    Trust in the Mentor-Youth Relationship and its Correlates with Frequency of Contact, Parental Involvement, and Academic Improvements

    Community mentoring programs target at-risk youth with the aim of providing them with a positive, stable adult presence in their lives. Relationship quality of the mentors and mentees has been linked to multiple external factors and youth outcomes. This study investigated mentor-mentee relationship quality (i.e., youth\u27s perceived trust in his or her mentor) and the associations between the amount of time the pair spent together per week, parents\u27 level of involvement in planning activities between their child and their mentor, and youth\u27s improvement in commitment to learning. The correlations between trust and time, and between trust and commitment to learning were non-significant. Due to limited parental responses, parental involvement was analyzed qualitatively. Poor response rates limited the analyses. Other limitations of the study and future directions are discussed

    The Effects of Training Multiple Mands Within Functional Communication Training on The Resurgence Of Problem Behaviors

    Resurgence is the reoccurrence of a previously reinforced behavior when, under similar circumstances, a more recently reinforced behavior is placed on extinction (Epstein, 1985). The resurgence of problem behavior within the context of functional communication training (FCT) may occur when reinforcement is inadvertently thinned or placed on extinction due to low implementation integrity throughout the course of the intervention (Lieving et al., 2004). Techniques evaluated to mitigate resurgence of problem behavior have included long-term exposure to extinction (Wacker et al., 2011), signaled schedule thinning (Fuhrman, Fisher, and Greer, 2016), and a combination of both techniques (Wacker et al., 2013). These studies, however, have demonstrated varied results. Training multiple mand modalities may be a way to program for generalization, by increasing a child’s response repertoire. The purpose of the current study is to evaluate the effects of training multiple mands on the resurgence of problem behavior after implementing FCT in a school setting and the social validity of conducting the assessments and interventions in this context as reported by school staff. Three students ages 15, 7, and 5 years, developmental disabilities and exhibiting communication deficits and problem behaviors were trained on an initial mand to gain access to a reinforcer. After resurgence was demonstrated following extinction of the initial mand, participants were taught two additional, functionally identical, mand modalities. A reversal design was used to evaluate differences in the resurgence of problem behavior when a participant’s preferred mand is placed on extinction but the additional two are available. A reduction in the resurgence of problem behaviors was observed for two of three participants following mand2 and mand3 training. In addition, an increase in rates of non-preferred mands was observed for two of three participants during extinction phases. It was concluded that, within classroom settings, training multiple mand modalities serving the same function is likely to reduce the resurgence of students’ problem behaviors to a greater degree than teaching one mand within FCT. Implications, future directions, and limitations are discussed

    Lateralization of mesial temporal lobe epilepsy with chronic ambulatory electrocorticography

    Objective Patients with suspected mesial temporal lobe (MTL) epilepsy typically undergo inpatient video–electroencephalography (EEG) monitoring with scalp and/or intracranial electrodes for 1 to 2 weeks to localize and lateralize the seizure focus or foci. Chronic ambulatory electrocorticography (ECoG) in patients with MTL epilepsy may provide additional information about seizure lateralization. This analysis describes data obtained from chronic ambulatory ECoG in patients with suspected bilateral MTL epilepsy in order to assess the time required to determine the seizure lateralization and whether this information could influence treatment decisions. Methods Ambulatory ECoG was reviewed in patients with suspected bilateral MTL epilepsy who were among a larger cohort with intractable epilepsy participating in a randomized controlled trial of responsive neurostimulation. Subjects were implanted with bilateral MTL leads and a cranially implanted neurostimulator programmed to detect abnormal interictal and ictal ECoG activity. ECoG data stored by the neurostimulator were reviewed to determine the lateralization of electrographic seizures and the interval of time until independent bilateral MTL electrographic seizures were recorded. Results Eighty-two subjects were implanted with bilateral MTL leads and followed for 4.7 years on average (median 4.9 years). Independent bilateral MTL electrographic seizures were recorded in 84%. The average time to record bilateral electrographic seizures in the ambulatory setting was 41.6 days (median 13 days, range 0–376 days). Sixteen percent had only unilateral electrographic seizures after an average of 4.6 years of recording. Significance About one third of the subjects implanted with bilateral MTL electrodes required \u3e1 month of chronic ambulatory ECoG before the first contralateral MTL electrographic seizure was recorded. Some patients with suspected bilateral MTL seizures had only unilateral electrographic seizures. Chronic ambulatory ECoG in patients with suspected bilateral MTL seizures provides data in a naturalistic setting, may complement data from inpatient video-EEG monitoring, and can contribute to treatment decisions. Key Points Ambulatory electrocorticograms were obtained in patients implanted with a responsive neurostimulator and bilateral mesial temporal intracranial electrodes. In patients with bilateral seizures, the average time to record bilateral electrographic seizures in the ambulatory setting was 41.6 days (median 13, range 0–376).     An update on oral cavity cancer:epidemiological trends, prevention strategies and novel approaches in diagnosis and prognosis

    In the UK, the incidence of oral cavity cancer continues to rise, with an increase of around 60% over the past 10 years. Many patients still present with advanced disease, often resulting in locoregional recurrence and poor outcomes, which has not changed significantly for over four decades. Changes in aetiology may also be emerging, given the decline of smoking in developed countries. Therefore, new methods to better target prevention, improve screening and detect recurrence are needed. High-throughput ‘omics’ technologies appear promising for future individual-level diagnosis and prognosis. However, given this is a relatively rare cancer with significant intra-tumour heterogeneity and variation in patient response, reliable biomarkers have been difficult to elucidate. From a public health perspective, implementing these novel technologies into current services would require substantial practical, financial and ethical considerations. This may be difficult to justify and implement at present, therefore focus remains on early detection using new patient-led follow-up strategies. This paper reviews the latest evidence on epidemiological trends in oral cavity cancer to help identify at risk groups, population-based approaches for prevention, in addition to potential cutting-edge approaches in the diagnosis and prognosis of this disease.     Chemical Cartography of the Sagittarius Stream with Gaia

    The stellar stream connected to the Sagittarius (Sgr) dwarf galaxy is the most massive tidal stream that has been mapped in the Galaxy, and is the dominant contributor to the outer stellar halo of the Milky Way. We present metallicity maps of the Sgr stream, using 34,240 red giant branch stars with inferred metallicities from Gaia BP/RP spectra. This sample is larger than previous samples of Sgr stream members with chemical abundances by an order of magnitude. We measure metallicity gradients with respect to Sgr stream coordinates (Λ,B)(\Lambda, B), and highlight the gradient in metallicity with respect to stream latitude coordinate BB, which has not been observed before. We find ∇[M/H]=−2.48±0.08×10−2\nabla \mathrm{[M/H]} = -2.48 \pm 0.08 \times 10^{-2} dex/deg above the stream track (B>B0B>B_0 where B0=1.5B_0=1.5 deg is the latitude of the Sgr remnant) and ∇[M/H]=−2.02±0.08×10−2\nabla \mathrm{[M/H]} =- 2.02 \pm 0.08 \times 10^{-2} dex/deg below the stream track (B<B0B<B_0). By painting metallicity gradients onto a tailored N-body simulation of the Sgr stream, we find that the observed metallicities in the stream are consistent with an initial radial metallicity gradient in the Sgr dwarf galaxy of ∼−0.1\sim -0.1 to −0.2-0.2 dex/kpc, well within the range of observed metallicity gradients in Local Group dwarf galaxies. Our results provide novel observational constraints for the internal structure of the dwarf galaxy progenitor of the Sgr stream. Leveraging new large datasets in conjunction with tailored simulations, we can connect the present day properties of disrupted dwarfs in the Milky Way to their initial conditions.Comment: 20 pages, 12 figures.     Ocular syphilis resurgence in an urban underserved community in the United States

    AIM: To evaluate the clinical characteristics, demographics, and visual outcomes of patients with ocular syphilis at an urban hospital to increase awareness and assist in earlier diagnosis and treatment of the resurgent disease. METHODS: A retrospective chart review was performed on patients with ICD-9 and ICD-10 diagnosis codes correlating with syphilis or syphilis-related ocular diseases between 2010 and 2019. Variables evaluated included age, gender, race, vision, ocular findings, human immunodeficiency virus (HIV) status and men who have sex with men status, recreational drug and alcohol use. RESULTS: Ocular syphilis was diagnosed in 40 patients (53 eyes) of a total of 229 patients who tested positive for syphilis via serum and/or cerebrospinal fluid treponemal testing from 2010-2019. Among patients with ocular syphilis, most patients were males, aged 45 or above and Black, and had no diagnosed HIV infection. Approximately 50% patients had 20/40 vision or better. Nearly 50% had non-granulomatous anterior uveitis as their initial presentation, and 49% of patients had involvement of the posterior segment. Neovascular glaucoma (5.7%), papillitis (7.5%), vasculitis (5.7%), and retinal detachment (5.7%) were rarer presentations of the disease and were associated with a poorer visual prognosis. CONCLUSION: Given the increased prevalence and protean manifestations of syphilis, early diagnosis and treatment are paramount. More studies on ocular syphilis are warranted to understand this resurging disease

    Inter-α-Inhibitor Blocks Epithelial Sodium Channel Activation and Decreases Nasal Potential Differences in ΔF508 Mice

    Increased activity of lung epithelial sodium channels (ENaCs) contributes to the pathophysiology of cystic fibrosis (CF) by increasing the rate of epithelial lining fluid reabsorption. Inter-α-inhibitor (IαI), a serum protease inhibitor, may decrease ENaC activity by preventing its cleavage by serine proteases. High concentrations of IαI were detected in the bronchoalveolar lavage fluid (BALF) of children with CF and lower airway diseases. IαI decreased amiloride-sensitive (IENaC) but not cAMP-activated Cl− currents across confluent monolayers of rat ATII, and mouse nasal epithelial cells grew in primary culture by 45 and 25%, respectively. Changes in IENaC by IαI in ATII cells were accompanied by increased levels of uncleaved (immature) surface α-ENaC. IαI increased airway surface liquid depth overlying murine nasal epithelial cells to the same extent as amiloride, consistent with ENaC inhibition. Incubation of lung slices from C57BL/6, those lacking phenylalanine at position 508 (∆F508), or CF transmembrane conductance regulator knockout mice with IαI for 3 hours decreased the open probability of their ENaC channels by 50%. ∆F508 mice had considerably higher levels the amiloride-sensitive fractions of ENaC nasal potential difference (ENaC-NPD) than wild-type littermates and only background levels of IαI in their BALF. A single intranasal instillation of IαI decreased their ENaC-NPD 24 hours later by 25%. In conclusion, we show that IαI is present in the BALF of children with CF, is an effective inhibitor of ENaC proteolysis, and decreases ENaC activity in lung epithelial cells of ∆F508 mice
