    Sonographic assessment of the normal limits of the spleen in healthy school children in South-East Nigeria

    Background/Objective: Ultrasonogrphy is a good modality for the  detection of splenomegaly even when it is not clinically palpable. The objective of this study was to establish the normal values of splenic length in healthy school children in South.East, Nigeria and to correlate them with body indices.Materials and Methods: This is a cross.sectional prospective study of 1315 children (633 boys and 682 girls) between the ages of 5 and 17 years. The splenic length was measured between the most superiomedial and the most inferiolateral margins, at the level of the hilum. Only the spleens that had normal shape and echotexture were measured. The mean splenic length and the 5th and 95th percentiles were determined for each age. The length was correlated with the sex, age, weight (WT), height, body mass index (BMI) and body surface area (BSA) of the subjects. A regression model for prediction of spleen dimension from age and body habitus was computed.Results: There was a significant correlation between splenic length and age, P < 0.001. Males had statistically significant longer spleen length than females. The splenic length correlated best with BSA, followed by body WT and least with BMI.Conclusions: This study noted racial variation between the established Nigerian values and results from other countries of the world. For the first time, a baseline value for splenic size for the Nigerian Children of various ages has been established with a regression model for predicting the splenic sizes.Key words: Children, echotexture, Nigeria, sonography, spleen,  splenomegaly, ultrasonograph

    Adult intussusception is a rare condition that is seen once in a while in surgical practice, 1. In this report we the case of a thirty –five year old Nigeria male on medical treatment of earlier diagnosed AIDS who had no evidence of cutaneous Kaposi sarcoma. He later presented with a palpable right lower quadrant mass and classic symptoms of intestinal obstruction for which he was referred to surgery. The findings at laparotomy were right ileo – colonic intussusception with polypoid lead point. There were also multiple intra – luminal growths involving the entire right colon, the caecum the appendix and the terminal ileum. A right hemi – colectomy with ileo – colonic anastamosis was performed, and the resulting specimen sent for histological examination confirmed Kaposi sarcoma. Assessment of the patient's clinical condition five months post – operatively revealed a significant improvement; patient regained weight from 43 kg on admission to 58.3 kg .We conclude that Kaposi sarcoma be included in the aetipathogenesis of adult intussusception KEY WORDS: Kaposi Sarcoma, Adult Intussusception, Surgical Outcome Nigerian Journal of Clinical Practice Vol.6(2) 2003: 117-11

    Kidney Parameters And Body Habitus Among South Eastern Nigerians

    Background; Imaging study of the kidney parameters helps in monitoring kidney sizes to enable early detection of deviations from normal, which may be early warning signs of a serious underlying disease state. Materials and Methods: The kidney size 120 subjects was studied by ultrasonography. Right and left kidneys were examined. The age of the subjects ranged from 15-70 yrs in both sexes. Sixty seven men (56%) and 53(44%) women participated in study. The ultraonographic assessment was performed with a multipurpose ultrasonographic scanner made by Philips Sterling with 3.5MHz transducer used for deep structures. The length, breadth and thickness of the kidney were measured. The analysis was done by computer with SPSS (version 100) package. Results and Discussion: The study established a mean kidney length of 104.771mm, breadth 59.59mm and thickness 40.13.59. McMinn stated Caucasian figures higher than the values from our study. These racial differences could be attributable to genetic and environmental factors. Left kidney length, breadth and thickness were found to be longer in males than in female, length 107.7mm (males) and 105.8 (females). Keywords: Kidney, Body Habitus, Ultrasound.
