    A dynamic emulator for physically based flow simulators under varying rainfall and parametric conditions

    This work presents a method to emulate the flow dynamics of physically based hydrodynamic simulators under variations of time-dependent rainfall and parametric scenarios. Although surrogate modelling is often employed to deal with the computational burden of this type of simulators, common techniques used for model emulation as polynomial expansions or Gaussian processes cannot deal with large parameter space dimensionality. This restricts their applicability to a reduced number of static parameters under a fixed rainfall process. The technique presented combines the use of a modified Unit Hydrograph (UH) scheme and a polynomial chaos expansion (PCE) to emulate flow from physically based hydrodynamic models. The novel element of the proposed methodology is that the emulator compensates for the errors induced by the assumptions of proportionality and superposition of the UH theory when dealing with non-linear model structures, whereas it approximates properly the behaviour of a physically based simulator to new (spatially-uniform) rainfall time-series and parametric scenarios. The computational time is significantly reduced, which makes the practical use of the model feasible (e.g. real time control, flood warning schemes, hydraulic structures design, parametric inference etc.). The applicability of this methodology is demonstrated in three case studies, through the emulation of a simplified non-linear tank-in-series routing structure and of the 2D Shallow Water Equations (2D-SWE) solution (FLOW-R2D) in two computational domains.     Colorectal cancer intrinsic subtypes predict chemotherapy benefit, deficient mismatch repair and epithelial-to-mesenchymal transition

    In most colorectal cancer (CRC) patients, outcome cannot be predicted because tumors with similar clinicopathological features can have differences in disease progression and treatment response. Therefore, a better understanding of the CRC biology is required to identify those patients who will benefit from chemotherapy and to find a more tailored therapy plan for other patients. Based on unsupervised classification of whole genome data from 188 stages I–IV CRC patients, a molecular classification was developed that consist of at least three major intrinsic subtypes (A-, B- and C-type). The subtypes were validated in 543 stages II and III patients and were associated with prognosis and benefit from chemotherapy. The heterogeneity of the intrinsic subtypes is largely based on three biological hallmarks of the tumor: epithelial-to-mesenchymal transition, deficiency in mismatch repair genes that result in high mutation frequency associated with microsatellite instability and cellular proliferation. A-type tumors, observed in 22% of the patients, have the best prognosis, have frequent BRAF mutations and a deficient DNA mismatch repair system. C-type patients (16%) have the worst outcome, a mesenchymal gene expression phenotype and show no benefit from adjuvant chemotherapy treatment. Both A-type and B-type tumors have a more proliferative and epithelial phenotype and B-types benefit from adjuvant chemotherapy. B-type tumors (62%) show a low overall mutation frequency consistent with the absence of DNA mismatch repair deficiency. Classification based on molecular subtypes made it possible to expand and improve CRC classification beyond standard molecular and immunohistochemical assessment and might help in the future to guide treatment in CRC patients.Intelligent SystemsElectrical Engineering, Mathematics and Computer Scienc

    La revista Arqueología cumple doce años; doce años de difundir los avances de nuestra disciplina en México, de establecer vínculos entre especialistas, así como de impulsar la discusión académica nacional y extranjera. En sus orígenes constituyó un reto editorial lanzado por un pequeño grupo de arqueólogos visionarios, pero con el paso del tiempo se ha ido convirtiendo en la publicación científica más importante del país, en su disciplina. Sus páginas han sido foro abierto a todo especialista que pueda y quiera enriquecer el discurso arqueológico nacional, desde sus muy particulares y variadas perspectivas. Su periodicidad y la calidad de sus contenidos han alimentado el entusiasmo y participación de los colaboradores, y sobre todo la dedicación de los editores, quienes han sido sostén principal en la construcción de la revista. Con pesar anunciamos el cambio del equipo editorial formado por la doctora Alba Guadalupe Mastache y el ingeniero Joaquín García-Bárcena, quienes imprimieron a esta tarea un sello especial hasta el número 26 de la publicación, conciliando la necesidad de mantener la continuidad y el nivel de calidad de la revista, con la salvaguarda de la pluralidad en las contribuciones, respetando en todo momento la libertad académica de los colaboradores. A ellos va nuestro más profundo reconocimiento por su inigualable labor, que lo mismo enfrentó problemas mayores de política editorial, que los detalles más pequeños de redacción. Desde ahora el compromiso ineludible del nuevo equipo editorial, tal como lo fue el anterior, será preservar la calidad de la revista y garantizar su continuidad, por lo que aseguramos nuestra mejor disposición para recibir y discutir trabajos, colaboraciones y observaciones. A partir del número 28 se abrirá un pequeño espacio de comentarios, debates y aclaraciones. Aunque en el pasado ha habido aportaciones de este tipo, éstas han sido circunstanciales y no han permitido establecer un ámbito permanente de debate científico, tan necesario para la construcción crítica denuestra disciplina. Esperamos que esta sección, lejos de la polémica estéril, contribuya a precisar datos y planteamientos, además de obligarnos a abrir públicamente nuestros comentarios académicos. Asimismo, estimularemos con mayor amplitud y profundidad las propuestas de trabajos que reporten avances de investigación y enfrenten al mismo tiempo la discusión de los problemas teóricos relacionados.</p

    Application of biofilm film array blood colture identification panel for rapid identification of the causative agents of ventilator associated pneumonia

    Objective: To evaluate the ability of the BioFire FilmArray Blood Culture Identification (BCID) panel to rapidly detect pathogens producing late-onset ventilator-associated pneumonia (VAP), a severe infection often produced by Gram-negative bacteria. These microorganisms are frequently multidrug resistant and typically require broad-spectrum empiric treatment. Methods: In the context of an international multicentre clinical trial (MagicBullet), respiratory samples were collected at the time of suspicion of VAP from 165 patients in 32 participating hospitals in Spain, Greece and Italy. Microorganisms were identified using the BCID panel and compared with results obtained by conventional microbiologic techniques. Results: Pseudomonas aeruginosa, Acinetobacter baumannii and Klebsiella pneumoniae were the most commonly identified species, representing 54.7% (70/128) of microorganisms. The BCID panel showed high global specificity (98.1%; 95% confidence interval, 96–100) and negative predictive values (96.6%) and a global sensitivity and positive predictive value of 78.6% (95% confidence interval, 70–88) and 87.3%, respectively, for these microorganisms. Importantly, the BCID panel provided results in only 1 hour directly from respiratory samples with minimal sample processing times. Conclusions: The BCID panel may have clinical utility in rapidly ruling out microorganisms causing VAP, specifically multidrug-resistant Gram-negative species. This could facilitate the optimization of empiric treatment

    Identification of genetic variants associated with Huntington's disease progression: a genome-wide association study

    Background Huntington's disease is caused by a CAG repeat expansion in the huntingtin gene, HTT. Age at onset has been used as a quantitative phenotype in genetic analysis looking for Huntington's disease modifiers, but is hard to define and not always available. Therefore, we aimed to generate a novel measure of disease progression and to identify genetic markers associated with this progression measure. Methods We generated a progression score on the basis of principal component analysis of prospectively acquired longitudinal changes in motor, cognitive, and imaging measures in the 218 indivduals in the TRACK-HD cohort of Huntington's disease gene mutation carriers (data collected 2008–11). We generated a parallel progression score using data from 1773 previously genotyped participants from the European Huntington's Disease Network REGISTRY study of Huntington's disease mutation carriers (data collected 2003–13). We did a genome-wide association analyses in terms of progression for 216 TRACK-HD participants and 1773 REGISTRY participants, then a meta-analysis of these results was undertaken. Findings Longitudinal motor, cognitive, and imaging scores were correlated with each other in TRACK-HD participants, justifying use of a single, cross-domain measure of disease progression in both studies. The TRACK-HD and REGISTRY progression measures were correlated with each other (r=0·674), and with age at onset (TRACK-HD, r=0·315; REGISTRY, r=0·234). The meta-analysis of progression in TRACK-HD and REGISTRY gave a genome-wide significant signal (p=1·12 × 10−10) on chromosome 5 spanning three genes: MSH3, DHFR, and MTRNR2L2. The genes in this locus were associated with progression in TRACK-HD (MSH3 p=2·94 × 10−8 DHFR p=8·37 × 10−7 MTRNR2L2 p=2·15 × 10−9) and to a lesser extent in REGISTRY (MSH3 p=9·36 × 10−4 DHFR p=8·45 × 10−4 MTRNR2L2 p=1·20 × 10−3). The lead single nucleotide polymorphism (SNP) in TRACK-HD (rs557874766) was genome-wide significant in the meta-analysis (p=1·58 × 10−8), and encodes an aminoacid change (Pro67Ala) in MSH3. In TRACK-HD, each copy of the minor allele at this SNP was associated with a 0·4 units per year (95% CI 0·16–0·66) reduction in the rate of change of the Unified Huntington's Disease Rating Scale (UHDRS) Total Motor Score, and a reduction of 0·12 units per year (95% CI 0·06–0·18) in the rate of change of UHDRS Total Functional Capacity score. These associations remained significant after adjusting for age of onset. Interpretation The multidomain progression measure in TRACK-HD was associated with a functional variant that was genome-wide significant in our meta-analysis. The association in only 216 participants implies that the progression measure is a sensitive reflection of disease burden, that the effect size at this locus is large, or both. Knockout of Msh3 reduces somatic expansion in Huntington's disease mouse models, suggesting this mechanism as an area for future therapeutic investigation
