Twenty-Five Years of Convoluted Health Reforms in Mexico
Núria Homedes is with UT Houston, Antonio Ugalde is with UT Austin.Mexico's neoliberal health care reforms began in 1983 as a condition for Mexico to receive loans from the World Bank (WB) and International Monetary Fund (IMF), which were needed because of the 1980s world recession. The first reform (1983) was a failed attempt to decentralize the Ministry of Health (MoH) by transferring financial responsibilities without devolving adequate decision-making authority to the states. The second reform (1994) advanced the decentralization of the MoH and attempted to increase the exposure of the major public social security scheme to private sector competition. In 2003, a third reform, the Seguro Popular (SP), emphasized improved access and services for the poor. Although accessibility has increased, the Mexican reforms have not resulted in significant reductions of health inequities, or in increased efficiency, productivity, or quality, despite their costs.Sociolog
From Scientists to Merchants: The Transformation of the Pharmaceutical Industry and its Impact on Health
The number of innovative drugs reaching the market has decreased steadily during the last several years to a handful per year. At the same time, the amount of resources allocated by the pharmaceutical industries to promotion and marketing has increased at a faster pace than those allocated to research and development of new products. The paper presents the hypothesis that for the large corporations, the production of me-too drugs is more profitable than to invest in research and development of innovative products. Gaining a market share of me-too drugs requires large investments in promotion and marketing, one result of which is a division of labor among pharmaceutical firms. Because small firms lack the large resources needed for promotion and marketing, they carry out an increasing share of the research and development and sell the patents to the large corporations
Desigualdad en el acceso a las vacunas: el fracaso de la respuesta mundial a la pandemia de COVID-19
This article summarizes the strategies used to rapidly develop COVID-19 vaccines and distribute them globally, with an emphasis on vaccines developed in western nations. It is based on interviews and information gathered regarding the response to the pandemic, both from international organizations and official documents from Brazil, Argentina, Colombia, Peru, and Mexico. While vaccine development has been hailed as successful, their global distribution has been highly unequal. We look at how the pandemic succeeded in mobilizing large quantities of government resources, and how citizens volunteered their bodies so that clinical trials could be completed quickly. However, patents prevented the expansion of manufacturing capacity, and the governments of a few wealthy countries prioritized the protection – and in some cases overprotection – of their citizens at the expense of protecting the rest of world’s population. Among the major beneficiaries of the global response to the pandemic are the leading vaccine companies, their executives, and investors. The article concludes with some of the lessons learned in this process.Este artÃculo resume las estrategias que se han utilizado para desarrollar rápidamente las vacunas COVID-19 y distribuirlas a nivel mundial. Se centra en las vacunas desarrolladas en los paÃses occidentales. Con base en entrevistas y recopilación de información existente sobre la respuesta a la pandemia, tanto de agencias internacionales como de documentos oficiales de Brasil, Argentina, Colombia, Perú y México se reconoce que, si bien el desarrollo de las vacunas ha sido un éxito, su distribución a nivel mundial ha sido muy desigual. Como veremos, la pandemia consiguió movilizar una gran cantidad de recursos gubernamentales y los ciudadanos prestaron sus cuerpos para que los ensayos clÃnicos se pudieran concluir rápidamente. Sin embargo, las patentes impidieron la expansión de la capacidad de fabricación y los gobiernos de unos pocos paÃses ricos priorizaron la protección y, en algunos casos, la sobreprotección de sus ciudadanos a expensas de la protección del resto de la población mundial. Entre los principales beneficiarios de la respuesta mundial a la pandemia se encuentran las principales empresas de vacunas, sus ejecutivos e inversores. La descentralización de los servicios de salud: de la teorÃa a la práctica
The current promoters of the decentralization of health services affirm that its implementation will improve the quality, equity and efficiency of health systems and at the same time will increase users' satisfaction. Additionally, those who advocate decentralization believe that it will facilitate community participation in health decisionmaking, and as a result it will strengthen the democratic process. In this study we discuss first the lack of consensus regarding the meaning of decentralization and the difficulties of measuring the degree of decentralization of a health system. We also identify the actors who have formulated the above hypotheses, examine the reasons behind its formulation, and analyze if the outcome of the decentralization of the Latin American health services confirm the hypotheses.Los actuales promotores de la descentralización de los servicios de salud afirman que su implementación mejorará la calidad, la equidad y la eficiencia de los sistemas de salud, y al mismo tiempo incrementará la satisfacción de los usuarios. Además, quienes abogan por la descentralización piensan que la descentralización facilitará la participación de las comunidades en la toma de decisiones relacionadas con la salud, y en consecuencia la democracia. En este trabajo se discute primero la falta de consenso que existe sobre el significado del concepto de descentralización y la dificultad de medir el nivel de descentralización de un sistema de salud. También se identifica a los actores que formularon estas hipótesis, se examinan las razones detrás de su formulación, y si los resultados alcanzados tras la descentralización de los servicios de salud en América Latina las confirman
El impacto de los investigadores fieles a la industria farmacéutica en la ética y la calidad de los ensayos clÃnicos realizados en Latinoamérica
This article explains the difficulties innovative pharmaceutical firms have in repaying shareholders with attractive dividends. The problem is the result of the expiration of the patents of blockbuster drugs and the difficulties that the firms have in bringing new blockbuster drugs to the market. One of the solutions companies have found has been to accelerate the implementation of clinical trials in order to expedite the commercialization of new drugs. Doing so increases the period in which they can sell drugs at monopoly prices. We therefore discuss how innovative pharmaceutical firms shorten the implementation time of clinical trials in Latin America and the consequences such actions have on the quality of the collected data, the protection of human rights of the subjects of experimentation, and compliance with the ethical principles approved in international declarations.Este artÃculo explica las dificultades que tienen las farmacéuticas innovadoras para retribuir a sus accionistas con dividendos atractivos. El problema es el resultado de la caducidad de las patentes de los medicamentos estrella (blockbusters) y las dificultades que tienen en desarrollar nuevos medicamentos estrella. Una solución que las empresas han encontrado es acelerar la ejecución de los ensayos clÃnicos para obtener, en el menor tiempo posible, el permiso de comercialización y asà incrementar el tiempo monopólico de ventas de los nuevos medicamentos. En este contexto, los autores describen la forma en que las farmacéuticas innovadoras acortan el tiempo de ejecución de los ensayos en América Latina y las consecuencias en la calidad de los datos que se obtienen, en la protección de los derechos humanos de los sujetos de experimentación, y en el cumplimiento de los principios éticos aprobados en las declaraciones universales

This article discusses the limitations of clinical trials in determining the safety and efficacy of therapeutic drugs in Latin America. A major limitation is the lack of transparency surrounding the implementation of clinical trials. The data gathered by research ethics committees, regulatory agencies and the pharmaceutical industry is inaccessible to outside parties; this secrecy is not explained by the need to protect industrial secrets but rather by the industry's need to commercialize as quickly as possible the drugs under experimentation. The covering up of ethical violations, errors, and even fraud is a tacit condition imposed by the industry in order to continue future clinical trials. The governments of the region have accepted the industry’s rationalization that clinical trials transfer scientific knowledge, benefit participants, and increase the flow of foreign capital coming in to the country, and in addition the results contribute to improving health in all nations. Based on evidence gathered from several Latin American countries and from independent researchers in other parts of the world, the authors refute the industry's arguments.En este artÃculo se presentan las limitaciones de los ensayos clÃnicos para determinar la seguridad y eficacia de los medicamentos en América Latina. Una de ellas es la falta de transparencia que rodea la implementación de los ensayos clÃnicos. No hay acceso a la información que obtienen los comités de ética en investigación, las agencias reguladoras y las empresas farmacéuticas, y el secretismo no responde a la necesidad de proteger los secretos industriales sino a permitir que los medicamentos en experimentación puedan comercializarse lo antes posible. El encubrimiento de las violaciones éticas, errores y en algunos casos fraudes es una condición tácita que imponen las empresas para que se sigan haciendo ensayos. Los gobiernos han aceptado la racionalización de la industria de que los ensayos clÃnicos transfieren conocimiento cientÃfico, benefician a los participantes, aportan divisas a la economÃa del paÃs y sus descubrimientos contribuyen a mejorar la salud de todas las naciones.
