Neurodegeneration in newborn rats following propofol anesthesia
In verschiedenen Untersuchungen konnte gezeigt werden, dass Substanzen, welche
ĂĽber den GABAA-Agonismus oder den NMDA-Antagonismus wirken, im Gehirn der
neugeborenen Ratte zu neurodegenerativen Effekten fĂĽhren. Dies wurde unter
anderem fĂĽr Barbiturate und Antiepleptika publiziert. So konnten Jevtovic-
Todorovic et al. zeigen, das eine Anästhesie mit Isofluran, Lachgas und
Midazolam im Tiermodel zu einem ausgeprägten Verlust von Neuronen sowie
persistierenden Gedächtnis- und Lerndefiziten führt. Es konnte außerdem durch
die Arbeitsgruppe um Dzietko gezeigt werden, dass die Applikation von
rekombinantem Erythropoietin bei sieben Tage alten Ratten die durch den NMDA-
Rezeptor-Antagonisten MK801 induzierte Apoptose, reduziert. Bei der Anästhesie
von Früh- und Neugeborenen wird häufig Propofol benutzt, obwohl es für diese
Altersklassen keine Zulassung gibt. Propofol wirkt ĂĽber eine Aktivierung der
GABAA-Rezeptoren und eine Hemmung der NMDA-Rezeptoren. Deshalb wird in der
vorliegenden Arbeit untersucht, ob die Applikation von Propofol bei der
neugeborenen Ratte zu einer Neurodegeneration führt, und ob die zusätzliche
Applikation von Erythropoietin neuroprotektive Effekte hat. Bei 3 bis 21 Tage
alten Wistar-Ratten wurde eine Anästhesie durch die intraperitoneale Gabe von
Propofol in unterschiedlichen Dosierungen durchgefĂĽhrt. Nach 24 Stunden
Überlebenszeit wurden die Tiere getötet und die Rattengehirne wurden mit der
DeOlmos-Kupfersilberfärbung oder der TUNEL-Färbung ausgewertet. Die numerische
Dichte der degenerierten Zellen wurde in 17 Hirnregionen mittels der
stereologischen Dissektionsmethode nach Gundersen bestimmt. Pro Tier wurde ein
kumulativer Schädigungsscore summiert und statistisch ausgewertet. Die
Kontrolltiere wurden wie die Versuchstiere behandelt, jedoch erhielten sie
kein Medikament. Um zu untersuchen, ob die zusätzliche Gabe von Erythropoietin
zu einer Reduktion der neurotoxischen Effekte im sich entwickelnden
Rattengehirn während einer Narkose mit Propofol führt, erhielt eine Gruppe
5.000 IE und eine weitere Gruppe 20.000 IE Erythropoietin pro kg
Körpergewicht. Die intraperitoneale Gabe von Erythropoietin erfolgte vor der
Narkose mit 3 x 30 mg Propofol pro kg Körpergewicht. In den Hirnschnitten der
am sechsten Lebenstag mit 3 x 30 mg Propofol pro kg Körpergewicht behandelten
Ratten, fand sich nach 24 Stunden Überlebenszeit eine signifikant erhöhte
Anzahl degenerierter Zellen pro mm3 im Vergleich zu den Kontrolltieren, welche
kein Medikament erhielten. Es zeigt sich eine Dosisabhängigkeit des Ausmaßes
der neurodegenerativen Wirkung nach der Applikation von Propofol. Die
Dosierung von 3 x 20 mg Propofol pro kg Körpergewicht führt zu keinem
Unterschied in der Neurodegeneration im Vergleich zu den Kontrollen. Die
Dosierung von 3 x 10 mg Propofol pro kg Körpergewicht verursachte eine
signifikant niedrigere Neurodegeneration im Vergleich zu den Kontrolltieren.
Zusätzlich konnte eine Altersabhängigkeit der Wirkung von Propofol auf das
sich entwickelnde Gehirn festgestellt werden. Bei den sechs Tage alten Tieren
zeigte sich nach der Gabe von 3 x 30 mg Propofol pro kg Körpergewicht eine
signifikant erhöhte Anzahl degenerierter Zellen und bei den 21 Tage alten
Tieren zeigt sich kein signifikanter Unterschied im Vergleich zu
Kontrolltieren. Die Tiere, die eine Dosis von 5.000 IE Erythropoietin pro kg
Körpergewicht vor der Gabe von Propofol erhielten, zeigten eine signifikant
niedrigere Anzahl degenerierter Zellen im Vergleich zu den nur mit Propofol
behandelten Tieren. Dagegen zeigten die Tiere, die 20.000 IE Erythropoietin
pro kg Körpergewicht zusätzlich zum Propofol erhielten, keinen signifikanten
Unterschied bezĂĽglich der Anzahl degenerierter Zellen (Summenscore) im
Vergleich zu den nur mit Propofol behandelten Tieren. Zusammenfassend kann mit
der vorliegenden Arbeit gezeigt werden, dass die Applikation von Propofol im
sich entwickelnden Gehirn der Ratte zu dosis- und altersabhängigen
neurodegenerativen Effekten fĂĽhrt. Eine Steigerung der Dosis fĂĽhrt zu einer
Zunahme der degenerierten Zellen im Gehirn. Zunehmendes Alter fĂĽhrt zu einer
Abnahme der Empfindlichkeit gegenüber Propofol. Die zusätzliche Gabe niedriger
Erythropoietin-Dosen hob die neurodegenerativen Effekte auf. In wieweit sich
dieses Tiermodell auf den Mensch übertragen lässt und ob die Zunahme
degenerierter Zellen klinische Folgen in der Entwicklung der Hirnleistungen
hat, lässt sich nur durch weitere Studien, insbesondere klinische
Verhaltensstudien, klären.Recently, it has been shown that the combined application of GABA-enhancing
and NMDA-blocking agents (midazolame, nitrous oxide and isoflurane) induces
widespread neurodegeneration in the brains of newborn rats. Propofol (6,2
Diisopropylphenol) acts via GABAA agonism as well as NMDA antagonism. The use
of propofol 1% for induction and maintenance of anesthesia is approved in
Germany for children older than 1 month. Despite of these restrictions off-
label use is common for anesthesia even in preterm infants and neonates. Three
to 21 day old Wistar rats received intraperitoneal injections with various
dosages of propofol and were separated from their mother during the
experimental phase. In every litter animals were randomized either for
anesthesia or controls. To maintain body temperature and prevent hypothermia,
animals were placed on a heating device. During anesthesia respiratory
frequency and skin color were observed to detect apnoea and hypoxia. Verum and
control animals received injections of PĂ„D II (crystalloid/glucose solution)
preventing hypoglycemia and hypovolemia. Control animals were separated from
their mother for the same period. After anesthesia animals were observed for
another 90 min until they were awake enough to be returned to their mother.
Animals were killed for perfusion and fixation with an injection of an
overdose of chloral hydrate 24 h after starting anesthesia. Brains were
examined histopathologically using the De Olmos cupric silver staining.
Degenerating cells were determined in sections of 17 different parts of the
brain by a stereological dissector. Additionally, a summation score of the
density of apoptotic cells was calculated for every brain. Six-day-old rats
received propofol doses of 30 mg/kg body weight every 90 min up to a
cumulative dose of 90 mg/kg. Compared to controls the brains showed a
significant higher neurodegenerative summation score. Additionally a dose
dependent effect could be demonstrated. The six-day-old rats receiving 3 x 20
mg propofol per kg body weight showed no difference in the neurodegeneration
comparised with the controls. The dosage of 3 x 10 mg propofol per kg body
weight causes a significant lower neurodegenerative summation score compared
to controls. Furthermore an age dependency has been demonstrated. Those six-
day-old rats receiving 30 mg/kg body weight every 90 min up to a cumulative
dose of 90 mg/kg, showed a significant higher neurodegenerative summation
score when compared to controls. The 21 day old rats that were treated with 3
x 30 mg propofol per kg body weight showed no difference in the
neurodegeneration compared to the controls. The significance of these results
for clinical anesthesia has not been completely clarified. Future studies
should focus on identification of safe anesthetic strategies for the
developing brain
Psychosocial work stress and parent-child bonding during the COVID-19 pandemic: clarifying the role of parental symptoms of depression and aggressiveness
Abstract Background Parental work stress and impaired mental health seem to have intensified during the current COVID-19 pandemic. Both can have a negative impact on parent-child bonding: psychosocial work stress in the course of a spillover effect from work to family and symptoms of impaired mental health as part of a crossover effect from parent to child. This potentially affects the child’s development in the long term. Method This cross-sectional study examined the relationship between psychosocial work stress and parent-child bonding during the early COVID-19 pandemic (May–June 2020). Symptoms of depression and aggressiveness were considered as mediators of this relationship. The sample consisted of employees in Eastern Germany (n = 380; 42.9% mothers, 57.1% fathers), aged 24–55 years, with children aged 0–36 months. Results In the total sample, an association was only found after adjusting for potential confounders, indicating that higher psychosocial work stress is associated with weaker bonding between the parent and child (β = 0.148, p = .017, 95% CI [0.566, 5.614]). The separate analyses for mothers and fathers did not reveal a statistically significant relationship between psychosocial work stress and parent-child bonding. In the total sample, the higher the psychosocial work stress was, the higher were the parental symptoms of depression (β = 0.372, p < .001, 95% CI [3.417, 5.696]) and aggressiveness β = 0.254, p < .001, 95% CI [1.008, 3.208]). The mental health symptoms in turn were related to weaker parent-child bonding (symptoms of depression β = 0.320, p < .001, 95% CI [0.345, 0.749]; symptoms of aggressiveness β = 0.394, p < .001, 95% CI [0.697, 1.287]). The results furthermore suggested that parental mental health symptoms mediate the association between psychosocial work stress and parent-child bonding (symptoms of depression, ab = 2.491, 95% CI [1.472, 3.577] and of aggressiveness, ab = 2.091, 95% CI [1.147, 3.279]). The mediation effect was also found in the separate analyses for the mothers and fathers. Discussion The results of this study during the early COVID-19 pandemic in Germany highlight the importance of prevention as well as intervention measures in relation to psychosocial work stress that may play a debilitating role in the context of family relationships. In addition, the results suggest that both employers and employees should be made aware of the importance of psychosocial work stress, as it can have a negative impact on mental health, which in turn may have a major influence on family relationships
Utilizing Moist or Dry Swabs for the Sampling of Nasal MRSA Carriers? An <i>In Vivo</i> and <i>In Vitro</i> Study
<div><p>This study investigates the quantitative bacterial recovery of Methicillin-resistant S<i>taphylococcus aureus</i> (MRSA) in nasal screenings by utilizing dry or moistened swabs within an <i>in vivo</i> and an <i>in vitro</i> experimental setting. 135 nasal MRSA carriers were each swabbed in one nostril with a dry and in the other one with a moistened rayon swab. Quantitative bacterial recovery was measured by standard viable count techniques. Furthermore, an anatomically correct artificial nose model was inoculated with a numerically defined suspension of MRSA and swabbed with dry and moistened rayon, polyurethane-foam and nylon-flocked swabs to test these different settings and swab-materials under identical laboratory conditions. <i>In vivo</i>, quantities of MRSA per nostril in carriers varied between <10<sup>1</sup> and >10<sup>7</sup> colony forming units, with a median of 2.15x10<sup>4</sup> CFU. However, no statistically significant differences could be detected for the recovery of MRSA quantities when swabbing nasal carriers with moist or dry rayon swabs. <i>In vitro</i> testing confirmed the <i>in vivo</i> data for swabs with rayon, polyurethane and nylon-flocked tips, since pre-moistening of swabs did not significantly affect the quantities of retrieved bacteria. Therefore, pre-moistening of swabs prior to nasal MRSA sampling provides no advantage in terms of recovering greater bacterial quantities and therefore can be omitted. In addition, this situation can be mimicked in an <i>in vitro</i> model, thereby providing a useful basis for future <i>in vitro</i> testings of new swab types or target organisms for screening approaches.</p></div
