    Study of role of fine needle aspiration biopsy in orbital and eyelid lesions.

    AIM : To study the role of FNAC in eyelid and orbital lesions in comparision to biopsy. Materials and methods : 40 patients with eyelid (24) and orbital lesions (16) were subjected to FNAC and then later to Biopsy and the results were compared and tabulated. Only firm eyelid lesions >5mm in size and anterior orbital lesions were included in the study. RESULTS : This shows that FNAC in eyelid and orbital lesions is a reliable diagnostic tool with histopathological correlation of >80 % in orbital lesions and correlation of 91.3%The sensitivity of FNAC for malignant lesions to be 88% and its specificity to be 100%.The positive predictive value of FNAC for malignant lesions is 100% and negative predictive value is 89%. CONCLUSION : FNAC gives rapid results and correlates highly with Tissue biopsy proven HPE, FNAC can be considered as a primary investigation in the diagnosis of eyelid and anterior orbital lesions and can be done safely as an outpatient procedure

This paper will analyze how AI can help streamline arbitration, reduce costs, ensure faster dispute resolution, and improve accessibility. By using machine learning algorithms and natural language processing techniques, AI systems can analyze large volumes of legal text, extract relevant information, recognize patterns, and predict case outcomes. In addition, AI-driven chatbots could provide users with instant support and assistance in navigating the complex arbitration process. However, ethical considerations such as privacy and bias must be taken into account to ensure that AI does not compromise fairness or jeopardize confidentiality in arbitration proceedings. The article concludes with an examination of the transformative impact that artificial intelligence will have on the future of arbitration and emphasizes the need for continued research and collaboration to realize its full potential while preserving the integrity of arbitration practice

    Assessing adaptation of the psychometric properties of the General Health Questionnaire (GHQ-12) using the Rasch Measurement Model among Indian teenagers

    The General Health Questionnaire (GHQ-12) is widely accepted as an instrument used to examine diagnosable psychiatric disorders. It is also routinely used as a measure of psychological morbidity. This study aimed to assess and establish the psychometric properties of the GHQ-12 among Indian teenagers. The tool was administered to 212 adolescents aged from 12 to 18 years. Following two-stage cluster sampling, the data were collected from Aligarh district, India. Fleiss Kappa analysis was used to determine test reliability and showed an overall value of 0.94, based on the rater agreement for the instrument. Furthermore, the Rasch measurement model was used, with values of 0.79 and 0.83 for person and item reliability, respectively. Moreover, the value of unidimensionality was found to be 37.9%. Additionally, item fit statistics and item analysis were conducted for the instrument. Based on the preliminary data and findings, the study provides primary evidence for the reliability and validity of GHQ-12. Hence, the questionnaire can lead to more multi-site studies in India

    CO 2 -wettability of sandstones exposed to traces of organic acids: Implications for CO 2 geo-storage

    Wettability of CO 2 -brine-mineral systems plays a vital role during geological CO 2 -storage. Residual trapping is lower in deep saline aquifers where the CO 2 is migrating through quartz rich reservoirs but CO 2 accumulation within a three-way structural closure would have a high storage volume due to higher CO 2 saturation in hydrophobic quartz rich reservoir rock. However, such wettability is only poorly understood at realistic subsurface conditions, which are anoxic or reducing. As a consequence of the reducing environment, the geological formations (i.e. deep saline aquifers) contain appreciable concentrations of various organic acids. We thus demonstrate here what impact traces of organic acids exposed to storage rock have on their wettability. Technically, we tested hexanoic acid, lauric acid, stearic acid and lignoceric acid and measured wettability as a function of organic acid concentration at realistic storage conditions (i.e. 25 MPa and 323 K (50 °C)). In addition, measurements were also conducted at ambient conditions in order to quantify the incremental pressure effect on wettability. Clearly, the quartz surface turned significantly less water-wet with increasing organic acid concentrations, even at trace concentrations. Importantly, we identified a threshold concentration at ˜10 −6 M organic acid, above which quartz wetting behaviour shifts from strongly water-wet to an intermediate-wet state. This wettability shift may have important consequences for CO 2 residual trapping capacities, which may be significantly lower than for traditionally assumed water-wet conditions where CO 2 is migrating through quartz rich reservoirs. © 2019 Elsevier Lt

    Nanomaterial-based drilling fluids for exploitation of unconventional reservoirs: A review

    © 2020 by the authors. The world\u27s energy demand is steadily increasing where it has now become difficult for conventional hydrocarbon reservoir to meet levels of demand. Therefore, oil and gas companies are seeking novel ways to exploit and unlock the potential of unconventional resources. These resources include tight gas reservoirs, tight sandstone oil, oil and gas shales reservoirs, and high pressure high temperature (HPHT) wells. Drilling of HPHT wells and shale reservoirs has become more widespread in the global petroleum and natural gas industry. There is a current need to extend robust techniques beyond costly drilling and completion jobs, with the potential for exponential expansion. Drilling fluids and their additives are being customized in order to cater for HPHT well drilling issues. Certain conventional additives, e.g., filtrate loss additives, viscosifier additives, shale inhibitor, and shale stabilizer additives are not suitable in the HPHT environment, where they are consequently inappropriate for shale drilling. A better understanding of the selection of drilling fluids and additives for hydrocarbon water-sensitive reservoirs within HPHT environments can be achieved by identifying the challenges in conventional drilling fluids technology and their replacement with eco-friendly, cheaper, and multi-functional valuable products. In this regard, several laboratory-scale literatures have reported that nanomaterial has improved the properties of drilling fluids in the HPHT environment. This review critically evaluates nanomaterial utilization for improvement of rheological properties, filtrate loss, viscosity, and clay- and shale-inhibition at increasing temperature and pressures during the exploitation of hydrocarbons. The performance and potential of nanomaterials, which influence the nature of drilling fluid and its multi-benefits, is rarely reviewed in technical literature of water-based drilling fluid systems. Moreover, this review presented case studies of two HPHT fields and one HPHT basin, and compared their drilling fluid program for optimum selection of drilling fluid in HPHT environment

    Clinicopathologic features and prognostic grouping of gastrointestinal stromal tumors (GISTs) in Pakistani patients: An institutional perspective

    Objectives: Gastrointestinal stromal tumors (GISTs) are rare tumors of gastrointestinal tract, prognosis of which largely depends upon histopathologic characteristics of resection specimens, which were not widely studied in our population. Therefore we aimed to evaluate the histopathologic characteristics of GISTs in our population and their prognostic grouping according to college of American pathologist’s guidelines. Results: Mean age of patients was 53.4 years (18–71 years). 92% of cases were of primary GISTs and stomach was the most common site (57.7%). 75% of cases were of spindle cell morphology and 53.8% belonged to high risk prognostic group. Comparison of stomach and intestinal GISTs showed that intestinal GISTs were found to be of high grade (70%) and of high risk prognostic group (75 and 80%) compared to stomach GISTs (43% were of high risk prognostic group), however this fnding was not statistically signifcant. GISTs are infrequent gastrointestinal tumors but early diagnosis and identifcation of adverse histological features are key to successful treatment. We found a large majority of GISTs to be located in stomach, however intestinal GISTs were found more likely to be associated with adverse prognostic parameters. However more large scale studies are warranted to establish this fnding

    Machine Learning Models for Predicting Breast Cancer Risk in Women Exposed to Blue Light from Digital Screens

    Background: Nowadays, there is a growing global concern over rapidly increasing screen time (smartphones, tablets, and computers). An accumulating body of evidence indicates that prolonged exposure to short-wavelength visible light (blue component) emitted from digital screens may cause cancer. The application of machine learning (ML) methods has significantly improved the accuracy of predictions in fields such as cancer susceptibility, recurrence, and survival. Objective: To develop an ML model for predicting the risk of breast cancer in women via several parameters related to exposure to ionizing and non-ionizing radiation.Material and Methods: In this analytical study, three ML models Random Forest (RF), Support Vector Machine (SVM), and Multi-Layer Perceptron Neural Network (MLPNN) were used to analyze data collected from 603 cases, including 309 breast cancer cases and 294 gender and age-matched controls. Standard face-to-face interviews were performed using a standard questionnaire for data collection. Results: The examined models RF, SVM, and MLPNN performed well for correctly classifying cases with breast cancer and the healthy ones (mean sensitivity> 97.2%, mean specificity >96.4%, and average accuracy >97.1%).  Conclusion: Machine learning models can be used to effectively predict the risk of breast cancer via the history of exposure to ionizing and non-ionizing radiation (including blue light and screen time issues) parameters. The performance of the developed methods is encouraging; nevertheless, further investigation is required to confirm that machine learning techniques can diagnose breast cancer with relatively high accuracies automatically

    General public awareness, knowledge and attitude toward COVID-19 infection and prevention: a cross-sectional study from Pakistan [version 2; peer review: 2 approved]

    Background: The aim of this study is to evaluate the knowledge, perceptions, and attitude of the public in Pakistan (using social media) towards COVID-19. Methods: A cross-sectional study was conducted amongst 1120 individuals nationwide. A self-developed, pre-tested questionnaire was used that comprised of sections covering demographic characteristics, medical history, hygiene awareness, COVID-19-related knowledge, and learning attitude. Descriptive statistics were used for frequencies, percentages, averages and standard deviations. Inferential statistics were done using the Student’s t-test and ANOVA. Results: The average age of participants was 31 years (range 18-60 years). In total 56 individuals (5%) had completed primary or secondary school education; 448 (40%) were employed (working from home) and 60% were jobless due to the COVID-19 crisis. Almost all the study subjects (1030 (92%)) were washing their hands multiple times a day. A total of 83% had awareness regarding quarantine time, 82% used face masks whenever they left their homes, 98% were aware of the origin of the disease, and 70% had knowledge regarding the most common symptoms of COVID-19. Conclusion: It can be concluded from the current study that female participants had higher level of education, and more awareness regarding the coronavirus. The majority of the participants followed proper hand washing regimes and washed their faces. Further knowledge and awareness should be promoted

    Long-term full-scale intelligent quotient outcomes following pediatric and childhood epilepsy surgery: A systematic review and meta-analysis

    OBJECTIVE: Cognitive measures are an important primary outcome of pediatric, adolescents, and childhood epilepsy surgery. The purpose of this systematic review and meta-analysis is to assess whether there are long-term alterations (≥ 5 years) in the Full-Scale Intelligence Quotient (FSIQ) of pediatric patients undergoing epilepsy surgery. METHODS: Electronic databases (EMBASE, MEDLINE, and Scopus) were searched for English articles from inception to October 2022 that examined intelligence outcomes in pediatric epilepsy surgery patients. Inclusion criteria were defined as the patient sample size of ≥ 5, average follow- up of ≥5 years, and surgeries performed on individuals ≤ 18 years old at the time of surgery. Exclusion criteria consisted of palliative surgery, animal studies, and studies not reporting surgical or FSIQ outcomes. Publication bias was assessed using a funnel plot and the Quality in Prognosis Studies (QUIPS) toolset was used for quality appraisal of the selected articles. A random-effects network meta-analysis was performed to compare FSIQ between surgical patients at baseline and follow-up and Mean Difference (MD) was used to calculate the effect size of each study. Point estimates for effects and 95% confidence intervals for moderation analysis were performed on variables putatively associated with the effect size. RESULTS: 21,408 studies were screened for abstract and title. Of these, 797 fit our inclusion and exclusion criteria and proceeded to full-text screening. Overall, seven studies met our requirements and were selected. Quantitative analysis was performed on these studies (N = 330). The mean long-term difference between pre- and post- operative FSIQ scores across all studies was noted at 3.36 [95% CI: (0.14, 6.57), p = 0.04, I2 = 0%] and heterogeneity was low. CONCLUSION: To our knowledge, this is the first meta-analysis to measure the long-term impacts of FSIQ in pediatric and adolescent epilepsy patients. Our overall results in this meta-analysis indicate that while most studies do not show long-term FSIQ deterioration in pediatric patients who underwent epilepsy surgery, there was an increase of 3.36 FSIQ points, however, the observed changes were not clinically significant. Moreover, at the individual patient level analysis, while most children did not show long-term FSIQ deterioration, few had significant decline. These findings indicate the importance of surgery as a viable option for pediatric patients with medically refractory epilepsy

    Variation of hepatic enzymes with Vitamin B12 and D3 levels in cirrhotic patients

    OBJECTIVE: To determine variation of hepatic enzymes Vitamin B12 and D3 levels in cirrhotic patients.DESIGN: Cross sectional study.SETTING: Cirrhotic patients in Abbasi Shaheed Hospital.PARTICIPANTS: 250 patients in Abbasi Shaheed Hospital with diagnosis of cirrhosis including 141 males and 109 females.VARIABLE PARAMETERS: They include mean ALT, GGT, Alkaline phosphatase levels along with Vitamin B12 and D3 levels in blood.RESULTS: Vitamin B12 levels were 1249.59±487.01pg/ml and 1422.28±627.75pg/ml in males and females respectively while Vitamin D3 levels were found to be 17.15±10.45 nmol/L in males and 14.80±14.24 nmol/L in females. Vitamin B12 levels were found to be positively correlated with the elevation of ALT and were negatively correlated with elevation of ALT, GGT and Alkaline Phosphatase. The ALT levels were 50.0±21.88 in males and 14.80±14.24 in females, Alkaline phosphatase to be 311.46±107.98 in males while female Alkaline phosphatase were 346.47±101.60. GGT levels to be 41.70±10.62 in males and 45.01±13.74 in females.CONCLUSION: Cirrhotic patients suffering from severe hepatocellular damage have their elevated levels of Vitamin B12 and depressed Vitamin D3 levels in plasma accompanied by a positive association with elevated ALT and GGT plasma level
