive practice in the surveyed institutions. It is necessary
to adequate services in order to promote comprehensive care for children with OI,
leading to improved quality of life and to effective social inclusion
CARACTERÍSTICAS CLÍNICO-FUNCIONAIS DE INDIVÍDUOS COM OSTEOGÊNESE IMPERFEITA
Objetivo: Descrever o perfil clínico e funcional de crianças e adolescentes com OI. Material e métodos: Pesquisa transversal, de caráter observacional e descritivo, realizada através de análise das últimas fichas de avaliação fisioterapêutica anexadas aos prontuários dos pacientes atendidos em Centro de Referência para tratamento de Osteogênese Imperfeita na região sudeste do Brasil. Foram analisados dados clínicos da amostra, além de estruturas e funções do corpo de 141 indivíduos com diversas formas clínicas de OI. Este projeto está vinculado ao estudo "Centro de Referência em Osteogênese Imperfeita - Protocolos clínicos e terapêuticos voltados para a Osteogênese Imperfeita", cadastrado no CEP/IFF (n° 1.740. 985; 2.777.502) como parte do Registro Nacional de Osteogênese Imperfeita. Resultados: O antireabsorptivo ósseo do grupo dos bisfosfonatos, pamidronato dissódico, foi a medicação mais utilizada em todos os tipos de OI. O atraso na aquisição da marcha, o uso de haste, o déficit de equilíbrio, a presença de deformidades, a limitação de arco de movimento, o número de fraturas e a idade da primeira fratura foram as variáveis analisadas que variaram significativamente de acordo com a gravidade clínica da doença. Conclusão: Acredita-se que o conhecimento referente à funcionalidade dos pacientes com OI possa prevenir danos, promover a saúde, facilitando o acesso ao tratamento adequado e preconizado
Assistência fisioterapêutica na atenção primária à saúde infantil: uma revisão das experiências
OBJETIVO: Revisar as experiências de atenção fisioterapêutica dirigidas à população pediátrica descritas na literatura e analisar a produção de conhecimento sobre fisioterapia no contexto da atenção primária à saúde infantil (APSI). MÉTODOS: Foi realizada uma revisão sistemática conforme PRISMA, com busca nas seguintes bases de dados: MEDLINE, LILACS, SciELO, PubMed, Scopus, Cochrane; banco de teses da CAPES; e System for Information on Grey Literature in Europe (SIGLE). Foram utilizados os descritores "atenção primária à saúde", "fisioterapia", "lactente ou criança" e seus correspondentes na língua inglesa e espanhola, sem restrição de ano de publicação. RESULTADOS: Analisamos 13 artigos de seis países, reunidos em três eixos temáticos: dilemas profissionais (três artigos), competências e habilidades específicas para a APSI (sete artigos) e relatos de prática (quatro artigos). Os dilemas profissionais mencionados foram a ampliação do papel do fisioterapeuta para incluir ambientes comunitários, compartilhando a tomada de decisão com as famílias, e o trabalho em colaboração com outros serviços de saúde para identificar as necessidades da criança. As competências e habilidades citadas foram a identificação de sintomas clínicos e socioculturais para além das condições musculoesqueléticas, o diagnóstico fisioterapêutico precoce, a prevenção contra o uso excessivo de medicamentos e a capacidade de trabalhar em equipe. Os relatos de prática discorreram sobre estimulação em crianças com quadros neurológicos, tratamento respiratório e grupos com mães de crianças com esses acometimentos. CONCLUSÕES: O baixo número de estudos sugere desconhecimento quanto ao modo como a fisioterapia se insere na APSI e, provavelmente, quanto às habilidades profissionais necessárias nesse ambiente. Assim, são necessários mais estudos para fornecer dados sobre a área e um esforço de qualificação continuada por parte dos fisioterapeutas
Respiratory manifestations in late-onset Pompe disease: a case series conducted in Brazil
ABSTRACT Objective: To describe respiratory function in a series of patients with late-onset Pompe disease after the definitive diagnosis and before enzyme replacement therapy. Methods: This was a cross-sectional study involving patients with a definitive molecular diagnosis of late-onset Pompe disease. The data analyzed included age at symptom onset; age at definitive diagnosis; type of initial symptoms; time from symptom onset to diagnosis; FVC in the sitting and supine positions; six-minute walk distance; and locomotor ability. Analyses were carried out using frequencies, medians, minimum values, and maximum values. Results: Six patients were included in the study. The median age at symptom onset was 15 years (range, 13-50 years), and the median age at diagnosis was 39.5 years (range, 10-64 years). The median time from symptom onset to diagnosis was 8 years (range, 0-45 years). In all cases, the initial manifestation of the disease had been motor weakness. The median FVC in percentage of the predicted value (FVC%) in the sitting and supine positions was 71.0% (range, 22.9-104.6%) and 58.0% (range, 10.9-106.9%), respectively. The median ΔFVC% was 24.5% (range, −4.59 to 52.40%).The median six-minute walk distance was 391.7 m (range, 97-702 m) . Conclusions: In this case series, the time from symptom onset to diagnosis was long. Although respiratory signs or symptoms were not the initial manifestations of the disease, 66.7% of the patients showed reduced FVC% in the sitting and supine positions at diagnosis
