    Self-medication with antibiotics: questionnaire survey among primary care center attendants

    Purpose The aim of this study was to determine the frequency and reasons for self-medication with antibiotics within the last 12 months among primary healthcare center attendants aged 18 or over, in Ankara. Methods A questionnaire was applied by face to face interview technique to 2696 subjects attending at five primary healthcare centers. Information on demographic characteristics, intention to self-medication with antibiotics and self-medication within last 12 months were collected. Results It was found that 28% of the subjects were storing antibiotics at home. The percentage of self-administering antibiotics was 19.1% in the last 12 months. The most common reasons for self-administration of antibiotics were sore throat (59.6%), fever (46.2%), and cough (40.0%). Other reasons were dental infection, rheumatism, and fatigue. According to age groups, the most common self-medicating group was those aged 40–49 with 23% while the least self-usage was in the 60–69 age groups with 11.8%. Male sex, being single, educational level of secondary school or higher, being employed and not having social security increased self-administration of antibiotics ( p<0.05). Conclusions The study indicated the need for legal regulations regarding the sale of antibiotics without prescription and, health education is required for the community to decrease the inappropriate use of antibiotics and self-medication

    CT-guided transthoracic biopsy: histopathologic results and complication rates

    PURPOSEWe aimed to investigate the effectiveness and complications of transthoracic CT-guided biopsy techniques.METHODSA total of 94 CT-guided percutaneous transthoracic biopsy procedures performed in 85 patients were retrospectively evaluated. Core biopsy technique was used in 87 procedures and transthoracic fine-needle aspiration biopsy was used in seven procedures.RESULTSDiagnostic results were achieved in 79 of 94 biopsy procedures. Pathology results were malignant in 54 patients, suspicious for malignancy in three patients, benign in five patients, and benign nonspecific in 17 patients. Specific diagnoses were obtained in 59 patients (62.8%) using core biopsy, but no specific diagnosis could be reached with transthoracic fine-needle aspiration biopsy. Complications included pneumothorax in 27 patients (28.7%) and parenchymal hemorrhage during and after the procedure in eight patients (8.5%). CONCLUSIONSCT-guided percutaneous transthoracic needle biopsy is a highly accurate procedure for histopathological diagnosis of thoracic masses. In addition, percutaneous transthoracic biopsy has an acceptably low complication rate and it reduces the need for more invasive surgical procedures

    Postpartum Depression Frequency and Quality of Life Among a Group of Mothers Having a Child Aged 2 Weeks-18 Months

    Objective: The aim was to determine the frequency of postpartum depression (PPD), its correlates and the effect on the quality of life.Material and Methods: This study was conducted among 708 mothers having a child aged 2 weeks-18 months. A questionnaire on descriptive features and PPD risk factors; Edinburgh Postpartum Depression Scale and WHOQOL-BREF quality of life scale were used for the data collection. Results: The rate of PPD frequency was 15%. The gestational age, mental problems and anxiety during pregnancy, emotional changes in the premenstrual period, past depression/PPD history, depression/PPD history in the family, satisfaction with the marriage, thinking that the baby affected the marriage adversely were the factors related with PPD. The quality of life was lower in women with PPD than those without PPD.Conclusion: The frequency of PPD is quite high and PPD is decreasing the quality of life of the mother

    Aim: The present study aimed to evaluate systolic and diastolic myocardial function in children and adolescents with congenital adrenal hyperplasia. Methods: The study included 44 children with the diagnosis of classic congenital adrenal hyperplasia and 39 healthy children whose age, pubertal status, and gender were similar to those of the patient group. Anthropometric parameters and 17-hydroxyprogesterone levels were measured, and bone age was calculated. The average daily hydrocortisone dose was calculated over the last 1-year file records. Hyperandrogenic state was defined according to bone age SD score (>= 2) and 17-hydroxyprogesterone levels (>10 ng/ml). Echocardiographic examinations were assessed by conventional two-dimensional Doppler echocardiography and tissue Doppler imaging. Results: Patients had higher morphological parameters, such as left ventricular end-systolic diameter, interventricular septal thickness at end diastole, left ventricular posterior wall thickness at end diastole, left ventricular mass and index, than the control group (p < 0.05). On pulsed-wave and tissue Doppler echocardiography, significant subclinical alterations were observed in systolic (isovolumic contraction time), diastolic (isovolumic relaxation time), and global left ventricular functional (myocardial performance index) parameters in the congenital adrenal hyperplasia group compared to the control group (p <0.05). In partial correlation analyses, after controlling the effect of hyperandrogenism, the mean hydrocortisone dosage was positively correlated with isovolumic relaxation time in congenital adrenal hyperplasia group (p < 0.05). Conclusion: This study demonstrated that the patients with congenital adrenal hyperplasia are at risk for left ventricular hypertrophy, systolic and diastolic myocardial subclinical alterations. Overtreatment may be responsible for the increased risk of myocardial dysfunction in patients with congenital adrenal hyperplasia
