Entre plan national et initiative locale, l'Ă©mergence d'une gouvernance territoriale des risques professionnels ?: Rapport final
A partir du cas des « RĂ©seaux de Veille et de PrĂ©vention » des risques professionnels (RVP) mis en place en Provence-Alpes-CĂŽte d'Azur au dĂ©but des annĂ©es 2000 dans le cadre du Contrat de Plan Etat-RĂ©gion 2000-2006, cette recherche s'est efforcĂ© de comprendre comment s'organisait et innovait l'action publique en matiĂšre de prĂ©vention des risques professionnels. 1. La difficile marche vers une gouvernance territoriale des risques du travail Reprenant les RVP et adoptĂ© dĂšs 2005, le Plan rĂ©gional SantĂ© Travail (PRST) est censĂ© concrĂ©tisĂ© un paradigme « Ă©pidĂ©miologique » qui, dans une logique de santĂ© publique, vise Ă dĂ©cloisonner les diffĂ©rentes composantes de l'action sanitaire. Soucieuse de prĂ©server son autonomie d'action ainsi que le bien fondĂ© d'une rĂ©gulation tripartite impliquant les partenaires sociaux, la direction rĂ©gionale du travail (DRTEFP) s'appuie sur la promotion d'un paradigme, alternatif et complĂ©mentaire, qualifiĂ© d'ergonomique : les atteintes Ă la santĂ© doivent ĂȘtre mises en relation avec les conditions organisationnelles de l'activitĂ© des personnes. Avec ce 2Ăšme paradigme, cette administration de l'Etat redonne du sens Ă une initiative de la RĂ©gion, Ă savoir les accords cadres tripartites de branche couvrant la formation, l'emploi et les conditions de travail. La conclusion et la mise en Ćuvre de nouveaux accords cadres dans la mĂ©tallurgie et la chimie-plasturgie, tout comme le renouvellement d'accords antĂ©rieurs dans des secteurs clĂ©s, tels que le BTP et l'hĂŽtellerie-restauration, constituent des tests probants d'une part, de la capacitĂ© de coopĂ©ration entre acteurs publics et d'autre part, de l'aptitude de ces derniers Ă mettre sur pied des dispositifs qui puissent intĂ©grer la gouvernance des divers risques du travail (versant emploi : exposition au chĂŽmage, obsolescence des qualifications ; versant santĂ© au travail : exposition aux accidents et maladies professionnelles) pour produire plus d'efficacitĂ© dans les gestions publiques et privĂ©es de ces risques. 2. Action collective et appropriation des dĂ©marches prĂ©ventives par de (trĂšs) petites entreprises La finalitĂ© de l'action collective Ă©tudiĂ©e, laquelle relevait initialement des RVP, Ă©tait double : d'une part, mettre Ă disposition des entreprises un guide pour l'Ă©laboration du document unique relatif Ă l'Ă©valuation des risques et, d'autre part, enclencher une action de formation permettant aux entreprises de s'approprier la dĂ©marche pour faire de la prĂ©vention un investissement durable. Au total, Ă l'Ă©chelle des deux professions âHĂŽtellerie-restauration, rĂ©paration automobile- dans les Alpes Maritimes, l'impact de la dĂ©marche reste limitĂ© ; d'ailleurs, les entreprises â non touchĂ©es par l'action collective - ne semblent pas plus Ă©loignĂ©es de la prĂ©vention des risques. L'appropriation durable de la mĂ©thode d'Ă©valuation des risques par les artisans et leurs employĂ©s requiert un service d'accompagnement et de suivi plus consĂ©quent. Cette appropriation est trĂšs dĂ©pendante des types d'entreprise concernĂ©es. On identifie â en prenant l'exemple de la rĂ©paration automobile - trois configurations qui dessinent des relations Ă la main d'Ćuvre spĂ©cifiques et aux normes lĂ©gales en matiĂšre de prĂ©vention : la TPE « indĂ©pendante » et traditionnelle ; la TPE « managĂ©riale » avec un mode de gestion plus industriel, la TPE « entrepreneuriale » qui s'appuie sur une haute maĂźtrise technique et des prestations annexes de qualitĂ©. Plus l'entreprise s'apparente au modĂšle de l'entreprise indĂ©pendante traditionnelle, plus elle met Ă distance la prĂ©vention. Il en ressort qu'une dĂ©marche d'intermĂ©diation doit Ă©chapper au « confort » d'une approche standardisĂ©e des entreprises si elle veut s'adapter Ă la pluralitĂ© des logiques de fonctionnement des TPE visĂ©es. 3. La formation Ă la prĂ©vention des risques, une composante de la qualification professionnelle ? Depuis 15 ans, l'investissement politique et cognitif sur l'enseignement Ă la prĂ©vention des risques professionnels en formation initiale a Ă©tĂ© important. L'objectif est de faire de cet enseignement une dimension reconnue comme telle de la formation professionnelle des jeunes. Au vu des quelques CFA et lycĂ©es professionnels visitĂ©s, nombre de facteurs convergent pour rendre assez abstraite la formation Ă la prĂ©vention des risques. Entreprises et Ă©tablissements de formation ne coopĂšrent pas assez pour faire de la prĂ©vention une composante reconnue de la qualification professionnelle
The Aids policy cycle in Western Europe: From exceptionalism to normalization
In every Western European country the occurrence of Aids has led to exceptional innovations in prevention, patient care, health policy and questions of civil rights. This exception can be explained not only by the fact that a health catastrophe was feared, but also civilizational harm in the field of civil rights. Despite national differences, this brought about similar exceptionalist alliances consisting of health professionals, social movements and those affected. With the failure of a catastrophe to arise signs of fatigue in the exceptionalist alliance and increasing possibilities of medical treatment, exceptionalism in Europe is drawing to a close. The paper elucidates specific aspects of each of the four roughly distinguishable phases in this process: Approx. 1981 - 1986: emergence of exceptionalism. The underlying reasons for exceptionalism are investigated in this paper. Approx. 1986 - 1991: consolidation and performance of exceptionalism. The paper investigates the exceptionalist policy model, more specifically some nationally different factors in the polity and politics that help to explain the different forms of policies. Approx. 1991 - 1996: exceptionalism crumbling, steps toward normalization. The forces driving the process of normalization are investigated. Since 1996: normalization, normality. The changes made in the management of HIV and Aids are elucidated using examples from the fields of health care, primary prevention and drug policies. Aids health-policy innovations, and their risks and opportunities in the course of normalization are investigated. Three possible paths of development are identified: stabilization, generalization and retreat. The chances of utilizing innovations developed in connection with Aids for the modernization of health policy in other fields of prevention and patient care vary from country to country with the degree to which Aids exceptionalism has been institutionalized and the distance of these innovations from medical/therapeutic events. The contribution made by European countries to containing the global Aids crisis is inadequate. --
Die Normalisierung von Aids in Westeuropa: Der Politik-Zyklus am Beispiel einer Infektionskrankheit
Das Auftreten von Aids hat in allen westeuropĂ€ischen LĂ€ndern zu exzeptionellen Innovationen in PrĂ€vention, Krankenversorgung, Gesundheitspolitik und BĂŒrgerrechtsfragen gefĂŒhrt. Diese Ausnahmen vom normalen Verlauf der Gesundheitspolitik erklĂ€ren sich vor allem daraus, daĂ neben einer gesundheitlichen Katastrophe infolge von Aids auch ZivilisationsbrĂŒche auf dem Gebiet der BĂŒrgerrechte befĂŒrchtet wurden. Dies brachte eine trotz groĂer nationaler Unterschiede im Grundmuster Ă€hnliche âexceptionalist allianceâ aus beteiligten Gesundheitsberufen, sozialen Bewegungen und Betroffenen hervor, die den mangels wirksamer Therapien von der Medizin nicht besetzten Handlungsraum produktiv nutzte. Mit dem Ausbleiben der Katastrophe, ErmĂŒdungserscheinungen der âexceptionalist allianceâ und zunehmenden Therapiemöglichkeiten der Medizin geht der âexceptionalism' in Europa jedoch schrittweise zu Ende. In diesem ProzeĂ werden bei national unterschiedlichen Entwicklungsmustern vier Phasen unterschieden: Circa 1981 - 1986: Entstehung des âexceptionalism'. Hierzu werden die Ursachen des âexceptionalism' untersucht. Circa 1986 - 1991: Praxis und Konsolidierung des âexceptionalism'. Dargestellt werden sowohl das Policy-Modell des âexceptionalism' als auch lĂ€nderspezifische Konfigurationen der Polities und Politics, die zu unterschiedlichen Inhalten der politischen Entscheidungsprozesse (Policies) in bezug auf Aids fĂŒhrten. Circa 1991 - 1996: Auflösung des âexceptionalism' und erste Anzeichen der Normalisierung. Untersucht werden die GrĂŒnde des Normalisierungsprozesses. Seit 1996: Normalisierung, NormalitĂ€t. Darstellung des verĂ€nderten Umgangs mit HIV und Aids. Dazu werden Beispiele aus den Bereichen Krankenversorgung, PrimĂ€rprĂ€vention und Drogenpolitik betrachtet. Es wird untersucht, welche gesundheitspolitischen Innovationen der âAids-exceptionalism' in Westeuropa hervorbrachte und auf welche Risiken und Chancen diese Innovationen im Zuge der Normalisierung treffen. Es werden drei mögliche Entwicklungspfade identifiziert: Stabilisierung, Generalisierung und RĂŒckwĂ€rtsentwicklung. Die Chancen, in Zusammenhang mit Aids entwickelte Innovationen fĂŒr die Modernisierung der Gesundheitspolitik auch fĂŒr andere Felder der PrĂ€vention und der Krankenversorgung zu nutzen, variieren lĂ€nderspezifisch. In welchem Umfang âAids-exceptionalism' gesundheitspolitische Innovationen auch in anderen Bereichen anstoĂen kann, hĂ€ngt letztlich vom jeweils erreichten Grad der Institutionalisierung des âexceptionalism' und der Entfernung dieser Neuerungen vom konventionellen medizinisch-therapeutischen Geschehen ab. Modelers' Perception of Mathematical Modeling in Epidemiology: A Web-Based Survey
International audienceBackground: Mathematical modeling in epidemiology (MME) is being used increasingly. However, there are many uncertainties in terms of definitions, uses and quality features of MME. Methodology/Principal Findings: To delineate the current status of these models, a 10-item questionnaire on MME was devised. Proposed via an anonymous internet-based survey, the questionnaire was completed by 189 scientists who had published in the domain of MME. A small minority (18%) of respondents claimed to have in mind a concise definition of MME. Some techniques were identified by the researchers as characterizing MME (e.g. Markov models), while othersâat the same level of sophistication in terms of mathematicsâwere not (e.g. Cox regression). The researchers' opinions were also contrasted about the potential applications of MME, perceived as higly relevant for providing insight into complex mechanisms and less relevant for identifying causal factors. The quality criteria were those of good science and were not related to the size and the nature of the public health problems addressed. Conclusions/Significance: This study shows that perceptions on the nature, uses and quality criteria of MME are contrasted, even among the very community of published authors in this domain. Nevertheless, MME is an emerging discipline in epidemiology and this study underlines that it is associated with specific areas of application and methods. The development of this discipline is likely to deserve a framework providing recommendations and guidance at various steps of the studies, from design to report
Predicting the Lay Preventive Strategies in Response to Avian Influenza from Perceptions of the Threat
Background: The identification of patterns of behaviors that lay people would engage in to protect themselves from the risk of infection in the case of avian influenza outbreak, as well as the lay perceptions of the threat that underlie these risk reduction strategies. Methodology/Principal Findings: A population-based survey (N = 1003) was conducted in 2008 to understand and describe how the French public might respond to a possible outbreak. Factor analyses highlighted three main categories of risk reduction strategies consisting of food quality assurance, food avoidance, and animal avoidance. In combination with the fear of contracting avian influenza, mental representations associated with the manifestation and/or transmission of the disease were found to significantly and systematically shape the behavioral responses to the perceived threat. Conclusions/Significance: This survey provides insight into the nature and predictors of the protective patterns that might be expected from the general public during a novel domestic outbreak of avian influenza

La dĂ©marche de qualitĂ© des soins transforme lâart mĂ©dical en exercice scientifiquement fondĂ©, lâautonomie professionnelle en situation dâinterdĂ©pendance systĂ©mique, la dĂ©finition de la technique mĂ©dicale en construction pluridisciplinaire et politique. ModĂšle dâorganisation et dâĂ©valuation de lâaction collective de soins dans une perspective dâefficacitĂ© curative et de maĂźtrise de la dĂ©pense, elle repose sur lâĂ©laboration de nouvelles mĂ©thodes et techniques normatives qui, visant Ă encadrer les comportements professionnels, affectent les intĂ©rĂȘts, les modes dâorganisation, les valeurs des divers acteurs liĂ©s par le systĂšme, et apparaissent ainsi comme des enjeux majeurs de nĂ©gociations et de conflits.Quality of care (QC), understood as a set of norms defining âhow to treat the patient rightâ in the public interest, is a new programme for organizing health systems that is being developed in all advanced pluralist democracies. QC aims to be a systemic means for regulating health-care supply in a social and political context that pays ever more attention to âdangersâ in the health sector and to the rising cost of health. It entails drawing up new operational specifications based on methods and techniques for assessing actual practices in health care in terms of expenditures and overall effectiveness. QCâs characteristics are analysed: feedback between the definition of a strategy for care and its assessment through experiments, the use of systems of information, the tracking of errors, auditing, etc. QC aims at providing, on a technical basis, an efficient social and political response to the growing âcomplexificationâ of health care. It thus tries to integrate into the notion of âqualityâ the products, structures, procedures, information and co-ordination that takes place in the process of providing care. Though not yet widely implemented in France, QC, as it gradually spreads, significantly redistributes power among health-care providers. It creates major changes in cognitive, normative and organisational arrangements, that, in turn, modify interests, organisations, values and norms among health professionals. Moreover, it brings new actors into the game. QC thus turns out to be a major social and occupational issue for reforming health systems
Des politiques de santé à leur management : un sujet sans objet ou une nécessité sans capacités ?
Les politiques publiques de santĂ© spĂ©cifiques d'une pathologie se prĂ©sentent sous la forme d'une intervention programmĂ©e des pouvoirs publics dans un secteur habituellement sous l'autoritĂ© absolue de la profession mĂ©dicale structurĂ©e autour de la maladie. Que ce soit sous la forme d'une alternative au modĂšle curatif ou d'une rĂ©gulation visant Ă modifier des pratiques mĂ©dicales jugĂ©es inadaptĂ©es, ces politiques publiques n'arrivent pas, en France, Ă trouver leur traduction concrĂšte. A partir de deux Ă©tudes de cas rĂ©alisĂ©es entre 1989 et 1992, l'analyse des processus de dĂ©cisions permet d'expliquer pourquoi ces politiques ne parviennent pas Ă leurs objectifs ; l'absence de management public, concept et pratique totalement inconnus dans ce secteur, rĂ©duit leur portĂ©e aux enjeux des acteurs de leur mise en Ćuvre.Setbon Michel. How has AIDS been put on the political agenda ? The conditions wherein decisions were made about testing for AIDS are pointed out ; and the policy of testing is analyzed as well as various actors' positions on this issue. This policy's operational and institutional effects are examined.Cet article vise à rendre compte de la mise en agenda politique d'une maladie telle que le Sida.
