Conflicting or complementary role of computed tomography (CT) and positron emission tomography (PET)/CT in the assessment of thymic cancer and thymoma: Our experience and literature review
Background: To evaluate the role of computed tomography (CT) and positron
emission tomography (PET)/CT in patients with thymic cancer and thymoma at
initial staging.
Methods: We retrospectively reviewed CT and PET/CT scans of 26 patients with a
thymic cancer (n = 9) or thymoma (n = 17). Chest CT findings documented were
qualitative and quantitative. Both qualitative and semiquantitative data were recovered
by PET/CT. The comparisons among histological entities, outcome, and qualitative
data from CT and PET/CT were made by non-parametric analysis.
Results: PET/CT resulted positive in 15/17 patients with thymoma. CT was available
in 5/9 (56%) patients with thymic cancer and in 3/17 with thymoma. All quantitative
CT parameters were significantly higher in patients with thymic cancer than
thymoma (maximum axial diameter: 45 vs. 20 mm, maximum longitudinal diameter:
69 vs. 21 mm and volume: 77.91 vs. 4.52 mL; all P < 0.05). Conversely, only
metabolic tumor volume (MTV) and total lesion glycolysis were significantly different
in patients with thymic cancer than thymoma (126.53 vs. 6.03 cm3 and 246.05
vs. 20.32, respectively; both P < 0.05). After a median follow-up time of 17.45
months, four recurrences of disease occurred: three in patients with thymic cancer
and one with a type B2 thymoma. CT volume in patients with recurrent disease was
102.19 mL versus a median value of 62.5 mL in six disease-free patients. MTV was
higher in the recurrent than disease-free patient subset (143.3 vs. 81.13 cm3),
although not statistically significant (P = 0.075).
Conclusion: Our preliminary results demonstrated that both morphological and
metabolic volume could be useful from a diagnostic and prognostic point of view in
thymic cancer and thymoma patients. A large multi-center clinical trial experience
for confirming the findings of this study seems mandatory

Refining Pheromone Lures for the Invasive Halyomorpha halys (Hemiptera: Pentatomidae) Through Collaborative Trials in the United States and Europe
Brown marmorated stink bug, Halyomorpha halys, is native to Asia and has invaded North America and Europe inflicting serious agricultural damage to specialty and row crops. Tools to monitor the spread of H. halys include traps baited with the two-component aggregation pheromone (PHER), (3S,6S,7R,10S)-10,11-epoxy-1-bisabolen-3-ol and (3R,6S,7R,10S)-10,11-epoxy-1-bisabolen-3-ol, and pheromone synergist, methyl (2E,4E,6Z)-decatrienoate (MDT). Here, an international team of researchers conducted trials aimed at evaluating prototype commercial lures for H. halys to establish relative attractiveness of: 1) low and high loading rates of PHER and MDT for monitoring tools and attract and kill tactics; 2) polyethylene lure delivery substrates; and 3) the inclusion of ethyl (2E,4E,6Z)-decatrieonate (EDT), a compound that enhances captures when combined with PHER in lures. In general, PHER loading rate had a greater impact on overall trap captures compared with loading of MDT, but reductions in PHER loading and accompanying lower trap captures could be offset by increasing loading of MDT. As MDT is less expensive to produce, these findings enable reduced production costs. Traps baited with lures containing PHER and EDT resulted in numerically increased captures when EDT was loaded at a high rate, but captures were not significantly greater than those traps baited with lures containing standard PHER and MDT. Experimental polyethylene vial dispensers did not outperform standard lure dispensers; trap captures were significantly lower in most cases. Ultimately, these results will enable refinement of commercially available lures for H. halys to balance attraction and sensitivity with production cost
KIT/PDGFRA Variant Allele Frequency as Prognostic Factor in Gastrointestinal Stromal Tumors (GISTs): Results From a Multi-Institutional Cohort Study
Background: The patient selection for optimal adjuvant therapy in gastrointestinal stromal tumors (GISTs) is provided by nomogram based on tumor size, mitotic index, tumor location, and tumor rupture. Although mutational status is not currently used to risk assessment, tumor genotype showed a prognostic influence on natural history and tumor relapse. Innovative measures, such as KIT/PDGFRA-mutant-specific variant allele frequency (VAF) levels detection from next-generation sequencing (NGS), may act as a surrogate of tumor burden and correlate with prognosis and overall survival of patients with GIST, helping the choice for adjuvant treatment.
Patients and methods: This was a multicenter, hospital-based, retrospective/prospective cohort study to investigate the prognostic role of KIT or PDGFRA-VAF of GIST in patients with radically resected localized disease. In the current manuscript, we present the results from the retrospective phase of the study.
Results: Two-hundred (200) patients with GIST between 2015 and 2022 afferent to 6 Italian Oncologic Centers in the EURACAN Network were included in the study. The receiver operating characteristic (ROC) curves analysis was used to classify "low" vs. "high" VAF values, further normalized on neoplastic cellularity (nVAF). When RFS between the low and high nVAF groups were compared, patients with GIST with KIT/PDGFRA nVAF > 50% showed less favorable RFS than patients in the group of nVAF ≤ 50% (2-year RFS, 72.6% vs. 93%, respectively; P = .003). The multivariable Cox regression model confirmed these results. In the homogeneous sub-population of intermediate-risk, patients with KIT-mutated GIST, the presence of nVAF >50% was statistically associated with higher disease recurrence.
Conclusion: In our study, we demonstrated that higher nVAF levels were independent predictors of GIST prognosis and survival in localized GIST patients with tumors harboring KIT or PDGFRA mutations. Atmospheric Dispersion Modelling and Spatial Analysis to Evaluate Population Exposure to Pesticides from Farming Processes
This work originates from an epidemiological study aimed to assess the correlation between population exposure to pesticides used in agriculture and adverse health effects. In support of the population exposure evaluation two models implemented by the authors were applied: a GIS-based proximity model and the CAREA atmospheric dispersion model. In this work, the results of the two models are presented and compared. Despite the proximity analysis is widely used for these kinds of studies, it was investigated how meteorology could affect the exposure assessment. Both models were applied to pesticides emitted by 1519 agricultural fields and considering 2584 receptors distributed over an area of 8430 km2. CAREA output shows a considerable enhancement in the percentage of exposed receptors, from the 4% of the proximity model to the 54% of the CAREA model. Moreover, the spatial analysis of the results on a specific test site showed that the effects of meteorology considered by CAREA led to an anisotropic exposure distribution that differs considerably from the symmetric distribution resulting by the proximity model. In addition, the results of a field campaign for the definition and planning of ground measurement of concentration for the validation of CAREA are presented.
