    Medical Treatment for Burn Patients with Eating Disorders: A Case Report

    There have been many cases of burn patients who also suffer from psychiatric problems, including eating disorders. We present a case of a 38-year-old female with an eating disorder and depression who became light-headed and fell, spilling boiling water from a kettle on herself at home sustaining partial thickness and full thickness burns over 5% of her total body surface area: left buttock and right thigh and calf. Eating disorders (in the present case, anorexia nervosa) cause emaciation and malnutrition, and consent for hospitalization from the patient and/or family is often difficult. During the medical treatment of burns for these patients, consideration not only of physical symptoms caused by malnutrition but also the psychiatric issues is required. Therefore, multifaceted and complex care must be given to burn patients with eating disorders

    Towards the Development of a Korean EXP Checklist and the Possibility of Estimating Levels of Experiencing through Non-verbal Expressions

    韓国の心理臨床の現場では体験過程尺度(EXPスケール)はほとんど活用されていない。そのため、本研究のひとつの側面は、体験過程の推定に使いやすいEXPチェックリストを韓国に紹介することであった。三宅(2007)のEXPチェックリストを久保田ら(印刷中)はEXPCKIIver. 1.1 に改良しており、筆者はそのEXPCK-IIver.1.1 を韓国語に翻訳した。EXP5段階の各レベルを反映する10編のサンプル・ビデオを韓国語で作成した。これらのクリップは複数の評定者が動画と日本語訳逐語記録をもとにEXPCK-IIver1.0 で評定し正答率を算出した。その結果を考慮して、7編のビデオクリップと韓国語に訳したEXPCK-IIver1.1 を韓国のトクソン女子大学に送付し、共同研究を準備した。また、日本で行なった評定の分析と評定者に対するアンケートを行ない、評定者がどのようなことに注目してEXP レベルを判定するかを調査した。一部のビデオを再編集したところ、評定結果の正答率は5 段階別にパーセント表記すると、100% Very Low;87.5% Low;75% Middle;87.5% High;100% Very High となっていた。また、評定者は言語的情報だけではなく、沈黙、表情、声、身体の動きなどの非言語情報にも注目していたことがわかった。ビデオが韓国語であったにもかかわらず、比較的高い正答率で評定することができたのは、そのためと考えることができる。体験過程様式の教育には、非言語的情報にも注目する必要があることが考察された。The Experiencing Scales are not used among psychotherapy researchers and practitioners in Korea. One aspect of this study is to introduce to Korea the EXP Checklist, a paper and pencil method for estimating modes of experiencing. Th e authors translated into Korean the EXPCK-II ver.1.1 (Kubota et al., in press), a checklist for estimating modes of experiencing, which was based on an earlier version by Miyake et al. (2007). Ten sample video segments produced in Korean were edited to reflect the five levels of experiencing. Japanese raters rated these video segments with translated transcripts. After considering rating correctness, seven of these video segments were sent to Duksung Women’s University in Korea for the purpose of future collaborative research. The ratings by the Japanese raters were analyzed, and open-ended questions asking for what the raters referred to during the ratings were studied. The correctness of ratings shown in percentages were 100% for Very Low, 87.5% for Low, 75% for Middle, 87.5% for High, and 100% for Very High. Moreover, it was found that raters also considered non-verbal expressions such as pauses, facial expressions, tone of voice, and gestures for reference during the ratings. These non-verbal expressions may have contributed to the high levels of accuracy in the ratings.     Combined treatment using Mohs' paste and neoadjuvant chemotherapy for giant gluteal soft tissue sarcoma with malignant fungating wound: a case report

    A malignant fungating wound is a cutaneous infiltration of malignant tumor or metastatic lesion that develop into ulceration. Local control is often difficult to obtain, because the Quality of Life of patients can decrease considerably due to bleeding, exudation, odor and pain from the wound. There are few studies in the literature that report the use of Mohs' paste for soft tissue sarcoma with malignant fungating wound. We report a case resulting in good local control for a patient with dedifferentiated liposarcoma with gluteal ulceration by the combined use of Mohs' paste and chemotherapy as a pre-operative adjuvant therapy. Mohs' paste controlled the infection, odor and exudation in approximately 2 weeks, and good visualization of the surgical field was obtained due to tumor volume reduction.     The high-affinity binding of Clostridium botulinum type B neurotoxin to synaptotagmin II associated with gangliosides GT1b/GD1a

    Abstract125I-labeled botulinum type B neurotoxin was shown to bind specifically to recombinant rat synaptotagmins I and II. Binding required reconstitution of the recombinant proteins with gangliosides GT1b/GD1a. Scatchard plot analyses revealed a single class of binding site with dissociation constants of 0.23 and 2.3 nM for synaptotagmin II and synaptotagmin I, respectively, values very similar to those of the high- (0.4 nM) and low-affinity (4.1 nM) binding sites in synaptosomes. The high-affinity binding of neurotoxin to synaptosomes was specifically inhibited by a monoclonal antibody recognizing with the amino-terminal region of synaptotagmin II. These results suggest that this region of synaptotagmin II participates in the formation of the high-affinity toxin binding site by associating with specific gangliosides

    The Establishment of Japan Library School under the Allied occupation

    Before the WWII, there were few educational institutions of library science in Japan. During the Allied occupation period, some library training courses were held, ex. at Doshisha and Kyoto Universities, but they were on the lines of the pre-war framework, stressing on how to administrate a library. In 1948,Robert B. Downs suggested to establish a library institution at the University of Tokyo, in vain. Then in 1950,GHQ/SCAP began a program of establishing Japan Library School (JLS) with support of ALA, in order to train professional Japanese librarians. This represents a shift of occupational library policy from reformation as a whole to specification to library training. Robert L. Gitler, the director of JLS, chose Keio University, favoring its westernized philosophy. With financial support of Rockefeller Foundation, JLS went into orbit. There held Americanized library curriculum, laying stress on understanding of library\u27s role in society as well as technical methods. It joined Japanese and American librarianship

    Insight into the population dynamics of pathogenic bacteria causing grapevine crown gall in snowfall areas: snow cover protects the proliferation of pathogenic bacteria

    Grapevine crown gall (GCG) is a significant bacterial disease caused by tumorigenic Allorhizobium vitis (TAV) and is prevalent worldwide. TAV infects grapevines through wounds such as freezing injuries. Although grapevines typically avoid being wounded under snow cover, GCG occurs in many commercial vineyards in snowy regions. This study investigated the TAV population in GCG gall tissues, grapevine skins, and snow on grapevine skins from six infected vineyards located in Hokkaido, Japan, an area known for heavy snowfall. TAV was isolated not only from gall tissues but also from skins and snow on skins throughout the year. Hierarchical Bayesian model (HBM) analysis revealed that the number of TAV cells in gall tissues was affected by cultivar and low temperature, while those in skins were affected by location and low temperature. Additionally, Bayesian changepoint detection (BCD) showed that the number of TAV cells in gall and skin tissues increased during winter, including the snowfall season. Furthermore, the TAV population in grapevine skins under the snow was significantly higher than those above the snow, indicating that TAV under the snow is protected by the snow and can survive well during the snowfall season. This study highlights the ability of TAV to overwinter on/in galls and skins under the snow and act as inoculum for the next season

    Library and Information Science Examination: A Report on Provisional Implementation in Japan / Hiroya Takeuchi, Akira Nemoto, Makiko Miwa... [et.al] al.

    This paper aims to describe the details of the challenging Library and Information Science (LIS) examinations, which the Japan Society of Library and Information Science (JSLIS) has conducted annually on a trial basis since 2007, and discuss some of observations. For such purposes, its objective, coverage of subject areas, examination style and reporting of results to students are described. We also discuss our experiences and observations of the provisional implementation of the LIS examination over the past four years. The results of the examination indicated that senior students achieved higher scores than junior students in general, and that the students from university LIS faculties achieved generally higher scores than did those from the Shisho certificate courses, which cover a more limited subject area of LIS than LIS specialty programs. These results suggest that the examination is a good measure of student proficiency, which reflects the quality of the educational programs. The problems to be solved before full-scale implementation can be summarized as follows: 1) the lack of standard textbooks covering the scope of the examination; 2) the low economic base for an annual examination; 3) lack of awareness of the need for this examination in standardizing the curricula for LIS education; and 4) the lack of a mechanism to implement the examination nationwide to facilitate participation by students living far from examination venues

    The present study clearly demonstrated the effects of complete cerebral ischemia and selective ischemia on brain choline metabolism in ddY mice The complete cerebral ischemia mouse was produced by cervical dislocation, and the selective ischemia mouse was produced by bilateral occlusion of the common carotid arteries. The following results were obtained 1 Complete cerebral ischemia increased brain choline contents rapidly and remarkably for 10 minutes After 10 minutes, the rate of increase of brain choline contents suddenly fell. 2 The increased level of choline contents by complete cerebral ischemia differed among the brain regions (cerebral cortex, hippocampus, medulla oblongata, cerebellum) tested 3 Selective ischemia increased choline contents in the cerebral cortex and hippocampus but not in the medulla oblongata or cerebella. 4. As with complete ischemia, hypoxia induced by N_2 gas inhalation increased choline contents in all brain regions. 5. Hypoglycemia induced by insulin administration did not increase brain choline contents However, the ischemia under a hypoglycemia state produced a greater choline increase than ischemia alone 6 Complete cerebral ischemia decreased the contents of both phosphatidylcholme (PtdCh) and glycerophosphocholine (GlyCh) in the mouse brain. The present results suggest that a main cause for the ischemic increase of brain choline contents is hypoxia by cessation of blood flow, and a low energy state caused by hypoglycemia partly contributes to this increase Furthermore, it is suggested that the ischemic increase of brain choline depends on the choline accumulation that results from decomposition of both PtdCh and GlyC
