多段階計算による溶媒和効果についての理論化学的研究:低精度計算によるサンプリングと高精度計算の組み合わせ

A 52-year-old man was referred to our hospital for best supportive care of malignant brain tumor(glioblastoma). He had intractable diarrhea due to enteral tube-feeding using liquid formula and severe peri-anal pain. He was applied to semi-solid diet with tube feeding. After two days, intractable diarrhea was stopped. State of stool was improved type 7 to type 4 or 5 by the Bristol Stool Scale. The duration of bed bound time decreased 9 hours to 2 hours in one day. He was able to discharge from hospital and receive medical care at home. Nutrition support using semi-solid diet with tube feeding might be effective for home medical care and improve the Quality of Life of patient

A69-year-old man was referred to our hospital for aluminum potassium sulfate and tannic acid (ALTA)sclerosing therapy for bleeding internal hemorrhoids. After two months from the first ALTA sclerosing therapy, he had recurrence with bleeding on defecation. The ALTA injection on the left anterior hemorrhoidal artery(one o’clock position)stopped bleeding. One o’clock position hemorrhoidal artery existed in 58.6% and called fourth hemorrhoidal artery. Aluminum potassium sulfate and tannic acid(ALTA)are able to induce noninvasive sclerosis and involution of hemorrhoids through initiation of inflammatory reaction. A77-year-old man was admitted to our hospital for an incarcerated hemorrhoid. We first performed the conservative treatment. After three weeks, we performed ALTA sclerosing therapy. Although he had necrosis at the injection site and an anoderm ulcer on the1st post-operative day, he had few of pain and fever. He was cured on the32nd post-operative day. Although he had recurrence on the306th post-operative day, we performed re-ALTA sclerosing therapy. After that, he had no relapse and had good quality of life. For incarcerated hemorrhoids, the ALTA sclerosing therapy might be performed after some period of the conservative treatment
Effect of nalfurafine hydrochloride on the basal pressure of the sphincter of Oddi in anesthetized rabbits
Background: Opioid analgesics, which are classified as μ-opioid receptor agonists, are known to induce spasms or contraction of the sphincter of Oddi (SO), thereby inducing or exacerbating biliary diseases such as biliary obstruction, gallbladder dysfunction, cholelithiasis, pancreatitis, biliary dyskinesia, cholangitis, and cholecystitis. However, effects of κ-opioid receptor agonists on SO contraction have not been clarified. In the present study, we investigated the effect of nalfurafine hydrochloride (nalfurafine), (E)-N-[17-(cyclopropylmethyl)-4,5α-epoxy-3,14-dihydroxymorphinan-6β-yl]-3-(furan-3-yl)-N-methylprop-2-enamide monohydrochloride, a selective κ-opioid receptor agonist, on spontaneous contraction of rabbit SO.Methods: SO contraction was measured using manometry in anesthetized rabbits. Rabbits were anesthetized with intravenous administration of 25 mg/kg sodium pentobarbital. An open tip catheter was inserted into the common bile duct toward the SO ampullae. Saline was perfused through the lumen of the open tip catheter at a constant rate of 6 ml/hr using a syringe pump. Nalfurafine, morphine, and pentazocine were intravenously (i.v.) administered and perfusion pressure was recorded.Results: Morphine (0.3 mg/kg, i.v.) and pentazocine (3 mg/kg, i.v.) were found to increase SO perfusion pressure, suggesting that these opioid analgesics may cause SO contraction. In contrast, nalfurafine (0.2 μg/kg, i.v.) decreased the perfusion pressure, indicating that this κ-opioid receptor agonist suppresses SO contraction.Conclusions: These findings suggest that nalfurafine is unlikely to induce or exacerbate biliary diseases and may be safely used in patients with these disorders

Mesenteric malignant lymphoma is comparative rare and has a poor prognosis in anadvanced case.Here, we report a patient with intestinal obstruction due to mesenteric malignantlymphoma treated successfully by gastrointestinal bypass operation, chemo-and radiotherapy.A 67-year-old man was admitted to our hospital because of abdominal pain andnausea. An abdominal CT scan revealed a huge tumorous lesion with a soft tissue density,which involved descending colon. Since the symptoms was persisted after conservativetreatment, gastrointestinal bypass procedures were surgically formed. During the laparotomy,an open incisional biopsy was performed. Histological examination showed non-Hodgkinlymphoma with diffuse, large-sized and B-cell type. This case was classified into high-risk groupacccording to International Prognostic Index (IPI). After the operation, 5 courses of combinedchemotherapy (THP-COP) and 30 Gy radiation were performed. These multidisciplinarytreatment is considered to be very effective. Since then complete response and goodpatient’s QOL has been attained for these 3.5 years
Establishing allowable value of Electric Conductivity for monitoring groundwater based on the benchmark dose concept and its application
Recently, a number of studies have pointed out that Electric Conductivity (EC) would be a very effective measure for monitoring the quality of ground water possibly polluted by sewage sludge or incineration ash leachate. However, few have explicitly determined the allowable limit of the EC value.
