A standardized formula for aesthetic mandibular reconstruction using an osteocutaneous fibular free flap
Ameloblastoma is the most common benign odontogenic tumor of the jaw, and expansional growth of a huge untreatedĀ ameloblastoma can result in disturbances in facial aesthetics and function, such as difficulty with mouthĀ opening, swallowing, chewing, breathing, neurologic deficits, and pathologic fractures. Radical wide resection withĀ safety margins and subsequent reconstruction is generally recommended. A fibular free flap (FFF) is commonlyused to reconstruct the mandible in order to adequately restore both aesthetic appearance and function. The aim ofĀ this brief clinical report is to present a case of huge ameloblastoma after wide resection with free safety margins, andĀ describe the immediate one-step mandibular reconstruction using a vascularized composite FFF. The sterolithographic(rapid prototype, RP) model, a wax pattern of the resected mandible, and a surgical fibular stent made fromĀ the wax pattern were constructed preoperatively. The Expression Patterns of FAM83H and PANX2 Are Associated With Shorter Survival of Clear Cell Renal Cell Carcinoma Patients
FAM83H is primarily known for its role in amelogenesis; however, recent reports suggest FAM83H might be involved in tumorigenesis. Although the studies of FAM83H in kidney cancer are limited, a search of the public database shows a significant association between FAM83H and pannexin-2 (PANX2) in clear cell renal cell carcinomas (CCRCCs). Therefore, we evaluated the clinicopathological significance of the immunohistochemical expression of FAM83H and PANX2 in 199 CCRCC patients. The expression of FAM83H and PANX2 were significantly associated with each other. In univariate analysis, individual, and co-expression pattern of FAM83H and PANX2 was significantly associated with shorter overall survival (OS) and relapse-free survival (RFS) of CCRCC patients: nuclear expression of FAM83H (OS; P < 0.001, RFS; P < 0.001), cytoplasmic expression of FAM83H (OS; P < 0.001, RFS; P < 0.001), nuclear expression of PANX2 (OS; P < 0.001, RFS; P < 0.001), cytoplasmic expression of PANX2 (OS; P < 0.001, RFS; P < 0.001), co-expression pattern of nuclear FAM83H and nuclear PANX2 (OS; P < 0.001, RFS; P < 0.001). In multivariate analysis, nuclear expression of FAM83H (OS; P < 0.001, RFS; P = 0.003) and the co-expression pattern of nuclear FAM83H and PANX2 (OS; P < 0.001, RFS; P < 0.001) were independent indicators of shorter survival of CCRCC patients. Cytoplasmic expression of FAM83H was associated with shorter RFS (P = 0.030) in multivariate analysis. In Caki-1 and Caki-2 CCRCC cells, knock-down of FAM83H decreased PANX2 expression and cell proliferation, and overexpression of FAM83H increased PANX2 expression and cell proliferation. These results suggest that FAM83H and PANX2 might be involved in the progression of CCRCC in a co-operative manner, and their expression might be used as novel prognostic indicators for CCRCC patients
An Outbreak of Trichinellosis with Detection of Trichinella Larvae in Leftover Wild Boar Meat
The clinical diagnosis of trichinellosis can be difficult due to lack of pathognomonic signs or symptoms. In Korea, since the first report of human infection by Trichinella spiralis in 1997 following the consumption of raw badger meat, there have been occasional trichinellosis outbreaks. We describe an outbreak of 12 cases of trichinellosis in Korea and implicate raw wild boar meat as the culprit. A total of 27 larvae of Trichinella (0.54 larvae per gram of meat) were recovered from the leftover raw wild boar meat
Subconjuctival Loa loa with Calabar Swelling
Loa loa is unique among the human filariae in that adult worms are occasionally visible during subconjuntival migration. A 29-yr-old African female student, living in Korea for the past 5 yr without ever visiting her home country, presented with acute eyelid swelling and a sensation of motion on the left eyeball. Her symptoms started one day earlier and became worse over time. Examination revealed a threadlike worm beneath the left upper bulbar conjunctiva with mild eyelid swelling as well as painless swelling of the right forearm. Upon exposure to slit-lamp illumination, a sudden movement of the worm toward the fornix was noted. After surgical extraction, parasitologic analysis confirmed the worm to be a female adult Loa loa with the vulva at the extreme anterior end. On blood smear, the microfilariae had characteristic features of Loa loa, including sheath and body nuclei up to the tip of the tail. The patient also showed eosinophilia (37%) measuring 4,100/ĀµL. She took ivermectin (200 Āµg/kg) as a single dose and suffered from a mild fever and chills for one day. This patient, to the best of our knowledge, is the first case of subconjunctival loiasis with Calabar swelling in Korea
Controllable synthesis of molybdenum tungsten disulfide alloy for vertically composition-controlled multilayer
The effective synthesis of two-dimensional transition metal dichalcogenides alloy is essential for successful application in electronic and optical devices based on a tunable band gap. Here we show a synthesis process for Mo<inf>1-x</inf>W<inf>x</inf>S<inf>2</inf> alloy using sulfurization of super-cycle atomic layer deposition Mo<inf>1-x</inf>W<inf>x</inf>O<inf>y</inf>. Various spectroscopic and microscopic results indicate that the synthesized Mo<inf>1-x</inf>W<inf>x</inf>S<inf>2</inf> alloys have complete mixing of Mo and Watoms and tunable band gap by systematically controlled composition and layer number. Based on this, we synthesize a vertically composition-controlled (VCC) Mo<inf>1-x</inf>W<inf>x</inf>S<inf>2</inf> multilayer using five continuous super-cycles with different cycle ratios for each super-cycle. Angle-resolved X-ray photoemission spectroscopy, Raman and ultraviolet-visible spectrophotometer results reveal that a VCC Mo<inf>1-x</inf>W<inf>x</inf>S<inf>2</inf> multilayer has different vertical composition and broadband light absorption with strong interlayer coupling within a VCC Mo<inf>1-x</inf>W<inf>x</inf>S<inf>2</inf> multilayer. Further, we demonstrate that a VCC Mo<inf>1-x</inf>W<inf>x</inf>S<inf>2</inf> multilayer photodetector generates three to four times greater photocurrent than MoS<inf>2</inf>-and WS<inf>2</inf>-based devices, owing to the broadband light absorption. &#169; 2015 Macmillan Publishers Limitedopen1
