    Management of intertrochanteric fracture in a bilateral below knee amputee

    Intertrochanteric fracture in a patient with bilateral below knee amputation is rare and the management is challenging. The major difficulty is in applying optimal amount of traction for reduction of the fracture due to absence of foot. We present the case of an elderly female with bilateral below knee amputation presenting with a minimally displaced right intertrochanteric fracture. The patient was managed using reverse boot technique for applying traction, obtaining reduction and fixed with right proximal femoral nail. The patient was able to return to her pre-injury mobility status on the third postoperative day. We highlight a method of traction application using this simple modification of the fracture table so that adequate amount of traction can be applied safely

    An electric screwdriver was designed to work in an automatic cycle on assembly lines. A characteristic feature of the screwdriver is the use of a low power motor in comparison with commonly used devices. The work of tightening the threaded joint takes place at the expense of the kinetic energy of the components of the drive system and the working screwdriver. It has been proved that the ability of the screwdriver to perform the tightening work is determined by the sum of the mass moment of inertia of the working components system and the mass moment of inertia of the motor's rotor, reduced to the axis of the screwdriver bit. The process of tightening is characterized by the number of screwing pulses, screwing torque values at the end of each pulse and the time of screwing process. The limitation of the value of the transmitted tightening torque takes place through the applied overload coupling.  The construction work of the screwdriver was preceded by the development of a mathematical model of the process of screwing and balance of torques acting during each phase of operation was carried out. This allowed the calculation of the value of the kinetic energy of components of the drive and operating system translated into the tightening work. The kinematic conditions to be met by the screwdriver at the end of a single tightening pulse were determined in order to accumulate kinetic energy and start another tightening pulse.Электрическая отвертка была разработана для автоматической работы на сборочных линиях. Характерной особенностью отвертки является использование двигателя малой мощности по сравнению с обычно применяемыми устройствами. Работа по натяжению резьбового соединения происходит за счет кинетической энергии компонентов системы привода и рабочей отвертки. Доказано, что способность отвертки выполнять затяжные работы определяется суммой массового момента инерции системы рабочих компонентов и массового момента инерции ротора двигателя, приведенного к оси наконечника отвертки. Процесс затягивания характеризуется количеством импульсов завинчивания, значениями момента завинчивания в конце каждого импульса и временем процесса завинчивания. Ограничение величины передаваемого момента затяжки происходит через применяемую перегрузочную муфту. Строительным работам по отвертке предшествовала разработка математической модели процесса завинчивания и балансировки крутящих моментов, действующих на каждом этапе работы. Это позволило рассчитать значение кинетической энергии компонентов привода и операционной системы в переводе на затяжные работы. Кинематические условия, которые должны быть выполнены отверткой в конце одного импульса затяжки, были определены для накопления кинетической энергии и запуска другого импульса затяжки

    It Takes Two to Tango: When and Where Dual Nutrient (N & P) Reductions Are Needed to Protect Lakes and Downstream Ecosystems

    Preventing harmful algal blooms (HABs) is needed to protect lakes and downstream ecosystems. Traditionally, reducing phosphorus (P) inputs was the prescribed solution for lakes, based on the assumption that P universally limits HAB formation. Reduction of P inputs has decreased HABs in many lakes, but was not successful in others. Thus, the "P-only" paradigm is overgeneralized. Whole-lake experiments indicate that HABs are often stimulated more by combined P and nitrogen (N) enrichment rather than N or P alone, indicating that the dynamics of both nutrients are important for HAB control. The changing paradigm from P-only to consideration of dual nutrient control is supported by studies indicating that (1) biological N fixation cannot always meet lake ecosystem N needs, and (2) that anthropogenic N and P loading has increased dramatically in recent decades. Sediment P accumulation supports long-term internal loading, while N may escape via denitrification, leading to perpetual N deficits. Hence, controlling both N and P inputs will help control HABs in some lakes and also reduce N export to downstream N-sensitive ecosystems. Managers should consider whether balanced control of N and P will most effectively reduce HABs along the freshwater-marine continuum

    The Treatment With the SGLT2 Inhibitor Empagliflozin Modifies the Hepatic Metabolome of Male Zucker Diabetic Fatty Rats Towards a Protective Profile

    [Abstract] The EMPA-REG OUTCOME (Empagliflozin, Cardiovascular Outcome Event Trial in patients with Type 2 Diabetes Mellitus (T2DM)) trial evidenced the potential of sodium-glucose cotransporter 2 (SGLT2) inhibitors for the treatment of patients with diabetes and cardiovascular disease. Recent evidences have shown the benefits of the SGLT2 inhibitor empagliflozin on improving liver steatosis and fibrosis in patients with T2DM. Metabolomic studies have been shown to be very useful to improve the understanding of liver pathophysiology during the development and progression of metabolic hepatic diseases, and because the effects of empagliflozin and of other SGLT2 inhibitors on the complete metabolic profile of the liver has never been analysed before, we decided to study the impact on the liver of male Zucker diabetic fatty (ZDF) rats of a treatment for 6 weeks with empagliflozin using an untargeted metabolomics approach, with the purpose to help to clarify the benefits of the use of empagliflozin at hepatic level. We found that empagliflozin is able to change the hepatic lipidome towards a protective profile, through an increase of monounsaturated and polyunsaturated glycerides, phosphatidylcholines, phosphatidylethanolamines, lysophosphatidylinositols and lysophosphatidylcholines. Empagliflozin also induces a decrease in the levels of the markers of inflammation IL-6, chemerin and chemerin receptor in the liver.     Mineralogy and olivine cooling rate of the Dhofar 019 shergottite

    Dhofar 019 is a new basaltic shergottite found in Oman. It is mainly composed of pyroxenes, plagioclase glass, and olivine. Olivine grains show variable size distributions from large (∿1mm) non-euhedral zoned grains with magnesian cores (Fa_) to small euhedral homogeneous grains with variable compositions (Fa_). This may suggest that some of the Dhofar 019 olivines are xenocrysts and others are phenocrysts. The Dhofar 019 olivines are compositionally distinct from those in other shergottites. Pyroxenes are extensively zoned, and pigeonite cores are usually surrounded with augite rims, suggesting undercooling of magma to a similar degree as that of EETA 79001 (lithology A) and Zagami. There appears to be a compositional change in major and minor elements from the magnesian pyroxenes to ferroan pyroxenes for both pigeonite and augite. The magnesian parts have similar compositions to EETA 79001 (lithology A) pyroxenes, and the ferroan parts are similar to Zagami pyroxenes. Plagioclase zoning in Dhofar 019 more closely resembles that of Zagami. Spinel compositions in Dhofar 019 overlap those of EETA79001,and the Dhofar 019 chromite may be a xenocryst like EETA79001. The cooling rate of Dhofar 019 calculated from Fe-Mg zoning profiles of olivine is 0.05-0.1℃/hour. This corresponds to a burial depth of ∿5m from the martian surface, possibly in a lava flow. Thus, the groundmass crystallization of Dhofar 019 occurred near the martian surface, although it is likely that Dhofar 019 has experienced a complex history
