    Social Integration and the Ala Loa: Reconsidering the Significance of Trails in Hawaiian Exchange

    A large network of coastal trails on Hawai'i Island was recently designated as a National Historic Trail, but our understanding of the trail has been limited to historical documentation supported by scant archaeological data. The current study is based upon an archaeological survey of a 2-mile section of the trail in Kona where it crosses through a dozen ahupua 'a (traditional Hawaiian land divisions), and considers the significance of the coastal trail in relation to Hawaiian land use and exchange. Findings suggest that a trail paved with waterworn stones followed the same straight route as the current trail, which has been affected by numerous historical era modifications. Along with similar known examples in Kona, this finding calls into question a common assumption that all precontact and early historical Hawaiian coastal trails meandered along the contours of the coast. In addition to supporting chiefly endeavors such as the collection of tribute during the annual Makahiki festival, it is suggested that the straight trail may have supported regular exchange of domestic commodities across ahupua 'a boundaries in the prehistoric or premissionary eras, and broadens our anthropological perceptions of interdistrict exchange in relation to ahupua 'a economics.     The development of a new corporate specific health risk measurement instrument, and its use in investigating the relationship between health and well-being and employee productivity

    BACKGROUND: There is a growing body of evidence linking health and well-being to key business issues. Despite this, corporate uptake of workplace health promotion programmes has been slow outside the USA. One possible reason for this is the lack of a generally available health risk measure that is quick and easy to administer and produces data that is rich enough to inform and direct subsequent employee health promotional interventions. METHODS: We report on the development and validation of the health and well-being (HWB) assessment, a free to use health risk appraisal questionnaire that has been specifically developed for use in the corporate setting. The HWB assessment focuses upon modifiable health issues that directly impact upon business drivers. Development involved interviews with business leaders to ascertain their key areas of focus, scientific and general literature review to find evidence for health status having an impact upon these areas, and end user testing. Three UK-based organisations (insurance, telecommunications and consumer goods sectors) participated in the research. A total of 2224 employees completed the HWB assessment, the short-form 36 (SF-36) and the World Health Organisation Health and Work Performance questionnaire (WHO-HPQ) as part of the validation process. RESULTS: The HWB assessment is a twenty item questionnaire covering ten areas of health and well-being. Completion of the HWB assessment generates a global health risk score and ten sub-scores corresponding to the ten areas covered. It is easy to use and quick to complete (average completion time was eight minutes) and showed good internal consistency and test-retest reliability. Statistically significant correlations with similar SF-36 variables were observed. A significant negative correlation between HWB score and productivity decrement, as measured by the WHO-HPQ, was observed (r = -0.4). Individuals with HWB scores above the 25(th )percentile were more likely to achieve workplace productivity standards than those with scores below the 25(th )percentile (OR 3.62, 95% confidence limits 2.93 – 4.47). CONCLUSION: The HWB assessment generates reliable business focused health risk data that can be used to direct and target appropriate interventions within corporate populations. It may also be useful in quantifying the financial impact health status issues have upon organisations

    The role of regulatory mechanisms for control of plant diseases and food security — case studies from potato production in Britain

    Being aware of the potentially devastating impacts of plant diseases on food security, governments have designed and employ plant health legislation to prevent or inhibit the worst impacts. The development of such policies in Britain, and latterly in Europe, can be closely linked to disease events that have occurred in the potato sector. We analyse early and current examples of policies governing potato diseases in Britain to identify the decision processes leading to the implementation of such phytosanitary policies and how they have evolved over time and in response to different disease threats. Reasons for developing and implementing phytosanitary policies include the desire to prevent pathogens being introduced (entering and establishing in a new area), the protection of export markets, and the lack of effective control measures. Circumstances in which regulatory policies would not be appropriate could include situations where a disease is already widely distributed, unacceptable costs, lack of exclusion measures, or difficulties of disease diagnosis. We conclude that in general, government policies have worked well in protecting British potato growing over the last one hundred years, despite of the failures of some of the policies discussed here. They have also contributed much to the development of plant health policies for other crops. Voluntary grower initiatives are a new mechanism complementing existing formal policies with an additional level of security that allows individual growers to take on additional responsibility rather than relying entirely on government legislation

    The effect of high correlated colour temperature office lighting on employee wellbeing and work performance

    BACKGROUND: The effects of lighting on the human circadian system are well-established. The recent discovery of 'non-visual' retinal receptors has confirmed an anatomical basis for the non-image forming, biological effects of light and has stimulated interest in the use of light to enhance wellbeing in the corporate setting. METHODS: A prospective controlled intervention study was conducted within a shift-working call centre to investigate the effect of newly developed fluorescent light sources with a high correlated colour temperature (17000 K) upon the wellbeing, functioning and work performance of employees. Five items of the SF-36 questionnaire and a modification of the Columbia Jet Lag scale, were used to evaluate employees on two different floors of the call centre between February and May 2005. Questionnaire completion occurred at baseline and after a three month intervention period, during which time one floor was exposed to new high correlated colour temperature lighting and the other remained exposed to usual office lighting. Two sided t-tests with Bonferroni correction for type I errors were used to compare the characteristics of the two groups at baseline and to evaluate changes in the intervention and control groups over the period of the study. RESULTS: Individuals in the intervention arm of the study showed a significant improvement in self-reported ability to concentrate at study end as compared to those within the control arm (p < 0.05). The mean individual score on a 5 point Likert scale improved by 36.8% in the intervention group, compared with only 1.7% in the control group. The majority of this improvement occurred within the first 7 weeks of the 14 week study. Substantial within group improvements were observed in the intervention group in the areas of fatigue (26.9%), alertness (28.2%), daytime sleepiness (31%) and work performance (19.4%), as assessed by the modified Columbia Scale, and in the areas of vitality (28.4%) and mental health (13.9%), as assessed by the SF-36 over the study period. CONCLUSION: High correlated colour temperature fluorescent lights could provide a useful intervention to improve wellbeing and productivity in the corporate setting, although further work is necessary in quantifying the magnitude of likely benefits

    Non cell autonomous upregulation of CDKN2 transcription linked to progression of chronic hepatitis C disease

    Chronic hepatitis C virus infection (C-HC) is associated with higher mortality arising from hepatic and extrahepatic disease. This may be due to accelerated biological aging; however, studies in C-HC have thus far been based solely on telomere length as a biomarker of aging (BoA). In this study, we have evaluated CDKN2 locus transcripts as alternative BoAs in C-HC. Our results suggest that C-HC induces non-cell-autonomous senescence and accelerates biological aging. The CDKN2 locus may provide a link between C-HC and increased susceptibility to age-associated diseases and provides novel biomarkers for assessing its impact on aging processes in man

    Clostridium difficile infection among veterans health administration patients

    OBJECTIVETo report on the prevalence and incidence of Clostridium difficile infection (CDI) from 2009 to 2013 among Veterans Healthcare Administration patientsDESIGNA retrospective descriptive analysis of data extracted from a large electronic medical record (EMR) databaseSETTINGData were acquired from VHA healthcare records from 2009 to 2013 that included outpatient clinical visits, long-term care, and hospitalized care as well as pharmacy and laboratory information.RESULTSIn 2009, there were 10,207 CDI episodes, and in 2013, there were 12,143 CDI episodes, an increase of 19.0%. The overall CDI rate increased by 8.4% from 193 episodes per 100,000 patient years in 2009 to 209 episodes per 100,000 patient years in 2013. Of the CDI episodes identified in 2009, 58% were identified during a hospitalization, and 42% were identified in an outpatient setting. In 2013, 44% of the CDI episodes were identified in an outpatient setting.CONCLUSIONThis is one of the largest studies that has utilized timely EMR data to describe the current CDI epidemiology at the VHA. Despite an aging population with greater burden of comorbidity than the general US population, our data show that VHA CDI rates stabilized between 2011 and 2013 following increases likely attributable to the introduction of the more sensitive nucleic acid amplification tests (NAATs). The findings in this report will help establish an accurate benchmark against which both current and future VA CDI prevention initiatives can be measured.Infect. Control Hosp.
