    Preparing for an Aging Nation: The Need for Academic Geriatricians

    According to the author of this issue brief, a strong commitment by the government to a regular stream of financial support for geriatrics would encourage medical centers to establish or expand programs, enhance their ability to attract funds from private sources, and ultimately produce a sufficient number of academic geriatricians. Given the impending retirement of the baby boom generation, this process should begin as soon as possible. This issue brief highlights the critical need for academic geriatricians and presents a realistic proposal to help address the problem

    Quantum double of a (locally) compact group

    We generalise the quantum double construction of Drinfel'd to the case of the (Hopf) algebra of suitable functions on a compact or locally compact group. We will concentrate on the *-algebra structure of the quantum double. If the conjugacy classes in the group are countably separated, then we classify the irreducible *-representations by using the connection with so-called transformation group algebras. For finite groups, we will compare our description to the result of Dijkgraaf, Pasquier and Roche. Finally we will work out the explicit examples of SU(2) and SL(2,R).Comment: LaTeX2e, 18 pages.     L'heĢmineĢgligence (HN) est un trouble neuropsychologique qui indique une incapaciteĢ totale ou partielle de repeĢrer, inteĢgrer ou s'orienter vers des stimuli sensoriels preĢsents dans l'heĢmi-espace controlateĢral aĢ€ une leĢsion touchant le plus souvent l'heĢmispheĢ€re ceĢreĢbral droit. Chaque expression clinique de la pathologie peut eĢ‚tre analyseĢe selon le peĢrimeĢ€tre d'action deĢfinit par l'espace qui se trouve autour de l'eĢ‚tre humain et qui peut eĢ‚tre diviseĢ en trois zones: l'espace personnel, l'espace peĢripersonnel et l'espace extrapersonnel. Cette eĢtude a pour but d'eĢvaluer l'efficaciteĢ d'une seĢance d'adaptation prismatique (AP) sur la description d'objets ou d'images dans l'espace lointain et dans l'espace proche chez des patients preĢsentant une HN. L'AP consiste en un exercice de pointage effectueĢ par le patient alors qu'il porte des lunettes avec des prismes qui deĢvient le champ visuel vers le coĢ‚teĢ ipsilateĢral aĢ€ la leĢsion. Au cours de l'AP classique, le patient agit dans l'espace peĢripersonnel ("proche"). Aucune eĢtude n'a aĢ€ preĢsent deĢtermineĢ si l'AP est aussi efficace dans l'espace extrapersonnel ("lointain"). En comparant les performances d'un groupe de patients HN dans une taĢ‚che de description d'images, preĢsenteĢes dans l'espace proche et lointain, avant et apreĢ€s une seĢance d'AP aĢ€ celles d'un autre groupe de patient HN effectuant les meĢ‚mes descriptions mais sans seĢance d'AP entre les deux, nous avons pu deĢmontrer que l'ameĢlioration lors de la deuxieĢ€me passation de la taĢ‚che eĢtait plus importante pour le groupe ayant eu une seĢance d'AP. Cette ameĢlioration eĢtait d'autant plus importante lorsque les eĢleĢments aĢ€ deĢcrire se situaient dans l'espace gauche que dans l'espace droit. Finalement, des analyses post-hoc plus deĢtailleĢes ont montreĢ une ameĢlioration significative pour le groupe ayant eu une seĢance d'AP lorsque les eĢleĢments eĢtaient situeĢs dans l'espace lointain et du coĢ‚teĢ gauche uniquement. L'ensemble de ces reĢsultats montre que l'AP a un effet beĢneĢfique sur les deĢficits d'HN dans l'espace proche et dans l'espace lointain, cependant, le dernier reĢsultat montre que dans notre taĢ‚che l'effet beĢneĢfique de l'AP sur les deĢficits aĢ€ gauche est plus important dans l'espace lointain que dans l'espace proche. Ces reĢsultats ont des implications importantes en ce qui concerne la reĢhabilitation et la qualiteĢ de vie des patients preĢsentant une HN extrapersonnelle: la deĢmonstration d'un effet beĢneĢfique de l'AP sur l'HN dans l'espace lointain pourrait permettre l'eĢlaboration d'une strateĢgie de prise en charge plus cibleĢe et adapteĢe aux neĢcessiteĢs de chaque patient et ameĢliorer ainsi leur autonomie dans la vie de tous les jours

    Approaches to assessment in time-limited Mentalization-Based Therapy for Children (MBT-C)

    In this article we describe our clinical approach to assessment, formulation and the identification of a therapeutic focus in the context of time-limited Mentalization-Based Treatment for Children (MBT-C) aged between 6 and 12. Rather than seeing the capacity to mentalize as a global construct, we set out an approach to assessing the developmental 'building blocks' of the capacity to mentalize the self and others, including the capacity for attention regulation, emotion regulation, and explicit mentalization. Assessing the child's strengths and vulnerabilities in each of these domains provides a more nuanced picture of the child's mentalizing capacities and difficulties, and can provide a useful approach to case formulation. The article sets out an approach to assessment that includes a consideration of mentalizing strengths and difficulties in both the child and the parents, and shows how this can be used to help develop a mutually agreed treatment focus.     ``Plug and play'' systems for quantum cryptography

    We present a time-multiplexed interferometer based on Faraday mirrors, and apply it to quantum key distribution. The interfering pulses follow exactly the same spatial path, ensuring very high stability and self balancing. Use of Faraday mirrors compensates automatically any birefringence effects and polarization dependent losses in the transmitting fiber.     The Clinical Challenge of Mentalization-based Therapy with Children Who are in "Pretend Mode"

    The ā€œpretend modeā€ is one of the so-called ā€œpre-mentalizing modes of thinkingā€, which were first introduced by Target and Fonagy over 20 years ago. In a series of papers about play and reality, ā€œpretend modeā€ thinking was understood as a mode of pre-mentalizing thinking which is typical in the early years, but which can reappear in a more problematic way in adults. Although the concept of pretend mode was first introduced in a developmental context, as a clinical term it has primarily been discussed in the context of adult or adolescent psychotherapy. This paper suggests that the pretend mode is a valuable clinical concept for therapists working with school-age children, but that its use in this context needs some clarification. After reviewing how pretend mode has been understood as a normal part of early development, this paper goes to demonstrate the various roles of pretend mode in clinical work with school-age children and sets out a number of clinical strategies that may be used in therapeutic work
