    Modeling the water regime of the Kura cascade of reservoirs in the Republic of Azerbaijan

    This study was conducted in connection with the recent increasing attention to saving water resources in Azerbaijan and to studying the condition of the largest cascade of reservoirs on the Kura River. The article highlights some aspects of hydrological modeling of this cascade. Modeling occurs in several stages: an orographic scheme is created, a system of differential equations compiled on the basis of this scheme is solved, and the results are displayed. To carry out the simulation, a program was developed in the MATLAB environment. The program works in 2 modes – design and working. In the design mode, an orographic diagram is created, including the main objects of the cascade (reservoirs, rivers, canals) and their parameters are set (volume, water transfer, geodata about location). The program allows you to edit and add objects. When switching to the operating mode, according to the selected model, a system of differential equations is created corresponding to the current orographic scheme. The number of equations is equal to the number of reservoirs in the cascade. The equations are solved using MATLAB. The solution is carried out sequentially, starting from the first equation of the system. Then the results found are substituted into the 2nd equation. With each subsequent equation its complexity increases. The results are displayed in the form of obtained formulas and in the form of graphs reflecting the state of the cascade. Thus, the developed program has the ability to carry out visual design, and can also be successfully used to model similar reservoir cascades

    RECAP-KG: Mining Knowledge Graphs from Raw GP Notes for Remote COVID-19 Assessment in Primary Care

    Clinical decision-making is a fundamental stage in delivering appropriate care to patients. In recent years several decision-making systems designed to aid the clinician in this process have been developed. However, technical solutions currently in use are based on simple regression models and are only able to take into account simple pre-defined multiple-choice features, such as patient age, pre-existing conditions, smoker status, etc. One particular source of patient data, that available decision-making systems are incapable of processing is the collection of patient consultation GP notes. These contain crucial signs and symptoms - the information used by clinicians in order to make a final decision and direct the patient to the appropriate care. Extracting information from GP notes is a technically challenging problem, as they tend to include abbreviations, typos, and incomplete sentences. This paper addresses this open challenge. We present a framework that performs knowledge graph construction from raw GP medical notes written during or after patient consultations. By relying on support phrases mined from the SNOMED ontology, as well as predefined supported facts from values used in the RECAP (REmote COVID-19 Assessment in Primary Care) patient risk prediction tool, our graph generative framework is able to extract structured knowledge graphs from the highly unstructured and inconsistent format that consultation notes are written in. Our knowledge graphs include information about existing patient symptoms, their duration, and their severity. We apply our framework to consultation notes of COVID-19 patients in the UK COVID-19 Clinical Assesment Servcie (CCAS) patient dataset. We provide a quantitative evaluation of the performance of our framework, demonstrating that our approach has better accuracy than traditional NLP methods when answering questions about patients

    Design and physical operation principles of new microchannel avalanche photodiode (MC APD) with gain up to 10^5 and linearity range improved an order of magnitude compared to known similar devices. A distinctive feature of the new device is a directly biased p-n junction under each pixel which plays role of an individual quenching resistor.     Study of biochemical markers in newborns with necrotizing enterocolitis

    Aim. To study the level of biochemical markers to optimize the diagnosis and prognosis of necrotizing enterocolitis in newborns. Methods. 110 newborns with necrotizing enterocolitis were observed in the intensive care unit at the age of 1 to 28 days. According to the stages of necrotizing enterocolitis, all examined newborns were divided into three groups. Group 1 consisted of 49 newborns (40.5%) with necrotizing enterocolitis stage I, group 2 included 48 newborns (39.7%) with necrotizing enterocolitis stage II and group 3 included 13 newborns (10.7%) with necrotizing enterocolitis stage III. In 40 newborns with necrotizing enterocolitis, matrix metalloproteinase-2, -9, -17, cathelicidin, transferrin in the blood and fecal calprotectin in the feces were measured by ELISA. Results. Comparative analysis demonstrated that matrix metalloproteinase-2 was increased in newborns from group 1 by 6.9 times, in group 2 - by 8.3 times and in group 3 - by 10.7 times. Similarly, the level of metalloproteinase-9 was increased in group 1 by 3 times, in group 2 by 3.4 times, and in group 3 by 4.5 times compared to the newborns from the control group. The concentration of metalloproteinase-17 in newborns from groups 1 and 2 was almost the same and increased on average by 2.5 times, and by 3.6 times in group 3 compared to the control. In examined newborns, the highest level of cathelicidin and lowest level of transferrin were observed in necrotizing enterocolitis stage III, which indicates the more severe course of the disease and may be a predictor of changes in treatment tactics. So, taking into account the diagnostic value of fecal calprotectin (75%), it can be used as a noninvasive marker of inflammation in the intestine. Conclusion. The established changes in the level of biochemical markers (metalloproteinases, cathelicidin and transferrin in the blood and fecal calprotectin in feces) have diagnostic and prognostic value in the diagnosis, prediction of outcomes and optimization of treatment tactics of necrotizing enterocolitis in neonatal practice

    The Result of Timely Therapy for Whipple's Disease: a Clinical Case

    Аim:Β to provide basic information on Whipple's disease necessary for timely diagnosis and treatment, using the example of clinical observation.Key points.Β Whipple's disease is a rare systemic infectious disease that internists, gastroenterologists, rheumatologists, and other physicians may encounter. The incidence of Whipple's disease is extremely low and amounts to 1 case per 1,000,000–10,000,000 people. The low prevalence of pathology can lead to underdiagnosis in favour of more common diseases. This, in turn, may worsen the patient's prognosis, as it will delay the time for establishing the correct diagnosis and initiating the necessary therapy. A 50-year-old man complained of losing 10 kg of weight over 5 months, abdominal pain and bloating, pain in the joints of feet, and shoulders, accompanied by swelling and hyperemia. The disease began with articular syndrome followed by diarrhea and manifestations of malabsorption (iron deficiency anemia, hypoalbuminemia, hypercholesterolemia). The diagnosis was established on the basis of morphological changes in biopsy samples of the postbulbar part of the duodenum. The identified changes were represented by thickening of the villi and accumulations in the stroma of large macrophages (CD68+) with wide light cytoplasm containing abundant accumulations of PAS-positive, negative when stained with carbol fuchsin according to Ziehl β€” Nielsen and auramine-rhodamine (under microscopy in luminescence mode) short rods, as well as numerous optically empty small and larger cavities. Treatment with intravenous injections of ceftriaxone 2 g per day for 14 days and trimethoprim/sulfamethoxazole 1920 mg per day for 8 months led to improved health, normalization of laboratory parameters, endoscopic and morphological findings. Treatment with trimethoprim/sulfamethoxazole is planned to be continued for up to 12 months or longer if necessary.Conclusion.Β Timely diagnosis and initiation of antibiotic therapy will help to avoid late complications of the disease, including death

    Tamoxifen is a drug of choice for endocrine therapy of hormone receptor- positive breast cancer in women of reproductive age. The metabolic activity of tamoxifen is determined by the activity of the CYP2D6 enzyme encoded by the CYP2D6 gene: under the action of the enzyme, tamoxifen converts into the metabolically active form called endoxifen.
