Factores de riesgo cardiovascular en pacientes con alopecia androgenética: síndrome metabólico y ateromatosis carotidea
Introducción: La relación entre la alopecia androgenética (AAG) y la afectación cardiovascular ha sido
objeto de estudio por algunos autores en las últimas décadas, obteniéndose diferentes resultados en los
distintos estudios epidemiológicos. El objetivo de este trabajo es conocer la prevalencia del síndrome
metabólico y el grado de arterioesclerosis carotídea, en los pacientes con alopecia androgenética de inicio
precoz (antes de los 35 años), y realizar una comparación con un grupo control de pacientes
dermatológicos sin alopecia.
Material y Métodos: Treinta pacientes (21 varones y 9 mujeres) diagnosticados de alopecia androgenética
de inicio precoz en el Servicio de Dermatología del Hospital San Cecilio, durante el período comprendido
desde noviembre de 2007 a Mayo de 2008, así como treinta controles son estudiados. En ambos grupos se
analizaron los criterios de síndrome metabólico que propone la ATP-III (obesidad, trigliceridemia, HDL-C,
tensión arterial y glucemia), y la presencia de placa de ateroma carotidea medida mediante ecografía
doppler. Otros factores de riesgo cardiovascular y estudio hormonal fueron también analizados.
Resultados: El 50% de los pacientes con alopecia androgenética cumplen criterios de síndrome metabólico
frente al 13% del grupo control (P<0.05). Nueve pacientes con alopecia androgenética presentaron placa
de ateroma, cuatro de ellos en ambas carótidas, y en dos casos había afectación del flujo sanguíneo. En el
grupo control se encontró placa carotídea unilateral en 2 pacientes (P<0,05), sin afectación del flujo. El IMC
(índice de masa corporal) en ambos grupos fue similar (valor medio de 27,3 para el grupo control y 28 para
los pacientes con alopecia), sin embargo la obesidad abdominal y la hipertrigliceridemia, analizados de
forma independiente, alcanzaron valores significativamente más altos en el grupo de alopécicos.
Los niveles de testosterona fueron similares en ambos grupos, aunque los valores de SHBG (globulina
transportadora de hormonas sexuales) fueron superiores para el grupo control, sin alcanzar significación
estadística. Sin embargo los niveles de insulina y aldosterona resultaron ser mayores en el grupo de
pacientes con AAG. (P<0,05)
Discusión: La elevada frecuencia con la que se presenta el síndrome metabólico y la ateromatosis
carotídea, en los pacientes con alopecia androgenética hace necesario un screening en este grupo para
detectar precozmente a aquellos individuos en riesgo, e iniciar tratamiento preventivo antes de que se
establezca la enfermedad cardiovascular.Introduction: The link between androgenetic alopecia (AGA) and cardiovascular disease has be studied by
several authors in recent decades, with different results being obtained from the epidemiological studies
carried out. The purpose of this paper is to determine the prevalence of metabolic syndrome and the degree
of carotid arteriosclerosis in patients with early-onset AGA (before the age of 35) treated at our hospital,
and make a comparison with a control group of dermatology patients without AGA.
Material and Methods: Thirty patients (21 males and 9 females) diagnosed with early-onset AGA by the
Dermatology Unit at San Cecilio Hospital between November 2007 and May 2008, plus a control group of
another 30 patients, divided identically by sex, who were being treated for other pathologies. In both
groups the criteria for metabolic syndrome proposed by the ATP-III (obesity, triglyceridaemia, HDL-C, blood pressure and glycaemia), and the presence of carotid atheromatous plaque measured by Doppler
ultrasound. Other cardiovascular risk factors and hormone studies were also considered.
Results: 50% of the AGA patients met metabolic syndrome criteria, compared with 13% in the control
group (P<0.05). Of the AGA patients, 9 presented atheromatous plaque, 4 of them in both carotids, and in 2
cases the blood flow was affected. In the control group unilateral carotid plaque was found in 2 patients
(P<0,05), without the blood flow being affected.
The BMI (body-mass index) was similar in the two groups (mean value of 27.3 for the control group and 28
for the AGA patients); however, abdominal obesity and hypertriglyceridaemia, analysed
separately,reached significantly higher values in the alopecia group. Testosterone levels were similar in the
two groups, although SHBG (sex-hormone-binding globulin) was higher in the control group, although not
statistically significant. However, insulin and aldosterone levels were higher in the AGA group (P<0,05).
Discussion: The high frequency with which metabolic syndrome and carotid atheromatosis are found in
patients with androgenetic alopecia (AGA) makes it appropriate for this group to be screened with a view to
the early detection of individuals at risk, in order that preventive treatment may begin before
cardiovascular disease (CVD) sets in
Association of 25-hydroxyvitamin D with Metabolic Syndrome in Patients with Psoriasis: A Case-control Study
Vitamin D deficiency is associated with higher cardiovascular risk and metabolic syndrome (MeS) criteria. The main objective of this study was to analyse the association of 25-hydroxyvitamin D (25-OHD) serum levels with MeS (National Cholesterol Education Program Adult Treatment Panel-III criteria) in 46 Spanish patients with psoriasis, but without arthritis and systemic treatment, and 46 control subjects, matched by sex and age. The patients with psoriasis showed significantly lower level of 25-OHD than controls (30.5 vs. 38.3 ng/ml; p =0.0001). Patients with MeS had significantly lower serum levels of 25-OHD than those without MeS (24.1 +/- 7.5 vs. 32.8 +/- 8.9,p =0.007), and a negative correlation was found between 25-OHD and waist circumference, diastolic blood pressure, fasting glucose, and triglyceridaemia. In the control group no significant correlation between 25-OHD and MeS was found. Although the sample was small, our results suggest a potential protective role for 25-OHD in the metabolic profile of patients with psoriasis without arthritis
Vitamin D and melanoma
La relación entre la vitamina D y diversos tumores ha sido objeto de estudio por varios autores en los últimos
años. El papel protector de la vitamina D se ha confirmado en estudios epidemiológicos para tumores como el
de colon y se ha sugerido en el melanoma. La vitamina D podría tener un papel antitumoral, de tal manera
que una menor actividad de la vitamina D estaría relacionada con un mayor riesgo de melanoma. La mayor
parte de la vitamina D se obtiene a través de la radiación ultravioleta. Surge así la controversia entre el
beneficio de la vitamina D y su función antitumoral, frente a la radiación ultravioleta, carcinógeno conocido
que altera el ADN de las células y aumenta el riesgo de cáncer cutáneo. El objetivo de nuestra revisión es
proporcionar un análisis detallado que hace referencia al actual interés en la vitamina D y el melanoma.The relationship between vitamin D and various tumors has been studied by several authors in recent years.

ultraviolet radiation, a carcinogen known to alter the DNA of cells and increases the risk of skin cancer. The

Melanomas in the elderly: clinical and epidemiological features
El melanoma es un tumor muy agresivo derivado de los melanocitos con una incidencia creciente y unas
tasas altas de mortalidad en la población anciana. Se ha diseñado un estudio retrospectivo para conocer las
características clínicas y epidemiológicas de los melanomas intervenidos en la población de pacientes
mayores de 65 años en el Hospital San Cecilio de Granada durante el año 2009. El valor medio del espesor de
Breslow observado en los pacientes ancianos ha sido muy elevado (2,6 mm) lo que ensombrece el pronóstico
de esta enfermedad. Es muy importante la sospecha diagnostica por parte de los médicos de atención
primaria y geriatras para realizar una derivación precoz a unidades especializadas para iniciar un tratamiento
adecuado.Melanoma is an aggressive tumor derived from melanocytes with increasing incidence and high mortality
rates in the elderly population. We have designed a retrospective study to know the clinical and
epidemiological features of melanoma in patients aged more than 65 years old undergoing surgery in
Hospital Clinic in Granada (Spain) in 2009. Mean Breslow thickness observed in elderly patients has been
very high (2.6 mm) which worsens prognosis of this disease. It is very important to the suspected diagnosis
by primary care physicians and geriatricians for an early referral to specialized units to initiate appropriate
treatments
Urticarial vasculitis: a retrospective study of 20 cases and diagnostic-therapeutic protocol
La urticaria vasculitis es una enfermedad crónica que se caracteriza por episodios urticariales o de
angioedema que puede asociarse a niveles bajos de complemento. Hemos realizado un estudio
descriptivo retrospectivo que incluye a 20 pacientes con urticaria vasculitis diagnosticados en los últimos
5 años donde analizamos los síntomas más frecuentes y los tratamientos que se han empleado. A
continuación se ha diseñado un protocolo diagnóstico-terapéutico de Urticaria Vasculitis para orientar el
manejo de estos pacientes, en el que se incluye una breve definición de esta entidad así como las

Cutaneous manifestations of scleroderma: a therapeutic challenge
La esclerodermia es una conectivopatía crónica y multisistémica con alta morbi-mortalidad y de
patogenia desconocida. Los principales síntomas son consecuencia del depósito de colágeno y el daño
endotelial a nivel de corazón, pulmón, tubo digestivo, riñón y piel. La afectación cutánea puede llegar a ser
muy incapacitante y empeorar de forma marcada la calidad de vida de los pacientes. En la presente
revisión se repasan las principales manifestaciones dermatológicas de esta enfermedad, su diagnóstico y
las posibilidades de tratamiento.Scleroderma is a chronic and multisystemic connective tissue disease with high morbidity and mortality
