Rate adaptation for wireless video streaming based on error statistics
This paper presents a new rate-control algorithm for live video streaming over wireless IP networks, which is based on selective frame discarding. In the proposed mechanism excess 'P' frames are dropped from the output queue at the sender using a congestion estimate based on packet loss statistics obtained from RTCP feedback and from the Data Link (DL) layer. The performance of the algorithm is evaluated through computer simulation. This paper also presents a characterisation of packet losses owing to transmission errors and congestion, which can help in choosing appropriate strategies to maximise the video quality experienced by the end user. Lymphopenia in Patients Submitted to Assisted Ventilation Due to Aggravated Chronic Respiratory Failure: a Prospective Study
Objectivo: avaliar e caracterizar a linfopenia em doentes admitidos numa unidade de cuidados intensivos para suporte ventilatório por exacerbação de insuficiência respiratória crónica e eventual relação com a gravidade da doença.
Material e métodos: estudo prospectivo com 6 meses de duração e mais 6 meses de seguimento após
alta da unidade. Incluídos 24 doentes, 22 homens, com APACHE II médio de 19,7, 3 dos quais com
possibilidade de seguimento após a alta. Foram colhidas análises para determinação das subpopulações linfocitárias na admissão e a cada 7 dias de ventilação mecânica. Excluídos doentes com sinais de infecção ou imunossupressão prévia, à excepção dos corticóides.
Resultados: a linfopenia foi encontrada em 79,2 % dos doentes com depleção de todas as subpopulações linfocitárias sendo mais expressiva a depleção de linfócitos B CD19+. Esta linfopenia não se relacionou com os níveis séricos de cortisol, e apesar de se relacionar com uma maior gravidade clínica não esteve associada a uma maior mortalidade. O registo evolutivo no internamento mostrou tendencialmente uma recuperação da linfopenia.
Conclusões: a linfopenia é frequente em doentes ventilados por exacerbação de doença respiratória
crónica. Trata-se de uma linfopenia não selectiva, que recupera ao longo do internamento, mais acentuada ao nível dos linfócitos B CD19+. Estes doentes apresentam índices de gravidade maior mas sem diferenças na mortalidade. O seguimento ambulatório destes doentes mostrou-se difícil e foi inconclusivo

ERAS® protocol improves survival after radical cystectomy: A single-center cohort study.
To evaluate Enhanced recovery after surgery (ERAS®) protocol on oncological outcomes for patients treated with radical cystectomy (RC) for urothelial carcinoma of the bladder (UCB).
A prospectively maintained single-institutional database comprising 160 consecutive UCB patients who underwent open RC from 2012 to 2020 was analyzed. Patients receiving chemotherapy and those with a urinary diversion other than ileal conduit were excluded. Patients were divided into two groups according to the perioperative management (ERAS® and pre-ERAS®). The study aimed to evaluate the impact of the ERAS® protocol on survival at five years after surgery using a Kaplan-Meier log-rank test. A multivariable Cox proportional hazards model was used to identify prognostic factors for cancer-specific (CSS) and overall survival (OS).
Of the 107 patients considered for the final analysis, 74 (69%) were included in the ERAS® group. Median follow-up for patients alive at last follow-up was 28 months (interquartile range [IQR] 12-48). Five-years CSS rate was 74% for ERAS® patients, compared to 48% for the control population (P = 0.02), while 5-years OS was 31% higher in the ERAS® (67% vs. 36%, P = .003). In the multivariable analysis, ERAS® protocol and tumor stage were independent factors of CSS, while ERAS®, tumor stage so as total blood loss were independent factors for OS.
A dedicated ERAS® protocol for UCB patients treated with RC has a significant impact on survival. Reduction of stress after a major surgery and its potential improvement of perioperative patient's immunity may explain these data
Robustness of Cellular Automata in the Light of Asynchronous Information Transmission
International audienceCellular automata are classically synchronous: all cells are simultaneously updated. However, it has been proved that perturbations in the updating scheme may induce qualitative changes of behaviours. This paper presents a new type of asynchronism, the beta -synchronism, where cells still update at each time step but where the transmission of information between cells is disrupted randomly. We experimentally study the behaviour of beta-synchronous models. We observe that, although many eff ects are similar to the perturbation of the update, novel phenomena occur. Collagen matricryptin promotes cardiac function by mediating scar formation

A peptide mimetic of a collagen-derived matricryptin (p1159) was shown to reduce left ventricular (LV) dilation and fibrosis after 7 days delivery in a mouse model of myocardial infarction (MI). This suggested p1159 long-term treatment post-MI could have beneficial effects and reduce/prevent adverse LV remodeling. This study aimed to test the potential of p1159 to reduce adverse cardiac remodeling in a chronic MI model and to elucidate p1159 mode-of-action.
Materials and methods
Using a permanent occlusion MI rodent model, animals received p1159 or vehicle solution up to 28 days. We assessed peptide treatment effects on scar composition and structure and on systolic function. To assess peptide effects on scar vascularization, a cohort of mice were injected with Griffonia simplicifolia isolectin-B4. To investigate p1159 mode-of-action, LV fibroblasts from naïve animals were treated with increasing doses of p1159.
Key findings
Matricryptin p1159 significantly improved systolic function post-MI (2-fold greater EF compared to controls) by reducing left ventricular dilation and inducing the formation of a compliant and organized infarct scar, which promoted LV contractility and preserved the structural integrity of the heart. Specifically, infarcted scars from p1159-treated animals displayed collagen fibers aligned parallel to the epicardium, to resist circumferential stretching, with reduced levels of cross-linking, and improved tissue perfusion. In addition, we found that p1159 increases cardiac fibroblast migration by activating RhoA pathways via the membrane receptor integrin α4.
Significance
Our data indicate p1159 treatment reduced adverse LV remodeling post-MI by modulating the deposition, arrangement, and perfusion of the fibrotic scar.ECU Open Access Publishing Support Fun

Actinobacillus Endocarditis Associated with Hypertrophic Cardiomyopathy
Infective endocarditis can be associated with complex clinical presentations, sometimes with a difficult multi-disciplinary management. Actinobacillus actinomycetemcomitans belongs to the Haemophilus species, Actinomycetemcomitans, Cardiobacterium hominis, Eikenella corrodens and Kingella species group, responsible for 5% to 10% of infective endocarditis in native heart valves. These organisms have slow fastidious growth pattern, often associated with negative cultures, and cause systemic embolism with abscess formation. The authors present the case of a 59-year-old man, admitted due to fever of unknown origin, with a personal history of obstructive hypertrophic cardiomyopathy and recent dental manipulation. The diagnosis of mitral valve's endocarditis was established after a transoesophageal ecocardiography, with a late isolation of A actinomycetemcomitans in blood culture. Despite the institution of antibiotic therapy, the patient suffered from multiple episodes of septic embolism: skin, mucosae, cerebral abscesses, spondylodiscitis and uveitis.
