    Corticospinal and reticulospinal contacts on cervical commissural and long descending propriospinal neurons in the adult rat spinal cord; evidence for powerful reticulospinal connections

    Descending systems have a crucial role in the selection of motor output patterns by influencing the activity of interneuronal networks in the spinal cord. Commissural interneurons that project to the contralateral grey matter are key components of such networks as they coordinate left-right motor activity of fore and hind-limbs. The aim of this study was to determine if corticospinal (CST) and reticulospinal (RST) neurons make significant numbers of axonal contacts with cervical commissural interneurons. Two classes of commissural neurons were analysed: 1) local commissural interneurons (LCINs) in segments C4-5; 2) long descending propriospinal neurons (LDPNs) projecting from C4 to the rostral lumbar cord. Commissural interneurons were labelled with Fluorogold and CST and RST axons were labelled by injecting the b subunit of cholera toxin in the forelimb area of the primary somatosensory cortex or the medial longitudinal fasciculus respectively. The results show that LCINs and LDPNs receive few contacts from CST terminals but large numbers of contacts are formed by RST terminals. Use of vesicular glutamate and vesicular GABA transporters revealed that both types of cell received about 80% excitatory and 20% inhibitory RST contacts. Therefore the CST appears to have a minimal influence on LCINs and LDPNs but the RST has a powerful influence. This suggests that left-right activity in the rat spinal cord is not influenced directly via CST systems but is strongly controlled by the RST pathway. Many RST neurons have monosynaptic input from corticobulbar pathways therefore this pathway may provide an indirect route from the cortex to commissural systems. The cortico-reticulospinal-commissural system may also contribute to functional recovery following damage to the CST as it has the capacity to deliver information from the cortex to the spinal cord in the absence of direct CST input

    Perceived Differences in Knowledge About Interactive Technology Between Young Adults and Their Parents

    This study was designed to investigate generational differences in knowledge about interactive technology (i.e., cell phones, social networking, email, video chat) between parents and their young adult children. Parents (n = 555) and young adults (n = 604) residing in the United States provided information about their knowledge in the use of interactive technology. Young adult children also reported their perceptions of their parents’ technological knowledge for the various technologies. Comparisons of young adult self-reported technological knowledge and their parents’ own reports of technological knowledge revealed that young adults were perceived to be much more knowledgeable than their parents (by both the young adults and their parents) regardless of the technology medium. The largest differences between parents and their young adult children were associated with newer interactive technologies, with the largest gap between parent and young adult knowledge in the area of social networking. Perceived differences between parents and their young adult children were smaller among the technologies that have been in use longer (such as such as e-mail), and larger among the newer modes of interactive technology (e.g., video chat)

    Physical Drivers of Phytoplankton Bloom Initiation in the Southern Ocean's Scotia Sea

    Abstract: The Scotia Sea is the site of one of the largest spring phytoplankton blooms in the Southern Ocean. Past studies suggest that shelf‐iron inputs are responsible for the high productivity in this region, but the physical mechanisms that initiate and sustain the bloom are not well understood. Analysis of profiling float data from 2002 to 2017 shows that the Scotia Sea has an unusually shallow mixed‐layer depth during the transition from winter to spring, allowing the region to support a bloom earlier in the season than elsewhere in the Antarctic Circumpolar Current. We compare these results to the mixed‐layer depth in the 1/6° data‐assimilating Southern Ocean State Estimate and then use the model output to assess the physical balances governing mixed‐layer variability in the region. Results indicate the importance of lateral advection of Weddell Sea surface waters in setting the stratification. A Lagrangian particle release experiment run backward in time suggests that Weddell outflow constitutes 10% of the waters in the upper 200 m of the water column in the bloom region. This dense Weddell water subducts below the surface waters in the Scotia Sea, establishing a sharp subsurface density contrast that cannot be overcome by wintertime convection. Profiling float trajectories are consistent with the formation of Taylor columns over the region's complex bathymetry, which may also contribute to the unique stratification. Furthermore, biogeochemical measurements from 2016 and 2017 bloom events suggest that vertical exchange associated with this Taylor column enhances productivity by delivering nutrients to the euphotic zone

    Return to Sport and Athletic Function in an Active Population After Primary Arthroscopic Labral Reconstruction of the Hip

    Background: Labral reconstruction has been advocated as an alternative to debridement for the treatment of irreparable labral tears, showing favorable short-term results. However, literature is scarce regarding outcomes and return to sport in the nonelite athletic population. Purpose: To report minimum 1-year clinical outcomes and the rate of return to sport in athletic patients who underwent primary hip arthroscopy with labral reconstruction in the setting of femoroacetabular impingement syndrome and irreparable labral tears. Study Design: Case series; Level of evidence, 4. Methods: Data were prospectively collected and retrospectively analyzed for patients who underwent an arthroscopic labral reconstruction between August 2012 and December 2017. Patients were included if they identified as an athlete (high school, college, recreational, or amateur); had follow-up on the following patient-reported outcomes (PROs): modified Harris Hip Score (mHHS), Nonarthritic Hip Score (NAHS), Hip Outcome Score–Sport Specific Subscale (HOS-SSS), and visual analog scale (VAS); and completed a return-to-sport survey at 1 year postoperatively. Patients were excluded if they underwent any previous ipsilateral hip surgery, had dysplasia, or had prior hip conditions. The proportions of patients who achieved the minimal clinically important difference (MCID) and patient acceptable symptomatic state (PASS) for mHHS and HOS-SSS were calculated. Statistical significance was set at P =.05. Results: There were 32 (14 females) athletes who underwent primary arthroscopic labral reconstruction during the study period. The mean age and body mass index of the group were 40.3 years (range, 15.5-58.7 years) and 27.9 kg/m2 (range, 19.6-40.1 kg/m2), respectively. The mean follow-up was 26.4 months (range, 12-64.2 months). All patients demonstrated significant improvement in mHHS, NAHS, HOS-SSS, and VAS (P \u3c.001) at latest follow-up. Additionally, 84.4% achieved MCID and 81.3% achieved PASS for mHHS, and 87.5% achieved MCID and 75% achieved PASS for HOS-SSS. VAS pain scores decreased from 4.4 to 1.8, and the satisfaction with surgery was 7.9 out of 10. The rate of return to sport was 78%. Conclusion: At minimum 1-year follow-up, primary arthroscopic labral reconstruction, in the setting of femoroacetabular impingement syndrome and irreparable labral tears, was associated with significant improvement in PROs in athletic populations.     The journey through care : palliative care for children and young people, and their families : what? when? how? : a realist inquiry

    Background: The World Health Organisation defines palliative care as “an approach to care that aims to improve the quality of life for people living facing the problems associated with life-threatening illness, through the prevention and relief of suffering”. The number of children and young people with life limiting and life-threatening conditions is rising. All could potentially benefit from palliative care, but there are significant inequalities in the provision of such care to children internationally, including the availability of specialist paediatric palliative care services. Research Aims: To examine the delivery of healthcare, including palliative care, for children with life-limiting and life-threatening conditions and their families. The study uses a realist approach to provide understanding into how palliative care is delivered most effectively, and when, leading to policy relevant recommendations. Methods: 1. Development of a programme theory (PT) through systematic and realist literature reviews. 2. 41 serial interviews with 31 participants from 14 families; 10 children with life-limiting or life-threatening conditions and 21 family members. 3. Four focus groups with children’s palliative care professionals. 4. Thematic / realist analysis to describe the hidden mechanisms (M), triggered in certain contexts (C), to produce desired outcomes (O). Context-Mechanism-Outcome configurations (CMOCs) are used to refine and refute the PT. Findings: Children and their families are vulnerable experts, negotiating a healthcare system that can be rigid and fragmented (C). The delivery of palliative care depends on interpersonal relationships with healthcare professionals (C), who are able to bear witness to the child and family situation (M), underpinned by trust and respect (M). Important child and family outcomes include feeling heard and supported (O). In organisations, an environment that values these relationships and legitimises palliative care as an approach (C) through leadership and role modelling (M) as well as the development of specialist services (C), leads to more equitable palliative care (O). Conclusion: The realist approach provides increased understanding and description of important child and family outcomes that underpin policy goals in palliative care, achieved in certain contexts. Future service and commissioning models should propose a whole system approach. Achieving this requires the unwavering commitment of system leaders, recognition of the unique situations of children and families, and individualised palliative care. Service design should place greater emphasis on the need for trusted relationships, should nurture and support professionals who have the motivation and capacity to provide palliative care, and should integrate specialist paediatric palliative care effectively into existing services

    Application of Structured Decision Making to Wildlife Management in Montana

    Good decision-making is essential to conserving wildlife populations. Whereas there may be multiple ways to address a problem, perfect solutions rarely exist. Managers are therefore tasked with identifying optimal decisions that will best achieve desired outcomes. Structured decision making (SDM) is a method of decision analysis used to identify the most effective, efficient, and realistic optimal decisions while accounting for values and priorities of the decision maker. The stepwise process includes identifying the management problem, defining objectives for solving the problem, developing alternative approaches to achieve the objectives, and formally evaluating which alternative is most likely to accomplish the objectives. The SDM process can be more effective than informal decision-making because it provides a transparent way to quantitatively evaluate decisions for addressing multiple management objectives while incorporating science, uncertainty, and risk tolerance. We illustrate the application of this process to management needs, including an SDM-based decision tool developed to identify optimal decisions for proactively managing risk of pneumonia epizootics in bighorn sheep (Ovis canadensis). Pneumonia epizootics are a major challenge for managers, including in terms of knowing how or when to manage risk. The decision tool facilitates analysis of alternative decisions for how to manage herds based on predictions from a risk model, herd-specific objectives, and predicted costs and benefits of each alternative. Managers can be confident resulting decisions are most effective, efficient, and realistic because they explicitly account for important considerations managers implicitly weigh when making decisions, including competing management objectives, uncertainty in potential outcomes and risk tolerance

    Seasonal Variation in Sex Ratios Provides Developmental Advantages in White-tailed Deer, Odocoileus virginianus

    Since Trivers and Willard first proposed their hypothesis concerning the adaptive advantages of producing a particular offspring sex in relation to maternal condition in 1973, it has been at the forefront of scientific research concerning sex ratios with most subsequent studies focusing on maternal condition as a key contributor to variations in sex ratios. Another factor that could greatly influence sex ratios, although has been only infrequently examined in mammalian species, is birth date. We investigated how birth date influenced offspring sex ratios in White-tailed Deer (Odocoileus virginianus). Since date of birth can greatly influence an individual’s fitness and reproductive success, we suggest that birth date may be an alternative strategy in determining offspring sex ratios. Since it has been shown that the lifetime reproductive fitness of a mother can be increased by producing a particular sex during a particular time, we hypothesized that more male offspring should be born earlier in the season due to their increased reproductive potential from being born at this time. Offspring born earlier will have a head start in development and therefore have greater potential for increased body size and dominance later in life, traits that greatly influence male reproductive success. In this study, we found that maternal condition did not affect offspring sex ratio in a captive population of White-tailed Deer in Michigan; however, birth date did. We found that more males tended to be born during the second and fourth birthing periods, while more females were born during the first, third and fifth periods. In addition, we found that males born earlier in the season had greater mass the following spring than those born later, a trend that was not as dramatic in females. These results lend moderate support to our hypothesis that in White-tailed Deer offspring sex will tend to vary according to timing of birth and relative reproductive advantages gained by a particular sex being born at that time

    Preschool Participation and BMI at Kindergarten Entry: The Case for Early Behavioral Intervention

    Preschool years (ages 3–5) are a critical period in growth and development. Emerging studies suggest that preschool attendance may be linked to future weight, and perhaps obesity. This study examined relationships between public preschool attendance, demographic variables, and weight at kindergarten entry. Participants included 2,400 children entering kindergarten in 2006. Height and weight were used to calculate a child's BMI category based on CDC norms. At kindergarten entry, 17% of participants were overweight, and 18% were obese. Children attending a public preschool were at an increased risk for overweight (OR = 1.06) and obesity (OR = 1.34) at kindergarten entry, χ2(2) = 6.81, P = .03 relative to children who did not attend preschool. No significant trends relationships between demographics and weight status were found, but demographic variables are summarized descriptively. Policy and clinical implications are provided
