    Clinical, Pathological, and Surgical Outcomes for Adult Pineoblastomas

    Introduction Pineoblastomas are uncommon primitive neuroectodermal tumors that occur mostly in children; they are exceedingly rare in adults. Few published reports have compared the various aspects of these tumors between adults and children. Methods The authors report a series of 12 pineoblastomas in adults from 2 institutions over 24 years. The clinical, radiologic, and pathologic features and clinical outcomes were compared with previously reported cases in children and adults. Results Patient age ranged from 24 to 81 years, and all but 1 patient exhibited symptoms of obstructive hydrocephalus. Three patients underwent gross total resection, and subtotal resection was performed in 3 patients. Diagnostic biopsy specimens were obtained in an additional 6 patients. Pathologically, the tumors had the classical morphologic and immunohistochemical features of pineoblastomas. Postoperatively, 10 patients received radiotherapy, and 5 patients received chemotherapy. Compared with previously reported cases, several differences were noted in clinical outcomes. Of the 12 patients, only 5 (42%) died of their disease (average length of survival, 118 months); 5 patients (42%) are alive with no evidence of disease (average length of follow-up, 92 months). One patient died of unrelated causes, and one was lost to follow-up. Patients with subtotal resections or diagnostic biopsies did not suffer a worse prognosis. Of the 9 patients with biopsy or subtotal resection, 4 are alive, 4 died of their disease, and 1 died of an unrelated hemorrhagic cerebral infarction. Conclusions Although this series is small, the data suggest that pineoblastomas in adults have a less aggressive clinical course than in children

The field of geoarchaeology has typically been defined as either geology pursued within an archaeological framework or (sometimes the reverse) as archaeology framed with the help of geological methodology. Either way, the formalized objectives of geoarchaeology define a broad range of pursuits, from placing archaeological sites into relative and absolute temporal context through the application of stratigraphic principles and absolute dating techniques, to understanding the natural processes of site formation, to reconstructing the landscapes that existed around a site or group of sites at the time of occupation. The editors of this volume have generally followed the lead of G.R. Rapp and C.L. Hill (2006, Geoarchaeology : the earth-science approach to archaeological interpretation) by stressing the importance of multiple viewpoints and methodologies in applying geoscience techniques to evaluate the archaeological record. In the broadest sense, then, Geoarchaeology of St. Catherines Island applies multiple earth science concepts, techniques, or knowledge bases to the known archaeological record and the processes that created that record. This volume consists of 16 papers presenting the newest research on the stratigraphic and geomorphological evolution of the St. Catherines Island landscape. Of particular interest are presentations addressing the relative timing and nature of sedimentation, paleobiology, sea level change, stream capture, hydrology, and erosional patterning evident on St. Catherines Island (and to some degree the rest of the Georgia Bight).     The Pursuit of Training Meritorious Learners of Diverse Backgrounds: Mayo Clinic College of Medicine and Science

    © 2020 Mayo Foundation for Medical Education and Research The doors of the Mayo Clinic College of Medicine and Science (MCCMS) are open to all meritorious learners, including learners who come from communities that have endured longstanding and profound health and economic disparities. In our contemporary world, upward mobility in socioeconomic status is often a function of successful attainment of higher education. One may justifiably ask if all sociodemographic groups in the United States have equal access to higher education so they can gain knowledge and acquire skill sets often necessary to lead a productive life. Several biopsychosocial factors may determine as to whether those that see the “mountain” of higher education at a distance will eventually succeed at surmounting obstacles to rise to the highest peaks. In this article, our earlier experiences are analyzed with the goal of offering insights into novel approaches to train meritorious learners from all sociodemographic groups, including those who come from underserved communities. In addition, we highlight an apolitical and academically rigorous social cognition model that informs the contemporary academic agenda of diversity and inclusion, and we trace its root to the work of the founding father of experimental social psychology
