    Uretero-Internal Pudendal Artery Fistula with Longterm Indwelling of Ureteral Stent: A Case Report

    A 74-year-old woman presenting with bilateral ureteral stricture was referred to our hospital. She had undergone radical hysterectomy and adjuvant irradiation therapy for cervical cancer in 2000. Double-J stents were inserted in both the ureters and replaced at regular intervals. Eighteen months after ureteral stenting, she complained of gross hematuria and was managed with hemostatic agents. During a routine replacement of the right double-J stent, massive bleeding was observed from the urethra which continued intermittently. The source of bleeding was not identified on computed tomography and angiography. We kept her at rest, which reduced the bleeding. However, she required intermittent transfusions. Angiography was performed at the time of bleeding on March 5, 2011. A uretero-internal pudendal artery fistula was found, and coil embolization was performed. Thereafter, hematuria did not recur up to the last followup in July 2011

    ユーラシアカワウソ(Lutra lutra)はユーラシア大陸に広く分布する中型食肉目である。本種には11の亜種がおり,欧州亜種L. l. lutra,東南アジア亜種L. l. barang,中国亜種L. l. chinensis などが国内外の動物園や水族館で飼育されている。欧州の動物園や水族館にはA-line(L. l. lutra)とB-line(L. l. lutraとL. l. barangが交雑した可能性がある系統)という2つのlineが存在し,日本の動物園や水族館にも導入されている。本来は同じ亜種内で繁殖がおこなわれるが,日本では本種の個体数が少ないためA-lineとB-lineで繁殖がおこなわれている。実際にB-lineが東南アジア亜種の遺伝子を持っているか評価がおこなわれ,B-lineとA-lineの間に違いがあることが示された。ただし先行研究では,解析配列が307bpと短いことが問題として上げられる。本研究ではミトコンドリア(mt)DNA Cytochrome bの全長配列(1140bp)をA-line,B-line各1個体,中国亜種2個体から決定し,先行研究で決定されたB-lineの配列を含む4配列を加えて配列比較,系統解析をおこなった。その結果,A-line,B-lineそれぞれが特徴的な変異サイトを示し,系統解析では先行研究と同じようにB-lineは中国亜種とクレイドを形成し,A-lineは欧州亜種独自のクレイドを形成した。よって解析したB-lineのmtDNAは東南アジア亜種に由来する可能性がある。2015年現在,日本のB-line個体は全て本研究で解析したB-lineの子や孫である。亜種間交雑が示唆された国内のB-lineは,本種の繁殖・維持に活用できるが,A-lineや中国亜種の系統維持に活用することはできない。Eurasian otter (Lutra lutra) is a mid-sized species belonging to the order Carnivora and is distributed across a wide area of the Eurasian continent. This species comprises 11subspecies. The European subspecies (L. l. lutra), the Southeast Asian subspecies (L. l. barang), and the Chinese subspecies (L. l. chinensis) are reared in European and Japanese zoos and aquariums. Among these, the European facilities rear two lines, which were named A-line (L. l. lutra) and B-line (this is likely a crossbreed of L. l. lutra and L. l. barang); the Japanese facilities introduced these two lines. The mitochondrial (mt) DNA partial cytochrome b (cytb) sequence (307bp) of the A- and B-lines detected previously have shown slight but characteristic differences between the two lines. However, the previous study had limitations, as only one sample of the B-line was considered and the sequence was found to be short. Although, typically, breeding involves mating of individuals within the same subspecies, the A- and B-lines were crossbred in the Japanese facilities, which is likely because only small numbers of the Eurasian otter were reared in these facilities, thereby making cross-breeding imperative to the conservation of Eurasian otter in this country. In this study, we determined the complete cytb sequence (1140bp) from one individual each of the A- and B-lines, and two individuals of the Chinese subspecies. These sequences were compared and subjected to phylogenetic analysis, along with four individuals of Eurasian otter (which includes one B-line individual). The B-line individual showed diagnostic nucleotides at two positions, whereas the A-line individual showed diagnostic nucleotides at four positions. In the phylogenetic tree, two B-line individuals formed a monophyletic group with the two individuals of Chinese subspecies, and an A-line individual formed another monophyletic group with the other European individual. These results suggest that mtDNA sequence of B-line was derived from some Asian subspecies, and it differed from the A-line sequence (European subspecies). Moreover, because these results suggest that hybridization between the European subspecies and some Asian subspecies occurred in the B-line, the breeding program involving B-line individuals is not appropriate to the conservation of subspecies-specific strains

    本学における統合実習指導の実態や課題を解決に導くために研修会を企画・運営したので報告する。研修会では統合実習開始当初からこれまでの指導体制に関する課題と各実習施設の工夫や取り組みについてグループワークをし、情報共有することができた。研修会を終えて、グループ毎のまとめをもとに、研修会担当者らで検討した結果、統合実習の指導体制、臨地実習PBL展開、夜間実習計画・指導、の3点が課題として集約できた。また大学と実習施設の連携強化のために必要な内容として、初めての実習施設で学生が力を発揮できるための学習環境の整備、臨地実習指導者と教員間の実習目標・指導方法・評価方法の共有、学生の心身両面のサポートも含めて教員と指導者の役割の明確化など、指導体制の連携強化の重要点が示唆された。Abstract:Planning workshops to guide in solving challenges and the realities of practice teaching methods at a university were conducted. Since leadership is challenging and the sharing of initiatives and information is important, there was a need to devise a training and work method for each practice of the integration exercise.The results of each group, based on consultation, training of personnel, training, integrated training in the education system, along with clinical practice PBL deployment, night-time practice planning and guidance, were summarized into three points. These key points were: strengthening leadership, such as clarification of the role of teachers and leaders; including support for shared training goals which can aid students at practice facilities for the fi rst time at a university for practical training necessary to foster a cohesive learning environment; and aiding clinical instructors and teachers, including teaching methods for evaluation of the student’s mental condition

    Electron-Deficient Heteroacenes that Contain Two Boron Atoms: Near-Infrared Fluorescence Based on a Push–Pull Effect

    Electron-deficient heteroacenes that contain two tricoordinate boron atoms in their acene skeletons and planarized phenyl ether moieties at their periphery were synthesized via the borylation of the corresponding silicon-bridged precursors. X-ray crystallographic analysis revealed quinoidal structures, which give rise to two-step reversible redox processes for both the reduction and oxidation. These compounds exhibit intense absorption and sharp fluorescence bands with vibronic structures in the near-infrared (NIR) region. These properties originate from the push–pull effect along the long axis of the molecule derived from the electron-donating ether moiety and the electron-accepting boron moieties. Of particular note is the NIR emission of the thienothiophene-centered heteroacene, which has a maximum at 952 nm with a narrow band width of 309 cm–1 in cyclohexane. A Franck–Condon analysis revealed the crucial role of the sterically less-hindered thienothiophene spacer in achieving this sharp emission band

    Spatial distribution of summer temperature in tokyo wards : observation results of metros

    The urban heat island phenomena in summer over Tokyo Wards was studied by using the data obtained from METROS (Metropolitan Environmental Temperature and Rainfall Observation System), which is the high density heat island phenomena observation network. The observational results clarified the detailed spatial distributions of temperature never known before. Concerning the spatial distribution of daily maximum temperature, the region with a relatively high temperature extends from the central Tokyo district toward the northern and northwestern area. On the other hand, relatively high values in the daily minimum temperature are distributed around the coastal area of Tokyo Bay including the central Tokyo district. Since the central Tokyo district has significantly high temperatures both in the daytime and in the nighttime, and also raises the temperature in the adjacent north and northeast area, it is necessary to take countermeasures preferentially to this district

    Abstract This study was conducted to improve the rate capability and cyclability of cathodes for lithium‐ion batteries (LIBs) with a hybrid cathode structure. The through‐holed LIB cathode material and activated carbon layers formed on each side of a current collector were drilled with a picosecond pulsed laser beam to prepare the cathode structure. The hybrid cathodes exhibited excellent rate capabilities of 93 % capacity retention at 100 C. The results were dependent on the weight percentage of the activated carbon relative to the total weight of the active materials and on the difference in discharge/charge voltages between the LIB cathode and activated carbon materials. The cathode had cycle stability at 50 C during 100 cycles. The performance characteristics of the hybrid cathode, the through‐holed and nontreated LIB cathodes, and the nontreated activated carbon cathodes were compared. In the Ragone plot, the hybrid cathode was located in the region where conventional through‐holed and nontreated cathodes would not be located
