Spectropolarimetry of Supernovae
Overwhelming evidence has accumulated in recent years that supernova
explosions are intrinsically 3-dimensional phenomena with significant
departures from spherical symmetry. We review the evidence derived from
spectropolarimetry that has established several key results: virtually all
supernovae are significantly aspherical near maximum light; core-collapse
supernovae behave differently than thermonuclear (Type Ia) supernovae; the
asphericity of core-collapse supernovae is stronger in the inner layers showing
that the explosion process itself is strongly aspherical; core-collapse
supernovae tend to establish a preferred direction of asymmetry; the
asphericity is stronger in the outer layers of thermonuclear supernovae
providing constraints on the burning process. We emphasize the utility of the
Q/U plane as a diagnostic tool and revisit SN 1987A and SN 1993J in a
contemporary context. An axially-symmetric geometry can explain many basic
features of core-collapse supernovae, but significant departures from axial
symmetry are needed to explain most events. We introduce a spectropolarimetry
type to classify the range of behavior observed in polarized supernovae.
Understanding asymmetries in supernovae is important for phenomena as diverse
as the origins of gamma-ray bursts and the cosmological applications of Type Ia

Modeling the transfer of land and water from agricultural to urban uses in the Middle Rio Grande Basin, New Mexico.
Social and ecological scientists emphasize that effective natural resource management depends in part on understanding the dynamic relationship between the physical and non-physical process associated with resource consumption. In this case, the physical processes include hydrological, climatological and ecological dynamics, and the non-physical process include social, economic and cultural dynamics among humans who do the resource consumption. This project represents a case study aimed at modeling coupled social and physical processes in a single decision support system. In central New Mexico, individual land use decisions over the past five decades have resulted in the gradual transformation of the Middle Rio Grande Valley from a primarily rural agricultural landscape to a largely urban one. In the arid southwestern U.S., the aggregate impact of individual decisions about land use is uniquely important to understand, because scarce hydrological resources will likely limit the viability of resulting growth and development trajectories. This decision support tool is intended to help planners in the area look forward in their efforts to create a collectively defined 'desired' social landscape in the Middle Rio Grande. Our research question explored the ways in which socio-cultural values impact decisions regarding that landscape and associated land use. Because of the constraints hydrological resources place on land use, we first assumed that water use, as embodied in water rights, was a reasonable surrogate for land use. We thought that modeling the movement of water rights over time and across water source types (surface and ground) would provide planners with insight into the possibilities for certain types of decisions regarding social landscapes, and the impact those same decisions would have on those landscapes. We found that water rights transfer data in New Mexico is too incomplete and inaccurate to use as the basis for the model. Furthermore, because of its lack of accuracy and completeness, water rights ownership was a poor indicator of water and land usage habits and patterns. We also found that commitment among users in the Middle Rio Grande Valley is to an agricultural lifestyle, not to a community or place. This commitment is conditioned primarily by generational cohort and past experience. If conditions warrant, many would be willing to practice the lifestyle elsewhere. A related finding was that sometimes the pressure to sell was not the putative price of the land, but the taxes on the land. These taxes were, in turn, a function of the level of urbanization of the neighborhood. This urbanization impacted the quality of the agricultural lifestyle. The project also yielded some valuable lessons regarding the model development process. A facilitative and collaborative style (rather than a top-down, directive style) was most productive with the inter-disciplinary , inter-institutional team that worked on the project. This allowed for the emergence of a process model which combined small, discipline- and/or task-specific subgroups with larger, integrating team meetings. The project objective was to develop a model that could be used to run test scenarios in which we explored the potential impact of different policy options. We achieved that objective, although not with the level of success or modeling fidelity which we had hoped for. This report only describes very superficially the results of test scenarios, since more complete analysis of scenarios would require more time and effort. Our greatest obstacle in the successful completion of the project was that required data were sparse, of poor quality, or completely nonexistent. Moreover, we found no similar modeling or research efforts taking place at either the state or local level. This leads to a key finding of this project: that state and local policy decisions regarding land use, development, urbanization, and water resource allocation are being made with minimal data and without the benefit of economic or social policy analysis
Computerized clinical documentation system in the pediatric intensive care unit
BACKGROUND: To determine whether a computerized clinical documentation system (CDS): 1) decreased time spent charting and increased time spent in patient care; 2) decreased medication errors; 3) improved clinical decision making; 4) improved quality of documentation; and/or 5) improved shift to shift nursing continuity. METHODS: Before and after implementation of CDS, a time study involving nursing care, medication delivery, and normalization of serum calcium and potassium values was performed. In addition, an evaluation of completeness of documentation and a clinician survey of shift to shift reporting were also completed. This was a modified one group, pretest-posttest design. RESULTS: With the CDS there was: improved legibility and completeness of documentation, data with better accessibility and accuracy, no change in time spent in direct patient care or charting by nursing staff. Incidental observations from the study included improved management functions of our nurse manager; improved JCAHO documentation compliance; timely access to clinical data (labs, vitals, etc); a decrease in time and resource use for audits; improved reimbursement because of the ability to reconstruct lost charts; limited human data entry by automatic data logging; eliminated costs of printing forms. CDS cost was reasonable. CONCLUSIONS: When compared to a paper chart, the CDS provided a more legible, compete, and accessible patient record without affecting time spent in direct patient care. The availability of the CDS improved shift to shift reporting. Other observations showed that the CDS improved management capabilities; helped physicians deliver care; improved reimbursement; limited data entry errors; and reduced costs
Prevention of febrile neutropenia: use of granulocyte colony-stimulating factors
There is good evidence to suggest that dose intensity is important when considering the effectiveness of adjuvant chemotherapy in patients with breast cancer. However, the development of chemotherapy-induced febrile neutropenia can lead to reduction in dose intensity and other treatment modifications, which may negatively affect patient outcomes. Febrile neutropenia can be prevented by the use of primary prophylactic treatment, notably with granulocyte colony-stimulating factors. This practice is supported by international guidelines, all of which recommend that primary prophylaxis with granulocyte colony-stimulating factors should be used with chemotherapy where the risk of febrile neutropenia is 20% or greater

Integrated system dynamics toolbox for water resources planning.
Public mediated resource planning is quickly becoming the norm rather than the exception. Unfortunately, supporting tools are lacking that interactively engage the public in the decision-making process and integrate over the myriad values that influence water policy. In the pages of this report we document the first steps toward developing a specialized decision framework to meet this need; specifically, a modular and generic resource-planning ''toolbox''. The technical challenge lies in the integration of the disparate systems of hydrology, ecology, climate, demographics, economics, policy and law, each of which influence the supply and demand for water. Specifically, these systems, their associated processes, and most importantly the constitutive relations that link them must be identified, abstracted, and quantified. For this reason, the toolbox forms a collection of process modules and constitutive relations that the analyst can ''swap'' in and out to model the physical and social systems unique to their problem. This toolbox with all of its modules is developed within the common computational platform of system dynamics linked to a Geographical Information System (GIS). Development of this resource-planning toolbox represents an important foundational element of the proposed interagency center for Computer Aided Dispute Resolution (CADRe). The Center's mission is to manage water conflict through the application of computer-aided collaborative decision-making methods. The Center will promote the use of decision-support technologies within collaborative stakeholder processes to help stakeholders find common ground and create mutually beneficial water management solutions. The Center will also serve to develop new methods and technologies to help federal, state and local water managers find innovative and balanced solutions to the nation's most vexing water problems. The toolbox is an important step toward achieving the technology development goals of this center

Impact of effective prevention and management of febrile neutropenia
Chemotherapy-induced febrile neutropenia is costly in both financial and human terms. The associated costs can be reduced substantially through the development and implementation of national policies and locally agreed protocols for the prevention and management of febrile neutropenia. Patients, the NHS, healthcare professionals and the broader community all stand to benefit from a commitment to effective management of this common and predictable side effect of some chemotherapy regimens for early-stage breast cancer
What we talk about when we talk about "global mindset": managerial cognition in multinational corporations
Recent developments in the global economy and in multinational corporations have placed significant emphasis on the cognitive orientations of managers, giving rise to a number of concepts such as “global mindset” that are presumed to be associated with the effective management of multinational corporations (MNCs). This paper reviews the literature on global mindset and clarifies some of the conceptual confusion surrounding the construct. We identify common themes across writers, suggesting that the majority of studies fall into one of three research perspectives: cultural, strategic, and multidimensional. We also identify two constructs from the social sciences that underlie the perspectives found in the literature: cosmopolitanism and cognitive complexity and use these two constructs to develop an integrative theoretical framework of global mindset. We then provide a critical assessment of the field of global mindset and suggest directions for future theoretical and empirical research

Alcohol, metabolic risk and elevated serum gamma-glutamyl transferase (GGT) in Indigenous Australians
<p>Abstract</p> <p>Background</p> <p>The interaction between overweight/obesity and alcohol intake on liver enzyme concentrations have been demonstrated. No studies have yet examined the interaction between metabolic syndrome or multiple metabolic risk factors and alcohol intake on liver enzymes. The aim of this study was to examine if alcohol consumption modifies the effect of metabolic risk on elevated serum GGT in Indigenous Australians.</p> <p>Methods</p> <p>Data were from N = 2609 Indigenous Australians who participated in a health screening program in rural far north Queensland in 1999-2000 (44.5% response rate). The individual and interactive effects of metabolic risk and alcohol drinking on elevated serum GGT concentrations (≥50 U/L) were analyzed using logistic regression.</p> <p>Results</p> <p>Overall, 26% of the population had GGT≥50 U/L. Elevated GGT was associated with alcohol drinking (moderate drinking: OR 2.3 [95%CI 1.6 - 3.2]; risky drinking: OR 6.0 [4.4 - 8.2]), and with abdominal obesity (OR 3.7 [2.5 - 5.6]), adverse metabolic risk cluster profile (OR 3.4 [2.6 - 4.3]) and metabolic syndrome (OR 2.7 [2.1 - 3.5]) after adjustment for age, sex, ethnicity, smoking, physical activity and BMI. The associations of obesity and metabolic syndrome with elevated GGT were similar across alcohol drinking strata, but the association of an adverse metabolic risk cluster profile with elevated GGT was larger in risky drinkers (OR 4.9 [3.7 - 6.7]) than in moderate drinkers (OR 2.8 [1.6 - 4.9]) and abstainers (OR 1.6 [0.9 - 2.8]).</p> <p>Conclusions</p> <p>In this Indigenous population, an adverse metabolic profile conferred three times the risk of elevated GGT in risky drinkers compared with abstainers, independent of sex and ethnicity. Community interventions need to target both determinants of the population's metabolic status and alcohol consumption to reduce the risk of elevated GGT.</p
