    Sternal plate fixation for sternal wound reconstruction: initial experience (Retrospective study)

    <p>Abstract</p> <p>Background</p> <p>Median sternotomy infection and bony nonunion are two commonly described complications which occur in 0.4 - 5.1% of cardiac procedures. Although relatively infrequent, these complications can lead to significant morbidity and mortality. The aim of this retrospective study is to evaluate the initial experience of a transverse plate fixation system following wound complications associated with sternal dehiscence with or without infection following cardiac surgery.</p> <p>Methods</p> <p>A retrospective chart review of 40 consecutive patients who required sternal wound reconstruction post sternotomy was performed. Soft tissue debridement with removal of all compromised tissue was performed. Sternal debridement was carried using ronguers to healthy bleeding bone. All patients underwent sternal fixation using three rib plates combined with a single manubrial plate (Titanium Sternal Fixation System<sup>Ā®</sup>, Synthes). Incisions were closed in a layered fashion with the pectoral muscles being advanced to the midline. Data were expressed as mean Ā± SD, Median (range) or number (%). Statistical analyses were made by using Excel 2003 for Windows (Microsoft, Redmond, WA, USA).</p> <p>Results</p> <p>There were 40 consecutive patients, 31 males and 9 females. Twenty two patients (55%) were diagnosed with sternal dehiscence alone and 18 patients (45%) with associated wound discharge. Thirty eight patients went on to heal their wounds. Two patients developed recurrent wound infection and required VAC therapy. Both were immunocompromised. Median post-op ICU stay was one day with the median hospital stay of 18 days after plating.</p> <p>Conclusion</p> <p>Sternal plating appears to be an effective option for the treatment of sternal wound dehiscence associated with sternal instability. Long-term follow-up and further larger studies are needed to address the indications, benefits and complications of sternal plating.</p

    Differences in carbon emissions reduction between countries pursuing renewable electricity versus nuclear power

    Two of the most widely emphasized contenders for carbon emissions reduction in the electricity sector are nuclear power and renewable energy. While scenarios regularly question the potential impacts of adoption of various technology mixes in the future, it is less clear which technology has been associated with greater historical emission reductions. Here, we use multiple regression analyses on global datasets of national carbon emissions and renewable and nuclear electricity production across 123 countries over 25 years to examine systematically patterns in how countries variously using nuclear power and renewables contrastingly show higher or lower carbon emissions. We find that larger-scale national nuclear attachments do not tend to associate with significantly lower carbon emissions while renewables do. We also find a negative association between the scales of national nuclear and renewables attachments. This suggests nuclear and renewables attachments tend to crowd each other out

    Fluxes of CO , water vapor, and sensible heat were measured by the eddy covariance method above a young ponderosa pine plantation in the Sierra Nevada Mountains (CA) over two growing seasons (1 June-10 September 1997 and 1 May-30 October 1998). The Mediterranean-type climate of California is characterized by a protracted summer drought, with precipitation occurring mainly from October through May. While drought stress increased continuously over both summer growing seasons, 1998 was wetter and cooler than average due to EL Nino climate patterns and 1997 was hotter and drier than average. One extreme 3-day heat wave in 1997 (Days 218-221) caused a step change in the relationship between H O flux and vapor pressure deficit, resulting in a change in canopy conductance, possibly due to cavitation of the tree xylem. This step change was also correlated with decreased rates of C sequestration and evapotranspiration; we estimate that this extreme climatic event decreased gross ecosystem production (GEP) by roughly 20% (4 Ī¼mol C m s ) for the rest of the growing season. In contrast, a cooler, wetter spring in 1998 delayed the onset of photosynthesis by about 3 weeks, resulting in roughly 20% lower GEP relative to the spring of 1997. We conclude that the net C balance of Mediterranean-climate pine ecosystems is sensitive to extreme events under low soil moisture conditions and could be altered by slight changes in the climate or hydrologic regime.
