Morphine/codeine ratio, a key in investigating a case of doping
Introduction: Consumption of codeine can lead to positive urine test for morphine in athletes. Morphine is classified as a prohibited doping drug while Codeine is not. Morphine/codeine ratio is used in forensic medicine to distinguish the consumption of codeine from abuse of morphine and other narcotics. Case Presentation: We present an athlete with positive urine test for morphine with a history of consumption of codeine. The disciplinary committee came to conclusion that the athlete had not consumed morphine and did not violate doping code based on morphine/codeine ratio. Conclusions: Analysis of codeine to morphine metabolism rate is needed when we are using morphine/codeine ratio to rule out abuse of narcotics. WADA should consider analysis for the CYP2D6 alleles (main metabolizer of codeine) in case of including morphine/codeine ratio in future prohibited list. The possibility of ultra-rapid CYP2D6 cannot be ruled out in certain results of morphine/codeine near the cut point. © 2015, Sports Medicine Research Center
Power Flow Control in Multi-Terminal HVDC Grids Using a Serial-Parallel DC Power Flow Controller
© 2013 IEEE. Multi-terminal HVDC (MT-HVDC) grids have no capability of power flow control in a self-sufficient manner. To address this important issue, utilization of dc-dc high power and high-voltage converters is motivated. However, proposing suitable partial-rated dc-dc converters as well as their suitable modeling and control in both primary and secondary control layers as well as the stability analysis are the existing challenges that should be alleviated beforehand. This paper addresses the control of power flow problem through the application of a power converter with a different connection configuration, namely, serial parallel dc power flow controller (SPDC-PFC). The SPDC-PFC input is the transmission line voltage, and its output is transmission line current. Therefore, employing a full-power dc-dc converter is avoided as a merit. Additionally, in this paper, the common two-layer MT-HVDC grid control framework comprised of primary and secondary layers is efficiently modified in order to integrate the SPDC-PFC. A differential direct voltage versus active power droop control scheme is applied to the SPDC-PFC at the local control layer, guaranteeing dynamic stability, while an extended dc power-flow routine - integrating the SPDC-PFC - is developed at the secondary control layer to ensure the static stability of the entire MT-HVDC grid. The proposed control framework enables the SPDC-PFC to regulate the flow of current/power in the envisioned HVDC transmission line. From the static and dynamic simulation results conducted on the test CIGRE B4 MT-HVDC grid, successful operation of the proposed SPDC-PFC and control solutions are demonstrated by considering power flow control action. In more detail, the SPDC-PFC successfully regulates the compensated lines' power to the desired reference both in static and dynamic simulations by introducing suitable compensation voltages. In addition, good dynamic performance under both SPDC-PFC power reference and wind power-infeed change is observed
Functional Independence Measure in Iran: A Confirmatory Factor Analysis and Evaluation of Ceiling and Floor Effects in Traumatic Brain Injury Patients
Background: The functional independence measure (FIM) is one of the most important assessment instruments for motor and cognitive dependence in rehabilitation medicine; however, there is little data about its confirmatory factor analysis (CFA) and ceiling/floor effects from other countries and also in Iranian patients.
Objectives: The aim of this study was to evaluate a two-factor model (motor and cognitive independence as latent variables) and ceiling/floor effects for FIM in Iranian patients with traumatic brain injuries (TBI).
Patients and Methods: In this cross-sectional study, 185 subacute TBI patients were selected from emergency and neurosurgery departments of Poursina Hospital (the largest trauma hospital in northern Iran, Rasht) using the consecutive sampling method and were assessed for functional independence.
Results: The results of this study showed that the floor effect was not observed; however, ceiling effects were observed for the FIM total score and its subscales. The confirmatory factor analysis showed that the chi-square/df ratio was 2.8 for the two-factor structure and the fit indices for this structural model including root mean square error of approximation (RMSEA) = 0.099, normed fit index (NFI) = 0.96, tucker lewis index (TLI) = 0.97, comparative fit index (CFI) = 0.97 were close to standard indices.
Conclusions: Although ceiling effects should be considered for rehabilitation targets, the two-factor model of FIM (motor and cognitive independence) has an eligible fitness for Iranian patients with TBI
Development of a DNA-Liposome Complex for Gene Delivery Applications
The association structures formed by cationic liposomes and DNA(Deoxyribonucleic acid)-liposome have been effectively utilized as gene carriers in transfection assays. In this research study, cationic liposomes were prepared using a modified lipid film hydration method consisting of a lyophilization step for gene delivery applications. The obtained results demonstrated that the mean particle size had no significant change while the polydispersity (PDI) increased after lyophilization. The mean particle size slightly reduced after lyophilization (520 ± 12 nm to 464 ± 25 nm) while the PDI increased after lyophilization (0.094 ± 0.017 to 0.220 ± 0.004). In addition. The mean particle size of vesicles increases when DNA is incorporated to the liposomes (673 ± 27 nm). According to the Scanning Electron Microscopy(SEM) and transmission electron microscopy (TEM) images, the spherical shape of liposomes confirmed their successful preservation and reconstitution from the powder. It was found that liposomal formulation has enhanced transfection considerably compared to the naked DNA as negative control. Finally, liposomal formulation in this research had a better function than Lipofectamine® 2000 as a commercialized product because the cellular activity (cellular protein) was higher in the prepared lipoplex than Lipofectamine® 2000
SURVEY THE MUTATION OF FGB (BETA FIBRINOGEN) AND FV (FACTOR V LEIDEN), FACTOR XIII AND FACTOR II (PROTHROMBIN), IN PATIENTS WITH RECURRENT ABORTIONS ALONG WITH NORMAL KARYOTYPE
Some pregnancies are abnormal in human genetically and end with the spontaneous abortion, which is the most common problem of pregnancy. The recurrent abortions are often referred to as multifactorial disease that one of which is thrombosis. The thrombosis in placenta capillaries seems to disturb the blood circulation between the mother and the fetus and eventually lead to abortion. Recently, studies have shown that genetic basis for thrombophilia relates with recurrent abortion. The aim of this study is the survey of G1691A and G4070A mutations in the Factor V gene, -455G>A mutation in the gen of XIII factor, G103T mutation in Beta fibrinogen and A20210G mutation in the thrombin gene. The samples were collected from 60 patients referred to Tehran Imam Khomeini hospital .DNA was extracted from patients' blood samples by multiple PCR simultaneously containing different mutations were duplicated then the existence of mutation was evaluated by the strip technique. The genes mutation of G1691A in Factor V, G4070A in Factor V, G103T in Beta fibrinogen, -455G>A in the XIII factor and G20210A were identified 6.6, 45, 36, 40 and 3.3 respectively. Studies on the other population showed that frequency of examined mutations varies with other communities. Anyway, more samples are required in order to obtain more accurate statistics related to the frequency of mutations
Training time frame, a possible key to literature discrepancy on fasting athletes
There is a discrepancy in the literature as to whether fasting during Ramadan affects athletic performance. We suggest in this letter that the time frame for training is a confounding variable that has been missed in the literature. © 2016 The Author(s)
