    Experimental morphological and functional study of gentamicin cochleotoxicity using the regular dose given to neonates

    TEMA: a gentamicina é um antibiótico que atua nas infecções causadas por bacilos Gram-negativos. Seu efeito colateral mais importante é a ototoxicidade. As ototoxicoses são afecções iatrogênicas provocadas por fármacos que alteram a orelha interna, podendo afetar o sistema coclear e/ou vestibular, alterando duas funções importantes: a audição e o equilíbrio. Os principais grupos pediátricos que recebem antibióticos aminoglicosídeos são recém-nascidos com infecções graves na UTI neonatal. OBJETIVOS: verificar a ocorrência de lesão às células ciliadas externas (CCE) pela gentamicina com os esquemas de dose única de 4mg/Kg/dia e de 2,5mg/Kg/dia a cada 12 horas, por meio de um estudo anatômico por microscopia eletrônica de varredura (MEV) e estudo funcional através das emissões otoacústicas por produto de distorção (OEAPD). Forma de estudo experimental. MÉTODO: foram avaliadas 26 cobaias albinas através das EOAPD pré e pós-tratamento com gentamicina. Para a avaliação anatômica por MEV, as cobaias foram sacrificadas em tempo programado após a administração das drogas via intramuscular. RESULTADOS: a avaliação do estado funcional das CCE mostrou preservação das OEAPD em todas as cobaias. Os resultados da MEV, depois de fotografados foram analisados através da contagem do número de CCE da espira basal da cóclea em determinado campo fotográfico. CONCLUSÃO: não foram observadas lesões ou alterações no funcionamento das células ciliadas externas mediante a dosagem aplicada em cobaias albinas, de 4mg/Kg/dia (dose única) e 2,5mg/Kg/dia a cada 12 horas, utilizadas por 10 e 14 dias.BACKGROUND: gentamicin is an antibiotic that acts in Gram-negative bacilli infections, having as a side effect ototoxicity. Ototoxicity is an iatrogenic disturb provoked by drugs that modify the internal ear, affecting the cochlear and/or vestibular system and causing alterations in two important functions: equilibrium and audition. The main pediatric groups that receive aminoglicosides antibiotics are newborns who present serious infections in Neonate intensive care units. AIM: to verify the occurrence of external cilliary cells (ECC) caused by gentamicin with single dose schemas of 4mg/kg/day and 2,5mg/kg/day every 12 hours, through a morphological - scanning electronic microscopy (SEM) and functional - distortion product otoacoustic emissions (DPOAE) experimental study. METHOD: 26 albino guinea pigs were evaluated through DPOAE pre and post gentamicin treatment. The guinea pigs were sacrificed in the programmed time after the intramuscular administration of the drugs for anatomic analysis using MEV. RESULTS: the evaluation of the functional state of the ECC indicated preservation of the DPOAE in all of the guinea pigs. The SEM results, after being photographed were analyzed in terms of the number of the ECC in the cochlear basal turn in a specific photographic field. CONCLUSION: lesions or alterations in the functioning of the ECC of albino guinea-pigs after the use of 4mg/Kg/day and 2,5mg/Kg/day every 12 hours for a period 10 and 14 days were not observed

    Persistence of the otoprotective effect. How long does otoprotection against amikacin lasts?

    Há comprovação de que o fenômeno de resistência ocorre quando a dose não lesiva da amicacina protege as células ciliadas contra a ototoxicidade da própria amicacina. OBJETIVO: O objetivo deste trabalho é verificar se o fenômeno de resistência é temporalmente persistente. MÉTODO: Estudo experimental com 14 cobaias albinas (Cavia porcellus) divididas em três grupos. Avaliação da função auditiva por emissões otoacústicas por produto de distorção (EOAPD): na pré-exposição à amicacina, no 15º dia de aplicação da dose não lesiva, no final da aplicação da dose lesiva e antes da decapitação. RESULTADOS: O Grupo A (controle) apresentou função auditiva e padrão histológico normais. No Grupo B (amicacina 20mg/kg/dia intramuscular por 30 dias e dose lesiva (400 mg/kg/dia) por 12 dias) e no Grupo C (mesmo esquema do grupo B, porém mantidos por 60 dias e sacrificados), as OEA-PD confirmaram função auditiva normal no período pré-exposição e manutenção do padrão após dose não lesiva, porém, houve perda importante da função auditiva após término do período de aplicação da dose lesiva. CONCLUSÃO: Não houve manutenção do fenômeno da autodefesa estendida por um período de 30 a 60 dias após a aplicação de doses lesivas de amicacina.There is evidence that a "resistance phenomenon" occurs when a none-damaging dose of amikacin protects the hair cells from ototoxicity. Our goal is to prove that this resistance is persistent. Method: Experimental study - 14 albino guinea pigs (Cavia porcellus) divided into three groups. The auditory function was assessed by distortion product otoacoustic emissions (DPOAE): before exposure to amikacin, on the 15th day after the non-damaging dose was injected, at the end of the damage dose injection and prior to decapitation. Results: Group A (control) presented normal hearing and histological pattern. Group B (amikacin 20 mg/kg/day (IM) for 30 days and affecting dose (400 mg/kg/day) for 12 days and Group C (same protocol of Group B, but kept for 60 days and slaughtered), the DPOAE confirmed normal auditory function in the pre-exposure and maintenance of the standard-close; however, significant loss of auditory function after the end of the damaging dose injection. Conclusion: The protection phenomenon did not extended for a period of 30 to 60 days after the application of damaging doses of amykacin

    Comparison of the Use of Tympanic and Extratympanic Electrodes for Electrocochleography

    Objectives: To determine the differences between tympanic and extratympanic electrodes regarding recording technique, comfort and ease of execution of the exam, and quality of auditory potential tracings. Study Design: Prospective cross-section investigation. Methods: Determination of the summation potential/action potential (SP/AP) ratio by electrocochleography (EchoG) using tympanic and extratympanic electrodes and separate analysis of SP and AP regarding the amplitude recorded. Results: Twenty-three subjects (15 men and 8 women; mean age: 33.17 years) with normal tonal threshold audiometry were evaluated. EchoG analysis revealed no significant difference between the two tympanic electrodes. Eleven of the 23 subjects reported discomfort with the insertion of the tympanic electrode even with the use of topical xylocaine, whereas no complaints of discomfort were reported with the use of the extratympanic electrode. Conclusions: Both electrodes were effective for EchoG evaluation, but the extratympanic one was easier to insert and did not cause discomfort.     PURPOSE: To compare the role of transitory latex and sylastic® implants in tympanoplasty on the closure of tympanic perforations. METHODS: A randomized double-blind prospective study was conducted on 107 patients with chronic otitis media submitted to underlay tympanoplasty and divided at random into three groups: control with no transitory implant, latex membrane group, and sylastic® membrane group. RESULTS: Greater graft vascularization occurred in the latex membrane group (p<0.05). Good biocompatibility was obtained with the use of the latex and silicone implants, with no effect on the occurrence of infection, otorrhea or otorragy. CONCLUSION: The use of a transitory latex implant induced greater graft vascularization, with a biocompatible interaction with the tissue of the human tympanic membrane.<br>OBJETIVO: Investigar os efeitos da biomembrana de látex e silicone no fechamento de perfurações timpânicas. METODOS: Foram estudados 107 pacientes com otite média cronica simples submetidos à cirurgia de timpanoplastia. Os pacientes foram classificados aleatoriamente, por sorteio, em três grupos. Até o final da cirurgia, os cirurgiões não tomavam conhecimento se seria ou não utilizado qualquer material externamente ao enxerto de fáscia temporal. Neste momento, o paciente que fora previamente classificado por uma ordem aleatória de entrada como participante do estudo foi identificado como participante do grupo 1, 2 ou 3, sendo, respectivamente, ausência de implante transitório no primeiro, membrana de látex no segundo e membrana de sylastic® no terceiro grupo. RESULTADOS: Houve uma maior vascularização do enxerto no grupo em que foi utilizado implante transitório de membrana de látex (p<0,05). Obteve-se boa biocompatibilidade com uso de implantes de látex e silicone sem afetar as taxas de ocorrência de infecção, otorreia ou otorragia. A porcentagem de cicatrização da membrana timpânica foi equivalente nos três grupos, assim como a melhora auditiva (p<0,01). CONCLUSÃO: O uso do implante transitório de látex provocou um maior processo de vascularização do enxerto, com interação satisfatória com os tecidos da membrana timpânica humana

    Effects of hyperbaric oxygen treatment on auditory hair cells after acute noise damage

    Acute acoustic trauma (AAT) is a sudden sensorineural hearing loss caused by exposure of the hearing organ to acoustic overstimulation, typically an intense sound impulse, hyperbaric oxygen therapy (HOT), which favors repair of the microcirculation, can be potentially used to treat it. Hence, this study aimed to assess the effects of HOT on guinea pigs exposed to acoustic trauma. Fifteen guinea pigs were exposed to noise in the 4-kHz range with intensity of 110 dB sound level pressure for 72 h. They were assessed by brainstem auditory evoked potential (BAEP) and by distortion product otoacoustic emission (DPOAE) before and after exposure and after HOT at 2.0 absolute atmospheres for 1 h. The cochleae were then analyzed using scanning electron microscopy (SEM). There was a statistically significant difference in the signal-to-noise ratio of the DPOAE amplitudes for the 1- to 4-kHz frequencies and the SEM findings revealed damaged outer hair cells (OHC) after exposure to noise, with recovery after HOT (p = 0.0159), which did not occur on thresholds and amplitudes to BAEP (p = 0.1593). The electrophysiological BAEP data did not demonstrate effectiveness of HOT against AAT damage. However, there was improvement of the anatomical pattern of damage detected by SEM, with a significant reduction of the number of injured cochlear OHC and their functionality detected by DPOAE.Fundacao de Amparo a Pesquisa do Estado de Sao Paulo (FAPESP)[2007/59213-2

    Organophosphate-related ototoxicity: Description of the vestibulocochlear system ultrastructural aspects of guinea pigs

    Organophosphate toxic agents are used in agriculture and are currently part of the group of toxic agents which can lead to hearing loss, in which we have solvents, metals and asphyxiation agents. Aim: to analyze the acute ototoxic action of a group of organophosphate agents in the vestibulocochlear system. This is a prospective experimental study. Materials and Methods: we used male albino guinea pigs, broken down into three groups, to which we provided distilled water (group 1 - control), agrotoxic agent - 0.3mg/Kg/day (group 2), agrotoxic - 3 mg/Kg/day (group 3), during 7 seven consecutive days. The most used agrotoxic agent was Tamaron BR (metamidophos). The anatomical evaluation of the cochlea, saccule and utricle was carried out by means of electronic scanning microscopy after the use of the agrotoxic agent. Results: the guinea pigs submitted to the organophosphate presented cochlear morphological alterations with lesions on the three turns analyzed, as well as cilia alterations in the saccule and utricle, intensified according to the agent dosage. Conclusion: the morphological alterations seen in the hair cells exposed to daily doses of organophosphate promote evidences of an acute deleterious effect of agrotoxic agents on the vestibulocochlear system
