Confronting the opioid crisis: Practical pain management and strategies: AOA 2018 critical issues symposium
The United States is in the midst of an opioid crisis. Clinicians have been part of the problem because of overprescribing of narcotics for perioperative pain management. Clinicians need to understand the pathophysiology and science of addiction to improve perioperative management of pain for their patients. Multiple modalities for pain management exist that decrease the use of narcotics. Physical strategies, cognitive strategies, and multimodal medication can all provide improved pain relief and decrease the use of narcotics. National medical societies are developing clinical practice guidelines for pain management that incorporate multimodal strategies and multimodal medication. Changes to policy that improve provider education, access to naloxone, and treatment for addiction can decrease narcotic misuse and the risk of addiction
Benford’s law predicted digit distribution of aggregated income taxes: the surprising conformity of Italian cities and regions
The yearly aggregated tax income data of all, more than 8000, Italian municipalities are analyzed for a period of five years, from 2007 to 2011, to search for conformity or
not with Benford’s law, a counter-intuitive phenomenon observed in large tabulated data where the occurrence of numbers having smaller initial digits is more favored
than those with larger digits. This is done in anticipation that large deviations from Benford’s law will be found in view of tax evasion supposedly being widespread
across Italy. Contrary to expectations, we show that the overall tax income data for all these years is in excellent agreement with Benford’s law. Furthermore, we also
analyze the data of Calabria, Campania and Sicily, the three Italian regions known for strong presence of mafia, to see if there are any marked deviations from Benford’s
law. Again, we find that all yearly data sets for Calabria and Sicily agree with Benford’s law whereas only the 2007 and 2008 yearly data show departures from the law for Campania. These results are again surprising in view of underground
and illegal nature of economic activities of mafia which significantly contribute totax evasion. Some hypothesis for the found conformity is presented
Data on the annual aggregated income taxes of the Italian municipalities over the quinquennium 2007–2011
This dataset contains the annual aggregated income taxes of all the Italian municipalities over the years 2007-2011. Data are clustered over the Italian regions and provinces. The source of the data is the Italian Ministry of Economics and Finance. The administrative variations in Italy over the quinquennium have been taken into account. Data are useful to understand the economic structure of Italy at the microscopic level of municipalities. They can serve also for making comparisons between economical aspects and other features of the Italian cities
Medipix3 Demonstration and understanding of near ideal detector performance for 60 & 80 keV electrons
In our article we report first quantitative measurements of imaging
performance for the current generation of hybrid pixel detector, Medipix3, as
direct electron detector. Utilising beam energies of 60 & 80 keV, measurements
of modulation transfer function (MTF) and detective quantum efficiency (DQE)
have revealed that, in single pixel mode (SPM), energy threshold values can be
chosen to maximize either the MTF or DQE, obtaining values near to, or even
exceeding, those for an ideal detector. We have demonstrated that the Medipix3
charge summing mode (CSM) can deliver simultaneous, near ideal values of both
MTF and DQE. To understand direct detection performance further we have
characterized the detector response to single electron events, building an
empirical model which can predict detector MTF and DQE performance based on
energy threshold. Exemplifying our findings we demonstrate the Medipix3 imaging
performance, recording a fully exposed electron diffraction pattern at 24-bit
depth and images in SPM and CSM modes. Taken together our findings highlight
that for transmission electron microscopy performed at low energies (energies
<100 keV) thick hybrid pixel detectors provide an advantageous and alternative
architecture for direct electron imagin

Dynamic multifactor hubs interact transiently with sites of active transcription in Drosophila embryos.
The regulation of transcription requires the coordination of numerous activities on DNA, yet how transcription factors mediate these activities remains poorly understood. Here, we use lattice light-sheet microscopy to integrate single-molecule and high-speed 4D imaging in developing Drosophila embryos to study the nuclear organization and interactions of the key transcription factors Zelda and Bicoid. In contrast to previous studies suggesting stable, cooperative binding, we show that both factors interact with DNA with surprisingly high off-rates. We find that both factors form dynamic subnuclear hubs, and that Bicoid binding is enriched within Zelda hubs. Remarkably, these hubs are both short lived and interact only transiently with sites of active Bicoid-dependent transcription. Based on our observations, we hypothesize that, beyond simply forming bridges between DNA and the transcription machinery, transcription factors can organize other proteins into hubs that transiently drive multiple activities at their gene targets.Editorial noteThis article has been through an editorial process in which the authors decide how to respond to the issues raised during peer review. Wheat Genomics: Present Status and Future Prospects
Wheat (Triticum aestivum L.), with a large genome (16000 Mb) and high proportion
(∼80%) of repetitive sequences, has been a difficult crop for genomics research. However, the availability of extensive cytogenetics stocks has been an asset, which facilitated
significant progress in wheat genomic research in recent years. For instance, fairly dense
molecular maps (both genetic and physical maps) and a large set of ESTs allowed
genome-wide identification of gene-rich and gene-poor regions as well as QTL including
eQTL. The availability of markers associated with major economic traits also allowed
development of major programs on marker-assisted selection (MAS) in some countries,
and facilitated map-based cloning of a number of genes/QTL. Resources for functional
genomics including TILLING and RNA interference (RNAi) along with some new
approaches like epigenetics and association mapping are also being successfully used for
wheat genomics research. BAC/BIBAC libraries for the subgenome D and some
individual chromosomes have also been prepared to facilitate sequencing of gene space.
In this brief review, we discuss all these advances in some detail, and also describe
briefly the available resources, which can be used for future genomics research in this
important crop
Complex joint probabilities as expressions of determinism in quantum mechanics
The density operator of a quantum state can be represented as a complex joint
probability of any two observables whose eigenstates have non-zero mutual
overlap. Transformations to a new basis set are then expressed in terms of
complex conditional probabilities that describe the fundamental relation
between precise statements about the three different observables. Since such
transformations merely change the representation of the quantum state, these
conditional probabilities provide a state-independent definition of the
deterministic relation between the outcomes of different quantum measurements.
In this paper, it is shown how classical reality emerges as an approximation to
the fundamental laws of quantum determinism expressed by complex conditional
probabilities. The quantum mechanical origin of phase spaces and trajectories
is identified and implications for the interpretation of quantum measurements
are considered. It is argued that the transformation laws of quantum
determinism provide a fundamental description of the measurement dependence of

Results and prospects on registration of reflected Cherenkov light of EAS from cosmic particles above 10^{15} eV
We give an overview of the SPHERE experiment based on detection of reflected
Vavilov-Cherenkov radiation (Cherenkov light) from extensive air showers in the
energy region E>10^{15} eV. A brief history of the reflected Cherenkov light
technique is given; the observations carried out with the SPHERE-2 detector are
summarized; the methods of the experimental datasample analysis are described.
The first results on the primary cosmic ray all-nuclei energy spectrum and mass
composition are presented. Finally, the prospects of the SPHERE experiment and
