    La necessitĂ  di affrontare l’emergenza evidenziata dalle recenti indagini OCSE-PISA sulle competenze dei quindicenni in matematica spinge ricercatori ed insegnanti ad elaborare metodologie sempre nuove e diverse di insegnamento/apprendimento. Tutti i tipi di risorse e le TIC in particolare diventano mezzi efficaci per proporre la matematica come strumento di pensiero capace di sollecitare il ragionamento. La creazione di una piccola comunitĂ  di apprendimento in rete, in cui i ragazzi possono imparare a cimentarsi con la scrittura collaborativa con la guida dei loro insegnanti, puĂČ essere un modo per stimolare quel confronto di idee tra i discenti che Ăš base per un apprendimento attivo

    Refinement of the diagnostic approach for the identification of children and adolescents affected by familial hypercholesterolemia: Evidence from the LIPIGEN study

    Background and aims: We aimed to describe the limitations of familiar hypercholesterolemia (FH) diagnosis in childhood based on the presence of the typical features of FH, such as physical sings of cholesterol accumulation and personal or family history of premature cardiovascular disease or hypercholesterolemia, comparing their prevalence in the adult and paediatric FH population, and to illustrate how additional information can lead to a more effective diagnosis of FH at a younger age.Methods: From the Italian LIPIGEN cohort, we selected 1188 (>= 18 years) and 708 (<18 years) genetically-confirmed heterozygous FH, with no missing personal FH features. The prevalence of personal and familial FH features was compared between the two groups. For a sub-group of the paediatric cohort (N = 374), data about premature coronary heart disease (CHD) in second-degree family members were also included in the evaluation.Results: The lower prevalence of typical FH features in children/adolescents vs adults was confirmed: the prevalence of tendon xanthoma was 2.1% vs 13.1%, and arcus cornealis was present in 1.6% vs 11.2% of the cohorts, respectively. No children presented clinical history of premature CHD or cerebral/peripheral vascular disease compared to 8.8% and 5.6% of adults, respectively. The prevalence of premature CHD in first-degree relatives was significantly higher in adults compared to children/adolescents (38.9% vs 19.7%). In the sub-cohort analysis, a premature CHD event in parents was reported in 63 out of 374 subjects (16.8%), but the percentage increased to 54.0% extending the evaluation also to second-degree relatives.Conclusions: In children, the typical FH features are clearly less informative than in adults. A more thorough data collection, adding information about second-degree relatives, could improve the diagnosis of FH at younger age

    Twelve Variants Polygenic Score for Low-Density Lipoprotein Cholesterol Distribution in a Large Cohort of Patients With Clinically Diagnosed Familial Hypercholesterolemia With or Without Causative Mutations

    : Background A significant proportion of individuals clinically diagnosed with familial hypercholesterolemia (FH), but without any disease-causing mutation, are likely to have polygenic hypercholesterolemia. We evaluated the distribution of a polygenic risk score, consisting of 12 low-density lipoprotein cholesterol (LDL-C)-raising variants (polygenic LDL-C risk score), in subjects with a clinical diagnosis of FH. Methods and Results Within the Lipid Transport Disorders Italian Genetic Network (LIPIGEN) study, 875 patients who were FH-mutation positive (women, 54.75%; mean age, 42.47±15.00 years) and 644 patients who were FH-mutation negative (women, 54.21%; mean age, 49.73±13.54 years) were evaluated. Patients who were FH-mutation negative had lower mean levels of pretreatment LDL-C than patients who were FH-mutation positive (217.14±55.49 versus 270.52±68.59 mg/dL, P<0.0001). The mean value (±SD) of the polygenic LDL-C risk score was 1.00 (±0.18) in patients who were FH-mutation negative and 0.94 (±0.20) in patients who were FH-mutation positive (P<0.0001). In the receiver operating characteristic analysis, the area under the curve for recognizing subjects characterized by polygenic hypercholesterolemia was 0.59 (95% CI, 0.56-0.62), with sensitivity and specificity being 78% and 36%, respectively, at 0.905 as a cutoff value. Higher mean polygenic LDL-C risk score levels were observed among patients who were FH-mutation negative having pretreatment LDL-C levels in the range of 150 to 350 mg/dL (150-249 mg/dL: 1.01 versus 0.91, P<0.0001; 250-349 mg/dL: 1.02 versus 0.95, P=0.0001). A positive correlation between polygenic LDL-C risk score and pretreatment LDL-C levels was observed among patients with FH independently of the presence of causative mutations. Conclusions This analysis confirms the role of polymorphisms in modulating LDL-C levels, even in patients with genetically confirmed FH. More data are needed to support the use of the polygenic score in routine clinical practice

    Serum Albumin Is Inversely Associated With Portal Vein Thrombosis in Cirrhosis

    We analyzed whether serum albumin is independently associated with portal vein thrombosis (PVT) in liver cirrhosis (LC) and if a biologic plausibility exists. This study was divided into three parts. In part 1 (retrospective analysis), 753 consecutive patients with LC with ultrasound-detected PVT were retrospectively analyzed. In part 2, 112 patients with LC and 56 matched controls were entered in the cross-sectional study. In part 3, 5 patients with cirrhosis were entered in the in vivo study and 4 healthy subjects (HSs) were entered in the in vitro study to explore if albumin may affect platelet activation by modulating oxidative stress. In the 753 patients with LC, the prevalence of PVT was 16.7%; logistic analysis showed that only age (odds ratio [OR], 1.024; P = 0.012) and serum albumin (OR, -0.422; P = 0.0001) significantly predicted patients with PVT. Analyzing the 112 patients with LC and controls, soluble clusters of differentiation (CD)40-ligand (P = 0.0238), soluble Nox2-derived peptide (sNox2-dp; P &lt; 0.0001), and urinary excretion of isoprostanes (P = 0.0078) were higher in patients with LC. In LC, albumin was correlated with sCD4OL (Spearman's rank correlation coefficient [r(s)], -0.33; P &lt; 0.001), sNox2-dp (r(s), -0.57; P &lt; 0.0001), and urinary excretion of isoprostanes (r(s), -0.48; P &lt; 0.0001) levels. The in vivo study showed a progressive decrease in platelet aggregation, sNox2-dp, and urinary 8-iso prostaglandin F2 alpha-III formation 2 hours and 3 days after albumin infusion. Finally, platelet aggregation, sNox2-dp, and isoprostane formation significantly decreased in platelets from HSs incubated with scalar concentrations of albumin. Conclusion: Low serum albumin in LC is associated with PVT, suggesting that albumin could be a modulator of the hemostatic system through interference with mechanisms regulating platelet activation

    Frequency of left ventricular hypertrophy in non-valvular atrial fibrillation

    Left ventricular hypertrophy (LVH) is significantly related to adverse clinical outcomes in patients at high risk of cardiovascular events. In patients with atrial fibrillation (AF), data on LVH, that is, prevalence and determinants, are inconsistent mainly because of different definitions and heterogeneity of study populations. We determined echocardiographic-based LVH prevalence and clinical factors independently associated with its development in a prospective cohort of patients with non-valvular (NV) AF. From the "Atrial Fibrillation Registry for Ankle-brachial Index Prevalence Assessment: Collaborative Italian Study" (ARAPACIS) population, 1,184 patients with NVAF (mean age 72 \ub1 11 years; 56% men) with complete data to define LVH were selected. ARAPACIS is a multicenter, observational, prospective, longitudinal on-going study designed to estimate prevalence of peripheral artery disease in patients with NVAF. We found a high prevalence of LVH (52%) in patients with NVAF. Compared to those without LVH, patients with AF with LVH were older and had a higher prevalence of hypertension, diabetes, and previous myocardial infarction (MI). A higher prevalence of ankle-brachial index 640.90 was seen in patients with LVH (22 vs 17%, p = 0.0392). Patients with LVH were at significantly higher thromboembolic risk, with CHA2DS2-VASc 652 seen in 93% of LVH and in 73% of patients without LVH (p &lt;0.05). Women with LVH had a higher prevalence of concentric hypertrophy than men (46% vs 29%, p = 0.0003). Logistic regression analysis demonstrated that female gender (odds ratio [OR] 2.80, p &lt;0.0001), age (OR 1.03 per year, p &lt;0.001), hypertension (OR 2.30, p &lt;0.001), diabetes (OR 1.62, p = 0.004), and previous MI (OR 1.96, p = 0.001) were independently associated with LVH. In conclusion, patients with NVAF have a high prevalence of LVH, which is related to female gender, older age, hypertension, and previous MI. These patients are at high thromboembolic risk and deserve a holistic approach to cardiovascular prevention

    Documentare e narrare le attività didattiche consente all’insegnante di riflettere sulle decisioni e sulle azioni compiute, permette di prendere piena coscienza dei processi attivati evitando di disperdere quegli imprevisti spunti educativi, che sicuramente sorgono nel corso della concreta attuazione, ma che troppo spesso rimangono ignorati o semplicemente dimenticati. La costruzione e l’integrazione di percorsi, prodotti e documenti, ma anche di strumenti e ambienti di formazione consente di racchiudere le attività all'interno e per mezzo di un'unica struttura formativa, generando un intimo collegamento tra il percorso didattico e la sua narrazione

    L’apprendimento collaterale costituisce la parte piĂč intima della formazione della persona, in quanto induce all’educazione e costruzione di atteggiamenti che, ancor piĂč delle singole conoscenze disciplinari, consentono di giungere ad uno sviluppo personale armonico e stabile. Tale processo si struttura durante tutto l’arco dell’esistenza e, in particolare negli insegnanti, si fonde profondamente con lo sviluppo professionale. In una duplice prospettiva esso riguarda le competenze socio-culturali a cui tendere personalmente e a cui educare i propri allievi. Le possibilitĂ  offerte dalla formazione on- line attraverso le piattaforme e-learning arricchiscono il panorama didattico-pedagogico. Oltre ai classici vantaggi dati dall’accesso semplice e rapido ai contenuti, indipendentemente dai vincoli di spazio e tempo, e dall’approccio costruttivista della conoscenza in un contesto collaborativo, la fruizione di un percorso formativo in rete per un insegnante in contesto formativo offre una serie di spunti di riflessione sulla necessitĂ  di continuare la ricerca del miglioramento di se stessi in una continua destrutturazione e ricostruzione delle proprie competenze.     I sistemi LMS offrono numerose funzionalità applicabili in campo didattico/pedagogico raggruppabili in tre categorie: istruzione, valutazione e comunicazione. Sicuramente questi sistemi si prestano a sollecitare un apprendimento formale, eppure le funzioni di tipo sociale, tipiche invece degli SNS, consentono di ampliare la proposta formativa aprendo il campo educativo all’apprendimento informale che, per mezzo di situazioni di scambio e condivisione di esperienze, idee e contatti sociali, produce conoscenza e crescita di competenze. Attraverso la descrizione e l’analisi quantitativa di alcune attività a connotazione sociale (forum e chat) organizzate su una piattaforma Moodle per studenti di scuola superiore, si considerano analogie e differenze tra queste due tipologie di ambiente per comprendere quali peculiarità pedagogiche concorrono ad ottenere un’unica struttura che ne sfrutti al meglio le caratteristiche nell’apprendimento per studenti in età scolare
