    Management considerations of massive hemoptysis while on extracorporeal membrane oxygenation.

    BACKGROUND: Veno-arterial extracorporeal membrane oxygenation (V-A ECMO) is a life-saving procedure in patients with both respiratory and cardiac failure. Bleeding complications are common since patients must be maintained on anticoagulation. Massive hemoptysis is a rare complication of ECMO; however, it may result in death if not managed thoughtfully and expeditiously. METHODS: A retrospective chart review was performed of consecutive ECMO patients from 7/2010-8/2014 to identify episodes of massive hemoptysis. The management of and the outcomes in these patients were studied. Massive hemoptysis was defined as an inability to control bleeding (\u3e300 mL/day) from the endotracheal tube with conventional maneuvers, such as bronchoscopy with cold saline lavage, diluted epinephrine lavage and selective lung isolation. All of these episodes necessitated disconnecting the ventilator tubing and clamping the endotracheal tube, causing full airway tamponade. RESULTS: During the period of review, we identified 118 patients on ECMO and 3 (2.5%) patients had the complication of massive hemoptysis. One case was directly related to pulmonary catheter migration and the other two were spontaneous bleeding events that were propagated by antiplatelet agents. All three patients underwent bronchial artery embolization in the interventional radiology suite. Anticoagulation was held during the period of massive hemoptysis without any embolic complications. There was no recurrent bleed after appropriate intervention. All three patients were successfully separated from ECMO. CONCLUSIONS: Bleeding complications remain a major issue in patients on ECMO. Disconnection of the ventilator and clamping the endotracheal tube with full respiratory and cardiac support by V-A ECMO is safe. Early involvement of interventional radiology to embolize any potential sources of the bleed can prevent re-hemoptysis and enable continued cardiac and respiratory recovery

    Home Food Production Before, During and Since Start of the COVID-19 Pandemic in Northern New England

    This brief details the results from three separate surveys of Northern New Englanders in Maine and Vermont in summer 2020, spring 2021, and spring 2022. A survey was conducted in summer of 2020 to understand the initial and continued impacts of the COVID-19 pandemic on food security, diet, and health outcomes. Two additional surveys were conducted using the same methods in spring 2021 and 2022 to continue to assess changes during the pandemic. All surveys were representative of the state populations on race and ethnicity and the data presented in this brief were weighted to be representative of income in both states. Collectively, the surveys represent more than 3,000 individuals. Key results include:1. Home Food Production (HFP) is common and has continued to increase among respondents since the beginning of the pandemic. Among respondents, 32% participated in HFP in 2020, 59% in 2021, and 65% in 2022. 2. The most common HFP activities in 2022 were gardening (48%) and food preservation (34%). 3. Food insecurity has been variable across years: 40% of households were food insecure in 2020, 31% of households were food insecure in 2021, and 39% of respondents were food insecure in 2022. 4. Food insecure households were more likely to participate in HFP (especially fishing, foraging, hunting, and raising animals for meat, dairy or eggs) compared to food secure households across all three surveys. 5. Pests (63%), weather (60%), and money for equipment or supplies (58%) were the biggest barriers for people participating in any HFP

    Glycolysis in the African Trypanosome: Targeting Enzymes and Their Subcellular Compartments for Therapeutic Development

    Subspecies of the African trypanosome, Trypanosoma brucei, which cause human African trypanosomiasis, are transmitted by the tsetse fly, with transmission-essential lifecycle stages occurring in both the insect vector and human host. During infection of the human host, the parasite is limited to using glycolysis of host sugar for ATP production. This dependence on glucose breakdown presents a series of targets for potential therapeutic development, many of which have been explored and validated as therapeutic targets experimentally. These include enzymes directly involved in glucose metabolism (e.g., the trypanosome hexokinases), as well as cellular components required for development and maintenance of the essential subcellular compartments that house the major part of the pathway, the glycosomes

    Effects of cochlear implantation on binaural hearing in adults with unilateral hearing loss

    A FDA clinical trial was carried out to evaluate the potential benefit of cochlear implant (CI) use for adults with unilateral moderate-to-profound sensorineural hearing loss. Subjects were 20 adults with moderate-to-profound unilateral sensorineural hearing loss and normal or near-normal hearing on the other side. A MED-EL standard electrode was implanted in the impaired ear. Outcome measures included: (a) sound localization on the horizontal plane (11 positions, −90° to 90°), (b) word recognition in quiet with the CI alone, and (c) masked sentence recognition with the target at 0° and the masker at −90°, 0°, or 90°. This battery was completed preoperatively and at 1, 3, 6, 9, and 12 months after CI activation. Normative data were also collected for 20 age-matched control subjects with normal or near-normal hearing bilaterally. The CI improved localization accuracy and reduced side bias. Word recognition with the CI alone was similar to performance of traditional CI recipients. The CI improved masked sentence recognition when the masker was presented from the front or from the side of normal or near-normal hearing. The binaural benefits observed with the CI increased between the 1- and 3-month intervals but appeared stable thereafter. In contrast to previous reports on localization and speech perception in patients with unilateral sensorineural hearing loss, CI benefits were consistently observed across individual subjects, and performance was at asymptote by the 3-month test interval. Cochlear implant settings, consistent CI use, and short duration of deafness could play a role in this result

    Food Security Impacts of the COVID-19 Pandemic: Following a Group of Vermonters During the First Year

    We surveyed a cohort of Vermonters three times during the first year of the COVID-19 pandemic to understand the impacts of the pandemic on food security and food access. The surveys were conducted in March/April 2020, June 2020, and March/April 2021. A total of 441 Vermonters responded to all three surveys. This brief shares changes in their experiences between March 2020 and March 2021. Key findings include: 1. Food insecurity rates increased during the pandemic and remained above pre-pandemic levels a year after the start of the pandemic. 2. 31.6% of respondents experienced food insecurity at some point in the first year of the pandemic. Of those, 46.9% were food insecure before the pandemic, but over half (53.1%) were newly food insecure. Of the respondents who experienced food insecurity at any point during the pandemic, 61.9% were still food insecure in March 2021. 3. People without a college degree, those with a job disruption, households with children, women, and younger people had greater odds of experiencing food insecurity in March 2021. 4. Fewer respondents were using federal food assistance programs and food pantries in March 2021 compared to earlier in the pandemic. 5. More than half of respondents (54.2%) reported suffering a job disruption (i.e. job loss, reduction in work hours or income, furlough) since March 2020 and 18.4% were still experiencing a job disruption in March 2021. 6. Vermonters today are less concerned about food becoming more expensive and possible loss of food assistance programs compared to earlier in the pandemic
