    Concomitant pulmonary tuberculosis and tuberculous appendicitis in a recipient of a renal transplant: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Tuberculosis is still a serious infection among recipients of renal transplants. Although the ileocecal region is the most affected part in intestinal tuberculosis, acute tuberculous appendicitis is quite a rare entity. We report a case of concomitant pulmonary tuberculosis and tuberculous appendicitis in a recipient of a renal transplant.</p> <p>Case presentation</p> <p>A 27-year-old Iranian woman, who had been the recipient of a renal transplant five years earlier, presented with a two-week history of coughing, fever and weight loss. The cause of her end-stage renal disease was chronic pyelonephritis. There were fine crackles noted during a chest examination, and a plain chest radiography showed fine miliary nodules throughout her entire lung fields. Sputum and bronchial aspirate examination was positive for acid-fast bacilli, suggestive of <it>Mycobacterium tuberculosis </it>infection. A chest computed tomography scan revealed widespread miliary nodules, compatible with miliary tuberculosis. She developed severe abdominal pain and abdominal surgery disclosed a perforated appendicitis. Histopathological examination of the resected appendix revealed widespread caseating epithelioid granulomas, suggestive of tuberculosis.</p> <p>Conclusion</p> <p>Our case report highlights a rare presentation of tuberculosis in a patient who has undergone renal transplant. Such unusual presentation of tuberculosis, particularly among patients receiving potent immunosuppressive protocols, should be considered by clinicians.</p

    Study On The Acoustic Characteristics Of Natural Date Palm Fibres: Experimental And Theoretical Approaches

    The present study deals with the acoustic performance of natural fibres originated from the date palm empty fruit (DPEFB) fibres which is mainly considered as agricultural waste. The fibres were processed and fabricated to be sound absorber samples with two different densities of 100 kg/m3 and 200 kg/m3 and with thicknesses of 10–40 mm. The normal incidence absorption coefficients of the sound absorbers were measured using an impedance tube based on ISO 10534-2. The effects of fibre density and sample thickness are discussed. The findings reveal that for density of 100 kg/m3 the absorption coefficient is 0.6–0.8 above 1.5 kHz for the samples with the thickness of 20 mm and 30 mm. For the thickness of 40 mm, the values even reached the value of 0.9. The values can reach 0.7–0.8 above 1 kHz for the density of 200 kg/m3. Mathematical model using the optimized Delaney-Bazley model with Nelder-Mead simplex method is shown to successfully predict the sound absorption coefficient of the fibre samples. The Johnson-Champoux-Allard model follows the trend of the absorption coefficient, but underestimates the measured data at high frequencies above 2.5 kHz

    Dirac-Born-Infeld action, Seiberg-Witten map, and Wilson Lines

    We write the recently conjectured action for transformation of the ordinary Born-Infeld action under the Seiberg-Witten map with one open Wilson contour in a manifestly non-commutative gauge invariant form. This action contains the non-constant closed string fields, higher order derivatives of the non-commutative gauge fields through the N*_N-product, and a Wilson operator. We extend this non-commutative D9D_9-brane action to the action for DpD_p-brane by transforming it under T-duality. Using this non-commutative DpD_p-brane action we then evaluate the linear couplings of the graviton and dilaton to the brane for arbitrary non-commutative parameters. By taking the Seiberg-Witten limit we show that they reduce exactly to the known results of the energy-momentum tensor of the non-commutative super Yang-Mills theory.     S-matrix elements and off-shell tachyon action with non-abelian gauge symmetry

    We propose that there is a unique expansion for the string theory S-matrix elements of tachyons that corresponds to non-abelian tachyon action. For those S-matrix elements which, in their expansion, there are the Feynman amplitudes resulting from the non-abelian kinetic term, we give a prescription on how to find the expansion. The gauge invariant action is an α\alpha' expanded action, and the tachyon mass mm which appears as coefficient of many different couplings, is arbitrary. We then analyze in details the S-matrix element of four tachyons and the S-matrix element of two tachyons and two gauge fields, in both bosonic and superstring theories, in favor of this proposal. In the superstring theory, the leading terms of the non-abelian gauge invariant couplings are in agreement with the symmetrised trace of the direct non-abelian generalization of the tachyonic Born-Infeld action in which the tachyon potential is consistent with V(T)=eπαm2T2V(T)=e^{\pi\alpha' m^2T^2}. In the bosonic theory, on the other hand, the leading terms are those appear in superstring case as well as some other gauge invariant couplings which spoils the symmetrised trace prescription.     Microextraction Methods for Preconcentration of Aluminium in Urine Samples

    Background: Analysis of aluminium (Al) in urine samples is required in management of a number of diseases including patients with renal failure. This work aimed to present dispersive liquid-liquid microextraction (DLLME) and ultrasound-assisted emulsification microextraction (USAEME) methods for the preconcentration of ultra-trace amount of aluminum in human urine prior to its determination by a graphite furnace atomic absorption spectrometry (GFAAS). Methods: The microextraction methods were based on the complex formation of Al3+ with 8-hydroxyquinoline. The effect of various experimental parameters on the efficiencies of the methods and their optimum values were studied. Results: Under the optimal conditions, the limits of detection for USAEME-GFAAS and DLLME-GFAAS were 0.19 and 0.30 ng mL−1, respectively and corresponding relative standard deviations (RSD, n=5) for the determination of 40 ng mL−1 Al3+ were 5.9% and 4.9%. Conclusion: Both methods could be successfully used to the analysis of ultra trace concentrations of Al in urine samples of dialysis patients

    Roadmap to recovery: Implemented and attitude toward school reopening strategies during the COVID-19 pandemic, a scoping review

    The novel coronavirus disease 2019 (COVID-19) has had various financial and life impacts on the world's population. Schools' regular activity and function during the pandemic require balancing the repercussions of suspending in-person education versus health threats. Furthermore, children are one of the prominent victims of the restricted quarantine strategies' effects, which may make them vulnerable to various mental health problems. In this study, we reviewed previously reported strategies and roadmaps regarding the reopening of schools during the COVID-19 pandemic. The following databases were searched from October to December 2021, via multi-step search strategies for “COVID-19,” “coronavirus,” “school reopening,” “roadmaps,” “reopening,” and “reopening strategies” Google Scholar, PubMed, Scopus, and Web of Science. A total of five papers with roadmaps focusing on reopening schools were included in this study. Fundamental issues and principles of these reviewed roadmaps were: 1) protecting the high-risk students and staff physically and mentally, 2) accelerating the vaccination of essential workers, staff, parents, and students, and 3) improving the COVID-19 testing capacity. Roadmaps for the reopening of the schools should describe some phases and steps for their strategies. Current roadmaps have not mentioned any phases and timelines for this process. Describing some health metrics in the roadmaps for progressing to the next step or returning to the previous ones is also necessary for all roadmaps and should be considered in further studies

    Asymptomatic Bilateral Ureteropelvic Junction Obstruction due to Supernumerary Renal Arteries

    A supernumerary renal artery is a common arterial variation, which warrants consi-derations in a variety of urologic and retroperitoneal operations. Supernumerary renal arteries can be associated with other uro-vascular variations and anomalies such as duplicated renal veins and ureters, aberrant origin of the gonadal arteries, persistence of fetal renal lobulation, and kidney malrotation. The role of crossing supernumerary renal arteries in ureteropelvic junction obstruc-tion (UPJO) is controversial. We report a healthy potential renal transplant donor with bilateral UPJO, which appeared to be secondary to supernumerary renal (inferior polar) arteries. We believe that the bilateral occurrence of asymptomatic UPJO associated with supernumerary renal arteries has not been previously reported

    اسماعیل جرجانی یکی از تأثیرگذارترین پزشکان و آناتومیست‌های ایرانی قرن دوازدهم میلادی است که بخش عمده دست نوشته‌هایش را در اواخر عمر پس از 70 سالگی به رشته تحریر درآورده است. کتاب جامع مرجع پزشکی جرجانی، با عنوان ذخیره خوارزمشاهی در حدود سال 1112م. نگارش یافته و قدیمی‌ترین دائرة المعارف پزشکی نوشته شده به زبان فارسی به شمار می‌آید. این کتاب یکی از کتاب‌های مرجع دانشجویان رشته پزشکی آن دوران بوده است. در این مقاله، شرح حال جرجانی به رشته تحریر درآمده و تفاسیر و تعابیری از توصیفات مشروح وی در خصوص اعصاب مغزی مربوط به حدود یک هزاره قبل ارائه گردیده است. دانشوران ایران باستان و دوران اسلامی از پیشگامان علوم مربوط به اعصاب مغزی به شمار می‌آیند. برخی از این توصیفات چنان روشن و رسا توسط جرجانی ارائه شده‌اند که سال‌های متمادی فراروی دانشمندان و محققین اروپایی در دوره پس از رنسانس قرار داشته‌اند.Jorjani was an influential Persian physician and anatomist of the 12th&nbsp;century who did most of his writing after his seventh decade of comprehensive textbook of medicine, zakhirey-e khwarazmshahi (The Treasure of the khwarazm Shah) was written in approximately considered to be the oldest medical encyclopedia written in Persian. This was an essential textbook for those studying medicine. We describe the life and times of jorjani and provide a translation and interpretations of his detailed descriptions of the cranial were written almost a millennium ago. Medieval Persian and Muslim scholars have contributed to our current knowledge of the cranial of these descriptions, such as the eloquent ones provided by jorjani, were original and have gone mostly unknown to post-Vesalian ars
