    Educação sexual para jovens : uma busca alem do reconhecimento da sexualidade

    Atualmente a sociedade tem vivenciado muitas mudanças sociais, políticas, econômicas e culturais, consequentemente tem seus valores ligados à sexualidade e família alterados. O que desperta curiosidade e conhecimento para os adolescentes têm gerado desconforto, medo e ansiedade nos adultos, que evidenciam despreparo frente a própria sexualidade. Os meios de comunicação têm abordado insastifatoriamente este desencontro de gerações, exibindo dados quantitativos que causam polêmica em meio à sociedade mas, que poucas vezes se preocupam em informar de maneira aprofundada assuntos como a prevenção de doenças ou mesmo da gravidez indesejada. A formação de professores e outros profissionais também não têm oferecido embasamento sobre este tema, de modo que dificil trabalhar com os educandos. Assim sendo, procurou-se analisar artigos, pesquisas e trabalhos relativos à sexualidade para auxiliar, tanto pais, quanto outros adultos no sentido de uma melhor educação e compreensão do processo vivido com seus ?lhos. O estudo sistemático do material possibilitou avaliar a carência sofrida por nossa sociedade que desconhece o adolescente como adulto futuro e dele, muitas vezes, tenta esconder sua capacidade de prevenir e controlar problemas que comprometem sua vida saudável

    Chronic Kidney Disease as an Independent Risk for Long-Term Adverse Outcomes in Patients Hospitalized With Heart Failure in Japan

    Background: Previous studies have demonstrated that renal dysfunction is common in patients with heart failure (HF), but it is not known whether chronic kidney disease (CKD) is associated with increased risks of long-term adverse outcomes in unselected HF patients encountered in current routine clinical practice in Japan. Methods and Results: The Japanese Cardiac Registry of Heart Failure in Cardiology (JCARE-CARD) prospectively studied a broad sample of patients hospitalized with worsening HF and their outcomes with an average of 2.4 years of follow-up. The study cohort (n=2,013) were classified into 3 groups by estimated glomerular filtration rate (eGFR): ≥60 (n=579), 30-59 (n=1,025), and <30 ml · min-1 · 1.73 m-2 or patients with dialysis (n=409); 1,372 patients (70.3%) had an eGFR <60 ml · min-1 · 1.73 m-2 and 62 patients were treated with dialysis. The multivariable adjusted risk for all-cause death or rehospitalization increased with reduced eGFR; an adjusted hazard ratio (HR) 1.520 (95% confidence interval (CI) 1.186-1.949) for eGFR 30-59 ml · min-1 · 1.73 m-2 (P=0.001) and HR 2.566 (95%CI 1.885-3.492) for eGFR <30 ml · min-1 · 1.73 m-2 or patients with dialysis (P<0.001). Conclusions: CKD is common in HF and was independently associated with long-term adverse outcomes in a broad cohort of Japanese patients. (Circ J 2009; 73: 1442 - 1447

    Fertilidade de gemas de videiras 'Niagara Rosada' de acordo com o sistema de condução

    O experimento teve por objetivo avaliar a influência dos sistemas de condução espaldeira e pérgula, na fertilidade das gemas da videira ' Niagara Rosada' na Região de Dourados-MS, durante o período de maio a junho de 2004. Coletaram-se bacelos provenientes de 24 vinhedos de plantas da cultivar Niagara Rosada enxertadas sobre o porta-enxerto IAC 572 'Jales', conduzidas em espaldeira e pérgula, que se encontravam na fase produtiva. Foram realizadas as seguintes avaliações: número de primórdios de inflorescência/gema primária (NPI/GP), número total de gemas/gema composta (NTG/GC) e número de gemas necrosadas (NGN). O delineamento foi em blocos casualizados, com 12 repetições, no esquema fatorial 2 x 10, usando-se como fatores dois sistemas de condução, em espaldeira e em pérgula; e dez posições da gema na vara, isto é, da basal à décima gema. Os blocos consistiram de 12 vinhedos de plantas conduzidas em espaldeira e outros 12 em pérgula. As médias dos dados foram comparadas pelo Teste de Tukey, a 5% de probabilidade. Verificou-se que, nas videiras conduzidas em pérgula, as médias de fertilidade de gemas são maiores que as do sistema em espaldeira, e a fertilidade de gemas é crescente da base à porção terminal das varas, nos dois sistemas de condução analisados

    The prevalence of obstructive sleep apnea in Japanese asthma patients

    Abstract Background Obstructive sleep apnea (OSA) occurs more commonly in asthma patients than in the general population because these conditions share some comorbidities. In Japan, the prevalence of OSA in the general population is reported to be approximately 20%; however, few reports have described the prevalence of OSA in asthma patients. Furthermore, the characteristics of Japanese patients with OSA and asthma are not clear. Methods Adult asthma patients were recruited from the outpatient departments of our institution between August 31, 2017, and March 31, 2019. In all included patients, the presence and severity of OSA were evaluated by the Epworth Sleepiness Scale (ESS) and a home sleep test (HST) using portable polysomnography (PSG). The rate of coexisting OSA in asthma patients and the characteristics of those patients according to the severity of OSA were investigated. Results Fifty-three patients were included. OSA was detected in 36 (67.9%) patients (mild, n = 15; moderate, n = 14; and severe, n = 7). Patients with OSA had significantly higher body mass index, Brinkman index, apnea-hypopnea index (AHI), and 3% oxygen desaturation index (ODI) values in comparison to those without OSA, while the percentage of the predicted value of forced vital capacity (%FVC) and lowest SpO2 levels were significantly lower. As the severity of OSA increased, age, brain natriuretic peptide level, AHI, and 3%ODI increased, and in contrast, FVC, %FVC, forced expiratory volume in one second (FEV1), percentage of the predicted value of FEV1 (%FEV1), Epworth Sleepiness Scale (ESS), 3%ODI, and lowest SpO2 levels decreased. In particular, the fact that the ESS value was inversely correlated with the severity of OSA in our patients was different from the general characteristics of OSA. Moreover, the AHI value was negatively correlated with FVC, %FVC, FEV1, and %FEV1. BMI was the only independent factor for the presence of OSA, and for asthma severity (FEV1, % of predicted), there was a weak correlation with smoking history. Conclusions This is the first report to investigate the prevalence of OSA in Japanese asthma patients, using an HST. This study suggests that an HST should be performed in addition to the sleep interview for asthma patients with refractory disease, a low pulmonary function, advanced age, and high BMI because the more severe the OSA, the lower the ESS value may be

    Peripheral T cell lymphoma not otherwise specified (PTCL-NOS) presenting as an endobronchial lesion: Case report and literature review

    Peripheral T cell lymphoma not otherwise specified (PTCL-NOS) is a rare entity of lymphoma. We herein report an even rarer case of a 68-year-old male with PTCL-NOS presenting as an endobronchial lesion, and review previously published cases in the literature. Initially, he was referred to our hospital for further investigation of the right upper lobe consolidation on chest radiograph. Computed tomography and 18F-fludeoxyglucose positron emission tomography revealed a right hilar mass with obstruction of the main bronchus and submandibular, right axillary and mediastinal lymphadenopathy. Pathological examination of the biopsy specimens from of the endobronchial lesion and subcutaneous nodule revealed PTCL-NOS.
