That students who have the drive to take responsibility and achievement of learning objectives, makes the learners to always be disciplined and focused in learning a subject. Therefore, students who have high motivation to learn, will get better learning outcomes compared to students with low learning motivation. Thus, if in improving learning outcomes of a subject, the need for efforts to increase student motivation. Motivation is realized in the form of action, is one factor that can be used to predict the progress of learning

Introduction: Blunt trauma is a rare cause of chylothorax. In this study an uncommon case of traumatic chylothorax after blunt trauma is described along with treatment options.   Report on a Case: A 50-year-old male was admitted to the hospital with sudden onset of shortness of breath for one day. He had a history of road traffic accident 2 days ago, in which he received a strong jerk while trying to avoid impact with the dashboard of the vehicle. He has no past medical history. On examination, He was fully conscious and oriented. Air entry on both sides of the chest was reduced. The x-ray of the chest revealed bilateral blunting of costo-phrenic angels due to pleural effusion. Pleural fluid was sent for biochemical, microbiological and histopathological analysis. Fluid Triglyceride and cholesterol levels were elevated. Culture and sensitivity were negative while cytopathological analysis revealed increased WBCs. CT scan of the chest did not reveal any malignancy or mediastinal lymph nodes.  Bilateral Chest intubation was done to treat severe shortness of breath, that drained milky white fluid. The patient was observed without any particular treatment other than chest intubation. The patient responded well and was discharged uneventfully after five days.  Conclusion: Despite its rarity, chylothorax can occur after blunt trauma. Diagnosis should be confirmed by laboratory testing. Conservative management with or without chest intubation is a common treatment option.  Keywords: Blunt chest trauma, Chylothorax, Triglyceride.  &nbsp

Economic Diversification and the Urban Image; Changing the Narrative on Street Vending

Street vending is a dynamic phenomenon of network of events, socio-economic and cultural factors while remaining a narration of place. At the metropolitan level, the narrative is negatively skewed towards street vending and its aesthetic reality, contemporaneously exploring hostile environmental interventions within the informal sector. This paper attempted to explore a counter-narrative asking; based on aesthetic experience, can the “desired” urban image be achieved by allowing street vendors proliferate in public spaces? This question was asked within the scope of the political-economy of diversification in Nigeria. Mapping over google satellite images over critical periods leading to demolitions and/or developments, this paper documented the spatial distribution of vendors to determine the urban centres that are hostile to vending activities and those that were not. The paper argued that, around public spaces such as parks and sidewalks, the precarious nature of vending activities lead to their diffidence in upgrades to stalls, tables and kiosks. With pictures from spaces that appear to approve of street vending tacitly, a pattern of upgrades in vending apparatus and kiosks were established. This paper proposes an integrative model of passive, active and tacit support that is required to influence the discourse of vending activities within the context of urban images produced in Nigerian. In conclusion and using sing Gouverneur (2014) concepts of receptors and transformers, this paper revealed that potential existing parks within a dense urban area could serve as transformers, creating an urban image that defies that “out of place” narrative associated with vendors

Objective: We aimed to investigate the frequency of dysnatremia among patients admitted with COVID-19 infection and its association with inpatient mortality. Methods: This retrospective longitudinal study was conducted for 12 weeks. Serum sodium levels were recorded at admission, during the hospital stay, and within 48 hours of discharge or death. Logistic regression was used to determine the predictors of mortality. Results: This study included 574 patients (69.7% men, age 55.6 ± 14.4 years). On admission, mean sodium was 135.9 ± 6.4 mEq/L; 39% had hyponatremia and 4.7% had hypernatremia. During admission, hypernatremia increased to 18.8%; maximum sodium in patients who survived was 140.6 ± 5.0 mEq/L versus 151.0 ± 9.9 mEq/L in those who died. The final sodium was 145.4 ± 9.4 mEq/L in patients who died versus 137.7 ± 3.7 mEq/L in those who survived (odds ratio [OR]: 1.22, 95% confidence interval [CI]: 1.13-1.32). Other predictors of mortality included ischemic heart disease (OR: 3.65, 95% CI: 1.39-9.61), acute kidney injury (OR: 6.07, 95% CI: 2.39-15.42), invasive ventilation (OR: 28.4, 95% CI: 11.14-72.40), and length of stay (OR: 0.91, 95% CI: 0.86-0.97).Conclusion: Hypernatremia was frequently observed in patients who were critically ill and died and may be considered a predictor of mortality in COVID-19 infection
