    Experimental Demonstration of Adaptive Quantum State Estimation

    The first experimental demonstration of an adaptive quantum state estimation (AQSE) is reported. The strong consistency and asymptotic efficiency of AQSE have been mathematically proven [ A. Fujiwara J. Phys. A 39 12489 (2006)]. In this Letter, the angle of linear polarization of single photons, the phase parameter between the right and the left circularly polarization, is estimated using AQSE, and the strong consistency and asymptotic efficiency are experimentally verified.     Candidates for Intensive Local Treatment in cIIIA-N2 Non-Small Cell Lung Cancer: Deciphering the Heterogeneity

    PurposeThe purpose of this study was to refine the heterogeneous clinical stage IIIA non-small cell lung cancer (NSCLC) with N2 nodes status (cIIIA-N2) by clinicopathological characteristics before treatment.Methods and MaterialsWe analyzed data of consecutive patients with cIIIA-N2 NSCLC diagnosed between 1997 and 2010 and treated by chemoradiation therapy (CRT). The appearance of the mediastinal lymph nodes (MLNs) was classified into discrete or infiltrative according to the criteria proposed by the American College of Chest Physicians. In addition, the extent of MLN involvement (MLNI) was classified as limited (close to the primary tumor) or extensive (including upper MLNI in the case of tumors in the lower lobes and vice versa).ResultsA total of 148 patients with cIIIA-N2 NSCLC was treated by CRT. The patient characteristics were as follows: males: 118; females: 30; median age: 62 years; appearance of the involved MLNs: 85 discrete, 63 infiltrative; extent of MLNI: 82 limited, 66 extensive; histology: 36 squamous, 112 nonsquamous. The median progression-free survival (PFS) and median overall survival (OS) in the entire subject population were 9.9 and 34.7 months, respectively. A discrete appearance of the involved MLNs and a limited extent of MLNI contributed significantly to a better PFS and OS. The percentages of cases with relapses within the irradiated field classified according to the characteristics of the MLNs were as follows; appearance of the MLNs (24.6% discrete, 18.9% infiltrative); extent of MLNI (25.9 limited, 17.9% extensive).ConclusionsThose with a discrete appearance of the involved MLNs and a limited extent of MLNI at diagnosis could show relatively more favorable outcomes and could be candidates for multimodality therapy

    Rupture and Rhythm: A Phenomenology of National Experiences

    This article investigates how people make sense of ruptures in the flow of everyday life as they enter new experiential domains. Shifts in being-in-time create breaks in the natural attitude that offer the opportunity to register national—or, for example, religious, gender, or class—experiences. People interpret ruptures in perception and proprioception by drawing connections with domains in which similar or contrasting kinds of disruption are evident. Normalizing the transition, rhythm—as both cadence and overall flow—helps people adjust to new circumstances, align action, and smooth subsequent ruptures. Based on extensive qualitative fieldwork, I examine the specific case of how novice and experienced tea ceremony practitioners in Japan move into, interpret, and normalize action within tea spaces

    Recurrent idiopathic pulmonary hemosiderosis after long-term remission presented with Sjogren's syndrome: Idiopathic no more?

    We report a case of recurrent idiopathic pulmonary hemosiderosis after a long-term remission presented with Sjögren's syndrome. The patient was diagnosed with IPH due to repeated pneumonia and blood sputum in his childhood. He was admitted to our hospital due to exertional dyspnea and dry cough with bilateral ground-glass opacity in chest computed tomography at the age of 32. Video-assisted thoracoscopic surgery was performed and the specimens showed nonspecific interstitial pneumonia pattern with diffuse, chronic alveolar hemorrhage, suggesting recurrence of IPH. He was also diagnosed with Sjögren's syndrome. Further immunological studies will reveal the pathogenesis of IPH. Keywords: Idiopathic pulmonary hemosiderosis, Sjögren's syndrom

    The psychological burden against nursing for physical complications held by nurses, without working experience outside of psychiatric ward

    本研究では,大学病院に勤務する精神科病棟以外での勤務経験がない看護師が抱く身体合併症看護に対する心理的負担について帰納的記述型研究を行った.その結果,【経験・知識・技術の不足による不安】【正しいアセスメントや身体ケアへの自信のなさ】【実践後に残る気がかり】【他スタッフへの懸念】【身体を看ることへのプレッシャー】【精神症状への対応とのジレンマ】の6つのカテゴリーが抽出された.精神科病棟以外での勤務経験がない看護師は,身体合併症看護の知識・技術,経験不足に加え,精神症状との兼ね合いや他スタッフとの関係性にも心理的負担を抱えながら身体合併症看護を実践していることが示された.In this study, inductive description type study was conducted on the psychological burden against nursing for physical complications held by nurses, working in university hospital, without working experience outside of psychiatric ward. As a result, six categories were extracted: [anxieties due to lack of experience/knowledge/technique], [lack of confidence on proper assessment and body care], [residual fear after practice], [concern about other staff], [pressure towards nursing body] and [dilemma with management for mental symptoms]. The study indicated that nurses without working experience outside of psychiatric ward practice nursing for physical complication, while holding psychological burden on balance with mental symptoms and relationship with other staff in addition to lack of knowledge, technique and experience for nursing of physical complication
