    How and why do South Asians attend GUM clinics? Evidence from contrasting GUM clinics across England

    Background: Improving access to sexual healthcare is a priority in the UK, especially for ethnic minorities. Though South Asians in the UK report low levels of sexual ill health, few data exist regarding their use of genitourinary medicine (GUM) services. Objectives: To describe reasons for attendance at GUM clinics among individuals of South Asian origin relative to patients of other ethnicities. Methods: 4600 new attendees (5% South Asian; n=226) at seven sociodemographically and geographically contrasting GUM clinics across England completed a questionnaire between October 2004 and March 2005, which were linked to routine clinical data. Results: South Asians were more likely than other groups to be signposted to the GUM clinic by another health service-for example, in women 14% versus 8% respectively (p=0.005) reported doing so from a family planning clinic. These women also reported that they would be less likely to go to the clinic if their symptoms resolved spontaneously compared with other women (51% vs 31%, p=0.024). However, relative to other clinic attendees, no differences in the proportions of South Asians who had acute STI(s) diagnosed at clinic were noted. Furthermore, South Asian men were more likely to report as their reason for attendance that they wanted an HIV test (23.4% vs 14.8%, p=0.005). Conclusion: Despite having similar STI care needs to attendees from other ethnic groups, South Asians, especially women, may be reluctant to seek care from GUM clinics, especially if their symptoms resolve. Sexual health services need to develop locally-delivered and culturally-appropriate initiatives to improve care pathways

    Equity and diversity in institutional approaches student-staff partnership schemes in higher education

Approaches to student engagement in teaching and learning must evolve to meet the changing needs of these diverse groups of students equitably. Student–staff partnership is an approach to student engagement embracing the diverse experiences of both students and staff. Growing evidence shows significant positive outcomes for students and staff. Practice, however, is dominated by small scale initiatives, limiting opportunities for equitable engagement. In institutions where partnership has been scaled-up, little research has explored these institutional partnership schemes. Our research addresses this gap by exploring institutional approaches to partnership across 11 universities in the United Kingdom. Results provide important lessons for improving equity in access to partnership schemes and the need for broader conceptions of scaling up partnership

    The Intensive English Program (IEP) high-beginners looked proud for the first time since they had started the course months earlier, their e-books projected dazzlingly on the wall of the classroom. The adult-education class turned into a sea of raised hands and smiling faces, each learner eager to stand in front of the class and read the unique, colorful paper book they had created. The preschoolers clamored to read another story so they could draw a new ending to that one as well. What do these three groups of learners have in common? They are all excited to read more because they have been reading with a purpose: to create a new book based on the text they had just read and share their creation with their peers

    The ‘Partnership Identity’ in Higher Education: Moving From ‘Us’ and ‘Them’ to ‘We’ in Student-Staff Partnership

    Student-staff partnerships in higher education re-frame the ways that students and staff work together as active collaborators in teaching and learning. Such a radical re-visioning of the relationships between students, staff, and the institutions within which they function is both potentially transformational and a significant challenge given the deeply entrenched identities, and attached norms, that form a part of the institutional culture. Explicit examination of the ways in which identity formation and navigation influences, and is influenced by, student-staff partnership is an important but under-explored area in the partnership literature. Drawing on structured reflective narratives focused on our own partnership experiences, we employ collaborative autoethnographic methods to explore this nexus through a social identity lens. Results highlight the need to move away from the labelling of dichotomous student/staff roles and identities in the context of partnership to a more nuanced conception that embraces the multiplicity of identity and diverse dimensions of meaning. We highlight the power of the normative conceptions that we attach to different identities, particularly where dissonance arises should those norms conflict. We discuss how this dissonance was particularly salient for us as we crossed the partnership threshold, only to find that the ethos underlying our new partnership identities contradicted the traditional hierarchical structure of the institutions within which we continued to function. Finally, we highlight the implications of these results for those engaging in student-staff partnerships in higher education and point the way toward potentially fruitful avenues of future research

    Capturing sexual contact patterns in modelling the spread of sexually transmitted infections : evidence using Natsal-3

    Background Understanding the spread of sexually transmitted infections (STIs) in a population is of great importance to the planning and delivery of health services globally. The worldwide rise of HIV since the 1980’s, and the recent increase in common STIs (including HPV and Chlamydia) in many countries, means that there is an urgent need to understand transmission dynamics in order to better predict the spread of such infections in the population. Unlike many other infections which can be captured by assumptions of random mixing, STI transmission is intimately linked to the number and pattern of sexual contacts. In fact, it is the huge variation in the number of new sexual partners that gives rise to the extremes of risk within populations which need to be captured in predictive models of STI transmission. Such models are vital in providing the necessary scientific evidence to determine whether a range of controls (from education to screening to vaccination) are cost-effective. Method and results We use probability sample survey data from Britain’s third National Survey of Sexual Attitudes and Lifestyles (Natsal-3) to determine robust distributions for the rate of new partnerships that involve condomless sex and can therefore facilitate the spread of STIs. Different distributions are defined depending on four individual-level characteristics: age, sex, sexual orientation, and previous sexual experience. As individual behaviour patterns can change (e.g. by remaining in a monogamous relationship for a long period) we allow risk-percentiles to be randomly redrawn, to capture longer term behaviour as measured by Natsal-3. We demonstrate how this model formulation interacts with the transmission of infection by constructing an individual-based SIS-P (Susceptible—Infected—Susceptible—Protected) transmission model for the spread of a generic STI, and observing overall population demographics when varying the transmission probability within a partnership, recovery rate and the level of population protection (e.g. from vaccination where applicable)

    A conceptual model for recruitment and retention : allied health workforce enhancement in Western Victoria, Australia

    Attracting and retaining allied health professionals in rural areas is a recognised problem in both Australia and overseas. Predicted increases in health needs will require strategic actions to enhance the rural workforce and its ability to deliver the required services. A range of factors in different domains has been associated with recruitment and retention in the allied health workforce. For example, factors can be related to the nature of the work, the personal needs, or the way an organisation is led. Some factors cannot be changed (eg geographical location of extended family) whereas others can be influenced (eg education, support, management styles). Recruitment and retention of allied health professionals is a challenging problem that deserves attention in all domains and preparedness to actively change established work practices, both individually as well as collectively, in order to cater for current and predicted health needs. Changes to enhance workforce outcomes can be implemented and evaluated using a cyclic model. The Allied Health Workforce Enhancement Project of the Greater Green Triangle University Department of Rural Health (GGT UDRH) is working towards increasing the number of allied health professionals in the south west of Victoria. Based on themes identified in the literature, an interactive model is being developed that addresses recruitment and retention factors in three domains: (1) personal or individual; (2) organisation; and (3) community.<br /

Participants completed the Marx and Gross audiotaped date-rape vignette and indicated if, and when, the man in the vignette should refrain from making further sexual advances. In order to examine the impact of social influences, participants completed the task alone or with an opposite sex confederate. Individuals that completed the task with an opposite sex confederate took much longer to make the risk recognition identification.     Trends in frequency of sexual activity and number of sexual partners among adults aged 18 to 44 years in the US, 2000-2018

    Importance: Sexual relationships are important for well-being and health. Recent trends in sexual activity among US adults are unknown. Objectives: To examine trends in reported frequency of sexual activity and number of sexual partners by sex and age and the association between measures of sexual activity and sociodemographic variables. Design, Setting, and Participants: In this survey study, repeat, cross-sectional analyses of participants aged 18 to 44 years from 10 rounds of the General Social Survey (2000-2018), a US nationally representative survey, were performed for men and women separately. Main Outcomes and Measures: Sexual frequency in the past year (sexual inactivity, once or twice per year, 1-3 times per month, or weekly or more) and number of sexual partners in the past year (0, 1, 2, or ≄3 partners). The association between measures of sexual activity and sociodemographic variables were assessed using logistic regression. Results: The study population included 4291 men and 5213 women in the analysis of sexual frequency and 4372 men and 5377 women in the analysis of number of sexual partners (mean [SD] age, 31.4 [7.6] years; survey response rate, 59.5%-71.4%). Between 2000-2002 and 2016-2018, the proportion of 18- to 24-year-old individuals who reported having had no sexual activity in the past year increased among men (18.9% vs 30.9%; age-adjusted odds ratio [aOR] for trend across survey periods, 1.20; 95% CI, 1.04-1.39) but not among women (15.1% vs 19.1%; aOR for trend, 1.03; 95% CI, 0.89-1.18). Smaller absolute increases in sexual inactivity were observed among those aged 25 to 34 years for both men (7.0% vs 14.1%; aOR for trend, 1.23; 95% CI, 1.07-1.42) and women (7.0% vs 12.6%; aOR for trend, 1.17; 95% CI, 1.01-1.35) but not among those aged 35 to 44 years. The increase in sexual inactivity coincided with decreases in the proportion reporting weekly or more sexual frequency (men aged 18-24 years: 51.8% vs. 37.4%; aOR for trend, 0.88 [95% CI, 0.79-0.99]; men aged 25-34 years: 65.3% vs 50.3%; aOR for trend, 0.87 [95% CI, 0.81-0.94]; women aged 25-34 years: 66.4% vs. 54.2%; aOR for trend, 0.90 [95% CI, 0.84-0.96]) or 1 sexual partner (men aged 18-24 years: 44.2% vs. 30.0%; aOR for trend, 0.88 [95% CI, 0.80-0.98]; women aged 25-34 years: 79.6% vs 72.7%; aOR for trend, 0.91 [95% CI, 0.84-0.99]) and occurred mainly among unmarried men (unmarried men aged 18-44 years: 16.2% vs 24.4%; aOR for trend, 1.14 [95% CI, 1.04-1.25]). Among married men and women, weekly or more sexual frequency decreased (men: 71.1 % vs 57.7%; aOR for trend, 0.86 [95% CI, 0.79-0.93]; women: 69.1% vs 60.9%; aOR for trend, 0.92 [95% CI, 0.86-0.99]). Men with lower income (aOR for men with an annual income of ≄50000vs50 000 vs 0-$9999, 0.37 [95% CI, 0.15-0.90]) and with part-time (aOR vs full-time employment, 2.08; 95% CI, 1.48-2.93) and no employment (aOR vs full-time employment, 2.08; 95% CI, 1.48-2.93) were more likely to be sexually inactive, as were men (aOR vs full-time employment, 2.94; 95% CI, 2.06-4.21) and women (aOR vs full-time employment, 2.37; 95% CI, 1.68-3.35) who were students. Conclusions and Relevance: This survey study found that from 2000 to 2018, sexual inactivity increased among US men such that approximately 1 in 3 men aged 18 to 24 years reported no sexual activity in the past year. Sexual inactivity also increased among men and women aged 25 to 34 years. These findings may have implications for public health
