    Asymptomatic stenosis in the cervical and thoracic spines of patients with symptomatic lumbar stenosis

    Study Design Retrospective study. Objective Studies on age-related degenerative changes causing concurrent stenoses in the cervical, thoracic, and lumbar spines (triple stenosis) are rare in the literature. Our objectives were to determine: (1) the incidence of asymptomatic radiologic cervical and thoracic stenosis in elderly patients with symptomatic lumbar stenosis, (2) the incidence of concurrent radiologic spinal stenosis in the cervical and thoracic spines, and (3) the radiologic features of cervical stenosis that might predict concurrent thoracic stenosis. Methods Whole-spine T2 sagittal magnetic resonance images of patients older than 80 and diagnosed with lumbar spinal stenosis between January 2003 and January 2012 were evaluated retrospectively. We included patients with asymptomatic spondylotic cervical and thoracic stenosis. We measured the anteroposterior diameters of the vertebral body, bony spinal canal, and spinal cord, along with the Pavlov ratio and anterior or posterior epidural stenosis at the level of the disk for each cervical and thoracic level. We compared the radiologic parameters between the subgroups of cervical stenosis with and without thoracic stenosis. Results Among the 460 patients with lumbar stenosis, 110 (23.9%) had concurrent radiologic cervical stenosis and 112 (24.3%) had concurrent radiologic thoracic stenosis. Fifty-six patients (12.1%) had combined radiologic cervical and thoracic stenosis in addition to their symptomatic lumbar stenosis (triple stenosis). Anterior epidural stenosis at C7–T1 was associated with a high prevalence of thoracic stenosis. Conclusions It appears that asymptomatic radiologic cervical and thoracic stenosis is common in elderly patients with symptomatic lumbar stenosis

    Introducing a New Talent Management System for Korea’s Public Officials

    This article criticizes that the personnel management system for Korean public officials is seriously lacking competition among public employees and fails to nurture their competence and confidence due to too much reliance on rank, seniority, closed recruitment, and excessive rotation of positions as well as due to the consequent problems of deep-seated ministry selfishness and rising political influence over public officials. To redress such a state of crisis, we suggest that the new talent management system for Korean public officials should be introduced through 3C (competition, competence, and confidence) strategies and three major reforms that include abolishment of the elite recruitment system at the fifth-level, abolishment of the career path in which most public employees serve for a single ministry for entire duration of their careers, and introduction of the matrix personnel management system that combines the vertical management with the horizontal one

    Study on Personal Characteristics and Emotional Affordances through User Activity Analysis

    Design methodology should address psychological and cognitive aspects based on human emotion and feeling as well as physical and functional aspects based on product structure. Donald Norman introduced perceived affordances as characteristics in the appearance of an object that gives clues for its proper operations. We conducted a case study in a public space - building lobby - used by many general people. User activities and behaviors were analyzed in several specific tasks given to twenty participants with similar cultural background in the lobby of a building they have never been to before. Then we identified affordances and building structure elements, called affordance features, critically related to those affordances. While performing the tasks in the same building lobby, user activities can be classified into several different groups for each task based on affordance features involved in their activities. These user activity differences are then compared with their personal cognitive characteristics derived from their perception and judgment preferences using intuitive and factual perception orientation and objective and subjective decision making inclination respectively. For user of less common activities for some tasks, personal cognitive characteristics have been identified. This study could help in affordance-based design so that alternative affordance features are designed considering diverse user cognitive characteristics

    Near-Infrared [Fe II] and H2 Line Observations of the Supernova Remnant 3C 396: Probing the Pre-supernova Circumstellar Materials

    We present the results of near-infrared [Fe II] and H2 line imaging and spectroscopic observations of the supernova remnant 3C 396 using the Palomar 5 m Hale telescope. We detect long, filamentary [Fe II] emission delineating the inner edge of the radio emission in the western boundary of the remnant in imaging observations, together with a bright [Fe II] emission clump close to the remnant center. There appears to be faint, diffuse [Fe II] emission between the central clump and the western filamentary emission. The spectroscopic observations determine the expansion velocity of the central clump to be ~56 km/s. This is far smaller than the expansion velocity of 3C 396 obtained from X-ray observations, implying the inhomogeneity of the ambient medium. The electron number density of the [Fe II] emission gas is < 2,000 cm-3. The H2 line emission, on the other hand, lies slightly outside the filamentary [Fe II] emission in the western boundary, and forms a rather straight filament. We suggest that the [Fe II] emission represents dense clumps in the wind material from the red supergiant phase of a Type IIL/b progenitor of 3C 396 which have been swept up by the supernova remnant shocks. The H2 emission may represent either the boundary of a wind bubble produced during the main-sequence phase of the progenitor or molecular clumps left over inside the bubble. We propose that the near-infrared [Fe II] and H2 emission observed in several supernova remnants of Type IIL/b SNe likely has the same origin.Comment: 24 page including 8 figures; Accepted by Ap
