    Comparison of the use of prenatal care services and the risk of preterm birth between pregnant women with disabilities and those without disabilities: A nationwide cohort study

    ObjectiveThe difficulties faced by pregnant women with disabilities in accessing health care may make them less likely to receive prenatal care. The aims of this study were to compare the number of prenatal services and the risk of preterm birth between pregnant women with and without disabilities.MethodsA total of 2999 pregnant women aged ≥20 years with birth records in 2011–2014 in Taiwan were enrolled. Data were obtained from the Registration File for Physical and Mental Disabilities and the National Health Insurance Research Database. A 1:4 matching between pregnant women with disabilities and those without disabilities was performed. The logistic regression analysis with generalized estimating equations was used to analyze.ResultsThe median of prenatal care services used by pregnant women with disabilities was 9.00 (interquartile range, IQR: 2.00). Pregnant women with disabilities used fewer services than those without disabilities (median, 10.00; IQR: 1.00). The disabled group (8.44%) had a significantly higher proportion of preterm births than did the non-disabled group (5.40%). The disabled group was at a 1.30 times higher risk of preterm births than was the non-disabled group.ConclusionsPregnant women with disabilities used significantly fewer prenatal care services and had a significantly higher risk of preterm birth than pregnant women without disabilities

    Enteric bacterial loads are associated with interleukin-6 levels in systemic inflammatory response syndrome patients

    AbstractBackgroundLoss of intestinal integrity is a critical contributor to excessive inflammation following severe trauma or major surgery. In the case of enterocyte damage, intestinal fatty acid-binding protein (IFABP) is released into the extracellular space. Excessive production of interleukin (IL)-6 can induce systemic inflammatory response syndrome (SIRS). However, the correlation of IL-6 with gut barrier failure and bacterial translocation in critically ill patients has not been well characterized.PurposesTo define the relationship between enteric bacterial loads and IL-6 levels in patients with SIRS.MethodsVariables related to prognosis and treatment were measured in 85 patients with SIRS upon admission to the emergency room. IL-6 and IFABP were measured using an enzyme-linked immunosorbent assay. Enteric bacterial loads in blood were measured through quantitative real-time polymerase chain reaction with primers specific for enteric bacteria.ResultsMultivariate analysis revealed a positive correlation between enteric bacterial loads and IL-6 levels in blood. Elevated IFABP concentration was associated with low blood pressure, high respiration rate, hyperglycemia, and high Sequential Organ Failure Assessment score. Elevated C-reactive protein concentrations were associated with higher soluble CD14 levels in blood.ConclusionEnterocyte damage is associated with hypotension and tachypnia in patients with SIRS. Gut function failure may permit enteric bacteria to enter the blood, thereby elevating IL-6 levels and inducing a systemic inflammatory response, resulting in multiple organ failure

    Depressive Neurosis Treated by Acupuncture for Regulating the Liver —A Report of 176 Cases

    ObjectiveTo observe therapeutic effect of acupuncture for regulating the liver on depressive neurosis.MethodsIn a multi-center randomized controlled trial, 440 patients were divided into 3 groups: Acupuncture group for regulating the liver (Acup., 176 cases) was treated by acupuncture at Siguan Points, i.e. bilateral Hegu (LI 4) and Taichong (LR 3), Baihui (GV 20) and Yintang (EX-HN3) plus ear-acupuncture, Prozac group (P., 176 cases) by oral administration of Prozac, and Non-acupoint needling group (NAN, 88 cases) by acupuncture at non-acupoints as acupuncture placebo. Self-rating Depression Scale (SDS) was examined before treatment, and one month, two and three months after treatment respectively to evaluate therapeutic effect, and Rating Scale for Side Effects (SERS) was used to evaluate the safety.ResultsAfter one month of treatment, SDS scores in Acup. Group were significantly lower than that in P. Group (P<0.05) and than that in NAN Group (P<0.01), and SDS scores in P. Group were lower than that in NAN Group (P<0.05), showing the SDS scores in Acup. Group <P. Group <NAN Group. After 2 months of treatment, SDS scores in Acup. Group were also significantly lower than that in P. Group (P<0.01) and than that in NAN Group (P<0.01), and SDS scores in P. Group were also lower than that in NAN Group (P<0.05), showing the SDS scores in Acup. Group <P. Group <NAN Group. After 3 months of treatment, SDS scores in Acup. Group were also significantly lower than that in P. Group (P<0.01) and than that in NAN Group (P<0.01), and SDS scores in P. Group were also lower than that in NAN Group (P<0.01), showing the SDS score in Acup. Group <P. Group <NAN Group. After treatment, SERS scores were 0.16±0.95, 6.51±5.09 and 0.23±1.36 in Acup. Group and NAN Group (P>0.05), showing the SERS scores in Acup. Group <NAN Group <P. Group. No side effect was found in Acup. and NAN groups.ConclusionThe therapeutic effect of acupuncture on depressive neurosis is better than or similar to that of Prozac but with less side effect

    Use of electrical impedance tomography to monitor dehydration treatment of cerebral edema: a clinical study

    In this study EIT was used to monitor brain impedance changes due to variations in cerebral fluid content during dehydration treatment of edema patients. 30 patients with cerebral edema were continuously imaged for two hours after the initiation of dehydration treatment. Results show that overall impedance across the brain increased significantly 5 minutes after dehydration treatment started. And different brain tissues have different reactions towards dehydration

    HIV self-testing among men who have sex with men in China: a qualitative implementation research study.

    BACKGROUND: HIV self-testing (HIVST) may expand HIV testing, but there have been few pilot programmes among men who have sex with men (MSM). This purpose of this study was to evaluate HIVST implementation among MSM in China using qualitative methods. METHODS: We undertook semistructured interviews among MSM and those organising HIVST programmes for MSM. Purposive sampling method was used to ensure men with different HIV serostatuses, ages and HIVST frequencies were included. Men were recruited from MSM community-based organisations and a local HIV clinic. An implementation science framework was used to interpret the findings. Two individuals used a standard code-based methodology to identify themes. RESULTS: Forty-two MSM and six stakeholders were interviewed. Our data showed many MSM and stakeholders preferred HIVST to facility-based testing. Most men reported that HIVST empowered MSM and informed sexual decision making. Many men noted that decreasing the HIVST price may increase demand. Some men noted that HIVST could be scaled up through social media and by modifying bulky packaging. Minimal adverse events were reported. CONCLUSIONS: HIVST may expand HIV testing and promote empowerment of MSM. Minimal adverse outcomes were noted, but further implementation research is needed

    A genetic variation map for chicken with 2.8 million single-nucleotide polymorphisms

    We describe a genetic variation map for the chicken genome containing 2.8 million single-nucleotide polymorphisms ( SNPs). This map is based on a comparison of the sequences of three domestic chicken breeds ( a broiler, a layer and a Chinese silkie) with that of their wild ancestor, red jungle fowl. Subsequent experiments indicate that at least 90% of the variant sites are true SNPs, and at least 70% are common SNPs that segregate in many domestic breeds. Mean nucleotide diversity is about five SNPs per kilobase for almost every possible comparison between red jungle fowl and domestic lines, between two different domestic lines, and within domestic lines - in contrast to the notion that domestic animals are highly inbred relative to their wild ancestors. In fact, most of the SNPs originated before domestication, and there is little evidence of selective sweeps for adaptive alleles on length scales greater than 100 kilobases
