    Anti-Inflammatory Activity of (Polyphenolic)-Sulfonates and Their Sodium Salts in Rodents

    A series of polyphenolic-sulfonated compounds were observed to have potent anti-inflammatory activity and were protective against induced endotoxic shock in mice at 8 and 16 mg/kg, I.P. These agents proved to be potent elastase inhibitors in human leukocytes and J774-AI and IC-21 mouse macrophages as well as prostaglandin cyclo-oxygenase inhibitors in J774-AI macrophages. The compounds from 5 to 50 ΌM inhibited TNFα release from IC-21 macrophages and IL-1 release from mouse P388D1 macrophages induced by LPS. The binding of these cytokines to high affinity receptors on target cells, e.g. L929 fibroblasts and IL-2 in HuT78 T lymphoma cells, were also suppressed by the agents. These compounds blocked the adhesion of leukocytes and macrophages to the plasma membranes of L929 fibroblasts

    Faculty-Led Virtual Level 1 Community Fieldwork during the COVID-19 Pandemic

    Fieldwork is an integral portion of occupational therapy education that ensures students have the opportunity to develop basic competencies in real world practice settings. The national shortage of fieldwork placements, particularly in the area of mental health, in combination with the COVID-19 pandemic, have led to the adoption of increasingly innovative fieldwork models. This retrospective, qualitative study investigates occupational therapy assistant students’ experiences of completing a faculty-led (i.e. where faculty served as the primary fieldwork educator) and virtual (i.e., where services were offered in a virtual environment) Level I fieldwork with a community-based peer led behavioral health agency. Twenty-three students completed a confidential survey describing their experiences in Fall 2020. A secondary analysis of students’ responses was performed using principles of thematic analysis, which yielded results centered on four themes: knowledge, skills, attitudes, and structure. Subcategories highlighted growth across multiple areas including knowledge of occupational therapy’s role in mental health, interpersonal skills, and use of technology and other resources. Students’ preconceived notions of individuals with mental illness were challenged and many reported increased confidence in their abilities to work with these individuals. Both positive and constructive feedback were provided regarding the overall virtual fieldwork experience. The faculty-led virtual fieldwork model was viable in supporting occupational therapy assistant students’ skills to engage people with mental health and substance use challenges in a community setting. The potential use of this model is discussed in light of the anticipated increase of behavioral health problems for many across the lifespan post-COVID-19 pandemic

    Does Gender Impact Intensity of Care Provided to Older Medical Intensive Care Unit Patients?

    Introduction. Women receive less aggressive critical care than men based on prior studies. No documented studies evaluate whether men and women are treated equally in the medical intensive care unit (MICU). The Therapeutic Intervention Scoring System-28 (TISS-28) has been used to examine gender differences in mixed ICU studies. However, it has not been used to evaluate equivalence of care in older MICU patients. We hypothesize that given nonsignificant, baseline health differences between genders at MICU admission, the level of care provided would be equivalent. Methods. Prospective cohort of 309 patients ≄60 years old in the MICU of an urban university teaching hospital. Explanatory variables were demographic data and baseline measures. Primary outcomes were TISS-28 scores and MICU interventions. We compare TISS-28 scores by gender using a statistical test of equivalence. Results. Women were older and had more chronic respiratory failure at MICU admission. Using equivalence limits of ±15% on gender-based scores of TISS-28, MICU interventions were equivalent. Supplementary analysis showed no statistically significant association between gender and mortality. Conclusions. In contrast with other reports from the cardiac critical care literature, as measured by the TISS-28, gender-based care delivered to older MICU patients in this cohort was equivalent

Frontiers in Marine Science, 8, (2021): 669530, https://doi.org/10.3389./fmars.2021.669530Scientists have long known that small-scale interactions of aquatic particles, plankton, and other organisms with their immediate environment play an important role in diverse research areas, including marine ecology, ocean optics, and climate change (Guasto et al., 2012; Prairie et al., 2012). Typically, the distribution of particles and other organisms in the water column tends to be quite “patchy,” i.e., non-homogeneous, both spatially and temporally (Durham and Stocker, 2012). Patchiness can manifest itself through well-known phenomena such as harmful algal blooms (HABs), phytoplankton and zooplankton “thin layers,” deep scattering layers, and schooling of marine organisms such as krill and fish. This non-homogeneous distribution can significantly influence predator-prey encounters and outcomes, export fluxes, marine ecosystem health, and biological productivity (Sullivan et al., 2010; Durham et al., 2013). Thus, there is a continuing need to study and characterize the small-scale biological-physical interactions between particles/organisms and their local environment, as well as the scaled-up effects of these small-scale interactions on larger-scale dynamics. These studies are also directly linked to broader research topics listed as part of the future “grand challenges” in marine ecosystem ecology, as outlined in Borja et al.     In this article we apply an Afrocentric Resilience Theory (relationship-resourced resilience) to the domain of education research partnerships. We posit academic flocking as an equitable research partnership approach aimed at developing education knowledge that responds to collective distress and supports collective quality education. We provide support for our supposition regarding academic flocking by overviewing the mutually beneficial development of an online, home-based learning resource with relevance in two transnational contexts and cultures, South Africa and the United States of America. Whereas the context of the argument is a COVID-19 related global need for evidence-based education resources, conceptually we draw on lenses of resilience and emancipatory, democratising methodology to make sense of academic flocking as a fundamental structure for research partnership equity and relevant education innovation. As such, academic flocking holds value as a transformative alternative for South-North researcher partnerships in generating useful, quality educational innovations to address critical needs.In diesem Beitrag wenden wir eine afrozentrische Resilienztheorie (beziehungsgestĂŒtzte Resilienz) auf einen Ansatz der Bildungsforschungspartnerschaften an. Wir stĂŒtzen uns auf die Annahme, dass „akademisches Flocking“ (Schwarmverhalten) eine Grundlage fĂŒr gleichberechtigte Forschungspartnerschaften bildet. Der Ansatz zielt darauf ab, Wissen ĂŒber Bildung zu generieren, das auf kollektive Notlagen reagiert und kollektive Prozesse der QualitĂ€tsbildung unterstĂŒtzt. Wir untermauern unsere Annahme ĂŒber „akademisches Flocking“, indem wir einen Überblick ĂŒber die fĂŒr beide Seiten vorteilhafte Entwicklung einer Online-Lernressource geben, die zu Hause eingesetzt werden kann. Dieser Ansatz wurde in zwei transnationalen Kontexten und Kulturen – SĂŒdafrika und den Vereinigten Staaten von Amerika – erprobt. Kontext der Argumentation ist der von der COVID-19-Pandemie verstĂ€rkt sichtbar gewordene globale Bedarf an der Bereitstellung evidenzbasierter Bildungsressourcen.     How Communication Failed or Saved the Day : Counterfactual Accounts of Medical Errors

    Communication breakdowns among clinicians, patients, and family members can lead to medical errors, yet effective communication may prevent such mistakes. This investigation examined patients\u27 and family members\u27 experiences where they believed communication failures contributed to medical errors or where effective communication prevented a medical error ( close calls ). The study conducted a thematic analysis of open-ended responses to an online survey of patients\u27 and family members\u27 past experiences with medical errors or close calls. Of the 93 respondents, 56 (60%) provided stories of medical errors, and the remaining described close calls. Two predominant themes emerged in medical error stories that were attributed to health care providers-information inadequacy (eg, delayed, inaccurate) and not listening to or being dismissive of a patient\u27s or family member\u27s concerns. In stories of close calls, a patient\u27s or family member\u27s proactive communication (eg, being assertive, persistent) most often saved the day. The findings highlight the importance of encouraging active patient/family involvement in a patient\u27s medical care to prevent errors and of improving systems to provide meaningful information in a timely manner

    The Effects of Amine-Carboxyborane Related Derivatives on UMR-106 Bone Metabolism

    The amine-carboxyboranes and related derivatives have been shown to be potent anti-inflammatory and anti-osteoporosis agents. Their action in part appears to be mediated by the modulation of cytokines, e.g. TNFα or IL-1. Previous studies have demonstrated that LPS induced macrophages release of TNFα maximally at 60 to 90 min. and IL-1 from 5 to 8 hr. The amine-carboxyboranes reduced significantly the release of these cytokines but also blocked TNFα high affinity binding to UMR-106 receptor at 90 min. at 10 ÎŒM, and IL-1 high affinity binding at 5 hr. at 12.5 ÎŒM. In addition, the agents suppressed IL-8 binding to CHO K1 high affinity receptor at 24 hr. at 50 ÎŒM and IL-2 binding to HuT-8 receptors at 25 ÎŒM at 90 min. and 5 hr. Correlation of metabolic events associated with osteoporosis showed that at 90 min., when TNFα receptor binding was reduced by the agents, calcium uptake into UMR-106 cells was reduced at 10 ÎŒM as well as the acid and alkaline phosphatases, and the prostaglandin cyclo-oxygenase activities and adhesion of leukocytes and macrophages to UMR-106 cell monolayers. At 5hr. when the agents reduced IL-1 binding to UMR-106 receptors, calcitonin and 1,25-dihydrovitamin D3 binding was reduced by the agents as was acid and alkaline phosphatase, and 5â€Č-lipoxygenase activities and white blood cell adhesion. At this time calcium uptake and proline incorporation was increased significantly by the agents. At later times e.g. 18-48 hr. calcium uptake was still increased, and NAG activity was inhibited in the presence of the agents. These effects may be related more to the inhibition of other cytokine receptor binding, e.g. IL-8. Thus, many of the observed metabolic effects of amine-carboxyboranes as antiosteoporosis agents can be correlated with their inhibition of cytokine high affinity binding to target cell receptors

    A Randomized, Double-Blinded, Phase II Trial of Gemcitabine and Nab-Paclitaxel Plus Apatorsen or Placebo in Patients with Metastatic Pancreatic Cancer: The RAINIER Trial.

    Lessons learnedThe addition of the heat shock protein 27 (Hsp27)-targeting antisense oligonucleotide, apatorsen, to a standard first-line chemotherapy regimen did not result in improved survival in unselected patients with metastatic pancreatic cancer.Findings from this trial hint at the possible prognostic and predictive value of serum Hsp27 that may warrant further investigation.BackgroundThis randomized, double-blinded, phase II trial evaluated the efficacy of gemcitabine/nab-paclitaxel plus either apatorsen, an antisense oligonucleotide targeting heat shock protein 27 (Hsp27) mRNA, or placebo in patients with metastatic pancreatic cancer.MethodsPatients were randomized 1:1 to Arm A (gemcitabine/nab-paclitaxel plus apatorsen) or Arm B (gemcitabine/nab-paclitaxel plus placebo). Treatment was administered in 28-day cycles, with restaging every 2 cycles, until progression or intolerable toxicity. Serum Hsp27 levels were analyzed at baseline and on treatment. The primary endpoint was overall survival (OS).ResultsOne hundred thirty-two patients were enrolled, 66 per arm. Cytopenias and fatigue were the most frequent grade 3/4 treatment-related adverse events for both arms. Median progression-free survival (PFS) and OS were 2.7 and 5.3 months, respectively, for arm A, and 3.8 and 6.9 months, respectively, for arm B. Objective response rate was 18% for both arms. Patients with high serum level of Hsp27 represented a poor-prognosis subgroup who may have derived modest benefit from addition of apatorsen.ConclusionAddition of apatorsen to chemotherapy does not improve outcomes in unselected patients with metastatic pancreatic cancer in the first-line setting, although a trend toward prolonged PFS and OS in patients with high baseline serum Hsp27 suggests this therapy may warrant further evaluation in this subgroup
