    Cuantificación y aspecto en la frase nominal [un caudal de N]

    En este estudio se explora desde una perspectiva lingüística cognitiva la continuidad entre usos léxicos y usos semigramaticalizados del lexema caudal en la frase nominal del tipo [Det. caudalN1 de N2] en español peninsular escrito. Esta estructura da cabida a varios esquemas construccionales, yendo de [[Det. + N1] ( [de + N2]) ] con N1 como núcleo, y colocaciones [Det. N1 de N2] más o menos lexicalizadas, a [[Det. N1 de] N2] con N2 como núcleo. El significado del nombre caudal, vinculado al escenario fluvial, presenta varios componentes cuantitativos, espaciales, tempo-aspectuales y cualitativos que lo predisponen a la proyección metafórica a otros dominios que el hídrico. En la construcción binominal cuantificadora [[un caudal de] N2] caudal guarda algunas propiedades distribucionales nominales, y asimismo persisten ciertas facetas de su significado léxico. Si bien varía el grado de esquematización o blanqueo conceptual en función de N2 y del contexto más amplio, caudal no solo cuantifica N2 sino que al mismo tiempo le confiere una dimensión aspectual-valorativa concebida esencialmente en términos de fluidez, continuidad y acumulación, o sea, que sitúa a N2 en una perspectiva evolutiva.This paper explores from a cognitive linguistic perspective the continuity between lexical and semigrammaticalized uses of the lexeme caudal in the noun phrase of the type [Det. caudalN1 de N2] in written peninsular Spanish. This structure yields various constructional schemas, going from [[Det. + N1] ( [de + N2]) ] with N1 as nucleus, and more or less lexicalized [Det. N1 de N2] collocations, to [[Det. N1 de] N2] with N2 as nucleus. The meaning of the nominal caudal, linked to the fluvial scenery, presents various quantitative, spatial, tempo-aspectual and qualitative components that predispose it to metaphorical projection from the hydric domain to other domains. In the binominal quantifying construction [[un caudal de] N2] caudal keeps some nominal distributional properties, and certain facets of its lexical meaning persist as well. While the degree of schematization or semantic bleaching varies in function of N2 and of the wider context, caudal does not only quantify N2, it at the same time endows it with an aspectual-appreciatory dimension essentially conceived of in terms of fluidity, continuity and accumulation, i.e., situating N2 in an evolutionary perspective

Taking the hypothesis that position constraints are mostly preferential as a starting point, we develop a methodology based on statistical inference in order to provide a formal account of the relative importance of different groups of constraints. Results show the relative importance of lexical constraints and that frequency-based and length constraints are the best predictors. This suggests that the placement of adjectives not only depends on our knowledge of lexical items but also on the knowledge of the way in which we use them in discourse, i.e. on usage

    Curative effect of second curettage for treatment of gestational trophoblastic disease - Results of the Belgian registry for gestational trophoblastic disease.

    peer reviewedOBJECTIVE: We assessed the curative effect of a second curettage in patients with persistent hCG serum levels after first curettage for a gestational trophoblastic disease (GTD). STUDY DESIGN: This prospective observational study used the data of the Belgian register for GTD between July 2012 and January 2017. We analysed the data of patients who underwent a second curettage. We included 313 patients in the database. Primary endpoints were need for second curettage and chemotherapy. RESULTS: Thirty-seven patients of the study population (12 %) underwent a second curettage. 20 had persistent human chorionic gonadotropin hormone (hCG) elevation before second curettage. Of them, 9 patients (45 %) needed no further treatment afterwards. Eleven patients (55 %) needed further chemotherapy. Nine (82 %) were cured with single-agent chemotherapy and 2 patients (18 %) needed multi-agent chemotherapy. Of the 37 patients, patients with hCG levels below 5000 IU/L undergoing a second curettage were cured without chemotherapy in 65 % versus 45 % of patients with hCG level more than 5000 IU/L. Of the ten patients with a hCG level below 1000 IU/L, eight were cured without chemotherapy. CONCLUSIONS: Patients with post-mole gestational trophoblastic neoplasia can benefit from a second curettage to avoid chemotherapy, especially when the hCG level is lower than 5000 IU/L
