    Prevalence and contributing factors of low birth weight babies in institutional delivery

    Introductions: Low Birth Weight (LBW) is a sensitive indicator of socio-economic conditions and indirectly measures the health of mother and child. Though, the health situation of Nepal has improved substantially over the years, LBW rate is still high. The present study explores the prevalence and contributing factors of LBW babies delivered in Patan hospital. Methods: The data were collected retrospectively from patients’ record for babies born at Patan Hospital, Patan Academy of Health Sciences, Kathmandu, Nepal. Records were retrieved through systematic randomization and IBM SPSS 15.0 was used for analysis. Non-parametric tests were applied with correlation done between dependent and independent variables. Results: A total of 4395 birth occurred during the study period of which 527 met the study criteria and remaining were of normal birth weight. The prevalence of LBW was 11.99% and the average LBW was calculated as 2074.97 ± 344.425 grams. The factors associated with LBW included preterm babies (Pearson correlation coefficient= 0.554, p= 0.001 < 0.05) and oligohydamnious (Spearman’s correlation coefficient= 0.307, p= 0.019 < 0.05). Conclusions: This study suggests preterm babies and oligohydramnious to be contributing factors of LBW babies. Keywords: amniotic fluid index, intra uterine growth retardation, maternal risk factors, Patan Hospital, retrospective hospital based studyÂ

    Hydrogen rich Syngas by Steam gasification of Waste Lignocellulosic Biomass

    The depleting petroleum reserve, as well as increasing demand for crude oil, has attracted the research interest for alternative sources of energy. The surplus lignocellulosic biomass such as forest residues, crop residues, and weeds may be a potential source to derive biofuel by the thermochemical route. Gasification is an established technology for the utilization of biomass to obtain producer gas containing mainly H2 and CO as combustibles. The air-blown biomass gasifiers are widely installed where the resulting producer gas contains 55-60 per cent nitrogen and a lower percentage of H2 and CO with an H2/CO ratio of less than one. Gasification of lignocellulosic biomass using steam as an oxidant produces syn-gas with a lower amount of nitrogen, a higher percentage of hydrogen, and H2/CO > 2.1. Steam as an oxidant favors water gas shift reaction with the formation of hydrogen and enhances the H2/CO ratio. The ratio of steam to biomass was 1.20-1.25 so the water gas shift reaction and steam reforming of methane occur. The percentage of hydrogen in the syngas obtained by gascification of pine needles, camelina straw and Lantana biomass was 56.8, 68.7, and 72.8 per cent respectively. The present article describes the steam gasification of pine needle biomass, crop residue from Camelina sativa, and biomass from weed i.e. Lantana camara to produce syngas with a higher H2/CO ratio along with a high calorific value compared to the one with air-blown gasification

    Design and test of optical payload for polarization encoded QKD for Nanosatellites

    Satellite based Quantum Key Distribution (QKD) in Low Earth Orbit (LEO) is currently the only viable technology to span thousands of kilometres. Since the typical overhead pass of a satellite lasts for a few minutes, it is crucial to increase the the signal rate to maximise the secret key length. For the QUARC CubeSat mission due to be launched within two years, we are designing a dual wavelength, weak-coherent-pulse decoy-state Bennett-Brassard '84 (WCP DS BB84) QKD source. The optical payload is designed in a 12×9×5cm312{\times}9{\times}5 cm^3 bespoke aluminium casing. The Discrete Variable QKD Source consists of two symmetric sources operating at 785 nm and 808 nm. The laser diodes are fixed to produce Horizontal,Vertical, Diagonal, and Anti-diagonal (H,V,D,A) polarisation respectively, which are combined and attenuated to a mean photon number of 0.3 and 0.5 photons/pulse. We ensure that the source is secure against most side channel attacks by spatially mode filtering the output beam and characterising their spectral and temporal characterstics. The extinction ratio of the source contributes to the intrinsic Qubit Error Rate(QBER) with 0.817±0.001%0.817 \pm 0.001\%. This source operates at 200MHz, which is enough to provide secure key rates of a few kilo bits per second despite 40 dB of estimated loss in the free space channe

    The prevention of‑ and first response to‑ injuries in Nepal: A review of policies and legislation

    Background: Injuries, the cause of an estimated 4.5 million deaths annually and many more disabilities worldwide each year, are the predictable outcome of particular circumstances. One of the most effective ways to prevent injuries is through policy and legislation. The aim of this research study was to identify and critically review all policy and legislation in Nepal that had the potential to prevent injuries. Methods: We identified legislation and policy that met inclusion criteria through a stakeholder meeting, networks and contacts, and websites and electronic resources. Each included document was critically reviewed to identify areas of strength and opportunities for improvement. We compared the included documents against WHO’s recommendations of known effective interventions. Results: Sixty-two documents met the inclusion criteria for this review. Of these, 24 (38.7%) were exclusively related to road injuries, 11 (17.7%) to occupational injuries, 6 (9.7%) to injuries in the home and 5 (8.1%) to injuries at school; 30 (48.4%) documents included text related to the first response to injuries. Of 127 strategic recommendations by WHO that provided an area for policy or legislative focus, 21 (16.5%) were considered adequately met by Nepali policy and legislation, 43 (33.9%) were considered partially met and 63 (49.6%) were not met. Conclusion: We drew five conclusions from this critical policy review, which we have related to recommendations as follows: widening the scope of legislation and policy for injury prevention to emphasize injuries occurring at home or school; addressing the causes of injuries and promoting proven preventive measures; greater clarity on both individual and institutional roles and responsibilities; trustworthy data and quality evidence to inform decision-making; and financial investment and capacity-strengthening for injury prevention and first response. The current system of federal governance in Nepal has potential for strengthening injury prevention and first response at the central, provincial and local levels

    Epigenomic profiling of prostate cancer identifies differentially methylated genes in TMPRSS2:ERG fusion-positive versus fusion-negative tumors

    Background: About half of all prostate cancers harbor the TMPRSS2:ERG (T2E) gene fusion. While T2E-positive and T2E-negative tumors represent specific molecular subtypes of prostate cancer (PCa), previous studies have not yet comprehensively investigated how these tumor subtypes differ at the epigenetic level. We therefore investigated epigenome-wide DNA methylation profiles of PCa stratified by T2E status. Results: The study included 496 patients with clinically localized PCa who had a radical prostatectomy as primary treatment for PCa. Fluorescence in situ hybridization (FISH) "break-apart" assays were used to determine tumor T2E- fusion status, which showed that 266 patients (53.6 %) had T2E-positive PCa. The study showed global DNA methylation differences between tumor subtypes. A large number of differentially methylated CpG sites were identified (false-discovery rate [FDR] Q-value Conclusions: This study identified substantial differences in DNA methylation profiles of T2E-positive and T2E-negative tumors, thereby providing further evidence that different underlying oncogenic pathways characterize these molecular subtypes

    Responsive Operations for Key Services (ROKS): A Modular, Low SWaP Quantum Communications Payload

    Quantum key distribution (QKD) is a theoretically proven future-proof secure encryption method that inherits its security from fundamental physical principles. With a proof-of-concept QKD payload having flown on the Micius satellite since 2016, efforts have intensified globally. Craft Prospect, working with a number of UK organisations, has been focused on miniaturising the technologies that enable QKD so that they may be used in smaller platforms including nanosatellites. The significant reduction of size, and therefore the cost of launching quantum communication technologies either on a dedicated platform or hosted as part of a larger optical communications will improve potential access to quantum encryption on a relatively quick timescale. The Responsive Operations for Key Services (ROKS) mission seeks to be among the first to send a QKD payload on a CubeSat into low Earth orbit, demonstrating the capabilities of newly developed modular quantum technologies. The ROKS payload comprises a quantum source module that supplies photons randomly in any of four linear polarisation states fed from a quantum random number generator; an acquisition, pointing, and tracking system to fine-tune alignment of the quantum source beam with an optical ground station; an imager that will detect cloud cover autonomously; and an onboard computer that controls and monitors the other modules, which manages the payload and assures the overall performance and security of the system. Each of these modules have been developed with low Size, Weight and Power (SWaP) for CubeSats, but with interoperability in mind for other satellite form factors. We present each of the listed components, together with the initial test results from our test bench and the performance of our protoflight models prior to initial integration with the 6U CubeSat platform systems. The completed ROKS payload will be ready for flight at the end of 2022, with various modular components already being baselined for flight and integrated into third party communication missions

    Progress and challenges in glacial lake outburst flood research (2017–2021):a research community perspective

    Glacial lake outburst floods (GLOFs) are among the most concerning consequences of retreating glaciers in mountain ranges worldwide. GLOFs have attracted significant attention amongst scientists and practitioners in the past 2 decades, with particular interest in the physical drivers and mechanisms of GLOF hazard and in socioeconomic and other human-related developments that affect vulnerabilities to GLOF events. This increased research focus on GLOFs is reflected in the gradually increasing number of papers published annually. This study offers an overview of recent GLOF research by analysing 594 peer-reviewed GLOF studies published between 2017 and 2021 (Web of Science and Scopus databases), reviewing the content and geographical focus as well as other characteristics of GLOF studies. This review is complemented with perspectives from the first GLOF conference (7-9 July 2021, online) where a global GLOF research community of major mountain regions gathered to discuss the current state of the art of integrated GLOF research. Therefore, representatives from 17 countries identified and elaborated trends and challenges and proposed possible ways forward to navigate future GLOF research, in four thematic areas: (i) understanding GLOFs - timing and processes; (ii) modelling GLOFs and GLOF process chains; (iii) GLOF risk management, prevention and warning; and (iv) human dimensions of GLOFs and GLOF attribution to climate change.Fil: Emmer, Adam.
