    The Stories We Tell: Gender-Based Variances in Recovery Narratives

    Substance-related issues have long been a societal concern, yet there is a dearth of empirical evidence about effective treatments. One of the most prominent methods of resolving substance-related issues, Alcoholics Anonymous (AA), has been criticized for its white, Christian middle-class, heterosexual male provenance. Particularly, the utility of AA for women has been questioned. Yet, many women do find recovery within AA. Therefore, the question becomes less about the fundamental efficacy of AA and more about for whom does AA work. As such, the present study set out to analyze recovery narratives drawn from the primary AA text. The stories of four women and four men of early AA significance were selected for a feminist phenomenological analysis to determine how women may experience substance use and recovery differently from men. The study revealed that gender played a much more significant role in the experience of alcoholism and became less pertinent in the discussion of recovery. Both women and men were working to produce a counterstory within their recovery narratives, however women were working to do that on the dual fronts of gender and alcoholism. Finally, there was evidence of agency within the narratives that suggest that existing autonomy may be critical for those seeking recovery within AA

    Recovery-Informed Education as a Means of Institutional Sustainability for CRPs

    CRPs are uniquely positioned to be valuable assets for universities to address substance-related issues in the collegiate environment. Utilizing the experience of students and staff members engaged in recovery support services empowers CRPs to implement alcohol and other drug (AOD) programming for the general student population. This presentation will outline how CRPs can implement education programs within their university in order to provide wider support and resources as well as challenge injunctive norms on campus. By employing various education initiatives, CRPs can reach a broader range of students while also attaining sustainability within the university structure

    Characterizing Participation and Perceived Engagement Benefits in an Integrated Digital Behavioral Health Recovery Community for Women: A Cross-Sectional Survey

    Background: Research suggests that digital recovery support services (D-RSSs) may help support individual recovery and augment the availability of in-person supports. Previous studies highlight the use of D-RSSs in supporting individuals in recovery from substance use but have yet to examine the use of D-RSSs in supporting a combination of behavioral health disorders, including substance use, mental health, and trauma. Similarly, few studies on D-RSSs have evaluated gender-specific supports or integrated communities, which may be helpful to women and individuals recovering from behavioral health disorders. Objective: The goal of this study was to evaluate the SHE RECOVERS (SR) recovery community, with the following 3 aims: (1) to characterize the women who engage in SR (including demographics and recovery-related characteristics), (2) describe the ways and frequency in which participants engage with SR, and (3) examine the perception of benefit derived from engagement with SR. Methods: This study used a cross-sectional survey to examine the characteristics of SR participants. Analysis of variance and chi-square tests, as well as univariate logistic regressions, were used to explore each aim. Results: Participants (N=729, mean age 46.83 years; 685/729, 94% Caucasian) reported being in recovery from a variety of conditions, although the most frequent nonexclusive disorder was substance use (86.40%, n=630). Participants had an average length in recovery (LIR) of 6.14 years (SD 7.87), with most having between 1 and 5 years (n=300). The most frequently reported recovery pathway was abstinence-based 12-step mutual aid (38.40%). Participants reported positive perceptions of benefit from SR participation, which did not vary by LIR or recovery pathway. Participants also had high rates of agreement, with SR having a positive impact on their lives, although this too did vary by recovery length and recovery pathway. Participants with 1 to 5 years of recovery used SR to connect with other women in recovery at higher rates, whereas those with less than 1 year used SR to ask for resources at higher rates, and those with 5 or more years used SR to provide support at higher rates. Lifetime engagement with specific supports of SR was also associated with LIR and recovery pathway. Conclusions: Gender-specific and integrated D-RSSs are feasible and beneficial from the perspective of participants. D-RSSs also appear to provide support to a range of recovery typologies and pathways in an effective manner and may be a vital tool for expanding recovery supports for those lacking in access and availability because of geography, social determinants, or other barriers

    Evaluating the Intra-Rater and Inter-Rater Reliability of Fixed Tension Scale Instrumentation for Determining Isometric Neck Strength

    International Journal of Exercise Science 14(3): 563-577, 2021. PURPOSE: The purpose of this study was to evaluate the reliability of a fixed tension scale instrumentation, comparing the intra-rater and inter-rater reliability between seating and standing measurement techniques. Instrumentation developed from this study will be utilized to assess isometric neck strength in future studies comparing neck strengthening protocols. METHODS: Isometric neck strength for eight movements (cervical/capital flexion, cervical/capital extension, left/right lateral flexion, left/right cervical rotation) as well as anthropometric measurements were evaluated for thirty-one participants through the use of a novel neck strength assessment protocol. RESULTS: The fixed tension scale instrumentation and methods used in this study demonstrated good to excellent intra-rater reliability (ICC range from 0.78 to 0.97) as well as moderate to excellent inter-rater reliability (ICC range from 0.73 to 0.91) for both measurement techniques. PRACTICAL APPLICATIONS: This study will provide foundational knowledge for the reliable assessment of neck strength. Additionally, the findings will provide a cost-effective, portable, and reliable instrument for measuring isometric neck strength. CONCLUSIONS: Seated and standing measurement techniques demonstrated similar intra and inter-rater reliability. Inter-rater reliability tended to be lower with motions (capital flexion and extension) that required the participants to face directly towards or away from the instrumentation. This could be due to participant positioning or unfamiliarity with those specific movements. The assessment protocol utilized in this study demonstrated comparable inter-rater reliability to another cost-effective method for evaluating isometric neck strength

    Recommendations for Higher Educational Supports for Students Experiencing Homelessness in the Southeastern United States

    Homelessness creates tall barriers for many college students pursuing postsecondary education. It can also come along with many secondary difficulties, such as lack of family support, histories of abuse, trauma, neglect, and systemic poverty, all of which are added challenges when pursuing higher education. These barriers can limit access to higher education, making it difficult to remain enrolled, let alone graduate, and creating long-term negative effects such as long-term economic instability and a lack of self-sufficiency. The recent COVID-19 pandemic has only exacerbated many of these existing barriers, including low enrollment [1] and a decrease in financial aid completion [2]. New research from the National Student Clearinghouse Research Center found that of the “2.6 million students who entered college as first-time freshmen in the fall of 2019, only 74 percent returned for their second year — an unprecedented two percentage point drop, the lowest level since 2012”. However, the situation is more dire for low-income and minority students, and they have seen the most dramatic enrollment and persistence drops [3]. Historically, very few states have offered higher education policies that support homeless college students; however, states are beginning to respond because of the recent increase in student homelessness. The most recent example of new legislation supporting these students is Georgia Senate Bill 107, passed in May 2021, which amended Title 20 of the Official Code of Georgia Annotated relating to the postsecondary education of homeless and foster youth. Specifically, this bill provides: Tuition and room and board waivers at two-year colleges for foster care students In-state tuition at two- and four-year colleges for students under the age of 24 and experiencing homelessness Provisions for university staff to determine homelessness status, like the McKinney-Vento Act [4]. Considering the increase in the number of homeless students, states should focus on policies to support these students\u27 successful completion of a college degree. Such policies would be good for students, institutions, and state governments, as college graduates contribute more in taxes and are less reliant on government services [5]. This paper will examine policies and practices in seven southeastern states, draw conclusions, and make recommendations for higher education institutions, policymakers, and philanthropy.https://digitalcommons.kennesaw.edu/careresources/1003/thumbnail.jp

    Mucosal Infections and Invasive Potential of Nonencapsulated <i>Streptococcus pneumoniae</i> Are Enhanced by Oligopeptide Binding Proteins AliC and AliD.

    Nonencapsulated &lt;i&gt;Streptococcus pneumoniae&lt;/i&gt; (NESp) is an emerging human pathogen that colonizes the nasopharynx and is associated with noninvasive diseases such as otitis media (OM), conjunctivitis, and nonbacteremic pneumonia. Since capsule expression was previously thought to be necessary for establishment of invasive pneumococcal disease (IPD), serotype-specific polysaccharide capsules are targeted by currently licensed pneumococcal vaccines. Yet, NESp expressing oligopeptide binding proteins AliC and AliD have been isolated during IPD. Thus, we hypothesize AliC and AliD are major NESp virulence determinants that facilitate persistence and development of IPD. Our study reveals that NESp expressing AliC and AliD have intensified virulence compared to isogenic mutants. Specifically, we demonstrate AliC and AliD enhance murine nasopharyngeal colonization and pulmonary infection and are required for OM in a chinchilla model. Furthermore, AliC and AliD increase pneumococcal survival in chinchilla whole blood and aid in resistance to killing by human leukocytes. Comparative proteome analysis revealed significant alterations in protein levels when AliC and AliD were absent. Virulence-associated proteins, including a pneumococcal surface protein C variant (CbpAC), were significantly downregulated, while starvation response indicators were upregulated in the double mutant relative to wild-type levels. We also reveal that differentially expressed CbpAC was essential for NESp adherence to epithelial cells, virulence during OM, reduction of C3b deposition on the NESp surface, and binding to nonspecific IgA. Altogether, the rise in NESp prevalence urges the need to understand how NESp establishes disease and persists in a host. This study highlights the roles of AliC, AliD, and CbpAC in the pathogenesis of NESp. &lt;b&gt;IMPORTANCE&lt;/b&gt; Despite the effective, widespread use of licensed pneumococcal vaccines over many decades, pneumococcal infections remain a worldwide burden resulting in high morbidity and mortality. NESp subpopulations are rapidly rising in the wake of capsule-targeted vaccine strategies, yet there is very little knowledge on NESp pathogenic potential and virulence mechanisms. Although NESp lacks a protective capsule, NESp lineages expressing AliC and AliD have been associated with systemic infections. Furthermore, higher antibiotic resistance rates and transformation efficiencies associated with emerging NESp threaten treatment strategies needed to control pneumococcal infections and transmission. Elucidating how NESp survives within a host and establishes disease is necessary for development of broadened pneumococcal prevention methods. Our study identifies virulence determinants and host survival mechanisms employed by NESp with a high pathogenic potential. Moreover, our study also identifies virulence determinants shared by NESp and encapsulated strains that may serve as broad prevention and therapeutic targets

    How Will the Future of Work Shape Osh Research and Practice? a Workshop Summary

    Growth of the information economy and globalization of labor markets will be marked by exponential growth in emerging technologies that will cause considerable disruption of the social and economic sectors that drive the global job market. These disruptions will alter the way we work, where we work, and will be further affected by the changing demographic characteristics and level of training of the available workforce. These changes will likely result in scenarios where existing workplace hazards are exacerbated and new hazards with unknown health effects are created. The pace of these changes heralds an urgent need for a proactive approach to understand the potential effects new and emerging workplace hazards will have on worker health, safety, and well-being. As employers increasingly rely on non-standard work arrangements, research is needed to better understand the work organization and employment models that best support decent work and improved worker health, safety, and well-being. This need has been made more acute by the SARS-CoV-2 global pandemic that has resulted in dramatic changes in employment patterns, millions of lost jobs, an erosion of many economic sectors, and widespread disparities which further challenge occupational safety and health (OSH) systems to ensure a healthy and productive workplace. to help identify new research approaches to address OSH challenges in the future, a virtual workshop was organized in June 2020 with leading experts in the fields of OSH, well-being, research methods, mental health, economics, and life-course analysis. A paradigm shift will be needed for OSH research in the future of work that embraces key stakeholders and thinks differently about research that will improve lives of workers and enhance enterprise success. A more transdisciplinary approach to research will be needed that integrates the skills of traditional and non-traditional OSH research disciplines, as well as broader research methods that support the transdisciplinary character of an expanded OSH paradigm. This article provides a summary of the presentations, discussion, and recommendations that will inform the agenda of the Expanded Focus for Occupational Safety and Health (Ex4OSH) International Conference, planned for December 2021

    Surface proteins and pneumolysin of encapsulated and nonencapsulated Streptococcus pneumoniae mediate virulence in a chinchilla model of otitis media

    Streptococcus pneumoniae infections result in a range of human diseases and are responsible for almost one million deaths annually. Pneumococcal disease is mediated in part through surface structures and an anti-phagocytic capsule. Recent studies have shown that nonencapsulated Streptococcus pneumoniae (NESp) make up a significant portion of the pneumococcal population and are able to cause disease. NESp lack some common surface proteins expressed by encapsulated pneumococci, but express surface proteins unique to NESp. A chinchilla model of otitis media (OM) was used to determine the effect various pneumococcal mutations have on pathogenesis in both NESp and encapsulated pneumococci. Epithelial cell adhesion and invasion assays were used to examine the effects in relation to deletion of intrinsic genes or expression of novel genes. A mouse model of colonization was also utilized for comparison of various pneumococcal mutants. It was determined that pneumococcal surface protein K (PspK) and pneumolysin (Ply) affect NESp middle ear pathogenesis, but only PspK affected epithelial cell adhesion. Experiments in an OM model were done with encapsulated strains testing the importance of native virulence factors and treatment of OM. First, a triple deletion of the common virulence factors PspA, PspC, and Ply, (ΔPAC), from an encapsulated background abolished virulence in an OM model while a PspC mutant had detectable, but reduced amounts of recoverable bacteria compared to wildtype. Next, treatment of OM was effective when starting antibiotic treatment within 24 hrs with resolution by 48 hrs post treatment. Expression of NESp-specific virulence factor PspK in an encapsulated strain has not been previously studied, and we showed significantly increased adhesion and invasion of human epithelial cells by pneumococci. Murine colonization was not significantly increased when an encapsulated strain expressed PspK, but colonization was increased when a capsule mutant expressed PspK. The ability of PspK expression to increase colonization in a capsule mutant despite no increase in adhesion can be attributed to other functions of PspK, such as sIgA binding or immune modulation. OM is a substantial economic burden, thus a better understanding of both encapsulated pneumococcal pathogenesis and the emerging pathogen NESp is necessary for effective prevention and treatment
