    Tibialis anterior muscle hernia : a case of chronic, dull pain and swelling in leg diagnosed by dynamic ultrasonography

    BACKGROUND: Symptomatic muscle hernias are not uncommon in the lower extremities and are a rare cause of chronic leg pain. They are most commonly seen in the tibialis anterior muscle, occurring through fascial defects, usually after trauma. There are about 200 cases of muscle hernias described in the literature. The diagnosis is challenging as most of the patients present with non-specific chronic leg pain. Dynamic muscle ultrasonography at rest and at stress is often used for the diagnosis. CASE REPORT: We describe a case of tibialis anterior muscle hernia presenting with persistent dull pain and swelling along the anterior aspect of the leg on straining the leg muscles. Dynamic ultrasonography was performed, which showed a defect in the fascial sheath of the muscle through which the tibialis anterior muscle herniated and produced a focal bulge along the anterior aspect of the leg. Based upon physical examination and dynamic ultrasonographic findings, a diagnosis of tibialis anterior muscle hernia was made. CONCLUSIONS: Tibialis anterior muscle hernia is a rare diagnosis and should be included in the differential diagnosis in a patient with chronic leg pain and swelling. Dynamic ultrasound is crucial in confirming the diagnosis and should be done on straining the muscles of the affected limb

    This paper presents an idea to secure the border in a simple and more reliable way, The proposed idea has a simple design and has a very high range , it covers a very large border area securing the border area from illegal entries, smuggling ,illegal movement of weapon without much involvement of security forces near border. Afteranalysing the previous attack in our country and the number of accidents happening each year, it is evident that in most of the cases the security force are unaware of the entry of these terrorist hence in order to protect our nation from such attacks and terrorist activity there is a need to implement a security system which will be smart enough to detect these activity and give proper information about each and every activity happening near the border. In this paper, we propose building a security system that can be implemented by using advance feature like obstacle detection, obstacle identification, distance sensing and activity tracking, and quick communication between the source area and control room . If obstacle enters the border it automatically detects the obstacle give warnings and send the record of activity to control room. Also it eliminates the enemy in more efficient and better way

    CD34 is a highly glycosylated surface-expressed sialomucin and, because it is present on hematopoietic stem cells (HSCs), has demonstrated immense clinical utility in their enumeration in aphaeresis products, immunoaffinity purification for transplantation, and disease monitoring. The success of CD34 based reagents in identifying hematopoietic progenitors led to the assumption that CD34 is expressed on cells with regenerative potential and is sufficient for hematopoietic reconstitution in marrow-ablated recipients. However, its role has not been identified in substantial detail. 

With the advent of the fact that CD34 binds adapter protein like CRK-L in cytosol and CD34 knock out studies identified a a signaling role, CD34 antigen has been proposed to play a signaling function. Since it is a sialomucin, a member of the group adhesion molecules, we attempted to identify a role by over-expreesing its gene in cell lines. We report here that CD34 and engineered forms (Ser306 & Tyr318) significantly regulates adhesion to stromal cells, like mesenchymal stem cells and bone marrow ligands. These enhance binding of cells overexpressing CD34 by upregulating integrins and we therefore propose that such cells may effectively potentiate the success of transplantation through greater homing if they are used for transfusion

    Correlation of non-stress test with fetal outcome in term of Apgar score- a prospective observation study

    Background: Non-stress test (NST) is a graphical recording of changes in fetal heart activity and uterine contraction along with fetal movement when uterus is quiescent. The objectives of antepartum fetal surveillance are to prevent fetal death and avoidance of unnecessary intervention. This study using NST as a tool for routine antepartum fetal surveillance was trying to catch up those fetuses who might be at risk in womb and provide prompt intervention in otherwise considered normal pregnancies without any obvious high risk factor thus giving the best outcome in mothers.Methods: The objective of this study was to evaluate the correlation of the non-stress test with fetal outcome in pregnancies from 37-42 weeks of gestation.  This was a prospective observational study at Pannadhaya Rajkiya Mahila Chikitsalya Udaipur (Rajasthan) from February 2022 to July 2022. This study included 100 normal pregnant mothers from 37 weeks to 42 weeks who were subjected to NST.Results: The parameters of poor fetal outcome like Apgar score <7 at 5 minutes had increased incidences in the non reactive group.Conclusions: NST tells about acute fetal hypoxia and decision to delivery time can be made for those patients with fetal distress so that a major improvement in the outcome among parturient can be achieved with abnormal NST results. So This study suggested that the NST was found to be a good predictor of the healthy foetus even in normal pregnancies between 37-42 weeks of gestation and the probability of an adverse outcome such as poor Apgar score increases with a non reactive strip

    Genital myiasis is an infestation by fly larvae in genital organs, where they feed and develop as parasites. They can cause severe infection, inflammatory reaction and can be linked to psychiatric disturbances. Commonly cutaneous, ophthalmic, auricular and nasopharyngeal myiasis are seen, with genital myiasis being a rare condition. We reported a case of an 82-year-old postmenopausal female who presented with the complaint of intense pain in genital region. After clinical evaluation she was clinically diagnosed as a case of genital myiasis of prolapsed uterus. Myiasis of the genital organ is a rare clinical entity. Good personal hygiene and proper sanitary conditions are very crucial for prevention of myiasis

    Complicated crown-root fracture of maxillary central and lateral incisors is common in case of severe trauma or sports-related injury. It happens because of their anterior positioning in oral cavity and protrusive eruptive pattern. On their first dental visit, these patients are in pain and need emergency care. Because of impaired function, esthetics, and phonetics, such patients are quite apprehensive during their emergency visit. Successful pain management with immediate restoration of function, esthetics and phonetics should be the prime objective while handling such cases. This paper describes immediate treatment of oblique crown root fracture of maxillary right lateral incisor with reattachment procedure using light transmitting fiber post. After two and half years, the reattached fragment still has satisfying esthetics and excellent function

    ANTI MICRO-BIOLOGICAL ANALYSIS OF ARJUNARISTA

    Arjunarista, an imperative Ayurvedic formulation used for treatment of cardiovascular disorders and prepared by fermenting the decoction of specified plant materials i.e. Arjuna twak (Terminalia arjuna), Draksha phala (Vitis vinifera), Madhuk puspa (Madhuka indica), Dhataki puspa (Woodfordia fruticosa) and Jaggery (Saccharum officinarum). Aim: To assess anti-microbial analysis of Arjunarista. Setting and Design: Arjunarista was prepared as per process of Shandhan kalpana mentioned in manuscripts of Ayurveda with consideration of two variations. These variations were at the level of pot like earthen pot and porcelain pot and addition/deletion of flowers of Dhataki (Woodfordia fruticosa (L.) Kurz). Methods and Materials: Micro-biological analysis was done for total bacterial count, total fungal count and specific pathogens. Total bacterial count, total fungal count was done via plate count agar method and potato dextrose agar respectively. Test for specific pathogens like E. coli, Salmonella sp., Staphylococcus aureus &amp; Pseudomonas aeruginosa was done by Violet red bile agar, Salmonella Agar and Mueller Hinton Agar (for last two bacteria). Results: Total bacterial count (TBC) was absent in all batches except one sample of batch II and batch IV respectively. Total fungal count (TFC) was not present in all batches except one sample of batch I. Specific pathogens count were not present at all in any batches. Conclusion: Arjunarista showed anti micro-biological activity irrespective of variations taken in study

    A rare case of giant condyloma acuminata during pregnancy

    Genital warts (Condyloma acuminata) are a benign lesion caused by human papillomavirus (HPV) type-6 or type-11 and are usually transmitted sexually. During pregnancy, condyloma has a tendency to proliferate and may have recurrence. We have a case of G2P1L1 38 weeks previously normal vaginal delivered patient in our hospital with extensive genital warts. Patient was successfully managed by surgical excision and after that in follow up after 2 months for podophyllin resin application on remaining lesions. HPV infection presentation can range from asymptomatic to cervical cancer. Small genital wart lesion may become extensive and cumbersome during pregnancy and again regress after delivery in due course of time. HPV vaccination, sex education and early treatment of condyloma lesions should prevent and, in any case, improve the prognosis of this disease

    Projet COQWe propose an implementation and a proof of correctness of binary decision diagrams (BDDs), completely formalized in Coq. This not only shows that BDD algorithms are correct, but also yields a certified BDD algorithm running in Caml, using extraction, and even allows us to run BDD-based algorithms inside Coq itself, by reflection. The latter point paves the path for a smooth integration of symbolic model-checking in the Coq proof assistant. Extensive experimental results show that this approach works in practice, and is able to solve both relatively hard propositional problems and actual industrial hardware verification problems
