    Examining Organizational Justice in the Context of LMX and the Effect of LMX on Trust and Job Performance

    Services marketing and organizational behavior researchers have studied factors that have a positive influence on employees’ job performance. They viewed that the manager’s role in dealing with his/her subordinates is critical in producing the desired outcomes such as employee job performance. This study based on organizational justice and Leader-Member Exchange (LMX) theory, presents and tests a model, in which relationships among justice, LMX, trust, and job performance are examined. More specifically, the study examines the impact of organizational justice on the quality of LMX and the effect of LMX on employees’ attitudinal and behavioral aspects (trust and job performance). The study employs four dimensions of justice, two bases of trust and two types of job performance. By using a dimensional approach, the study attempts to investigate and comprehend dynamic and complex relationships among the variables. Using the data collected from restaurant employees in South Korea, the study finds that all dimensions of justice have a positive influence on the quality of LMX. LMX plays a significant role in affecting two bases of trust (cognitive trust and affective trust) and two dimensions of job performance (task performance and helping behavior). The study also found that not cognitive trust but affective trust influences task performance. The finding that distributive justice explains the largest portion of the variance in the quality of LMX suggests that distributive justice plays the most important role in building and maintaining the quality of manager-employee relationship. Employees care about fair distribution of the resources and outcomes that reflect the efforts they put into the work. The importance of quality of leader-member exchange is highlighted in this study. The high-quality relationship developed through the organization’s institutionalization of justice helps employees have trust in the leaders. It seems that justice is a mechanism through which managers and employees develop a highquality relationship. The study also found that affective trust has a significant positive effect on task performance, suggesting instilling and nurturing trust among employees is critical for improving their job performance

    Readmissions following elective radical total gastrectomy for early gastric cancer: A case-controlled study

    AbstractBackgroundReadmission after gastrectomy is one of the factors that reflect quality of life. Therefore, we analyzed the several factors related to readmissions after total gastrectomy for early gastric cancer.MethodsFrom January 2002 through December 2009, 102 consecutive patients who underwent radical total gastrectomy for early gastric cancer were enrolled in this study. We evaluated the incidence, cause, time point, and type of treatment for readmission after discharge; we compared the readmission and non-readmission groups in regard to clinicopathologic features and postoperative outcomes.ResultsThe readmission rate during the five years after total gastrectomy was 22 of 102 (21.6%). The most common cause for readmission was esophagojejunostomy stricture (5 cases). The treatment given for 31 readmissions included 23 conservative therapies, 3 radiologic or endoscopic interventions, and 5 re-operations. No significant differences were detected in the clinicopathologic feature, postoperative outcomes, or 5-year survival rates between the readmission and non-readmission group. No specific risk factor was found to be associated with readmission.ConclusionAlthough we could not determine a specific risk factor associated with readmission after radical total gastrectomy, prevention of readmission by evaluating the causes and treatments after radical total gastrectomy can improve the patient's quality of life

    Clinical significance of D-dimer levels during acute period in ischemic stroke

    Background Initial D-dimer level is a well-known prognostic parameter in patients with acute ischemic stroke (AIS). However, there have been no studies on the clinical significance of follow-up D-dimer levels. In this study, we evaluated the association between initial and follow-up D-dimer levels and early neurological deterioration (END) in patients with AIS. Methods We included consecutive patients with AIS who had a positive initial D-dimer test (> 0.55mg/L) between March 2021 and November 2022. The follow-up D-dimer test was performed on the 7th day after hospitalization and on the day of discharge if discharged earlier. END was defined as an increase of ≥ 2 in the total NIHSS score, or ≥ 1 in the motor NIHSS score within the first 7 days of admission. As medical conditions closely associated with the initial and follow-up D-dimer levels in AIS patients, we also evaluated the history of cancer, active cancer, and venous thromboembolism (VTE) that occurred during hospitalization together. Results A total of 246 patients with AIS were evaluated (median age: 87 years, male: 56.5%). In multivariable logistic regression analysis, the initial D-dimer level was closely associated with END after adjusting for confounders (adjusted odds ratio [aOR]: 1.48, 95% CI: 1.06–2.05). The follow-up D-dimer level also showed a close correlation with END (aOR: 1.60, 95% CI: 1.16–2.20). Regarding the analysis of the association between D-dimer levels and underlying cancer or VTE, the initial D-dimer level showed a statistically significant positive relationship only with active cancer (P = 0.024). On the other hand, the follow-up D-dimer level was found to be statistically significantly associated with a history of cancer (P = 0.024), active cancer (P = 0.001), and VTE (P = 0.001). Conclusions Initial and follow-up D-dimer levels were associated with END in AIS patients. Particularly, the follow-up D-dimer level showed a clear correlation not only with END but also with the underlying cancer or the occurrence of VTE during the acute period
