    Gender Based Violence and its Effects on Women’s Reproductive Health: The Case of Hatcliffe, Harare, Zimbabwe

    Gender based violence (GBV) negatively impacts on women’s reproductive health (R.H) and is contrary to human rights and RH statutory instruments. The study triangulates quantitative and qualitative research methods with women in the reproductive age group being the target group. The study noted that 95% of the respondents experienced physical violence, 31% rape by a stranger, 92% spousal rape and 65% forced marriages. Socio-cultural, religious, economic and policy implementation factors underlie a culture of silence that prevails among the victims of GBV. The study recommends economic empowerment of women and information, education, counselling pertaining to the negative effects of GBV to both males and females. There is need for education about grievance procedures and scaling up of policy enforcement in order to curb the gruesome effects of GBV.     Susceptibility of Grapevine Sucker and Green Shoot Wounds to Trunk Disease Pathogens

    Grapevine trunk disease fungi infect vines through openings, primarily pruning wounds. The main objective of this study was to understand the role of sucker wounds and wounds made by the removal of green shoots from the stems of potted grapevines as potential points of infection for grapevine trunk disease pathogens. Six wine and four table grape vineyards of different ages were sampled in differentproduction areas in the Western Cape grape region of South Africa. Isolations were made from 161 sucker wounds, and fungal pathogens were identified using morphology and DNA sequence analysis of the internal transcribed spacers (ITS1 and ITS2) and the 5.8S ribosomal RNA gene, the translation elongation factor 1alpha or the partial β-tubulin gene. The results show that 62% of the sucker wounds were infected by trunk disease pathogens, including Diaporthe ampelina, Diplodia seriata, Phaeomoniella chlamydospora, Phaeoacremonium minimum, Eutypella microtheca, Cryptovalsa ampelina and Neofusicoccum australe.  Diaporthe ampelina was the most common, followed by D. seriata and P. chlamydospora, in both the wine and table grape sucker wounds. Under glasshouse conditions, wounds made by the removal of young green shoots on one-year-old potted grapevine plants were inoculated with spore suspensions of D. ampelina, E. lata, N. parvum, P. minimum and P. chlamydospora. After four months, all the inoculated pathogens could be re-isolated. This study shows that grapevine sucker and green shoot wounds are susceptible to different grapevine trunk disease pathogens and may therefore play a role in the epidemiology of trunk diseases

    Maternal HIV viral load testing during pregnancy and postpartum care in Gauteng Province, South Africa

    Background. Pregnant and breastfeeding women living with HIV (WLHIV) are a target population for elimination of mother-to-child transmission of HIV (eMTCT). However, there are limited data on maternal virological responses during pregnancy and the postpartum period in South Africa (SA).Objectives. To review compliance of viral load (VL) testing with national guidelines and suppression rates during pregnancy and up to 9  months postpartum among WLHIV delivering in four tertiary hospitals in Gauteng Province, SA.Methods. All women who had a point-of-care HIV VL test using Xpert HIV-1 VL (Cepheid, USA) at delivery in four tertiary obstetric units in Gauteng between June 2018 and February 2020 were included. HIV VL tests of eligible women performed up to 9 months before and after delivery were extracted from the National Health Laboratory Service’s Corporate Data Warehouse. Proportions of women delivering who had antenatal and postpartum VL tests performed and their suppression rates were determined and expressed as percentages.Results. Of 4 989 eligible WLHIV (median age 31.1 years), 917 (18.4%) had a VL performed during the antenatal period; of these, 335 (36.5%) had a VL ≥50 copies/mL and 165 (18.0%) a VL ≥1 000 copies/mL. At delivery, 1 911 women (38.3%) had a VL ≥50 copies/mL and 1 028 (20.6%) a VL ≥1 000 copies/mL. Among 627 women (12.6%) with a VL test postpartum, 234 (37.3%) had a VL ≥50 copies/mL and 93 (14.8%) a VL ≥1 000 copies/mL. Overall, having a VL test performed during the antenatal period was associated with viral suppression at delivery and receiving a VL test postpartum (p<0.001). Women with a VL ≥50 copies/mL at delivery were more likely to be younger and to remain virally unsuppressed postpartum (p<0.001) compared with women with a VL <50 copies/mL.Conclusions. Fewer than 5% of WLHIV with a VL at the time of delivery received VL monitoring during the antenatal and postpartum periods in accordance with national guidelines. More than 80% of WLHIV delivering had no evidence of VL monitoring during the antenatal period, and they were more likely than women who received monitoring during the antenatal period to be virally unsuppressed at delivery and to receive no VL monitoring postpartum. Women with a high VL at delivery were likely to remain virally unsuppressed postpartum. These results emphasise the need for closer monitoring of and rapid reaction to high maternal VLs during pregnancy, at delivery and postpartum for attainment of eMTCT

    Irrigation development in Zimbabwe: understanding productivity barriers and opportunities at Mkoba and Silalatshani irrigation schemes

    Productivity barriers and opportunities influencing smallholder irrigation sustainability in Zimbabwe were identified using case studies of the Silalatshani and Mkoba irrigation schemes. The major barriers were poor infrastructure and soil fertility, and poor access to farm inputs, farm implements, functioning markets and agricultural knowledge, which resulted in low yields, food insecurity and negative farm income. Most irrigated land remains unused, and marketing of produce is uncoordinated. Mobile technologies provide opportunities for market information dissemination. Institutions are needed to continuously encourage dialogue among agricultural value chain stakeholders to allow irrigators to align their operations to market demands and improve the viability of irrigation systems

    Antimicrobial resistance among producers and non-producers of extended spectrum beta-lactamases in urinary isolates at a tertiary Hospital in Tanzania

    <p>Abstract</p> <p>Background</p> <p>Published data on the existence and magnitude of extended spectrum beta-lactamase (ESBL) production in urinary pathogens in local setting is limited. The aim of the present study was to determine the prevalence of antimicrobial resistance and ESBL production among <it>Escherichia coli </it>and <it>Klebsiella spp </it>from urine samples in a tertiary hospital. This was a cross sectional study conducted at Muhimbili National Hospital in Dar es Salaam, Tanzania.</p> <p>Findings</p> <p>A total of 270 <it>E.coli </it>and <it>Klebsiella spp </it>urinary pathogens from children and adults isolated from January to March 2010 were included in the study. <it>E. coli </it>and <it>Klebsiella spp </it>isolates were tested for antimicrobial susceptibility by the Clinical and Laboratory Standard Institute's disc diffusion method. These isolates were further screened for ESBL phenotype using cefotaxime and ceftazidime discs. Isolates with reduced sensitivity were confirmed using ESBL E-test strips. Of 270 isolates, 138 (51.1%) were <it>E. coli </it>and 132 (48.9%) were <it>Klebsiella spp</it>. ESBL was detected in 122 (45.2%) of all the isolates. ESBL- producing <it>E. coli </it>strains were significantly more resistance to cotrimoxazole (90.7%), ciprofloxacin (46.3%) and nalidixic acid (61.6%) than strains that did not produce ESBL (p < 0.05). Similarly, ESBL- producing <it>Klebsiella spp </it>strains were significantly more resistance to cotrimoxazole (92.6%), ciprofloxacin (25.0%), nalidixic acid (66.2%), and gentamicin (38.2%) than strains that did not produce ESBL (P < 0.05). Multi-drug resistance was found to be significantly (<it>P </it>< 0.05) more in ESBL producing isolates (90.5%) than non ESBL producers (68.9%). The occurrence of ESBL was significantly higher among isolates from inpatients than outpatients [95 (50.5%) vs. 27(32.9%)] (p = 0.008). The occurrence of ESBL was significantly higher among isolates from children than in adults [84 (54.9%) vs. 38(32.5%)] (p < 0.001).</p> <p>Conclusions</p> <p>High prevalence of ESBL-producing <it>E. coli </it>and <it>Klebsiella spp </it>strains was found among inpatients and children. Most of the ESBL- producing isolates were multi-drug resistant making available therapeutic choices limited. We recommend continued antibiotic surveillance as well comprehensive multi-center studies to address the emerging problem of ESBL-associated infections in order to preserve the continued usefulness of most antimicrobial drugs. Further more conducting molecular studies will help to evaluate the various ESBL types.</p

    An investigation of the perceived consequences to employees of reducing employment related trip end choices in Cape Town

    Paper presented at the 33rd Annual Southern African Transport Conference 7-10 July 2014 "Leading Transport into the Future", CSIR International Convention Centre, Pretoria, South Africa.In South Africa, the bench mark for transport expenditure is 10% of monthly income. In the global South “transport poverty” is not a foreign concept considering that most individuals use more than 10% of their income on transport. The driver of these high transport costs is the positioning of economic activities relative to residential areas. Additionally, concerns over Peak Oil and its impact on fuel price means transport will become even more unaffordable in the future. Using Cape Town as a study area, this paper explores the concept of “too much” choice in a job choice framework to investigate the hypothesis that, the provision of a large catchment area from which individuals seek jobs does not continue to yield an increase in utility, but that there is a point beyond which benefits from having more choice are negligible. The results suggest that in a job choice framework, an increase in catchment size does not result in an increase in utility, instead as the catchment size increases utility decreases. It is therefore concluded that there is an amount of accessibility that cities can provide that can be considered to be “sufficient” and still enable individuals to attain positive utility. From the findings in this study, providing “sufficient” accessibility can be achieved by locating future urban growth thereby reducing home to work distances which may translate to a reduction in transport costs.This paper was transferred from the original CD ROM created for this conference.     Identifying leverage points to transition dysfunctional irrigation schemes towards complex adaptive systems

    This article explores the value of Ostrom’s socio-ecological systems framework and Meadows’s leverage point hierarchy, as structured diagnostics, to define systemic problems and avoid approaches based on linear thinking. These frameworks were applied as an ex post analysis of an irrigation scheme in Zimbabwe, drawing on the scheme’s baseline condition and the intervention outcomes. Strong leverage points, particularly those driving feedback mechanisms and institutional design, interacted with other intervention points, initiating systemic change. This analysis suggests that dysfunctional schemes can be transitioned towards complex adaptive systems by using agricultural innovation platforms to identify systemic challenges and intervention points

    The importance of learning processes in transitioning small-scale irrigation schemes

    Many small-scale irrigation schemes are dysfunctional, and learning, innovation and evaluation are required to facilitate sustainable transitions. Using quantitative and qualitative data from five irrigation schemes in sub-Saharan Africa, we analyze how learning and change arose in response to: soil monitoring tools, which triggered a deep learning cycle; and agricultural innovation platforms, which helped develop a social learning system. Knowledge generation and innovation were driven by the incentives of more profitable farming. Learning and change spread to farmers without the tools, and learning at different levels resulted in extension and governance stakeholders facilitating profound institutional change
