Plasma levels of mannan-binding lectin-associated serine proteases are increased in type 1 diabetes patients with insulin resistance.
Activation of the lectin pathway of the complement system, as demonstrated by elevated levels of mannan-binding lectin proteins (MBL), contributes to vascular pathology in type 1 diabetes (T1D). Vascular complications are greatest in T1D individuals with concomitant insulin resistance (IR), however, whether IR amplifies activiation of the lectin pathway in T1D is unknown. We pooled pre-treatment data from two RCTs and performed a cross-sectional analysis on 46 T1D individuals. We employed estimated glucose disposal rate (eGDR), a validated IR surrogate with cut-points of: 8.7Â mg/kg/min to determine IR status, with lower eGDR values conferring higher degrees of IR. Plasma levels of MBL-associated proteases (MASP-1, MASP-2, MASP-3) and their regulatory protein MAp44 were compared among eGDR classifications. In a subset of 14 individuals, we assessed change in MASPs and MAp44 following improvement in IR. We found that MASP-1, MASP-2, MASP-3, and MAp44 levels increased in a stepwise fashion across eGDR thresholds with elevated MASPs and MAp44 levels conferring greater degrees of IR. In a subset of 14 patients, improvement in IR was associated with significant reductions in MASPs, but not MAp44, levels. In conclusion, IR in T1D amplifies levels of MASP-1/2/3 and their regulator MAp44, and improvement of IR normalises MASP-1/2/3 levels. Given that elevated levels of these proteins contribute to vascular pathology, amplification of the lectin pathway of the complement system may offer mechanistic insight into the relationship between IR and vascular complications in T1D
Hydrographic Properties and Inferred Circulation Over the Northeastern Shelves of the Gulf of Mexico During Spring to Midsummer of 1998
A hydrographic cruise was conducted 5-16 May 1998 over the northeastern shelves of the Gulf of Mexico, Observed distributions of temperature, salinity, oxygen, and nutrients were consonant with prior occurrences of upwelling, particularly near the head of DeSoto Canyon. Shipboard, moored, and satellite observations indicated these upwelling events were related to the presence of an anticyclonic circulation feature over the canyon. In addition, several cool water events occurred during spring in the nearshore region west of Pensacola; these may be attributed to atmospheric effects. High river discharges from rivers west of the Apalachicola during winter and spring likely resulted in the extensive surface distributions of low-salinity water observed from Mississippi Sound to Cape San Bias during the cruise. The combination of cool bottom temperatures and relatively low surface salinities over the inshore shelf west of Cape San Bias, with the usual seasonal warming, resulted in enhanced vertical stability. This stability likely inhibited vertical mixing and contributed to the development of the relatively low concentrations of dissolved oxygen observed in the bottom waters
Integrated Active Fire Retrievals and Biomass Burning Emissions Using Complementary Near-Coincident Ground, Airborne and Spaceborne Sensor Data
Ground, airborne and spaceborne data were collected for a 450 ha prescribed fire implemented on 18 October 2011 at the Henry W. Coe State Park in California. The integration of various data elements allowed near coincident active fire retrievals to be estimated. The Autonomous Modular Sensor-Wildfire (AMS) airborne multispectral imaging system was used as a bridge between ground and spaceborne data sets providing high quality reference information to support satellite fire retrieval error analyses and fire emissions estimates. We found excellent agreement between peak fire radiant heat flux data (less than 1% error) derived from near-coincident ground radiometers and AMS. Both MODIS and GOES imager active fire products were negatively influenced by the presence of thick smoke, which was misclassified as cloud by their algorithms, leading to the omission of fire pixels beneath the smoke, and resulting in the underestimation of their retrieved fire radiative power (FRP) values for the burn plot, compared to the reference airborne data. Agreement between airborne and spaceborne FRP data improved significantly after correction for omission errors and atmospheric attenuation, resulting in as low as 5 difference between AquaMODIS and AMS. Use of in situ fuel and fire energy estimates in combination with a collection of AMS, MODIS, and GOES FRP retrievals provided a fuel consumption factor of 0.261 kg per MJ, total energy release of 14.5 x 10(exp 6) MJ, and total fuel consumption of 3.8 x 10(exp 6) kg. Is facet joint distraction a cause of postoperative axial neck pain after ACDF surgery?
Introduction: Intervertebral distraction in anterior cervical discectomy and fusion (ACDF) has been postulated to injure the degenerative facet joints posteriorly and increase postoperative pain and disability. This study aims to determine if there is a correlation between the amount of facet distraction and postoperative patient reported outcomes.
Methods: A retrospective cohort analysis of patients undergoing ACDF for degenerative pathologies was performed. Each patient received lateral cervical spine x-rays at the immediate postoperative time point and were split into groups based on the amount of facet distraction measured on these films: Group A: \u3c 1.5 mm; Group B: 1.5-2.0 mm; and Group C: \u3e 2.0 mm. Patients reported outcome measures were obtained preoperatively and at 1-year postoperatively. Univariate and multivariate analyses were performed to compare outcomes between groups.
Results: A total of 229 patients were included with an average follow-up of 19.8 [19.0, 20.7] months with a mean facet joint distraction of 1.7mm. There were 87 patients in Group A, 76 patients in Group B, and 66 patients in Group C. Patients significantly improved across all outcome measures from baseline to postoperatively (p \u3c 0.05). There was no difference between groups at any time point with respect to outcome scores (p \u3e 0.05). Multiple regression analysis did not identify increasing distraction as a predictor of patient outcomes.
Conclusions: There were no significant differences between patient outcomes and the amount of facet distraction after ACDF surgery. Multivariate analysis did not find a correlation between facet distraction and overall HRQOL outcome
β-Glucosylceramide From Allergic Mothers Enhances Offspring Responsiveness to Allergen
In animals and humans, offspring of allergic mothers have increased responsiveness to allergen and the allergen-specificity of the offspring can be different than that of the mother. In our preclinical models, the mother's allergic responses influence development of the fetus and offspring by elevating numbers of cells in dendritic cell subsets. A major question is the identity of maternal factors of allergic mothers that alter offspring development of responsiveness to allergen. Lipids are altered during allergic responses and lipids are transported to the fetus for growth and formation of fetal membranes. We hypothesized that pro-inflammatory lipids, that are elevated in allergic mothers, are transported to the fetus and regulate fetal immune development. We demonstrate in this report that there was a significant 2-fold increase in β-glucosylceramides (βGlcCer) in allergic mothers, the fetal liver and her offspring. The βGlcCer were transported from mother's plasma, across the placenta, to the fetus and in breastmilk to the offspring. Administration of βGlcCer to non-allergic mothers was sufficient for offspring responses to allergen. Importantly, maternal administration of a clinically relevant pharmacological inhibitor of βGlcCer synthase returned βGlcCer to normal levels in the allergic mothers and her offspring and blocked the offspring increase in dendritic cell subsets and offspring allergen responsiveness. In summary, allergic mothers had increased βGlcCer that was transported to offspring and mediated increases in offspring DCs and responsiveness to allergen. These data have a significant impact on our understanding of mechanisms for development of allergies in offspring of allergic mothers and have the potential to lead to novel interventions that significantly impact risk for allergic disease early in life
Genome sequencing of the extinct Eurasian wild aurochs, Bos primigenius, illuminates the phylogeography and evolution of cattle
Background
Domestication of the now-extinct wild aurochs, Bos primigenius, gave rise to the two major domestic extant cattle taxa, B. taurus and B. indicus. While previous genetic studies have shed some light on the evolutionary relationships between European aurochs and modern cattle, important questions remain unanswered, including the phylogenetic status of aurochs, whether gene flow from aurochs into early domestic populations occurred, and which genomic regions were subject to selection processes during and after domestication. Here, we address these questions using whole-genome sequencing data generated from an approximately 6,750-year-old British aurochs bone and genome sequence data from 81 additional cattle plus genome-wide single nucleotide polymorphism data from a diverse panel of 1,225 modern animals.
Results
Phylogenomic analyses place the aurochs as a distinct outgroup to the domestic B. taurus lineage, supporting the predominant Near Eastern origin of European cattle. Conversely, traditional British and Irish breeds share more genetic variants with this aurochs specimen than other European populations, supporting localized gene flow from aurochs into the ancestors of modern British and Irish cattle, perhaps through purposeful restocking by early herders in Britain. Finally, the functions of genes showing evidence for positive selection in B. taurus are enriched for neurobiology, growth, metabolism and immunobiology, suggesting that these biological processes have been important in the domestication of cattle.
Conclusions
This work provides important new information regarding the origins and functional evolution of modern cattle, revealing that the interface between early European domestic populations and wild aurochs was significantly more complex than previously thought
In-Hospital Mortality Trends After Surgery for Traumatic Thoracolumbar Injury: A National Inpatient Sample Database Study
INTRODUCTION: Given the increasing incidence of traumatic thoracolumbar injuries in recent years, studies have sought to investigate potential risk factors for outcomes in these patients.
RESEARCH QUESTION: The aim of this study was to investigate trends and risk factors for in-hospital mortality after fusion for traumatic thoracolumbar injury.
MATERIALS AND METHODS: Patients undergoing thoracolumbar fusion after traumatic injury were queried from the National Inpatient Sample (NIS) from 2012 to 2017. Analysis was performed to identify risk factors for inpatient mortality after surgery.
RESULTS: Patients in 2017 were on average older (51.0 vs. 48.5, P = 0.004), had more admitting diagnoses (15.5 vs. 10.7, p \u3c 0.001), were less likely to be White (75.8% vs. 81.2%, p = 0.006), were from a ZIP code with a higher median income quartile (Quartile 1: 31.4% vs. 28.6%, p = 0.011), and were more likely to have Medicare as a primary payer (22.9% vs. 30.1%, p \u3c 0.001). Bivariate analysis of demographics and surgical characteristics demonstrated that patients in the in-hospital mortality group (n = 90) were older (70.2 vs. 49.6, p \u3c 0.001), more likely to be male (74.4% vs. 62.8%, p = 0.031), had a great number of admitted diagnoses (21.3 vs. 12.7, p \u3c 0.001), and were more likely to be insured by Medicare (70.0% vs. 27.0%, p \u3c 0.001). Multivariate regression analysis found age (OR 1.06, p \u3c 0.001) and Black race (OR 3.71, p = 0.007) were independently associated with in-hospital mortality.
CONCLUSION: Our study of nationwide, traumatic thoracolumbar fusion procedures from 2012 to 2017 in the NIS database found older, black patients were at increased risk for in-hospital mortality after surgery
